BACKGROUND: Autosomal dominant pathogenic variants in PLCG2 are associated with autoinflammation and PLCG2-associated antibody deficiency and immune dysregulation (APLAID), characterized by immunological abnormalities and multi-system inflammation. Descriptions of associated neurological manifestations and vascular anomalies are limited. We describe three patients with APLAID, vasculopathies, neuroinflammation, and cerebral vascular tortuosity. These cases highlight underrecognized and clinically important manifestations of APLAID.
CASE PRESENTATIONS: All three patients had de novo variants i

We report a rare presentation of renal-limited thrombotic microangiopathy (TMA). TMA is a clinicopathologic syndrome resulting from endothelial injury and activation of the coagulation and platelet pathways, leading to the formation of platelet-rich microthrombi within small vessels. This loss of blood flow due to the thrombi can result in tissue ischemia and end-organ injury. Systemic TMA classically manifests with the triad of microangiopathic hemolytic anemia (MAHA), thrombocytopenia, and organ dysfunction. However, TMA may be limited predominantly to the kidneys, resulting in acute or prog

Livedoid vasculopathy (LV) is a rare, chronic vascular disorder marked by painful, recurrent ulcerations that primarily affect the lower extremities. It is frequently associated with hypercoagulable states, although its pathophysiology remains incompletely understood. We present the case of a 25-year-old woman with a longstanding history of LV, elevated factor VIII, increased lipoprotein(a), recurrent kidney stones, and iron deficiency anemia who presented with worsening lower-extremity ulcerations. An extensive workup excluded other hypercoagulable and connective-tissue conditions, leaving el

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic vasculitis characterized by high clinical heterogeneity. The conventional binary classification based on antineutrophil cytoplasmic antibody (ANCA) status inadequately captures this heterogeneity and provides limited guidance for individualised treatment decisions. This study aimed to identify and validate clinical-biological endophenotypes in EGPA using unsupervised machine learning methods based on multidimensional real-world clinical data, to inform precision medicine approaches.
METHODS: In this retrospective s

Urticaria multiforme is a morphological variant of urticaria, clinically characterized by arciform urticarial plaques, more commonly observed in children, with a benign and self-limited course. We present the case of a 72-year-old female who acutely developed fever and a disseminated dermatosis characterized by arciform and annular urticarial plaques, erythemato-violaceous with a pale center, accompanied by angioedema of the upper lip. Given the clinical findings and in order to exclude differential diagnoses with greater systemic involvement, a skin biopsy was performed, revealing superficial

INTRODUCTION: ANCA-associated vasculitis (AAV) can have several pulmonary manifestations. There is limited knowledge of the epidemiology and complications of pulmonary manifestations in AAV, despite their association with increased mortality and reduced health-related quality of life.
METHODS: This study retrospectively analyzed a large, multicentre longitudinal cohort of individuals with AAV followed between 2006 and 2023. Data included diagnosis, demographics, comorbidities, time of disease onset, and pulmonary manifestations of disease. Measures of both disease activity and damage were rec

Cutaneous manifestations of vasculitic or vasculopathic processes can pose a diagnostic challenge, particularly when systemic symptoms are absent. We report a case of an adult patient who presented with nonpainful petechiae and blue-purple discoloration of the lower extremities three days after a self-limited upper respiratory infection. Laboratory testing revealed elevated erythrocyte sedimentation rate, white blood cell count, and C-reactive protein, as well as positive cytoplasmic antineutrophil cytoplasmic antibody (c-ANCA) titers. No other systemic findings or organ involvement were ident

BACKGROUND: Despite widespread use of tumor necrosis factor inhibitors (TNFi) as first-line therapy in rheumatoid arthritis (RA), up to 40% of patients fail initial treatment. Subsequent therapeutic choices remain poorly structured, with limited evidence-based guidance to inform individualized post-TNFi decision-making.
OBJECTIVE: To develop evidence-informed, profile-based recommendations to guide treatment selection after inadequate response to a first TNFi in RA, combining evidence and expert consensus.
DESIGN: Delphi-based consensus study informed by a PRISMA-guided scoping review (ScR)

OBJECTIVES: Since 2011, immune checkpoint inhibitors (ICI) have transformed the treatment of various cancers. However, our understanding of the autoimmune adverse events, particularly those affecting the nervous system, remains limited. These adverse events can cause significant disability or even death, yet there are currently no established guidelines or biomarkers to aid diagnosis and treatment. With this study, we aim to gain a deeper understanding of neurological adverse events and investigate potential predictive biomarkers.
METHODS: Between 19 December 2019 and 21 August 2021, 150 out

Based on the Chapel Hill consensus criteria, the primary forms of vasculitis are classified by the predominant size of the affected blood vessels into large, medium, or small vessels. The two main forms of large vessel vasculitis (LVV) are giant cell arteritis and Takayasu arteritis, both of which are more prevalent in women while showing distinct geographic patterns of prevalence. The pathogenesis of LVV is complex and multifactorial, involving a combination of genetic predisposition, environmental and geographic triggers, immune dysregulation, and aging or (premature) senescence of the immun

Cryoglobulinemic vasculitis is an immune-complex-mediated small-vessel vasculitis characterized by complement activation and endothelial injury. Its clinical manifestations are heterogeneous and may range from limited cutaneous involvement to systemic disease. We report the case of a 65-year-old woman who developed a symmetrical violaceous dermatosis involving the upper and lower extremities, accompanied by arthralgias and distal paresthesias. Laboratory investigations revealed marked hypocomplementemia and positive circulating cryoglobulins. Histopathological examination of a skin biopsy demo

Livedoid vasculitis is a vascular autoimmune disorder affecting the skin. It most commonly occurs in young women. The occurrence of this condition after the coronavirus 2019 (COVID-19) vaccination is rare. The available literature suggests limited use of exosomes in the treatment of livedoid vasculitis. We present the case of a 65-year-old Chinese man who developed livedoid vasculitis after COVID-19 vaccination. We posit that the etiology in this case may be either COVID-19 vaccine-induced immune stimulation or activation of the condition by a rare vaccine component. This case is unique, with

INTRODUCTION: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder caused by biallelic SMN1 variants, partially modulated by SMN2 copy number. Risdiplam, an oral SMN2 splicing modifier, has demonstrated efficacy in SMA. However, long-term adherence and persistence are key to sustaining benefit. We evaluated real-world adherence, persistence, and safety of risdiplam in a population-based cohort.
METHODS: This was a retrospective observational study including all genetically confirmed SMA type 1-3 patients treated with risdiplam in Spain between January 2020 and October 2025. Adherenc

A man in his 50s receiving immunomodulatory therapy for presumed Behçet's disease presented with odynophagia and jaw pain. Three months earlier, he had developed deep vein thrombosis and extensive superficial phlebitis with transmural inflammation on histopathology. Imaging revealed carotid artery vasculitis and unilateral inflammation around the temporal vessels. Ultrasound demonstrated wall thickening of the right temporal vein with normal temporal arteries. Persistent systemic inflammation and bicytopenia prompted bone marrow examination revealing myelodysplastic syndrome. Detection of a so

Our manuscript presents a rare case of idiopathic mesenteric phlebosclerosis (IMP) in a 73-year-old woman who experienced recurrent abdominal pain and diarrhea. Through a detailed clinical investigation, including laboratory tests, colonoscopy, biopsy, and enhanced abdominal CTA, we diagnosed the patient with IMP. Notably, we identified a potential link between the patient's long-term intake of a traditional Chinese herb containing Gardenia Fructus and the development of IMP. This finding is supported by existing literature, which suggests that genipin, a component of Gardenia Fructus, may cau

Acute hemorrhagic edema of infancy (AHEI) is a rare leukocytoclastic vasculitis of young children characterized by cockade-like purpura and edema, usually with a preserved general condition. Because of its dramatic presentation, it may mimic life-threatening disorders such as meningococcemia with purpura fulminans, leading to extensive investigations and unnecessary treatment. We report two cases of AHEI in boys aged 18 and 22 months. The first presented with widespread purpura after a recent fever, prompting a full sepsis workup and empirical ceftriaxone before AHEI was recognized. The second
