Granuloma faciale (GF) is an uncommon inflammatory dermatosis with characteristic clinical and histologic features. Very little is known about its pathogenesis. We used a battery of immunoperoxidase lymphocyte markers to study the population of hematopoietic cells present in a case of GF. The majority of non-myelocytic hematopoietic cells present were T-helper lymphocytes. It is particularly high compared to other series in the world. If diagnosis is easy for the clinician, much care must surround the pathological answer. BACKGROUND AND DESIGN: Pyoderma faciale was originally described by O'Leary and Kierland in 1940. It is characterized by the sudden onset of monstrous coalescent nodules and confluent draining sinuses confined to the face of young women in their early 20s. This report summarizes our results in 20 cases. The women were 15 to 46 years old (mean, 25 years).

Assisted ventilation at home for breathing insufficiencies requires long-term tracheotomy. Standard tracheotomy is frequently complicated by granuloma and tracheostomy stenosis. We have designed a permanent opening technique that we report on 9 patients. The technique prevents, as far as possible, granuloma and stenosis formation and provides a sufficiently large opening to reduce irritation during tracheotomy tube change. Surgery must be done under general anesthesia. High-dose long-term corticosteroid treatment can induce skin breakdown.

A 42 years old woman was seen October 1987 because of tumor on the left maxilla. Removal of the lesion and histologic examination conclude at myeloplaxe tumor. A second giant cell lesion appears in the right maxilla 2 years later. The diagnosis of brown tumor was confirmed by the discovery of a parathyroid adenoma. We discuss about this case diagnosis difficulty of brown tumor from the others giant cell lesions of the jaws.

While histopathologic analysis may offer some clues as to the pathogenesis of vasculitis, observations must be interpreted with caution, as there is considerable overlap in the histologic pattern. Granuloma faciale is an uncommon cutaneous condition characterized by brownish-red macules, papules, nodules, and plaques. Separate areas of granuloma faciale involving the nasal skin of a 32-year-old man were treated using electrosurgery, carbon dioxide laser, and dermabrasion. All modalities provided good cosmetic and functional outcomes. The relative advantages and disadvantages of each technique are presented.

The authors report a case of laryngeal abcess due to Pseudomonas Aeruginosa, of the preepiglottic space with destruction of the epiglottis and granulomatous reaction of the epiglottic endolaryngeal side. The differential diagnosis of the endoscopic image includes neoplasm, granulomatous diseases and laryngeal abscess. Biopsies revealed the diagnosis of laryngeal abscess without habitual causing factors. The treatment consisted in CO2 laser microsurgery with debulking, as complete as possible, of the granulomatous reaction in association with adapted antibiotherapy.

Cervico-facial actinomycosis is a chronic bacterial disease caused by actinomycetes. The diagnosis may be made on the specific anatomo-pathological appearance of the granulomas and be confirmed by anaerobic culture of the germs. In a series of 100 adults with tinea faciei various dermatologic manifestations were mimicked, viz.: discoid lupus erythematosus in 52 patients, lymphocytic infiltration in 15, seborrheic dermatitis in 11, rosacea in 8, contacts dermatitis in 7, polymorphous light eruption in 4 and granuloma faciale in 3. A case of cholesteatoma of the ethmoid extending to the orbit is reported. These cholesteatomas of facial sinuses are rarely reported, those documented involving localization in maxillary or frontal, or ethmoido-frontal or ethmoido-maxillary sinuses. Primary perisinusal cholesteatomas of embryonic origin must be distinguished from the rarer secondary iatrogenic or post-traumatic lesions. Histopathology allows distinction between epidermoid cysts and cholesterol type granulomas. Treatment is by surgery using a wide approach.

The implantation of biocompatible hydroxyapatite is used for human mandibular ridge augmentation in edentulous patients. Histologically, we studied such a material after a long period of implantation (1 1/2 to 2 years) in two patients. We report a case of tinea of the face caused by Trichophyton rubrum in which the histologic changes were consistent with granuloma faciale. We recommend that local infection with dermatophytes be considered in all cases of histologic granuloma faciale, especially when the clinical presentation is atypical for this dermatosis.
