BACKGROUND: Cognitive dysfunction (CD) is a common manifestation of central nervous system involvement in patients with systemic lupus erythematosus (SLE). Patients with SLE may develop CD insidiously at an early stage of the disease, and the lack of a standardized diagnostic test poses a major challenge in prompt diagnosis and management of these patients. This review summaries the current application of various magnetic resonance imaging (MRI) techniques for patients with SLE complicated with CD, aiming to identify potential quantitative neuroimaging biomarkers for patients with SLE and CD.

The complex nature of immune system behavior in both autoimmune diseases and transplant rejection can be understood through the lens of avalanche dynamics in critical-point systems. This paper introduces the concept of the "immunological avalanche" as a framework for understanding unpredictable patterns of immune activity in both contexts. Just as avalanches represent sudden releases of accumulated potential energy, immune responses exhibit periods of apparent stability followed by explosive flares triggered by seemingly minor stimuli. The model presented here draws parallels between immune sy

Inflammatory bowel disease (IBD) is an autoimmune disease (AD) characterized by chronic, relapsing intestinal inflammation. Systemic lupus erythematosus (SLE) is a complex autoimmune disease with multisystem involvement and overactivation of both innate and adaptive immunity. The extra intestinal manifestations (EIMs) that commonly occur in IBD include many of the organ sites that are affected by SLE. ADs are often comorbid with one another and may have shared underlying genetic features and architectures contributing to their pathogenesis and disease course. We performed both epidemiological

BACKGROUND: CD19-targeting chimeric antigen receptor (CAR) T-cell therapy has advanced treatment strategies for severe autoimmune diseases such as systemic lupus erythematosus (SLE), systemic sclerosis, and idiopathic inflammatory myopathy. Data regarding side-effects are mostly generated from patients with malignancies, but little is known about autoimmune disease-specific adverse events. This study aimed to describe autoimmune disease-specific adverse events that occur with CAR T-cell therapy.
METHODS: In this observational study, patients of any age with autoimmune disease receiving CD19-t

BACKGROUND: Eosinophilic esophagitis (EoE) is a chronic Type 2 inflammation. One might assume that the disease-related impact on daily life is significantly smaller, given the relatively localized disease distribution, compared to more systemic immune-mediated diseases. This study aimed to evaluate the disease burden among various immune-mediated diseases, including EoE, inflammatory bowel disease (IBD), systemic sclerosis (SSC) and lupus erythematosus (SLE).
METHODS: A web-based questionnaire assessed baseline characteristics, general quality of life and disease-related impairment across sev

BACKGROUND: Systemic lupus erythematosus (SLE) is a spontaneous systemic auto-immune condition for which the inciting factors and genetic basis are generally unknown. Although heterogeneous in its manifestations and severity, SLE involves chronic inflammation along with sustained autoantibody production. The root causes and pathophysiology of the inflammation and breaches of tolerance are incompletely understood, but neutrophils are thought to be important elements of the pathophysiology. Type I interferons (IFN) in the bloodstream and an IFN-stimulated gene (ISG) signature in circulating leuk

RATIONALE: Systemic lupus erythematosus (SLE) is a rare but serious autoimmune disorder that can present with diverse clinical manifestations, often making diagnosis challenging, particularly in pediatric patients. While thrombocytopenia is a known hematological manifestation of SLE, its occurrence as the sole initial symptom is uncommon.
PATIENT CONCERNS: An 11-year-old male presented with a 2-week history of painless localized bleeding, ecchymosis, and petechiae. He also experienced mild knee joint pain but had no fever, weight loss, or other systemic symptoms. Despite initial treatment for

Lupus myocarditis, a severe and often underdiagnosed complication of systemic lupus erythematosus (SLE), remains challenging to treat due to the complex inflammatory processes involved. While lymphangiogenesis has been implicated in lupus nephritis, its role in myocarditis is not well understood. This study investigates the potential of lymph node-targeted cyclosporine A (CsA) as a therapeutic approach for lupus myocarditis. Using a lupus mouse model, we explored the effects of localized immunosuppression through polymer nanoparticles designed to deliver CsA with lymph node-targeting functiona

By balancing immunity and tolerance, dendritic cells (DCs) are key regulators of immune responses. Recent studies have highlighted the crucial role of these cells in connective tissue diseases. Conventional DCs (cDCs) and plasmacytoid DCs (pDCs) exhibit distinct contributions to disease progression. In systemic lupus erythematosus and systemic sclerosis, pDCs primarily drive pathogenesis via excessive type I interferon production, whereas in rheumatoid arthritis (RA), cDCs play a major role in promoting autoreactive T cell activation. Emerging DC subsets, such as inflammatory DC3s and LAMP3+ D

Antiphospholipid syndrome (APS) is an autoimmune disorder characterized by arterial, venous, or small-vessel thromboembolic events and/or pregnancy morbidity in the presence of antiphospholipid antibodies. APS can occur in the setting of an underlying systemic autoimmune disease, particularly systemic lupus erythematosus (SLE). SLE is a chronic autoimmune disease that can affect virtually any organ of the body due to the production of antinuclear antibodies. During the course of their disease, many patients develop pulmonary manifestations, including alveolar hemorrhage. We describe a clinical

BACKGROUND: Systemic lupus erythematosus (SLE) is a heterogeneous autoimmune disease shaped by complex interactions involving genetic and environmental factors. Among these, the gut microbiome is emerging as potentially modulating immune responses and influencing disease susceptibility, progression, and activity.
OBJECTIVES: To synthesize current evidence on gut microbiome changes in adult SLE patients, framed along the clinical pathway - from diagnosis to treatment - to help bridge bench and bedside for microbiome-informed SLE care and research.
METHODS: A systematic search identified prima

OBJECTIVES: Lupus nephritis (LN) is a severe complication of systemic lupus erythematosus (SLE), characterised by kidney inflammation, tubular injury, and interstitial fibrosis. However, the spatial organisation of these heterogeneous cell populations and their regulatory mechanisms in LN remain poorly understood. The objective of this study was to investigate the regulatory mechanisms underlying region-specific kidney lesions and tubular damage in LN.
METHODS: We performed single-cell multiome and spatial transcriptomic analyses on kidney biopsy samples from patients with LN and controls, in

Tinea incognito is an incorrectly diagnosed form of fungal infection due to a changed clinical picture as a result of systemic or topical corticosteroids or even local immunomodulators. This type of skin lesion is most often located on the trunk but can affect any part of the body. We present a case report of 76-year-old woman with a history of systemic lupus erythematosus who was admitted to hospital because of extensive, painful, and burning erythematous and papular lesions in an annular pattern, covered with a thick, yellow crust, located on the scalp and neck. The skin lesions were accompa

Systemic lupus erythematosus (SLE) is a chronic systemic autoimmune disease with a wide array of manifestations in different organ systems. Involvement of the central nervous system (CNS), also known as neuropsychiatric SLE (NPSLE), often presents with seizures, mental status changes, focal neurological findings, and cognitive impairment. While many underlying pathophysiologic mechanisms have been described, no unifying etiology has been determined for NPSLE, thus complicating treatment. We highlight the case of a 41-year-old female with SLE who presented to the emergency department after sev

Calcium signaling is essential for the proper function of immune cells. Recent studies have shown that the scaffold protein, AHNAK1, is important for efficient calcium signaling and NFAT activation in T cells through its ability to properly localize calcium ion (Ca2+) channels at the plasma membrane. Interestingly, both T cells and B cells of systemic lupus erythematosus exhibit activation signaling anomalies with dysregulated Ca2+ response, and enhanced Ca2+ signaling has emerged as a target for treatment with SLE. Therefore, we hypothesized SLE patients may have autoantibodies (Abs) against

This article presents a case of neuromyelitis optica spectrum disorder (NMOSD) secondary to systemic lupus erythematosus (SLE). The patient initially presented with unexplained nausea, vomiting, intractable hiccups, and significant bradycardia (48 bpm). Cranial and spinal MRI findings were unremarkable, while serum aquaporin-4 immunoglobulin G (AQP4-IgG) antibody titers were markedly elevated (1:320). Following exclusion of alternative etiologies through comprehensive serological and neuroimaging investigations, the diagnosis of area postrema syndrome (APS) was confirmed according to the 2015
