Rational design of FAP-targeted sEVs delivered by microneedles for precision treatment of hypertrophic scars via ferroptosis in hypertrophic scar fibroblasts.

Hypertrophic scars (HS) lack cell-specific therapies because pathogenic hypertrophic scar fibroblasts (HSFs) are hard to inactivate without damaging human dermal fibroblasts (HDFs). Ferroptosis has emerged as a potential antifibrotic vulnerability of activated fibroblasts, yet its translational use is constrained by off-target toxicity of small-molecule inducers. Here, we report a targeted delivery strategy based on small extracellular vesicles (sEVs) surface-decorated with a fibroblast activation protein (FAP) ligand to preferentially deliver a widely used ferroptosis inducer erastin to HSFs.

Bioactive microneedles enabled by Ca-MOF-regulated collagenase for synergistic hypertrophic scar remodeling.

Hypertrophic scars resulting from pathological fibrosis and excessive collagen deposition cause significant functional and aesthetic impairment. Conventional therapies often fail to address the underlying molecular mechanisms of collagen overproduction. To overcome these limitations, we developed a novel layered dissolving microneedle system (MOF-CCH-MN) capable of delivering a synergistic therapeutic payload. This system incorporates Clostridium histolyticum collagenase (CCH) loaded within a calcium-based metal-organic framework (Ca-MOF), embedded in a γ-polyglutamic acid (γ-PGA) matrix. Char

Deoxyshikonin inhibits growth and induces apoptosis of hypertrophic scar-derived fibroblasts by downregulating FBXO expression through autophagy.

Hypertrophic scar (HS) is a fibro-proliferative disorder of cutaneous wound healing, which seriously impacts the patient's quality of life. Deoxyshikonin (DSK) as a natural shikonin derivative possesses various pharmacological effects, however, whether DSK is involved in anti-fibrotic function for HS remain unclear. This study aimed to investigate the effect of DSK on HS-derived fibroblasts (HSFs) growth and reveal its possible mechanism. HSFs were isolated from human HS tissues. The viability and proliferation of HSFs under DSK treatment were evaluated by CCK-8 and EdU assays. The evaluation

Effects of aesthetic suturing on wound healing and scar formation in patients with traffic accident-related injuries.

INTRODUCTION: Traffic accident-related lacerations frequently require urgent primary closure and may be complicated by irregular wound margins, heterogeneous contamination, and variable mechanical tension, all of which can influence scar maturation. This study evaluated the association between aesthetic suturing and clinical wound healing as well as scar-related outcomes in patients with traffic accident-related injuries. METHODS: In this retrospective observational study, 117 consecutive patients treated between March 2023 and September 2024 were categorized according to the closure approach

From RCT to mechanistic study: ATRA reverses myofibroblast activation by reprogramming glucose metabolism via HIC1 and PCK1/2 to attenuate hypertrophic scar formation.

BACKGROUND: Abnormal glucose metabolism often contributes to myofibroblast activation and the pathogenesis of skin fibrotic diseases. All-trans retinoic acid (ATRA), the active component of tretinoin cream, can regulate glucose metabolism and activate myofibroblasts. Importantly, investigating the potential of ATRA to inhibit myofibroblast activation by modulating glucose metabolism could reveal the translational significance of ATRA in attenuating hypertrophic scar (HS) formation. METHODS: We first conducted a multicenter, double-blind, randomized controlled trial (RCT) to compare the effect

ROS, identified as a driver for fibroblast activation, can be counteracted by glucocorticoid in LINC00605-dependent GMD machinery.

Fibroblast-to-myofibroblast transformation embodies a central facet of fibroblast activation, evidenced by elevated α-SMA levels and increased extracellular matrix synthesis. While excessive reactive oxygen species (ROS) are traditionally viewed as drivers of oxidative stress-related damage, emerging data implicate ROS in the pathological transformation of certain fibroblast subsets. However, the precise contributions of raised ROS to fibroblast activation remain to be defined. In this study, employing single-cell RNA sequencing and hypertrophic scars as a model of phenotypic transition, we de

Combined Mohs micrographic surgery and 5-aminolevulinic acid photodynamic therapy for nasal ala basal cell carcinoma: A case report.

Basal cell carcinoma (BCC) is a common skin malignancy, often occurring in cosmetically sensitive facial areas. We report a case of a 46-year-old woman with BCC of the right nasal ala. The lesion had persisted for nine years, gradually enlarging, with recent ulceration and crusting. Skin biopsy confirmed BCC. The patient underwent Mohs micrographic surgery (MMS), with the wound left to heal by secondary intention for 48 h, followed by sequential 5-aminolevulinic acid photodynamic therapy (ALA-PDT) once weekly for five consecutive weeks. A freshly prepared 20 % 5-aminolevulinic acid (ALA) solut

Research advances in the pathogenesis and photodynamic therapy of pathological scars.

This review summarizes recent advances in the pathogenesis of pathological scars, including hypertrophic scars and keloids, and their photodynamic therapy (PDT). The formation of pathological scars involves fibroblast dysfunction, imbalanced ECM metabolism, chronic inflammation, aberrant activation of multiple signaling pathways (e.g., TGF-β/Smad, JAK/STAT, PI3K/Akt/mTOR), epigenetic regulation, and biomechanical factors, against a background of genetic susceptibility. As a minimally invasive and targeted therapy, PDT acts through the generation of reactive oxygen species (ROS) upon light acti

Chitosan nanoparticles-incorporated polylactic acid/tacrolimus nanofiber composite membrane for tracheal scar inhibition and tracheal wound healing promotion.

Postoperative tracheal stenosis caused by excessive scar formation is a major complication of complex thoracic surgeries (e.g., sleeve lobectomy, lung transplantation) with limited effective preventive strategies. We hypothesized that a composite membrane integrating chitosan (CS), polylactic acid (PLA), and tacrolimus (TAC) via simultaneous electrospinning and electrospraying would inhibit tracheal scarring and promote tracheal wound healing by synergizing their respective advantages. Herein, we fabricated the CS nanoparticle-incorporated PLA/TAC nanofiber (CS/PLA/TAC) composite membrane and

Clinical Efficacy of Local Injection of Compound Betamethasone Versus Triamcinolone Acetonide in the Treatment of Hypertrophic Scars: A Retrospective Analysis of 126 Patients.

BACKGROUND: Hypertrophic scars (HTS) are common complications of wound healing characterized by excessive fibroblast proliferation and collagen deposition. HTS causes local elevation, pruritus, and pain, significantly impacting aesthetics and quality of life. While triamcinolone acetonide (TA) and compound betamethasone (CB) are standard treatments, direct comparative studies regarding their efficacy and recurrence rates remain limited. OBJECTIVE: To evaluate the clinical efficacy, safety, and recurrence rate over 6 months of local injection of TA and CB monotherapy in the treatment of HTS.

Refractory Keloids and Hypertrophic Scars: Immune Dysregulation and Neuroimmune Mechanisms Underlying Treatment Failure.

Keloids and hypertrophic scars are fibroproliferative disorders of wound healing characterized by excessive extracellular matrix deposition, constant inflammation, and high recurrence rates despite appropriate management. Conventional therapies, including surgical excision, corticosteroid injections, laser therapy, and radiation, can provide temporary relief. However, treatment failure remains common, specifically in refractory keloids. Recent findings suggest these outcomes cannot be fully explained by technical or mechanical factors alone, and pathological scarring may reflect underlying imm

Efficacy and Safety of Cryotherapy Combined With Intralesional Steroid for Keloid and Hypertrophic Scar: A Systematic Review and Meta-Analysis.

OBJECTIVE: Intralesional steroid injection is the first-line therapy for scars. Emerging evidence indicates that combining intralesional steroid injection with cryotherapy yields promising therapeutic outcomes for scar management. However, a consensus on the efficacy and safety of this combined regimen has not yet been reached. Consequently, the present meta-analysis aimed to assess the efficacy and safety of this combination compared with control groups. METHODS: A full-scale literature retrieval was performed across a range of databases to identify eligible randomized controlled trials (RCT

Porous GelMA microneedle with delivery of AIE molecules via HSF-derived exosomes for hypertrophic scar treatment.

Hypertrophic scarring (HS) occurs after surgery or wounding, leading to tissue dysfunction and damaging the appearance. However, no satisfactory treatment strategy is available because the demand for eliminating hypertrophic scar fibroblasts (HSFs) in HS remains unfulfilled. Here, aggregation-induced emission molecule (TTMN)-based porous gelatin methacryloyl microneedles (MNs) were fabricated. The TTMNs were encapsulated in HSF-derived exosomes, which were subsequently assembled in the MN patch. This MN patch exhibited good biocompatibility. When the porous MN patch was applied to the HS, it r

Exosome-Coated Boron-Containing Evodiamine Derivative Triggered by Pathology-Specific Reactive Oxygen Species Targets Myofibroblasts to Ameliorate Hypertrophic Scarring.

Targeting overactivated myofibroblasts is a potential treatment for hypertrophic scarring. Evodiamine, an indole alkaloid derived from Evodiae fructus, has attracted increasing attention for its antitumor efficacy to inhibit proliferation and promote apoptosis of tumor cells. However, its role on hypertrophic scarring remains unexplored. Hence, we engineered a boronic acid-functionalized evodiamine derivative (BED) triggered by pathology-specific reactive oxygen species (ROS) to target myofibroblasts to ameliorate hypertrophic scarring with minimal toxicity to normal fibroblasts. In vitro expe

Transcriptome-guided development of a fibrosis-reversal compound reduces skin scarring and allows regeneration via mitochondrial uncoupling.

Skin scarring impairs function and aesthetics. Current therapies show limited efficacy and cause iatrogenic dermal disruption (e.g., triamcinolone acetonide [TA], a first-line corticosteroid for keloids), with topical medications demonstrating inferior outcomes. Through transcriptome-guided evaluation, we develop FR-1 (fibrosis-reversal compound 1), a small molecule that reverses fibrosis in vitro by inhibiting fibroblast proliferation, suppressing α-smooth muscle actin (α-SMA), and remodeling the extracellular matrix (ECM) via collagen downregulation and matrix metalloproteinase-1 (MMP1) indu

Fluid silicone gels in the prevention and treatment of hypertrophic scars: a systematic review and meta-analysis of randomised controlled trials.

Hypertrophic scars are a benign yet potentially debilitating cutaneous manifestation, characterised by elevated, pigmented scarring confined within the wound boundaries. They can affect the patients both physically, through pruritus, pain, stiffness and contractures, and also psychologically through stigmatization, anxiety, depression, diminished self-esteem and daily activity disruption, reducing the quality of life. There are currently various treatments used, and although silicone gel fluid is widely recognised as a leading non-invasive treatment, there is inconsistent evidence of superiori

Adipose Stem Cell-Derived Apoptotic Vesicles Attenuate Hypertrophic Scarring by Targeting the CDC20/WNT Signaling Pathway.

Background: Apoptotic vesicles (ApoVs) derived from adipose stem cells (ASCs) have recently emerged as important mediators of tissue repair and are implicated in pathways relevant to hypertrophic scar (HS). Although ASCs exhibit potential in scar modulation, the therapeutic value of their apoptotic clearance products remains largely unexplored. Methods: In this study, we investigated the efficacy and mechanism of staurosporine (STS)-induced adipose stem cell derived apoptotic vesicles (ASCs-ApoVs) in mitigating HS. Western blot, RT-qPCR, and immunofluorescence were used to assess fibrotic mark
