Neutrophilic dermatoses.

Neutrophilic dermatoses are a group of rare heterogenous inflammatory disorders that share the presence of a sterile neutrophilic infiltrate on histopathology due to an underlying systemic illness, which can be a malignancy. This group of disorders can have diverse clinical presentations, including papules, plaques, nodules, vesiculopustular lesions, or ulcerations. We review the clinical and histopathologic features of several neutrophilic dermatoses that can be cutaneous manifestations of an underlying malignancy, including Sweet syndrome (acute febrile neutrophilic dermatosis), pyoderma gan

Dual-Site Inflammation During Adalimumab Therapy: Birdshot Chorioretinopathy Complicated by Paradoxical Psoriasis.

Birdshot chorioretinopathy is a chronic posterior uveitis that often requires systemic immunomodulatory therapy, yet biologic agents, particularly tumor necrosis factor alpha (TNF-α) inhibitors, may trigger paradoxical cutaneous inflammation and complicate multidisciplinary management. A 32-year-old man was evaluated for progressive right eye (RE) visual decline and long-standing floaters. Examination showed bilateral posterior uveitis with chorioretinitis and retinal vasculitis, more severe in the RE, with significant cystoid macular edema (CME) and leakage on fluorescein angiography (FA). In

Histopathological analysis of CD163 and myeloperoxidase expression in neutrophil-rich dermatoses.

BACKGROUND: Predominance of myeloperoxidase immunoreactive mononucleated cells in the dermal infiltrate defines histiocytoid Sweet's syndrome. These cells are believed to represent immature myeloid cells of granulocytic lineage, and the authors suppose that they could be present in other neutrophil-rich inflammatory dermatoses. AIM: This report aimed to investigate CD163 and/or MPO expression on mononuclear cells in dermatoses with tissue neutrophilia not previously considered. METHODS: A double immunostaining protocol with CD163 and MPO primary antibodies was applied to skin biopsies of 4 g

Sarcoidosis-like reaction associated with tocilizumab in a patient with giant cell arteritis: a case based review.

We report a 71-year-old man with confirmed giant cell arteritis (GCA) who underwent temporal artery biopsy. Following 2.5 years of combination therapy with tocilizumab (TCZ) (162 mg/w) and glucocorticoids (tapered over 2.5 years to prednisolone 1 mg/day), erythematous facial papules and red eye developed. Skin biopsy revealed multiple non-caseating epithelioid granulomas containing multinucleated giant cells, without evidence of infection. Serum angiotensin-converting enzyme levels and soluble interleukin-2 receptor levels were within normal limits, and imaging showed no systemic sarcoidosis i

Systemic IgA Vasculitis With Leukocytoclastic Features in a Dialysis-Dependent Patient With Primary IgA Nephropathy.

IgA vasculitis (IgAV) and IgA nephropathy (IgAN) are increasingly recognized as part of a disease spectrum sharing common pathogenic mechanisms involving galactose-deficient IgA1. While IgAN represents kidney-limited disease, IgAV is the systemic manifestation. The development of systemic IgA vasculitis in patients with established end-stage renal disease (ESRD) secondary to IgA nephropathy is rarely reported and presents unique diagnostic challenges. We present a case of a 51-year-old woman with ESRD secondary to IgA nephropathy on maintenance hemodialysis who presented with acute anemia, abd

Evidence-based guidelines for the diagnosis and management of adult-onset IgA vasculitis.

IgA vasculitis (IgAV) is an immune complex-mediated small-vessel vasculitis that typically affects the skin, gastrointestinal tract, kidneys and joints. Childhood-onset IgAV is a common disease and usually follows a self-limiting course, whereas adult-onset IgAV is considerably less frequent and is associated with a poorer prognosis. The diagnosis, assessment and management of adult-onset IgAV remain challenging owing to the absence of validated diagnostic criteria for adults and lack of IgAV-specific standardized disease activity scores. Short-term outcomes are mainly determined by gastrointe

Keratoconjunctivitis Caused By Dieffenbachia Plant Sap: A Case Series.

BACKGROUND Over 5000 plant species produce milky latex sap utilized in decoration and various industries. Sap exposure, however, can elicit toxic reactions, including acute keratoconjunctivitis and corneal stromal infiltration. The common ornamental plant Dieffenbachia can cause acute keratoconjunctivitis and the deposition of fine blue crystals within the corneal stroma. This report analyzes 3 cases of Dieffenbachia-induced keratitis and reviews existing literature on plant-induced keratitis. CASE REPORT Three patients presented with ocular irritation, pain, and redness following accidental e

Extraintestinal Cutaneous Manifestations in Inflammatory Bowel Disease Among Non-White Patients: A Retrospective Multicenter Study.

BACKGROUND: The presence of extraintestinal manifestations with cutaneous diseases in patients with inflammatory bowel disease (IBD) can pose significant complications. Though the prevalence of IBD has been increasing in racial and ethnic minority groups, most literature has characterized several cutaneous manifestations (CM) of IBD in patients of white skin, with a lack of studies describing these complications in patients with other skin tones. METHODS: Our study aimed to determine the rates of various CM of IBD in skin of color using a health care database to identify white and non-white p

Rocky Mountain Spotted Fever Presenting as an Ischemic Stroke: A Rare Manifestation.

Rocky Mountain spotted fever (RMSF) is among the most severe tick-borne rickettsial infections encountered in the United States. Its presentation ranges from the classic triad of fever, headache, and rash to central nervous system (CNS) conditions, including meningitis, encephalitis, and seizures. Notably, RMSF presenting as an ischemic stroke due to CNS vasculitis is rare, which can prove fatal if recognition is delayed. We present the case of a woman in her 50s with a history of well-controlled diabetes mellitus type 2 and hypertension, who presented with fever, chills, body aches, and rash.

Cutaneous manifestations of scrub typhus: a comprehensive review of diagnostic clues in a neglected tropical disease.

Scrub typhus, caused by Orientia tsutsugamushi, is a re-emerging zoonotic infection with significant diagnostic reliance on its cutaneous manifestations. The disease, transmitted by larval trombiculid mites, is endemic to the 'Tsutsugamushi Triangle' and extends to other regions globally. Dermatological features are pivotal in early recognition, with eschar formation being pathognomonic. Eschars, resembling 'cigarette burns', develop as necrotic lesions surrounded by erythema and often resolve with minimal scarring. Maculopapular rashes, although variably reported, predominantly affect the tru

Coronary Ectasia and Ventricular Arrhythmia in a Patient With Prior Tick Exposure and Borrelia burgdorferi IgG Seroreactivity: A Case Report.

Lyme borreliosis is a multisystem infectious disease caused by Borrelia burgdorferi and transmitted to humans through the bite of infected Ixodes ticks that most commonly affects the skin, joints, nervous system, and, less frequently, the heart. Lyme carditis typically presents with atrioventricular conduction abnormalities, whereas coronary vascular involvement is rarely described. We report a case of a 74-year-old man with exertional chest discomfort, dyspnea, and palpitations several months after an untreated tick bite. Electrocardiography demonstrated ventricular bigeminy with repolarizati

Erythema elevatum diutinum.

Erythema elevatum diutinum (EED) is a rare and chronic form of cutaneous vasculitis often associated with systemic diseases including such hematologic malignancies as immunoglobulin A monoclonal gammopathy, multiple myeloma, and non-Hodgkin lymphoma, as well as such autoimmune disorders and infections HIV and streptococcus. Clinically, EED presents as symmetrical red-to-brown papules, plaques, or nodules, primarily on extensor surfaces including the elbows, knees, and hands. These lesions may be asymptomatic or associated with burning, tenderness, or pruritus, and their chronic nature often le

Probable Coxsackievirus Infection in an Immunocompetent Adult With Genital and Oral Ulcerations Mimicking Herpes Simplex Virus Infection and Behçet's Disease.

Disseminated viral infections in immunocompetent adults are uncommon and may mimic autoimmune or bacterial conditions, including Behçet's disease and disseminated herpes simplex virus (HSV) infection. We report a case of a 33-year-old immunocompetent woman who developed severe, painful vulvar ulcerations, oral ulcers, and a painful maculopapular rash involving both lower extremities that evolved into pustules. She was afebrile and hemodynamically stable but demonstrated a marked systemic inflammatory response, including leukocytosis (white blood cell count: 21.3×10⁹/L; 85% neutrophils) and ele

Recurrent Lower Extremity Pseudocellulitis in a Liver Transplant Recipient: Suspected Tacrolimus-Associated Drug Eruption.

Cutaneous drug eruptions can closely mimic cellulitis, creating a diagnostic challenge in transplant recipients receiving multiple potentially contributory medications. We report recurrent lower-extremity pseudocellulitis in a 55-year-old liver transplant recipient whose clinical course raised suspicion for a tacrolimus-associated drug eruption. Over 11 months, she was hospitalized 10 times and received multiple therapeutic and prophylactic antimicrobial regimens without sustained improvement. Repeated microbiologic studies, including bacterial, fungal, and acid-fast bacillus tissue cultures,

Short-Term Surgical Outcomes of Curative Colorectal Resections from an Evolving Low-Volume Cancer Center in a Tier-2 City in India.

In vitro cultures play a promising role for production of pharmaceutically important plant secondary metabolites and the use of elicitation can mitigate the low productivity of active compounds. In the present study, the influence of cadmium chloride (CdCl2) elicitation on alkaloid yield was investigated in Rauvolfia serpentina. This heavy metal was employed to enhance the yield of reserpine and ajmalicine in leaf derived callus, leaves, stems and roots of in vitro grown cultures. Different concentrations [0.05 mM (C1), 0.10 mM (C2), 0.15 mM (C3) and 0.20 mM (C4)] of CdCl2 were added to the MS

Superficial Skin Ulcers as an Initial Manifestation of Systemic Lupus Erythematosus.

Cutaneous vasculitis is frequently present in patients with systemic lupus erythematosus (SLE), rheumatoid arthritis, and Sjögren's syndrome and is less common in dermatomyositis, systemic sclerosis, and relapsing polychondritis. Given the low prevalence of vasculitic involvement in this disease, the skin lesions described so far have had minimal semiological characterization. This has limited the provision of clear guidelines regarding their treatment, with most therapeutic interventions being extrapolated from idiopathic chronic leukocytoclastic vasculitis. We present the case of a female pa
