Non-thrombocytopenic palpable purpura is a characteristic lesion of cutaneous leukocytoclastic vasculitis. In association with manifestations in the gastrointestinal tract, kidney and/or joints, it forms the clinical entity of Henoch-Schönlein purpura. Among 27,510 inpatients in the years 1974 to 1989 from the CHDM Berne/St. Gallen, 2 developed one, and a further patient as many as 4 episodes of leukocytoclastic vasculitis limited to the skin, which were probably related to drugs (most often compounds containing a sulfonamide, related structures or a penicillin). In contrast, among 8 additiona

Ninety patients with chronic venous ulceration were investigated to determine the extent of the incompetence of the venous valves. They were then treated by established surgical methods to control reflux. All wore fitted compression stockings and were regularly followed up. Forty-two patients had valvular incompetence confined to the perforating and superficial veins. All their ulcers healed and only one developed a minor recurrence during the three year follow-up. Nineteen patients had additional but limited incompetence of the deep veins; all their ulcers healed, but three developed a recurr

A patient with hypocomplementemic urticarial vasculitic syndrome (HUV) is presented. This is an immunological pathology, limited to skin or multisystemic, that requires a differential diagnosis with erythematosus systemic lupus on the same occasions. The ever-present symptom is skin participation, such as urticaria-angioedema or fixed exanthema; biopsy shows necrotizing venulitis with polymorphonuclear infiltration and leukocytoclastic powder. Typical laboratory data are: diminished C3, C4 and C1q; C1 inhibition can be low or normal; the more characteristic finding is the presence of C1q assoc

PURPOSE: Assessment of the value of determination of antineutrophil cytoplasmic antibodies (ANCA) and its specificities for classification of patients with biopsy-proven necrotizing arteritis.
PATIENTS AND METHODS: The serum samples of 28 consecutive patients with biopsy-proven vasculitis involving medium- and/or small-sized arteries were tested for ANCA by an indirect immunofluorescence technique, by neutrophil extract enzyme-linked immunosorbent assay (ELISA), and by catching ELISA.
RESULTS: Eight patients had Churg-Strauss syndrome; six had myeloperoxidase (MPO) antibodies, and in the oth

Antineutrophil cytoplasmic autoantibodies (ANCA) are specific for constituents of neutrophil primary granules and monocyte lysosomes. There are different types of ANCA with different specificities. By indirect immunofluorescence microscopy using alcohol-fixed neutrophils as substrate, two major categories of ANCA can be recognized, one with cytoplasmic staining (C-ANCA) and the other with artifactual perinuclear staining (P-ANCA). Some C-ANCA have specificity for proteinase 3 (PR3-ANCA) and some P-ANCA have specificity for myeloperoxidase (MPO-ANCA), but there are additional C-ANCA and P-ANCA

A small clinically distinct group of patients with widespread tissue granulomata are described. The principal presenting symptoms are malaise, fever, and weight loss, although a wide variety of complaints are documented. Pulmonary involvement is uncommon. The granulomata are noncaseating with a few multinucleate giant cells and some surrounding chronic inflammatory infiltrate. There is no evidence of an associated arteritis. The disease has a relapsing and remitting course and although it may require treatment with immunosuppressive drugs, particularly if the kidneys are involved, the prognosi

The identification of hereditary and acquired complement deficiencies in humans has led to a better understanding of the biologic importance of the complement system in immunity and autoimmune disease. Although the understanding of the relevance of complement in the pathogenesis of disease is incomplete, several characteristic clinical syndromes associated with complement deficiencies have been recognized and should be known to the practicing clinician. In allergic diseases, one need recognize the C1 inhibitor deficiency syndromes which can present as severe, recurrent angioedema in childhood

The clinical features of 25 children with Henoch-Schönlein purpura are presented, with particular emphasis on the cutaneous features. Although every patient exhibited purpura on the buttocks and lower extremities, it was preceded by a maculopapular or urticarial rash in nine. Moreover, cutaneous features were preceded by arthralgia or abdominal pain by up to two weeks in 40% of the patients. Other dominant clinical features included arthralgia in 84%, abdominal pain in 76%, nephritis in 44%, and gastrointestinal bleeding in 40%. Laboratory studies reflected the acute, systemic nature of the il

Drawing on our experience of 16 cases and a review of the English literature, we propose that CSS is under-diagnosed because of exclusive emphasis upon pathologic recognition of the disorder. The classical histological picture comprises a necrotizing vasculitis, eosinophilic tissue infiltration and extravascular granulomas, but it is only found in a minority of cases, and is not pathognomonic of the condition (69, 108). On the other hand, the clinical pattern of the disorder is most distinctive, and CSS can be readily identified on clinical grounds. Typically, it begins with allergic rhinitis,

Patients with HCL are subject to a variety of medical problems. Many of these complications are caused by the cytopenias and splenomegaly produced by proliferating neoplastic cells. Infection is a common cause of morbidity in HCL, but it is not clear whether there is an inherent defect in the immune system. The incidence of infection is related to neutropenia and is increased by the administration of cytotoxic drugs and corticosteroids; such drugs should be used cautiously in these patients. Opportunistic or unusual pathogens occur frequently in HCL, but recovery from such infections is the ru

Potassium iodide therapy has a history of more than 150 years. It has been tried in many diseases in the past. However, with the development of modern medications indications for potassium iodide therapy are very limited. It is well known that potassium iodide is the drug of choice for sporotrichosis. Subacute nodular migratory panniculitis and erythema nodosum have also been treated successfully with this drug.

In Wegener's granulomatosis peripheral nerves are more often involved than central nervous structures; functional psychoses are secondary e.g. to renal complications. Neurologic-psychiatric symptoms are varied and result from local spread from the upper respiratory tract with separate granulomata and vasculitis. Striped muscle is involved in about 10% of cases. Specific vegetative disturbances of function do not appear to occur. This is also true for Granuloma gangrenescens. Its multicentric inflammatory, ulcerating and necrotic foci involve midline structures and often start at the anterior n

Infection invariably accompanies thermal injury. The degree to which a patient is jeopardized by infection is related to the size and depth of the burn, the density and virulence of the microorganisms colonizing the burn wound, and the competence of his immune defenses. The aim of topical therapy is to limit microbial colonization of the burn wound to levels below those associated with invasive infection of the viable tissue beneath the eschar. The use of effective topical and systemic antimicrobial agents has been associated with the emergence of other bacterial, fungal, and viral infections

Serum cryoglobulin levels were estimated in thirty-one patients with cutaneous vasculitis, twenty-nine patients with other skin disorders and forty-seven normal subjects. Eighteen of the normal sera contained mixed cryoglobulins—the upper limit of the normal range being 80 μg/ml. Twelve of the vasculitis patients had serum cryoglobulin levels above the normal range. All but one of these cryoglobulins were mixed cryoglobulins. A low serum C3 level was found in only one patient. The possible role of mixed cryoglobulins in the pathogenesis of vasculitis is discussed.

Hocken, A. G., and Bradshaw, G. (1970).Brit. J. industr. Med.,27, 56-60. Arsine poisoning. A case of acute arsine poisoning is described, occurring in an industrial metallurgy worker. The clinical course was of associated oliguric renal failure with acute haemolytic anaemia which was self-limiting but accompanied by marked non-thrombotic phlebitis. There was minor hepatocellular damage. Skin pigmentation was disproportionate to the elevation of serum bilirubin. Transient initial pulmonary oedema was regarded as a local irritative phenomenon. All systems underwent complete recovery. Renal funct

Moderate resistance to Escherichia coli and Serratia marcescens endotoxin was induced in rabbits. The E. coli endotoxin-resistant rabbits did not respond with a Shwartzman reaction if the skin preparation was carried out by E. coli endotoxin and the provocation by Serratia endotoxin. However, the same animals showed definite haemorrhagic reactions if their skin was prepared by endotoxin from other serological types, e.g. Serratia or Salmonella typhi endotoxin. Serratia endotoxin resistant rabbits did not display the Shwartzman reaction if the skin preparation was carried out by Serratia endoto
