Immunoglobulin deposition and plasma cell infiltration define distinct immunopathological patterns in skin diseases.

Immunoglobulin deposition in the skin is an important diagnostic and etiological feature in many skin diseases. However, the relationship between deposited immunoglobulins and the local presence of antibody-producing cells, such as B cells and plasma cells, remains poorly understood. In several autoimmune diseases, including systemic lupus erythematosus and bullous pemphigoid, marked immunoglobulin deposition is observed despite limited B-cell infiltration into the skin, suggesting that the deposited antibodies primarily originate as circulating autoantibodies. In contrast, diseases such as di

Glymphatic dysfunction as a potential mechanistic link to cognitive ımpairment in Behçet's disease: a hypothesis.

Behçet's disease (BD) is a chronic multisystem inflammatory disease in which cognitive impairment is increasingly recognized but remains incompletely understood. Although cognitive dysfunction is more common in patients with Neuro-Behçet's disease (NBD), it has also been reported in patients without clinically apparent neurological involvement, suggesting that mechanisms beyond focal inflammatory lesions may contribute to cognitive impairment. The glymphatic system is a brain-wide perivascular clearance pathway that maintains cerebral homeostasis by facilitating the exchange of cerebrospinal a

Ability of SLE-DAS and SLEDAI-2K to predict damage accrual in SLE: A real-world 7-year cohort study from Sweden.

OBJECTIVES: SLE remains a heterogeneous autoimmune disease that can lead to irreversible organ damage. SLE Disease Activity Score (SLE-DAS) is a recently developed tool with higher sensitivity to change than SLE Disease Activity Index-2000 (SLEDAI-2K). We compared SLE-DAS with other activity measures in terms of ability to predict organ damage. METHODS: Patients from Linköping University Hospital with ≥2 visits between 2008 and 2025 were included. SLEDAI-2K and Physician's Global Assessment (PhGA) were assessed prospectively, and SLE-DAS retrospectively. Organ damage was estimated annually us

Pulmonary hypertension in anti-synthetase syndrome: prevalence, comparative risk across autoimmune diseases, and impact on survival.

OBJECTIVES: Anti-synthetase syndrome (ASyS) is a distinct entity within the myositis spectrum characterized by interstitial lung disease (ILD) which raises the concern of increased pulmonary vascular pressures. However, the prevalence of pulmonary hypertension (PH) in real-world ASyS populations remains unclear. We investigated the prevalence and impact of PH in ASyS. METHODS: Data were obtained from a world-wide, physician-reported multicenter registry based on physician-diagnosed ASyS and/or a positive anti-synthetase antibody. Comparative analyses and multivariable logistic regression asse

Gender differences in the clinical features and quality of life of Japanese patients with systemic lupus erythematosus: a cross-sectional study based on the LUNA registry.

BACKGROUND: Gender differences in clinical manifestations and disease progression of systemic lupus erythematosus (SLE) have been well documented; however, there is a paucity of literature on the gender differences in the quality of life (QOL) of patients with SLE, especially in the context of organ-specific damage. This study aimed to explore gender differences in the clinical features and patient-reported outcomes among Japanese patients with SLE, with particular focus on the effect of joint and cardiac damage on gender-based disparities in QOL: previous studies have shown that articular and

Unraveling the impact of microplastics on autoimmune diseases: hidden dangers and environmental triggers.

Autoimmune diseases (ADs) are a heterogeneous group of disorders characterized by loss of immune tolerance against self-antigens, leading to local or systemic inflammation and subsequent tissue/organ damage. Until now, the etiology of ADs remains obscure. Growing evidence suggests that microplastics (MPs) may act as emerging environmental triggers in the initiation and progression of these disorders. MPs have been shown to modulate immune-related gene expression and induce excessive reactive oxygen species production in various immune cells, such as macrophages, T cells, and B cells. This may

Twin Presentation of Cutaneous Neonatal Lupus Associated With Maternal Anti-U1RNP Antibodies: A Case Report.

Neonatal lupus erythematosus (NLE) is a rare autoimmune condition caused by the transplacental transfer of maternal IgG autoantibodies, most commonly anti-Sjögren's-syndrome-related antigen A (anti-Ro/SSA) and anti-Sjögren's-syndrome-related antigen B (anti-La/SSB). NLE associated solely with anti-U1RNP antibodies is uncommon and typically limited to cutaneous manifestations, with extremely few cases reported in twins. We describe bi-chorionic bi-amniotic preterm twin girls who developed atypical erythematous lesions along the trajectory of epicutaneo-cava catheters during the second week of l

Lupus anticoagulant hypoprothrombinemia syndrome successfully treated with rituximab - case report.

Lupus anticoagulant-hypoprothrombinemia syndrome (LAHPS) is a rare, acquired coagulation disorder where antiphospholipid antibodies (aPL) target prothrombin. LAHPS should be suspected in patients with aPL presenting with bleeding rather than thrombotic events or prolonged prothrombin time. Prompt assessment of factor II levels is crucial to prevent life-threatening hemorrhage. Given its rarity, there is currently no established treatment guidelines. Management of bleeding episodes involves transfusion of fresh frozen plasma or prothrombin complex concentrate (PPC), activated factor concentrate

Photosensitive psoriasis mimicking cutaneous lupus erythematosus: a case report.

Photosensitive presentations of psoriasis are often under-recognized and may closely mimic other photodistributed dermatoses. We report the case of a 39-year-old male patient with no history of other photodermatoses who developed a progressive, non-pruritic rash, localized in sun-exposed skin areas, with marked exacerbation after phototherapy. Pronounced photosensitivity and atypical lesion distribution, involving the malar and nasal regions, complicated the initial differential diagnosis between photosensitive plaque psoriasis and cutaneous lupus erythematosus (CLE). Only further clinical eva

Damaged glomeruli in proliferative pediatric lupus nephritis exhibit a C5a-C5aR1 induced fibrotic transcriptional program.

Lupus nephritis (LN) is a leading cause of morbidity in pediatric systemic lupus erythematosus (pSLE) due to suboptimal kidney remission rates and the sequelae of prolonged intensive immunosuppressive therapy. LN is patchy, with some glomeruli severely damaged while others remain histologically unaffected in the same kidney. Using spatial transcriptomic technology, we interrogated microanatomic transcriptional differences between histologically damaged and unaffected glomeruli in pSLE LN to understand local drivers of renal injury. Despite SLE being a disease of Type I interferon (IFN), IFN ge

Expert collaborative evidence-based consensus for childhood lupus nephritis diagnosis and treatment: unifying clinical approach in Saudi Arabia.

BACKGROUND: Lupus nephritis (LN) affects approximately 60–70% of children with systemic lupus erythematosus (cSLE). Clinical practice for childhood lupus nephritis (cLN) management in Saudi Arabia varies significantly between pediatric rheumatologists and nephrologists. Unified, evidence-based national guidelines are needed to standardize care and improve outcomes. We aimed to develop the first evidence-based Saudi consensus recommendations for the diagnosis, monitoring, and treatment of childhood LN through collaboration between pediatric rheumatologists and nephrologists. METHODS: A multidis

A Unique Case of Lupus Erythematosus Profundus Presenting With Localized Scarring Alopecia and Perioral Swelling in a Pediatric Patient.

Lupus erythematosus profundus, also known as lupus panniculitis, is a rare subtype of cutaneous lupus erythematosus characterized by inflammatory involvement of the deep dermis and subcutaneous adipose tissue. It is predominantly reported in adults, whereas pediatric presentations remain uncommon. A five-year-old Middle Eastern girl presented with a two-month history of progressive localized alopecia involving the left parietal scalp, associated with perioral swelling predominantly affecting the upper lip. Examination showed an alopecic patch measuring approximately 4 × 2 cm with reduced folli

Deep profiling of lupus nephritis kidneys reveals dynamic changes in myeloid cells associated with disease progression.

OBJECTIVES: Lupus nephritis (LN) is a common, potentially fatal manifestation of systemic lupus erythematosus. We aimed to gain new insights into the immune responses underlying LN and their relation to the histologic heterogeneity observed in this disease, focusing on myeloid cells. METHODS: We used single-cell RNA-sequencing (scRNA-seq) data of dissociated kidney samples from 156 patients with LN and 30 healthy individuals. We applied spatial transcriptomics (ST), utilising a gene panel designed to capture all myeloid subsets identified in the scRNA-seq data, to profile kidney samples acqui

Circulating IL-33 as a sensitive alarmin in systemic lupus erythematosus: a biomarker for disease activity and skin-specific manifestations.

Interleukin-33 (IL-33), an alarmin predominantly released by keratinocytes and endothelial cells, plays a pivotal role in type 2 immune responses and has been linked to the pathogenesis of autoimmune diseases, including systemic lupus erythematosus (SLE). This study aimed to explore the relationship between plasma IL-33 levels and clinical features in SLE patients. A cross-sectional study was conducted involving 133 SLE patients and 81 normal controls. Plasma IL-33 levels were measured via enzyme-linked immunosorbent assay. Clinical parameters, such as SLEDAI-2K scores, photosensitivity, skin

Localized Discoid Lupus Erythematosus of the Auricular Concha: A Case Report Highlighting the Diagnostic Relevance of the Carpet Tack Sign.

Discoid lupus erythematosus is a chronic autoimmune inflammatory dermatosis characterized by erythematous, scaly plaques that may progress to permanent atrophy, scarring, and pigmentary alterations. The auricle is a recognized site of involvement in discoid lupus erythematosus; however, isolated lesions confined to the auricular conchae are less frequently described and may pose diagnostic challenges because of their clinical resemblance to other inflammatory, infectious, or neoplastic conditions of the ear. We report the case of a 35-year-old woman with a 15-year history of asymptomatic bilat

Children exposed to hydroxychloroquine in utero show no signs of retinal toxicity at age 5.

OBJECTIVE: Limited examinations from prior studies support the absence of retinal toxicity in infants exposed to maternal hydroxychloroquine (HCQ). However, the infant retina does not have adult morphology that could show typical signs of HCQ retinopathy. Using the Preventive Approach To Congenital Heart Block with Hydroxychloroquine (PATCH) study, children at age 5 (when adult morphology has been reached) were evaluated using modern imaging techniques. METHODS: Ocular coherence tomography (OCT) was performed (CIRRUS HD-OCT, Carl Zeiss Meditec) at age 5 on 10 PATCH children exposed to materna

Stress-induced IL-6 regulation in pediatric bone growth disorders: current insights and therapeutic strategies.

Stress-induced upregulation of interleukin-6 (IL-6) signaling and its downstream pathophysiological consequences have garnered considerable attention in recent years. However, no comprehensive review has specifically examined the association between stress-induced IL-6 and its implications for bone health in children. During childhood, linear growth and peak bone mass acquisition are tightly regulated processes. Hence, this review aims to investigate current evidence on stress-induced IL-6 upregulation and its detrimental effects on pediatric bone health. Findings from animal models, knockout
