Massive Bilateral Breast Hypertrophy in Pregnancy Due to Gestational Gigantomastia: A Case Report and Review of the Literature.

Gestational gigantomastia is a very rare disorder characterised by massive, bilateral breast enlargement during pregnancy. Clinically, it is characterised by rapid breast enlargement associated with severe mastodynia, back and shoulder pain, and erythema of the skin. In its most severe forms, it can lead to ulceration, necrosis, haemorrhage, or septicaemia. The underlying pathophysiology is not completely understood, but is thought to be multifactorial, involving hormonal hypersensitivity and autoimmune mechanisms. Histopathological confirmation is necessary for definitive diagnosis because it

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Single-cell RNA-seq reveals immune cell heterogeneity and increased Th17 cells in human fibrotic skin diseases.

BACKGROUND: Fibrotic skin disease represents a major global healthcare burden, characterized by fibroblast hyperproliferation and excessive accumulation of extracellular matrix components. The immune cells are postulated to exert a pivotal role in the development of fibrotic skin disease. Single-cell RNA sequencing has been used to explore the composition and functionality of immune cells present in fibrotic skin diseases. However, these studies detected the gene expression of all cells in fibrotic skin diseases and did not enrich immune cells. Thus, the precise immune cell atlas in fibrotic s

Fat-Free Mass: Friend or Foe to Metabolic Health?

BACKGROUND: Fat mass (FM) and fat-free mass (FFM) are body composition estimates commonly reported in research studies and clinical settings. Recently, fat-free mass indexed to height (fat-free mass index; FFMI) has been shown to be positively associated with impaired insulin sensitivity or insulin resistance. Consequently, hypertrophic resistance training which can increase FFM was also questioned. This paper sets out to evaluate these propositions. METHODS: In this narrative review, we discuss possible reasons that link FFMI to adverse metabolic health outcomes including the limitations of

A Classic Presentation of Acne Keloidalis Nuchae in a Black Male.

Acne keloidalis nuchae (AKN), also known as folliculitis keloidalis nuchae and nuchal keloid acne, is a chronic inflammatory skin disorder of the hair follicle. Predominantly occurring in the nuchal and occipital scalp, it can lead to keloid-like plaques and cicatricial alopecia. Primarily affecting people of color, more specifically African American individuals, it is associated with haircuts, friction, or trauma among a variety of other less common causes. The diagnosis is clinical with physical exam findings showing firm, dome-shaped papules or pustules, hypertrophic scars, inflammation, an

Dissolvable microneedles based on active components of Chinese Herbal Medicine for enhanced treatment of skin diseases.

According to World Health Organization (WHO) estimates, around 900 million people globally are affected by various skin diseases, which cause significant physical and psychological distress and place a heavy economic burden on healthcare systems. The primary challenges in treating skin diseases include the limited transdermal drug absorption due to the skin barrier, the side effects associated with medications, and the recurring nature of these conditions that lead to prolonged patient suffering. Microneedles (MNs) have emerged as a promising transdermal drug delivery technology, able to painl

Blue Light (λ = 453 nm) Significantly Reduces TGF-β-Induced Fibroblast Differentiation Through Reversible Disruption of Mitochondrial Respiration, Glycolysis, and ATP Production Rate.

Background/Objectives: Abnormal differentiation of human skin fibroblasts into myofibroblasts contributes to fibrotic skin disorders such as hypertrophic scars, keloids, and Dupuytren's disease. This process is characterized by increased fibroblast proliferation, enhanced differentiation into myofibroblasts, and reduced programmed cell death (apoptosis). We previously demonstrated that blue light irradiation (λ = 453 nm) significantly and dose-dependently inhibits both spontaneous and TGF-β-induced fibroblast differentiation. Methods: Because fibroblast differentiation depends on cellular ener

Efficacy and Safety of Runzaozhiyang Lotion for Chronic Hand Eczema: Protocol for a Randomized Controlled Trial.

BACKGROUND: Chronic hand eczema (CHE) is a prevalent inflammatory skin disease in clinical practice. Guidelines recommend the use of topical medications, including corticosteroids and calcineurin inhibitors; however, prolonged use of these drugs may result in adverse effects, such as skin atrophy, drug tolerance, and rebound phenomena. Moreover, these medications exhibit limited efficacy in managing chronic hypertrophic hand eczema. Traditional Chinese medicine (TCM) has a long history of treating patients. The Runzaozhiyang lotion (RZZYL) is often used in the Department of Dermatology of Guan

Pachydermoperiostosis Associated With a Rare SLCO2A1 Mutation: A Case Report and Literature Review.

Pachydermoperiostosis (PDP), also known as primary hypertrophic osteoarthropathy, is a rare genetic disorder characterized by digital clubbing, periostosis, and pachydermia. It typically presents during adolescence or early adulthood and is commonly associated with mutations in the HPGD or SLCO2A1 genes. Due to its nonspecific clinical features, PDP is often misdiagnosed, resulting in delayed treatment. We report the case of a 26-year-old Arab male with an eight-year history of progressive facial skin thickening, hyperhidrosis, and digital clubbing. Laboratory investigations were unremarkable,

Codon-optimized human Smad7 gene therapy enhances skeletal muscle mass and function in a murine model of Duchenne muscular dystrophy.

Commercial development of gene therapeutics often requires transitioning to human payload genes as initial proof-of-concept studies in animal models often use taxa-specific orthologs. Such transitions also provide opportunities to address potential secondary structure and immune-related subsequences as with human Smad7 cDNA, which was optimized by removing several repeats, potential hairpins and negative cis elements. Thermodynamic modeling at or above minimal free energy states revealed substantial improvements in secondary structure with fewer hairpins and improved diversity scores. Serotype

Pachydermoperiostosis Presenting With Gynecomastia and Low Insulin-Like Growth Factor-1 Levels: A Diagnostic Challenge.

Pachydermoperiostosis (PDP), or primary hypertrophic osteoarthropathy, is a rare genetic disorder characterized by digital clubbing, skin thickening, and periostosis. Due to its overlapping clinical features with secondary hypertrophic osteoarthropathy, it often poses a diagnostic challenge. We report a case of a 20-year-old male patient who presented with progressive digital clubbing, joint pain, and coarse facial features over four years. The patient also exhibited gynecomastia, an uncommon feature in PDP. Despite no history of underlying pulmonary, cardiac, or gastrointestinal disease, his

Looking beyond the scalpel: Assessing patient risk factors for complications following surgical excision of hidradenitis suppurativa.

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic inflammatory disease affecting intertriginous skin. Severe HS presents a reconstructive challenge due to high complication rates; however, risk factors for complications are understudied. This study examines patient-level risk factors for increased complications and operative techniques used at our institution. METHODS: We conducted an IRB-approved retrospective review of patients (≥18 years) who underwent primary or secondary wide local excision of HS between 12/1/2015-06/02/2022. Sociodemographic, intraoperative, and complication data

Dupilumab-Induced Generalized Lichen Planus: A Case Report.

BACKGROUND: Lichen planus (LP) is a chronic inflammatory skin disease of unknown cause. Common subtypes include generalized LP, linear LP, annular LP, and hypertrophic LP. Current clinical evidence suggests that lichen planus is a T-cell-mediated autoimmune disorder. Therefore, an increasing number of clinicians are opting for biologics or immunomodulators to manage lichen planus, but the reported evidence on their efficacy and safety remains limited. CASE SUMMARY: This case report describes a 32-year-old female with pre-existing localized lichen planus (LP) who developed generalized LP after
