Targeted-Penetrating Biomimetic Hybrid Transfersomes for Enhanced Transdermal Gene Transfection in Hypertrophic Scar Therapy.

Hypertrophic scar (HS) is a recurrent and inflammatory dermal fibrotic disorder, characterized by abnormal fibroblast proliferation and excessive extracellular matrix (ECM) deposition. Affecting over 310 million patients worldwide, while effective strategies to halt progression remain lacking. The TGF-β-mediated fibrotic cascade is the central driver of HS, and siRNA-based therapeutics offer a promising gene-level intervention owing to their high sequence specificity and low off-target risk. However, siRNA translation is limited by rapid degradation, poor transfection, insufficient targeting,

Implications of Dermatologic Disorders in Facial Cosmetic Surgery: A Systematic Review.

BACKGROUND: Invasive and minimally invasive cosmetic procedures in patients with autoimmune or inflammatory skin disorders can carry an increased risk of surgical site infection, disease exacerbation, atrophic/hypertrophic scarring, or suboptimal cosmetic results. As facial procedures continue to rise in popularity, increasing complications are being reported in association with underlying dermatologic disease. In this review, we aim to bring attention to these dermatologic conditions and the potential challenges that they pose to the facial plastic surgeon. METHODS: A comprehensive literatur

Hypertrophic cardiomyopathy associate with a PLN gene mutation in a child: a case report.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a common hereditary disease of the myocardium, caused by mutations in at least 20 genes, encoding cardiac sarcomere proteins and sarcomere-associated proteins, while genes related to calcium metabolism, such as phospholamban (PLN), have been reported rarely, in less than 1% of positive patients. This study aims to report a unique case of child's HCM caused by a missense variant in the PLN gene on chromosome 6 (NM_002667.5:c.106T>C, p.Cys36Arg). This mutation in children, not previously reported in the literature except for an adult, expand the k

Overactive PDGFRα and PDGFRβ promote distinct phenotypes of skeletal muscle fibrosis and stiffness, with PDGFRβ also driving muscle growth.

Fibrosis accumulates in skeletal muscle over time and leads to greater muscle rigidity, stiffness, and increased risk of injuries. However, investigations of appropriate experimental models to study the mechanisms through which muscle fibrosis occurs are often confounded by injury or disease. The contribution of platelet-derived growth factor receptors alpha and beta (PDGFRα or PDGFRβ) to muscle fibrosis is yet to be clarified. We hypothesized that both receptors would promote extracellular matrix (ECM) deposition and fibrosis, causing muscle stiffening and weakness, with sex-specific differen

Fractional Ablative CO2 Laser Treatment Results in Measurable Changes to Collagen and Galectin in Hypertrophic Scars.

Burn injuries often lead to hypertrophic scars (HTS), which negatively affect quality of life and skin function. Hypertrophic scars result from abnormal wound healing characterized by excess, disorganized collagen confined to the original wound. Fractional ablative CO₂ laser revision (FLSR) has shown promise in improving burn scar symptoms by inducing micro-wound healing, but its specific mechanism remains unclear. Progress is limited by a lack of standardized, objective metrics to evaluate treatment response. We hypothesized that FLSR would shift HTS collagen structure toward that of normal s

Renal function-improving action of Pentadecyl®, a multifunctional triglyceride from Aurantiochytrium limacinum, in a mouse model of chronic kidney disease.

Pentadecyl®, derived from Aurantiochytrium limacinum, is a triglyceride primarily composed of odd-numbered fatty acids such as pentadecanoic acid (C15). Pentadecyl® improves glucose tolerance in a mouse model of type II diabetes and promotes collagen production in human skin cells. However, its effects on kidney failure and chronic kidney disease (CKD) remain unexplored. The present study examined the effects of continuous Pentadecyl® administration on renal dysfunction in 5/6-nephrectomized mice (5/6Nx mice), a well-established model of CKD. Pentadecyl® (0.2 g/l) was administered orally via a

Impact of Scarring Disorders and Surgical Techniques on the Development of Capsular Contracture in Breast Augmentation.

BACKGROUND: Capsular contracture is a common complication following breast augmentation and a major indication for reoperation. While pocket contamination and aberrant inflammatory processes are suspected causes, a definitive etiology has not been identified. This study investigates if other disorders of aberrant scarring such as Dupuytren disease, hypertrophic scars, and keloids are clinically correlated with developing capsular contracture, as well as the use of certain surgical techniques. METHODS: A retrospective chart review of patients undergoing breast augmentation at a single institut

Primary hypertrophic osteoarthropathy complicated with myelofibrosis and compound heterozygous SLCO2A1 mutations: a case report and review of literature.

BACKGROUND: Primary hypertrophic osteoarthropathy (PHO) is a rare hereditary clinical syndrome characterized by digital clubbing, periostosis, and pachydermia. It is mainly caused by mutations in SLCO2A1 or HPGD, leading to impaired degradation and elevated levels of prostaglandin E2 (PGE2). In addition to the typical skeletal and skin manifestations, some patients may present with gastrointestinal or hematologic abnormalities, including anemia and myelofibrosis. This report describes a case of PHO with myelofibrosis and compound heterozygous SLCO2A1 mutations and review the literature. CASE

Innovative Nasal Splitting Technique for Dual Rhinoplasty and Rhinophyma Management: A Case Report and Literature Review.

Rhinophyma is a deforming disease that affects the nose mainly of Caucasian men. Progressive nasal skin thickening, telangiectasias, and hypertrophy of sebaceous appendages are frequently found in these patients. Many therapeutic modalities have been reported with good results. Among these, tangential excision is established as the surgical modality of choice. In advanced cases, it is not possible to distinguish nasal deformities resulting from rhinophyma from those resulting from malposition of the anatomic components that make up the nasal framework. We describe the use of a mixed technique

Unmasking Ehlers-Danlos Syndrome Through Reduction Mammaplasty: An Illustrative Case and a Systematic Literature Review.

Ehlers-Danlos syndrome (EDS) is characterized by joint hypermobility, skin hyperextensibility, and tissue fragility, which may worsen symptomatic macromastia. Concerns about elevated surgical risk have historically discouraged elective procedures in EDS patients; however, postoperative complications may themselves prompt an EDS diagnosis, as illustrated by our case. This relationship prompted the conduct of a systematic review examining outcomes of reduction mammaplasty in EDS. A 37-year-old woman underwent bilateral inferior pedicle reduction mammaplasty for symptomatic mammary hyperplasia. P

Thick filament molecular interfaces play a critical role in the pathogenesis of hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) variants in genes encoding the myosin heavy chain (MHC) (MYH7), myosin light chains (MYL2 and MYL3), and cardiac myosin binding protein-C (cMyBP-C, MYBPC3) lead to cardiac hypertrophy, with abnormal contractility, relaxation, and energy consumption. Here, we defined the structural consequences of pathogenic and benign missense variants in these genes by mapping 233 variants (MYH7, n = 175; MYBPC3, n = 41; MYL2, n = 12; MYL3, n = 5) onto a cryo-EM-based atomic model of the human cardiac thick filament. We identified HCM variants residing in 30 molecular interfa

Atypical Orogenital Lichen Planus in a Young Male: A Case Report.

INTRODUCTION: Lichen planus is a chronic inflammatory ailment with a wide range of symptoms that can mimic other dermatological and sexually transmitted conditions, especially when affecting the genitalia. Such overlap might make diagnosis difficult, particularly in uncommon circumstances. CASE PRESENTATION: We present a 21-year-old Middle Eastern male with a one-year history of progressive papular lesions that started in the glans penis and advanced to the oral mucosa, where they extended to the gingiva. The lesions were non-ulcerated, well-defined, dome-shaped papules ranging from pink to s

Long-Standing Unilateral Verrucous Papules of the Hand Revealing Hypertrophic Lichen Planus: A Diagnostic Challenge.

Lichen planus (LP) is an inflammatory disorder that can involve the skin, nails, hair follicles, and mucous membranes and may present with diverse clinical morphologies that mimic other dermatologic conditions. Hypertrophic (verrucous) LP is a chronic variant characterized by hyperkeratotic lesions that most often affect the lower extremities, which makes isolated hand involvement diagnostically challenging. We report a 65-year-old man with skin-colored, dermally embedded verrucous papules confined to the right hand with extension to the wrist, evolving for more than 40 years. He denied prurit

Multi-modal analysis of vascular remodelling in human obliterative bronchiolitis after lung transplantation.

Chronic lung allograft dysfunction (CLAD) is the leading cause of late morbidity and mortality after lung transplantation. Bronchiolitis obliterans syndrome (BOS), a major CLAD phenotype, is characterized by obliterative bronchiolitis (OB). While microvascular perturbations are described in BOS, their cellular and molecular characteristics remain unclear. Intact human CLAD lungs (n=3) underwent hierarchical phase-contrast tomography (HiP-CT) for three-dimensional vascular mapping. In OB-affected airways vs. controls (n=4 each), endothelial subtypes were characterized by multiplex immunofluores
