Scleroderma-Like Skin Changes and Multifocal Muscle Thickening in a Patient with Diffuse Large B-Cell Lymphoma: A Case Report.

BACKGROUND: Paraneoplastic rheumatic manifestations may precede the diagnosis of an occult malignancy and closely mimic primary autoimmune connective-tissue diseases. Scleroderma-like manifestations have rarely been reported in association with lymphoid malignancies, including diffuse large B-cell lymphoma (DLBCL). CASE PRESENTATION: A 47-year-old woman presented with progressive skin thickening, difficulty walking, reduced appetite, and weight loss. She had been diagnosed with scleroderma by an outside physician in early 2024 and had been treated with methotrexate 15 mg once weekly. Physical

The effects of plyometric training on adolescent sports performance: a systematic review and meta-analysis.

BACKGROUND: Plyometric training (PT) is a popular method of improving explosive power, speed, and agility in adolescent athletes. Nevertheless, earlier reviews have been mainly based on individual performance domains, individual sports or general youth groups. It is therefore not clear whether the effects of PT differ based on sex, type of sport, frequency of training or duration of intervention. This meta-analysis and systematic review investigated the impact of PT on jumping, sprinting, and agility performance in adolescents. METHODS: PubMed, Web of Science, and Embase were searched from da

[Filgotinib treatment of calcinosis in centromere-antibody-positive systemic sclerosis].

Centromere-antibody-positive limited cutaneous systemic sclerosis (LCSSc) is a common disease. It generally takes a benign course, but can still considerably reduce quality of life. Our patient-a woman born in 1987 who was first diagnosis 2017-suffered from pain and declining hand function due to sclerodactyly and calcinosis. Methotrexate (MTX) and analgesics were not effective. After commencing off-label treatment with filgotinib, the patient reported rapid improvement. An X-ray after 8 months showed a reduction in calcinosis load. Filgotinib is well tolerated. Sclerodactyly has resolved, and

Maternal and fetal outcomes in pregnancies affected by mixed connective tissue disease.

INTRODUCTION: Mixed connective tissue disease (MCTD) is an autoimmune condition with overlapping features of lupus, systemic sclerosis, and polymyositis. Maternal-fetal outcomes specific to MCTD remain poorly characterized; a scoping review was conducted. METHODS: Five databases were queried. Extracted data included demographics, antibody profiles, symptoms, treatments, and maternal-fetal outcomes. Descriptive analysis was performed. RESULTS: Of 269 articles, 33 met inclusion criteria, including 375 pregnancies. Common symptoms and disease manifestations included Raynaud's phenomenon, arthra

Clinical Associations of Anti-RNPC3 Autoantibodies in Mixed Connective Tissue Disease.

OBJECTIVE: The RNA-binding region-containing protein 3 (RNPC3) protein acts as a molecular bridge, promoting U11/U12 RNP complex formation. In previous reports, patients with systemic sclerosis (SSc) with anti-RNPC3 autoantibodies had an increased risk of interstitial lung disease (ILD), severe gastrointestinal (GI) disease, and cancer. The aim of this study was to compare phenotypic features of patients with mixed connective tissue disease (MCTD) with and without anti-RNPC3 autoantibodies. METHODS: A retrospective MCTD cohort was studied. Addressable laser bead immunoassay was used to detect

Physical rehabilitation interventions for hand function in people with systemic sclerosis.

Systemic sclerosis (SSc) is a complex systemic autoimmune rheumatic disease with marked clinical heterogeneity. Cutaneous manifestations affecting the hands and wrists include vascular insufficiency, early edema (puffy fingers), followed by progressive skin fibrosis and atrophy (sclerodactyly). This progressive skin tightening results in joint stiffness, deformity, functional impairment and reduced quality of life. Beyond skin changes, hand involvement may also include inflammatory arthritis, joint contractures, tendon friction rubs, Raynaud phenomenon, digital ulcers, acro-osteolysis, and cal

Castleman-Like Lymphadenopathy in a Patient with Mixed Connective Tissue Disease: A Case Report and Review of the Literature.

Differentiating reactive lymphadenopathies in the context of autoimmune disease from Idiopathic Multicentric Castleman Disease (iMCD) poses a significant diagnostic challenge. Castleman-like histological features have been described in various autoimmune disorders, necessitating a strict and comprehensive integration of clinical and laboratory findings to reach the correct diagnosis. Although the Castleman Disease Collaborative Network (CDCN) consensus guidelines list several autoimmune conditions as exclusion criteria for an iMCD diagnosis, mixed connective tissue disease (MCTD) is not curren

Rash-Negative Dermatomyositis-Spectrum Overlap Myositis With PM/Scl Positivity Presenting With Progressive Pharyngeal Dysphagia.

Dermatomyositis-spectrum inflammatory myopathy may present without characteristic cutaneous findings, and overlap phenotypes can delay recognition when early neuromuscular examination is not yet diagnostic. We describe the case of a patient with rheumatology-diagnosed dermatomyositis without rash and PM/Scl-positive overlap myositis with systemic sclerosis features whose course was marked by interstitial lung disease, progressive weakness, and objectively confirmed pharyngeal dysphagia. A 64-year-old female with interstitial lung disease presented with several weeks of polyarthralgia, severe f

Isolated Sclerodactyly Associated with Silica Exposure in a Stonemason: A Case Report.

INTRODUCTION: Sclerodactyly is commonly associated with systemic sclerosis (SSc), occurring alongside Raynaud's phenomenon and disease-specific autoantibodies. The combination of occupational silica exposure and SSc is known as Erasmus syndrome. However, isolated digital fibrosis in silica-exposed individuals without systemic features or SSc-specific autoantibody positivity has not yet been described. CASE PRESENTATION: A 40-year-old stonemason presented with a 6-month history of progressive fingertip sclerosis, fissuring, and pain, in combination with hand dermatitis, which was initially dia

Beyond Raynaud's: Atypical Peripheral Vascular Manifestations in a Case of CREST Syndrome.

Peripheral vascular disease (PVD) is a rare but severe symptom of CREST syndrome, which itself is a limited cutaneous sclerosis. Even though Raynaud's phenomenon is the gold standard of CREST, the development of critical limb ischemia and self-amputation is rare. We report a case of a 48-year-old female with CREST syndrome who presented with progressive ischemia in both upper and lower limbs, leading to spontaneous auto-amputation of multiple fingers/toes. The patient exhibited sclerodactyly, dry gangrene, delayed capillary refill, and tactile deficits in peripheral pulses. ANA and anti-Scl 70

Coexistence of sarcoidosis and systemic sclerosis with nailfold videocapillaroscopy documentation.

BACKGROUND: Sarcoidosis and systemic sclerosis (SSc) are both rare multisystem disorders whose aetiology is not fully understood. Their coexistence is uncommon but clinically significant. Nailfold videocapillaroscopy (NVC) is a validated non-invasive technique for assessing peripheral microvascular involvement in SSc, and its potential utility in sarcoidosis/SSc overlap has not been systematically reported. CASE PRESENTATION: We report a 62-year-old female non-smoker with confirmed SSc (anti-Scl-70 positive, anti-Pm/Scl75 positive, sclerodactyly, Raynaud's phenomenon) and concurrent pulmonary

CREST-associated ruptured right MCA aneurysm: A case report and review of the literature.

Intracranial aneurysms associated with systemic sclerosis and Calcinosis, Raynaud's phenomenon, Esophageal dysmotility, Sclerodactyly, and Telangiectasia (CREST) syndrome are rare, and their pathophysiological relationship remains uncertain. We report the case of a 24-year-old male with systemic sclerosis and clinical features suggestive of CREST syndrome who presented with subarachnoid hemorrhage caused by a ruptured right middle cerebral artery bifurcation aneurysm. The aneurysm was treated with microsurgical clipping, followed by stent-assisted coiling for a residual aneurysm neck identifie

Eosinophilic Fasciitis With Overlap Connective Tissue Disease: A Diagnostic Challenge in Progressive Skin Fibrosis.

Eosinophilic fasciitis (EF) is a rare connective tissue disorder characterized by inflammation and fibrosis of the deep fascia and adjacent subcutaneous tissue. Because its clinical presentation may resemble systemic sclerosis (SSc), diagnosis can be delayed, particularly in patients with an established autoimmune disease. We report a 48-year-old woman with undifferentiated connective tissue disease (UCTD), inflammatory arthritis, and Sjögren's disease overlap syndrome who developed progressive skin thickening and loss of range of motion despite treatment for inflammatory arthritis. Wernicke's encephalopathy in mixed connective tissue disease triggered by persistent vomiting: a case report and literature review.

Wernicke's encephalopathy (WE) is an acute, reversible but potentially disabling neurological syndrome caused by thiamine deficiency. It is increasingly recognized in non-alcoholic settings, particularly when persistent vomiting, poor intake, malabsorption, or systemic illness rapidly depletes limited thiamine stores. Patients with mixed connective tissue disease (MCTD) may be vulnerable because esophageal/gastrointestinal dysmotility, gastroesophageal reflux, chronic inflammation, and reduced oral intake can overlap and obscure nutritional complications. A 38-year-old woman with a 4-year hist

Paraneoplastic scleroderma: a patient with a history of breast cancer presenting with sclerodactyly and Raynaud's phenomenon.

A woman in her 70s with a past history of breast cancer in remission presented with several months of dyspnoea, skin tightness and Raynaud's phenomenon. Physical examination was significant for hypertension, hypoxaemia, sclerodactyly, diffuse crackles on auscultation and mild respiratory distress. Work-up revealed elevated serum creatinine, elevated right ventricular systolic pressure on echocardiography and nuclear bone scan findings of osseous metastatic disease. Captopril was initiated with concern for scleroderma renal crisis. The scleroderma panel was positive for RNA polymerase 11 and 15

A case of mixed connective tissue disease with usual interstitial pneumonia and methotrexate-induced erythema multiforme.

Mixed connective tissue disease (MCTD) is a rare autoimmune disorder characterized by overlapping features of various connective tissue diseases. We present a case of a 62-year-old Indian male with a 20-year history of skin tightness and dysphagia, accompanied by a low-grade fever persisting for 1 year. Physical examination revealed sclerodactyly, Raynaud's phenomenon, and perioral sclerosis. High-resolution computed tomography findings indicated a pattern consistent with usual interstitial pneumonia (UIP), characterized by honeycombing and traction bronchiectasis. Prevalence and clinical relevance of digital ulcers in systemic sclerosis patients from the real-life: the experience of the SPRING Registry of the Italian Society for Rheumatology.

INTRODUCTION: Digital ulcers (DU) are one of the most frequent manifestations in systemic sclerosis (SSc). The presence of DU seems to be a sentinel sign of internal organ involvement and is related to a poor prognosis of the disease. The aim of this study was to evaluate the prevalence and the relationship of DU with clinical manifestations/variants in a large SSc cohort from the SPRING registry. METHODS: SSc patients fulfilling the ACR/EULAR 2013 classification criteria without missing data on digital ulcers were enrolled in a cross-sectional study. Logistic regression models were built to
