Scleroderma calcinosis cutis score (SC2S): an imaging metric to quantify SSc-calcinosis cutis.

OBJECTIVES: Calcinosis cutis (CC) is disabling for SSc patients, and quantitative outcomes and treatments are needed. We performed computer-assisted mapping of CC lesions on CT exams and quantified CC during sodium thiosulphate (STS) treatment. METHODS: In a pilot study, SSc-CC patients underwent CT imaging of a painful lesion followed by 6-12 month 25% STS, intradermal monthly injections or twice daily cream, treatment. Post-treatment CT exams were obtained, and three assessors used open-source software (BioImageSuite/BIS Web) to map and quantify CC volume, termed scleroderma calcinosis cuti

Spatial transcriptomic profiling reveals fibroblast and inflammatory signatures in calcinosis of chronic cutaneous lupus.

Calcinosis cutis represents a debilitating complication of chronic cutaneous lupus erythematosus; however, its underlying molecular mechanisms remain poorly understood. We performed spatial transcriptomic profiling on skin biopsies from 4 patients with chronic cutaneous lupus erythematosus: 2 with calcinosis cutis and 2 without. Analysis of 87,730 cells across 6 major cell types revealed distinct molecular signatures associated with calcification. Fibroblasts in calcinosis cutis lesions exhibited significant upregulation of osteogenic and extracellular matrix genes, including periostin, collag

[Normophosphatemic familial tumoral calcinosis: description of a case with phosphate lowering therapy].

Normophosphatemic familial tumoral calcinosis (NFTC) is characterized by cutaneous calcifications with no metabolic disorders; it manifests in the first years of life; associated with mutations in SAMD9 gene. Chronic trauma and transient hyperphosphatemia play a leading role in calcification formation.Surgical treatment can remove calcifications, but it does not prevent recurrence and can cause complications. Various conservative therapy is used. Lowering of blood phosphate levels is considered a promising pathogenetic treatment method. We present a clinical case of a patient with NFTC associat

Single nucleus RNA sequencing of juvenile dermatomyositis skeletal muscle identifies altered angiogenic signaling.

Juvenile dermatomyositis (JDM) is a chronic multisystem vasculopathy and inflammatory myopathy characterized by proximal muscle weakness, distinct rash, and risk of complications such as calcinosis cutis, skin ulceration, and mortality. Molecular insight from diagnostic muscle biopsy histology is limited, and the mechanistic pathoetiology of JDM remains poorly defined. We used single nuclei transcriptomics to assess muscle samples from patients with newly diagnosed treatment-naïve JDM. As a control, we assessed muscle samples from patients with congenital (nemaline) myopathy (CM), a non-inflam

Increasing use of azoles combined with corticosteroids in Denmark.

INTRODUCTION: Calcinosis cutis (CC), the deposit of insoluble calcium salts in the skin and subcutaneous tissues, is a rare condition that occurs in autoimmune connective tissue diseases, including systemic sclerosis (SSc), dermatomyositis and in non-connective tissue diseases (non-CTD), particularly in patients with calciphylaxis and nephropathy, and in chronic ulcers. CC causes significant morbidity and impacts quality of life. This study evaluated CC management strategies of patients in a clinical setting. METHODS: A retrospective descriptive study was conducted at a university dermatology

[Filgotinib treatment of calcinosis in centromere-antibody-positive systemic sclerosis].

Centromere-antibody-positive limited cutaneous systemic sclerosis (LCSSc) is a common disease. It generally takes a benign course, but can still considerably reduce quality of life. Our patient-a woman born in 1987 who was first diagnosis 2017-suffered from pain and declining hand function due to sclerodactyly and calcinosis. Methotrexate (MTX) and analgesics were not effective. After commencing off-label treatment with filgotinib, the patient reported rapid improvement. An X-ray after 8 months showed a reduction in calcinosis load. Filgotinib is well tolerated. Sclerodactyly has resolved, and

Early detection and intervention in dermatomyositis: current evidence and clinical implications.

Dermatomyositis is a rare autoimmune disease characterized by muscle weakness and distinctive skin rashes. Patients typically present with symmetric proximal muscle weakness alongside extramuscular manifestations, including interstitial lung disease and skin infections. While the exact cause remains unknown, researchers believe genetics, infections, and underlying autoimmune processes contribute to its development. The disease likely involves inappropriate complement protein activation targeting the perimysium, which triggers blood vessel inflammation and destruction around muscle fascicles. T

Tofacitinib for Calcinosis Cutis Associated With Juvenile Dermatomyositis: An Open-Label Single-Arm Study.

OBJECTIVE: Calcinosis cutis (CC) is dystrophic calcification affecting 20% to 40% of patients with juvenile dermatomyositis (JDM). Management of CC is often challenging owing to its being refractory to usual therapeutic options. Type I interferon (IFN)-mediated immune dysregulation may be involved in the pathogenic role in CC, suggesting that altering JAK/STAT signaling through JAK inhibitors might offer a therapeutic benefit. METHODS: In this open-label, single-arm study, the primary objective was to study the effect of tofacitinib (MSN Pharmaceuticals) on the burden of CC in children (2-18

Calcinosis Cutis as the Leading Manifestation of Juvenile Dermatomyositis: A Case Report.

INTRODUCTION: Juvenile dermatomyositis (JDM) is a rare autoimmune inflammatory myopathy of childhood. Initial manifestations may be atypical and lead to delays in diagnosis. CASE REPORT: An 11-year-old boy with a history of trauma of the left knee was initially treated with systemic antibiotics without clinical improvement. On physical examination, ulcerated plaques composed of multiple fistulous tracts with abundant purulent discharge were observed. A skin biopsy revealed findings consistent with calcinosis cutis. The autoantibody panel was positive for anti-Mi-2 antibodies, along with eleva

Surgical management of autoimmune-associated calcinosis: a dermatomyositis case and case-based review.

Calcinosis cutis is a challenging complication of autoimmune connective tissue diseases, particularly dermatomyositis, and is associated with significant morbidity. Current medical treatments often show limited and inconsistent effectiveness, highlighting the need to better define the role of alternative approaches. To present a case of extensive dermatomyositis-associated calcinosis successfully managed with surgical excision and flap reconstruction, and to systematically review the role of surgical management in autoimmune-related calcinosis. A case report is presented together with a litera

Idiopathic Calcinosis Cutis Universalis in a Child.

Calcinosis cutis is the abnormal deposition of calcium salts in the skin and subcutaneous tissue. In children, it is rare and most often dystrophic, occurring in the setting of an underlying connective tissue disease, with juvenile dermatomyositis being the leading cause. Idiopathic calcinosis cutis universalis, defined by diffuse calcium deposition without preceding tissue damage, metabolic disturbance, or identifiable connective tissue disease, is exceptional in the pediatric population and remains a diagnosis of exclusion. We report the case of a nine-year-old girl with a two-year history o

Rapidly progressive calcinosis cutis in adult-onset dermatomyositis: A case report.

Calcinosis cutis is an uncommon but clinically significant complication of dermatomyositis, occurring in approximately 11%-20% of adult patients. We report a case of rapidly progressive, extensive calcinosis cutis in a 52-year-old woman with adult-onset dermatomyositis. Despite ongoing systemic immunosuppressive therapy, she developed biopsy-confirmed calcinosis within 3 months of initial presentation, with subsequent extension to the bilateral axillae, anterior chest wall, and proximal thighs causing functional impairment. Myositis-specific antibody panel results are pending. This case illust

Microneedling for Cutaneous Manifestations of Autoimmune Disease: A Scoping Review.

Microneedling has emerged as a promising, minimally invasive modality for treating cutaneous manifestations of autoimmune diseases, including alopecia areata, vitiligo, and dermatomyositis. This procedure induces controlled microinjuries that stimulate wound healing, collagen production, and tissue remodeling, while also enhancing transdermal drug delivery. This scoping review aimed to evaluate the current evidence on the efficacy of microneedling in managing autoimmune-related dermatologic conditions. The review was conducted in accordance with the PRISMA-ScR guidelines. Peer-reviewed studies
