Tyrosine kinase 2 inhibitors in the therapy of inflammatory and autoimmune diseases.

Tyrosine kinase 2 (TYK2) is a member of the Janus kinase (JAK) family that mediates intracellular signaling from cytokines such as interleukin 12 (IL-12), interleukin 23 (IL-23), and type I interferons (IFN-α/β) via the JAK–signal transducer and activator of transcription (JAK-STAT) pathway. These cytokines are central to the pathogenesis of autoimmune and inflammatory diseases, including psoriasis, psoriatic arthritis, systemic lupus erythematosus, and inflammatory bowel disease. Selective TYK2 inhibition has emerged as a novel therapeutic approach offering targeted modulation of immune respo

The regulatory mechanisms and treatment of HDAC6 in immune dysregulation diseases.

Histone deacetylase 6 (HDAC6) is a class IIb histone deacetylase that contains two catalytic domains and a zinc finger ubiquitin binding domain (ZnF-UBP). The deacetylation function of HDAC6 has been extensively studied with well-characterized substrates such as α-tubulin and Hsp90. Apart from its deacetylase activity, HDAC6 ZnF-UBP binds to unanchored ubiquitin of specific sequences and serves as a carrier for transport of aggregated proteins. subsequently, aggresomes is degraded by the autophagy-lysosome pathway. Additionally, Cells can utilize this HDAC6-dependent microtubule transport to a

Refractory Lupus Podocytopathy: The Role of Repeat Biopsy and Rituximab.

Lupus podocytopathy is a rare renal manifestation of systemic lupus erythematosus (SLE) that requires treatment strategies distinct from those used for proliferative lupus nephritis. In this report, the author describes a 28-year-old man with biopsy-proven class III lupus nephritis whose proteinuria progressed despite multiple induction regimens, including high-dose corticosteroids with mycophenolate mofetil, voclosporin, and cyclophosphamide-based therapy. A repeat kidney biopsy revealed the development of lupus podocytopathy with features of the focal segmental glomerulosclerosis variant. On

Beneath the Surface: A Rash That Uncovered Renal Autoimmunity.

We report the case of a 42-year-old male who presented with a generalized rash, acute kidney injury, anemia, and positive antinuclear antibodies (ANA). Serologic testing revealed elevated anti-double-stranded DNA (dsDNA) levels and hypocomplementemia, raising concern for systemic lupus erythematosus (SLE). Kidney biopsy confirmed Class III (focal proliferative) lupus nephritis according to the International Society of Nephrology/Renal Pathology Society 2003 classification. Immunofluorescence demonstrated full-house staining (positive for IgG, C3, C1q, and light chains) with weaker signals for

Imaging Features of Amyloid of the Breast: Review of 94 Cases From the Literature and 6 New Cases.

Breast amyloidosis is a rare disease that can be secondary to systemic disease or localized to the breast. The imaging findings are variable with features suspicious for breast malignancy such as calcifications, focal asymmetry, and mass that prompt the recommendation for biopsy. US correlates are also variable, though when present, most are irregular hypoechoic masses. The imaging features overlap with those of primary breast malignancy. Therefore, biopsy and histopathologic correlation are needed to confirm a concordant diagnosis and for amyloid typing. Because approximately half of cases ar

Environmental Exposures of Concern for Rheumatologists: Ultraviolet Radiation.

Climate change is increasing ultraviolet radiation (UVR) exposure, posing a significant threat to individuals with autoimmune diseases such as lupus erythematosus, dermatomyositis, and Sjögren's syndrome. UVR induces both local and systemic immune dysregulation through DNA damage, autoantigen exposure, and interferon-driven inflammation. While UVR's cutaneous effects are well studied, its role in systemic disease progression remains unclear. Limited epidemiologic data and inconsistent definitions of photosensitivity hinder clinical guidance. More robust research is needed to understand UVR's r

An Unusual Presentation of Antiphospholipid Syndrome With Seizures and Sagittal Sinus Thrombosis: A Case Report.

Cerebral venous sinus thrombosis (CVST) is a rare but potentially life-threatening form of stroke. It may present with nonspecific neurological symptoms, including headache, seizures, or focal deficits. One of the rare but important underlying causes is antiphospholipid syndrome (APS), an autoimmune prothrombotic condition associated with persistent antiphospholipid antibodies: lupus anticoagulant (LA), anticardiolipin (aCL), or anti-β2 glycoprotein I (anti-β2GPI). APS-related CVST is typically seen in young women, often with systemic autoimmune disease such as systemic lupus erythematosus. We

B cell depletion and BAFF receptor blockade with ianalumab (VAY736) for the treatment of moderate-to-severe systemic lupus erythematosus: a phase 2 randomised, double-blind, placebo-controlled trial with subsequent open-label treatment.

OBJECTIVES: B cell depletion or B cell activating factor (BAFF) blockade has shown benefits in systemic lupus erythematosus (SLE). We compared ianalumab, a monoclonal antibody targeting BAFF receptor (BAFF-R)-expressing B cells to lyse B cells and block BAFF-R, with placebo for SLE treatment combined with standard therapies. METHODS: Patients with active SLE were randomised (1:1) to monthly subcutaneous ianalumab 300 mg or placebo. The primary outcome was a composite of SLE Responder Index (SRI)-4 at week 28 in patients successfully achieving corticosteroid (CS) tapering criteria. Patients su

An Overview of the Pathogenesis of Cutaneous Lupus Erythematosus.

Background/Objectives: Cutaneous lupus erythematosus (CLE) is a complex autoimmune skin disease driven by genetic predisposition, environmental triggers, and immune dysregulation. Environmental factors such as ultraviolet radiation, smoking, and certain drugs can initiate disease onset by inducing keratinocyte apoptosis. The subsequent release of nucleic acids and danger-associated molecular patterns activates pattern recognition receptors (PRRs) on keratinocytes and immune cells, leading to the production of type I and type III interferons (IFNs) and pro-inflammatory cytokines. The objective

Urine-Derived CD105-Positive Exosome Promotes Macrophage M1 Polarization Through the TGF-β1/Smad3 Signaling Axis To Exacerbate Lupus Nephritis.

This study investigated the regulatory role of exosomal CD105 in macrophage polarization and its mechanisms in lupus nephritis (LN). Analysis of samples from 20 LN patients revealed significant CD105 overexpression in kidney tissues and urinary exosomes. Exosomes were isolated from the urine specimens and tested for CD105 expression levels. Exosomes overexpressing or knocking down CD105 were co-cultured with macrophages to detect the levels of M1/M2 markers (CD86, IL-6, ARG-1, and IL-10) and changes in the TGF-β1/Smad3 pathway. Rescue experiments were performed using the TGF-β1 agonist (80116-

Autoimmune Disease Flare-Up in a Case of Locally Advanced Breast Carcinoma Controlled by Surgical Resection.

Autoimmune diseases and malignancies share complex immunological interactions, but their coexistence in a single patient presents unique diagnostic and therapeutic challenges. We report the case of a 65-year-old woman with a history of autoimmune disease who presented with a locally advanced carcinoma of the breast. Despite ongoing immunosuppressive therapy, she developed a flare of autoimmune manifestations during the progression of her malignancy. The patient underwent surgical resection of the primary tumor, which not only achieved local oncological control but also resulted in a marked imp

From the Masterclasses in Dermatology 2025 Meeting: Practical Approaches to Cutaneous and Systemic Lupus for Dermatologists.

Cutaneous lupus erythematosus (CLE) may occur independently or in association with systemic lupus erythematosus (SLE). When systemic disease is present, CLE is the first manifestation in nearly one-third of cases. This positions dermatologists as key stakeholders in early detection of systemic disease, underscoring the importance of appropriate screening among this population. Various CLE subtypes carry distinct risks of systemic progression, with acute CLE closely tied to active SLE, subacute CLE conferring moderate risk, and most chronic subtypes (eg, localized discoid lupus) remaining limit

Lupus Enteritis as an Initial Manifestation in a Previously Undiagnosed Case of Systemic Lupus Erythematosus: A Case Report.

Systemic lupus erythematosus (SLE) is an autoimmune condition with the potential to impact all bodily organ systems. Lupus enteritis represents a rare complication observed in a subset of patients who exhibit nonspecific gastrointestinal symptoms. Typically, the diagnosis relies on imaging findings such as localized or widespread thickening of the bowel wall, abnormal enhancement of the bowel wall, a distinctive "comb sign" indicating engorged mesenteric vessels, the presence of ascites, and lymph node enlargement. Treatment usually involves the administration of corticosteroids and other immu

Viral warts, a common dermatological condition caused by human papillomavirus infection (HPV) infection, present a particular challenge for systemic lupus erythematosus (SLE) patients due to their compromised immunity, which increases the susceptibility to HPV infection and complicates treatment efforts. Imiquimod, an immunomodulatory agent, has demonstrated efficacy in managing warts. Furthermore, local hyperthermia, a non-invasive therapeutic modality, has demonstrated significant potential in the management of warts. We report the case of a SLE patient developed recalcitrant and extensive v
