Magnetic Resonance Imaging for Distinguishing Perianal Hidradenitis Suppurativa from Fistulizing Crohn Disease.

INTRODUCTION: Both hidradenitis suppurativa (HS) and Crohn disease (CD) are characterized by suppuration and granulomatous inflammation, which may result in formation of tunnels, sinus tracts, or fistulas. We sought to determine the differences in characteristics of fistulizing perianal CD vs. perianal HS on magnetic resonance imaging (MRI). METHODS: This retrospective cohort study included patients diagnosed with CD, HS, or both diseases who underwent medical examination at the Mayo Clinic between January 1, 1998, and July 31, 2021. We further selected patients who had documented perianal le

Deep Granuloma Annulare After Scalp Infection: Wolf's Isotopic Response in a Pediatric Patient.

We present a case of a pediatric patient with recurrent flares of deep granuloma annulare (GA) largely localized to the right scalp and face, corresponding to the area of a previously resolved scalp infection. This clinical pattern is suggestive of Wolf's isotopic response, a phenomenon that has been rarely reported in children. To our knowledge, this may represent one of the first documented cases of deep GA manifesting as Wolf's isotopic response in a pediatric patient.

Subcutaneous Granuloma Annulare: A Rare Case Associated With Acetazolamide.

Granuloma annulare (GA) is a benign, granulomatous skin condition characterized by ring-shaped papules or plaques on the dorsal aspects of the hands and/or feet. It is most frequently seen in children and young adults, with a female predominance. Although many cases are idiopathic, associations with certain diseases include autoimmune disorders, diabetes mellitus, and thyroid disease. Drugs have also been implicated including allopurinol, diclofenac, topiramate, amlodipine, thalidomide, and TNF-α inhibitors. The clinical and morphologic distinction from the interstitial granulomatous drug reac

Clinical updates of JAK inhibitors in cutaneous granulomatous diseases.

Cutaneous granulomatous diseases, characterized by persistent granuloma formation, often exhibit chronic and relapsing courses that are challenging to manage with conventional therapies. The Janus kinase (JAK)-signal transducer and activator of transcription (STAT) pathway plays a central role in mediating key cytokines involved in granuloma initiation and maintenance, such as IFN-γ, IL-6, IL-12, and IL-23. JAK inhibitors, by targeting this pathway, offer a promising therapeutic strategy for refractory cases. This review synthesizes current evidence supporting the efficacy of JAK inhibitors-in

Generalised granuloma annulare following ustekinumab therapy for palmoplantar psoriasis.

We report the case of a woman in her 70s who developed generalised granuloma annulare (GA) after 3 years of successful ustekinumab therapy for severe palmoplantar psoriasis. Ustekinumab, an interleukin (IL)-12/23 inhibitor, was ceased as the most likely trigger. GA remained persistent despite multiple treatments but resolved gradually over 5 years. This resolution was followed by a severe flare of her psoriasis, requiring initiation of guselkumab, an IL-23 inhibitor. This case illustrates a probable paradoxical T-helper (Th)1-mediated granulomatous reaction to ustekinumab, likely due to immune

Rapid and Successful Treatment of Generalized Granuloma Annulare With Deucravacitinib.

Granuloma annulare (GA) is a chronic granulomatous dermatosis that often proves resistant to conventional therapies. Increasing evidence implicates interferon-gamma-mediated JAK-STAT activation in GA pathogenesis, with tyrosine kinase 2 (TYK2) playing a central role through IL-12, IL-23, and type I interferon signaling. Deucravacitinib, a selective allosteric TYK2 inhibitor, has emerged as a potential therapy by targeting these pathways. We present the case of a 59-year-old woman with biopsy-confirmed interstitial-type GA that was unresponsive to topical corticosteroids. Initiation of deucrava

[Intralesional rituximab for refractory granuloma faciale].

A 60-year-old man presented with refractory granuloma faciale. After unsuccessful treatment with dapsone, hydroxychloroquine, and radiotherapy, treatment with intralesional rituximab was initiated. This led to a steady improvement in the lesions without serious side effects. The case shows that rituximab can be a treatment option for refractory cases of granuloma faciale, especially in the context of IgG 4-related autoimmune disease.

Diagnosis and management of central giant cell granulomas of the jaws: a European and multicenter study.

INTRODUCTION: Central giant cell granulomas (CGCGs) are benign but locally aggressive rare neoplasms that affect the bony skeleton, particularly the maxillofacial region. The purpose of this European multicenter study was to describe and assess the characteristics, diagnosis, management, and recurrence of CGCGs at different European oral and maxillofacial surgery centers. METHODS: The data from all the treated CGCGs from the involved oral and maxillofacial surgical units across Europe between January 1st, 2014 and December 31st, 2023 were recorded. RESULTS: A total of 76 patients, including

An unusual chest lesion with dual pathologies.

Granuloma faciale (GF) is a rare, benign, chronic inflammatory condition characterized by well-defined reddish-brown papules, nodules, or plaques, often demonstrating follicular accentuation and telangiectasia, which may be termed "peau d'orange." Typically, GF is localized to the facial area, but extrafacial variants have been reported, particularly on the trunk, scalp, and extremities. We report a 66-year-old male with a history of GF and multiple basal cell carcinomas (BCCs) who presented with a pearly plaque on the chest. Biopsy revealed extrafacial GF with a coincidental BCC, representing

Ocular Surface Granuloma Faciale: A Clinicopathologic Study of Two Novel Conjunctival Cases with a Review of the Literature Regarding Ophthalmic Involvement.

Granuloma faciale (GF) is a rare, idiopathic, localized vasculitis, usually without associated systemic autoimmune disease. Primarily involving the skin, GF can also occur on the ocular surface, mimicking neoplasia and other inflammatory processes. In this report, the authors report 2 novel cases of GF in the conjunctiva, and analyze the 4 published ocular cases to better appreciate the clinical and histopathologic features of GF involving the ocular surface, and to compare the findings with those in the skin. A pink-to-yellowish lesion of the superior bulbar conjunctiva in a 16-year-old femal

[Granuloma faciale and Takayasu arteritis in a child: a case report].

An 11-year-old boy presented with erythematous plaques over the bilateral mandibular and mental regions for 2 years, accompanied by cough and dyspnea for more than 2 months. Chest computed tomography angiography revealed marked stenosis of the right pulmonary artery, irregular aortic caliber, and aortic wall thickening. Histopathological examination of the skin lesion, including immunohistochemistry and special stains, confirmed a chronic suppurative inflammation. Whole-exome sequencing was negative. A final diagnosis of granuloma faciale and Takayasu arteritis was established. Combination the

Necrobiosis Lipoidica Diabeticorum: A Rare and Difficult-to-Treat Condition.

Necrobiosis lipoidica (NL) is a chronic granulomatous dermatologic condition usually associated with diabetes mellitus (DM). Early diagnosis and treatment can improve the quality of life of the patient and prevent the development of squamous cell carcinoma. Treatment options include corticosteroids, tacrolimus, cyclosporine, fumaric acid esters, biologics (including adalimumab, etanercept, and infliximab), and phototherapy with varying degrees of success. Here, we report a case of NL diabeticorum, which was effectively treated with immunosuppressants and keratolytics.

Necrobiosis lipoidica arising in a port wine stain treated with topical ruxolitinib.

We present a unique case of necrobiosis lipoidica (NL) arising within a port-wine stain (PWS), which, to our knowledge, has not been previously reported. NL is a rare granulomatous disorder often associated with diabetes mellitus, characterized by chronic inflammation and microvascular dysfunction. PWS, a congenital vascular malformation, results from defective endothelial differentiation and capillary malformation. The coexistence of these conditions suggests a potential link in their pathophysiological mechanisms, including inflammation, endothelial dysfunction, and shared signaling pathways

Diabetic Dermopathies as Predictive Markers of Cardiovascular and Renal Complications: A Narrative Review.

Background: Cutaneous manifestations are frequent in diabetes mellitus and may reflect systemic vascular injury. Among them, diabetic dermopathy, necrobiosis lipoidica, scleredema diabeticorum, bullosis diabeticorum, and eruptive xanthomas are clinically significant. Aim: To synthesize current evidence on the associative and potential predictive role of diabetic dermopathies as non-invasive indicators of cardiovascular and renal complications. Methods: A narrative review of studies published between 2010 and 2023 in PubMed, Scopus, and Web of Science was conducted, focusing on links between sp
