Nephritis has been a recognized complication of systemic lupus erythematosus since the early 1900s. Almost all lupus patients have some degree of renal involvement related to their condition, but a considerably smaller proportion of these patients actually progress to end-stage renal disease (ESRD). However, lupus patients are also susceptible to other primary renal insults that may significantly contribute to the deterioration in their renal function. We present a case of a patient with clinical and pathological evidence of lupus nephritis that progressed to ESRD and subsequently developed "r

INTRODUCTION: Short-term and long-term results of renal transplantation have improved over the past 15 years. However, there has been no change in the prevalence of recurrent and de novo diseases. A retrospective study was initiated through the Renal Allograft Disease Registry, to evaluate the prevalence and impact of recurrent and de novo diseases after transplantation.
MATERIALS AND METHODS: From October 1987 to December 1996, a total of 4913 renal transplants were performed on adults at the Medical College of Wisconsin, University of Cincinnati, University of California at San Francisco, U

Mothers of children with neonatal lupus erythematosus (NLE) and heart block, as well as patients with Sjögren's syndrome (SS) and systemic lupus erythematosus, have serum autoantibodies that recognize SS-A/Ro autoantigens including the 60-kDa ribonucleoprotein. By yeast 2-hybrid screening, we identified a novel 75-kDa protein (pp75) that interacts with the carboxyl 70% of 60-kDa SS-A/Ro. The specificity of interaction was confirmed using mammalian 2-hybrid and chemical crosslinking studies. Immunoprecipitation with radiolabeled HeLa cell extracts showed that pp75 was phosphorylated and associa

Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by the increased production of antibodies reactive with a variety of self and non-self antigens. A number of immunomodulatory therapies have been investigated for the disease process. Intragastric administration of low dose kidney extract (KE) three times weekly for 5 weeks and then weekly until 6 months of age in SLE mice, showed decreased anti-dsDNA antibody levels, less kidney damage and significantly prolonged survival compared with control phosphate buffered saline (PBS)-fed mice. The KE-fed mice also exhibited redu

UNLABELLED: Collapsing glomerulopathy in HIV and non-HIV patients: A clinicopathological and follow-up study.
BACKGROUND: Collapsing glomerulopathy (CG) is a pattern of renal injury that is seen in association with HIV infection and that is increasingly recognized in non-HIV patients.
METHODS: A review of native kidney biopsies with CG that were diagnosed between 1979 and 1997 in 18 HIV and 42 non-HIV patients is provided.
RESULTS: HIV and non-HIV patients with CG were similar in terms of age, sex ratio, serum creatinine, proteinuria, the extent of collapsing and sclerosing glomerular lesio

There is considerable evidence that the development of systemic lupus erythematosus (SLE) has a strong genetic basis. For more than 20 years, much effort has been made to understand the genetics of SLE. Association studies in humans suggest the existence of genetic effects by the alleles encoded in the HLA, deficiencies in the complement genes and the low-affinity variants of Fcgamma-receptors. In mouse models of SLE at least 13 loci have been identified, including the MHC, linked to lupus-related phenotypes such as nephritis and production of autoantibodies. Recently, linkage studies have bee

INTRODUCTION AND CLINICAL CASE: Chronic inflammatory demyelinating polyneuropathy (CIDP) is an acquired autoimmune disorder of unknown cause. CIDP most often occurs alone and not as a complication of other disorders but may accompany plasma cell dyscrasias, human immunodeficiency virus infection, systemic lupus erythematosus and others diseases. The association of CIDP and carcinoma has rarely been reported and its relevance is debated.
CONCLUSION: We report one patient with CIDP and prostatic adenocarcinoma. Steroids therapy were effective.

A review of routine histopathological samples and autopsies examined at the Department of Pathology, University of Malaya revealed 15 cases of amyloidosis of the lung. Two were localized depositions limited to the lung while in the remainder, lung involvement was part of the picture of systemic amyloidosis. Both cases of localized amyloidosis presented with symptomatic lung/bronchial masses and a clinical diagnosis of tumour. Histology revealed "amyloidomas" associated with heavy plasma cell and lymphocytic infiltration and the presence of multinucleated giant cells. In both cases, the amyloid

OBJECTIVE: To recruit a large cohort of sibling pairs with systemic lupus erythematosus (SLE) as a clinical and biologic resource for genetic studies in SLE.
METHODS: Complementary approaches were used to identify suitable families. The study was advertised in the newsletters of the Lupus Foundation of America and the Arthritis Foundation. Fliers were mailed to 250 clinical rheumatologists across the US, as well as to the local branches of the Lupus Foundation. All advertisements displayed a toll-free telephone number for interested patients to contact our group. Patients were then screened i

BACKGROUND: Granular immunoglobulin M (IgM) deposits at the dermo-epidermal junction (DEJ) are common in lupus erythematosus (LE); however, weak, interrupted, linear and granular patterns of IgM at the DEJ have been reported in sun-exposed skin in normal adults and in a heterogeneous group of diseases.
MATERIALS AND METHODS: We analyzed 200 patients with positive IgM direct tissue immunofluorescence deposition at the DEJ, alone or in combination with other immunoreactants, in order to determine the diagnostic significant of IgM deposition at the DEJ.
RESULTS: IgM deposition at the DEJ, commo

In murine models of SLE, particular patterns of abnormalities of social interaction and memory collectively known as neurobehavioral dysfunction (NBD) correlate with the occurrence of brain reactive autoantibodies. Study of the immunopathogenic effects of these antibodies has been limited by the absence of isolated autoantibodies and antigens. In order to identify the molecular targets, we isolated autoantibodies highly specific for brain plasma membranes from MRL/lpr mice. After immunoscreening a brain expression library with these brain specific autoantibodies, we identified a single cDNA cl

The anti-Sm test is thought to be specific for the diagnosis of systemic lupus erythematosus (SLE) and is one of the American College for Rheumatology (ACR) criteria for the classification of SLE. Locally, the sensitivity and specificity of this test for the diagnosis of SLE are unknown. We therefore aim to study the sensitivity and specificity of this test in our local context. A total of 1034 patient samples that were sent for routine testing of anti-Sm antibodies over a 30-month period, were included in the study. However, only 1031 patient samples were included in the final analysis as 3 c

OBJECTIVE: Acute transverse myelopathy (ATM) is a rare manifestation of systemic lupus erythematosus (SLE). The pathogenesis is unclear and the optimal management strategy is uncertain because of the lack of controlled trials. In this study, the clinical presentation, autoantibody profile, treatment, and outcome of cases of ATM in our local SLE population were retrospectively analyzed and compared with SLE controls.
RESULTS: Ten cases of ATM were identified among 315 patients with SLE studied, giving a prevalence of 3.2%. In 5 of the patients, ATM was the initial manifestation of SLE. The cer

OBJECTIVE: Antinuclear antibodies (ANA), rheumatoid factors (RF), and erythrocyte sedimentation rate (ESR) are among the most frequently requested tests in the diagnosis and investigation of connective tissue diseases (CTD). We evaluate the utilization patterns and predictive value of these tests in patients referred to rheumatologists by primary care physicians.
METHODS: We reviewed the records of all new patients referred by primary care physicians in 1994 to 2 rheumatologists practicing at the University of Alberta. Data extracted from the records included diagnostic tests requested by ref

Adjuvant-induced arthritis (AIA) in rats is a widely used autoimmune experimental model with many features similar to rheumatoid arthritis (RA). To identify potential genetic regulatory mechanisms in RA, we conducted genome-wide linkage analysis in F2 progeny of arthritis-susceptible Dark Agouti (DA) and relatively resistant Fischer 344 (F344) inbred rats. We compared the data with our previously reported investigation of collagen-induced arthritis (CIA), which was expanded in the follow-up study reported in this work. We found two quantitative trait loci (QTLs) in common, i.e., Aia1/Cia1 on c
