A wearable ultrasound-assisted microneedle system for the treatment of poorly permeable skin lesions.

Poorly permeable skin lesions (e.g., hypertrophic scars and psoriasis) exhibit impaired barrier function, making it difficult for drugs to penetrate to deeper layers. Microneedles (MNs) have emerged as a promising strategy to bypass the stratum corneum, but their efficacy in poorly permeable lesions is restricted by excessive insertion force, insufficient penetration depth, and limited drug diffusion. Herein, we develop a wearable ultrasound-assisted microneedle system (WUAMS) that integrates low-frequency ultrasound with titanium nanotubes (TNTs)-functionalized MNs for poorly permeable skin l

Mesenchymal Stem Cell-Derived Exosomal miR-29a-3p Improves Hypertrophic Scar by Inhibiting the Proliferation and Migration of Schwann Cells via the PDGFRB/PAK1 Axis.

Mesenchymal stem cell-derived exosomes (MSC-exo) can alleviate hypertrophic scar (HS) formation, whereas Schwann cells (SCs) promote HS formation. This study aimed to investigate whether MSC-exos attenuate HS by modulating SCs and to elucidate the underlying mechanisms. HS and normal skin tissues were obtained from patients. MSCs, SCs, and fibroblasts were isolated from BALB/c mice. SCs and HS mouse models were treated with MSC-exos. SCs under various treatments were co-cultured with fibroblasts. mRNA and protein levels were assessed by qRT-PCR, western blot, immunofluorescence, and immunohist

The Role and Mechanism of Nanozymes in Burn Wound Healing.

The treatment of burn wounds is a complex and lengthy process, including infection control, inflammation modulation, tissue regeneration, and scar management. Although significant progress has been achieved, numerous clinical challenges persist, especially the increased bacterial resistance, the risk of wound sepsis, and the issue of serious hypertrophic scar. In recent years, nanozymes have emerged as a hotspot in materials research and increasingly been applied to promote burn wound healing. Possessing multiple enzyme-like activities, nanozymes can integrate antibacterial, anti-inflammatory,

LncRNA TUG1 promotes hypertrophic scar formation via the miR-627/IGF1R axis.

Hypertrophic scars (HS) are fibrotic skin disorders driven by abnormal fibroblast activity. The molecular mechanisms underlying HS remain incompletely understood, particularly the role of non-coding RNAs. Expression levels of lncRNA TUG1, miR-627, and IGF1R were measured in HS tissues and fibroblasts using qRT-PCR and Western blotting. Dual-luciferase assays validated direct interactions. Functional effects of TUG1 and miR-627 on fibroblast proliferation and migration were assessed using MTT and Transwell assays. A rabbit ear model of HS was used to examine in vivo effects of TUG1 and miR-627

Modeling Human Hypertrophic Scars with Induced Pluripotent Stem-Cell-Derived Scar Organoids Versus Skin Organoids.

Hypertrophic scars are characterized by excessive collagen deposition, fibrotic remodeling, and functional impairment. However, the ability of current models is limited in recapitulating human pathology. This study presents a novel approach using induced pluripotent stem cell-derived scar organoids to model hypertrophic scar characteristics in vitro. Following established protocols, human pluripotent stem cells were differentiated into skin organoids and induced fibrotic transformation by treatment with TGF-β1 (10 ng/mL) and hypoxia (5% O2) from day 45 onward. Scar organoids exhibited signific

Anti-fibrotic effects of evogliptin in fibroblasts derived from post-burn hypertrophic scars.

Hypertrophic scar (HTS) frequently occurs after burn injury, characterized by persistent fibroblast activation, overexpression of α-smooth muscle actin (αSMA) and extracellular matrix (ECM) components. Complete scarless regeneration remains incompletely understood. Dipeptidyl peptidase-4 (DPP4) is a cell-surface serine protease, its inhibitors (DPP4i) have been associated with fibrosis. However, the functional role of evogliptin in HTS formation remains unclear. This investigated its effects on fibrosis markers in hypertrophic scar fibroblasts (HTSFs). Human normal fibroblasts (HNFs) and HTSFs

Advances in microneedle design for the delivery of drugs, proteins, and cells in the treatment of hypertrophic scars.

Hypertrophic scars (HS) represent a significant clinical challenge due to their complex pathophysiology and resistance to conventional therapies, often resulting in persistent symptoms such as itching, pain, and impaired joint mobility that compromise patients' quality of life. Current treatment modalities, including compression therapy, pharmacological agents, radiation, silicone gel, and laser therapies, have faced limitations primarily due to inadequate drug penetration into the dense fibrotic scar tissue. In this context, microneedle-mediated controlled delivery systems have emerged as a p

Stand-Alone Carboxytherapy Followed by Consolidation with Oral Hydrolyzed Marine Collagen Fragments: A Potential Sequential Bioregenerative Strategy for Hypertrophic Scars: A Case Report.

INTRODUCTION: Carboxytherapy, via the Bohr effect, a denser dermal environment, and preserved fibroblast function, along with oral administration of bioactive marine collagen fragments that target fibroblasts, are regenerative strategies that could improve scar elasticity, thickness, and hydration. Their sequential combination might be an innovative approach. METHODS: In March 2024, a 56-year-old woman began treatment of a large, hypertrophic, partially retracted post-surgical scar in the nasojugal area at the level of the right nasal wing. Ablative techniques were strictly contraindicated; t

The LncRNA GAS5/miR-205 ceRNA axis regulates fibroblast activation and may contribute to hypertrophic scarring.

Hypertrophic scars (HS) arise from abnormal wound healing characterized by excessive fibroblast proliferation, enhanced migration, and resistance to apoptosis. Although the long non-coding RNA GAS5 (LncRNA GAS5) has been implicated in several fibrotic disorders, its function in HS remains largely unclear. This study investigated whether GAS5 regulates fibroblast behavior through a competing endogenous RNA (ceRNA) mechanism involving miR-205. Human skin tissue fibroblasts (HSTFs) were transfected with a GAS5 overexpression plasmid (pcDNA-GAS5) or miR-205 mimics, and cell proliferation, migratio

Keratinocyte-derived exosomes serve as an active messenger platform for LINC01605 to amplify TGF-β1-induced fibroblasts activation.

Hypertrophic scarring (HS) represents a proliferative disorder that emerges due to aberrant wound healing processes and is characterized by the excessive accumulation of extracellular matrix components. Recent research has highlighted the pivotal role of exosomes-nanoscale extracellular vesicles-as crucial mediators of intercellular communication in both tissue repair and pathological states. This study investigates the mechanism by which TGF-β1 modulates the cargo of keratinocyte-derived exosomes to promote the transfer of the long non-coding RNA linc01605. The results reveal that exosomes se

A combinatorial therapeutic platform based on graphene-collagen cryogel and apoptotic vesicles for modulating hypertrophic scars.

Graphene-based collagen hydrogels have demonstrated anti-fibrotic potential in various diseases; however, their therapeutic efficacy in hypertrophic scars (HS) remains largely unexplored. Adipose stem cells (ASCs) and their extracellular vesicles have been shown to regulate HS progression, yet the role of ASCs derived apoptotic vesicles (ASCs-ApoVs) has not been fully investigated. In this study, we developed graphene-incorporated type I collagen cryogel (G-GEL(C)) and collected ASCs-ApoVs. We evaluated their individual and combined capacity to regulate HS and explored the potential of G-GEL(C

Laser-Assisted Drug Delivery of 5-Fluorouracil and Triamcinolone vs. Triamcinolone Monotherapy Does Not Lead to Different Outcomes in the Treatment of Burn Hypertrophic Scars.

BACKGROUND/OBJECTIVE: Fractional ablative CO2 laser is an effective treatment for hypertrophic scars (HTS) and can enhance topical drug penetration through laser-assisted drug delivery. Although intralesional triamcinolone is a standard treatment for HTS, it is associated with more adverse effects than topical steroids and has limited applicability for large scars. PATIENTS AND METHODS: This single-institution, prospectively collected, retrospective chart review included 55 patients who underwent laser-assisted drug delivery treatment of either triamcinolone alone or a combination of triamcin
