Evidence-based guidelines for the diagnosis and management of adult-onset IgA vasculitis.

IgA vasculitis (IgAV) is an immune complex-mediated small-vessel vasculitis that typically affects the skin, gastrointestinal tract, kidneys and joints. Childhood-onset IgAV is a common disease and usually follows a self-limiting course, whereas adult-onset IgAV is considerably less frequent and is associated with a poorer prognosis. The diagnosis, assessment and management of adult-onset IgAV remain challenging owing to the absence of validated diagnostic criteria for adults and lack of IgAV-specific standardized disease activity scores. Short-term outcomes are mainly determined by gastrointe

Extraintestinal Cutaneous Manifestations in Inflammatory Bowel Disease Among Non-White Patients: A Retrospective Multicenter Study.

BACKGROUND: The presence of extraintestinal manifestations with cutaneous diseases in patients with inflammatory bowel disease (IBD) can pose significant complications. Though the prevalence of IBD has been increasing in racial and ethnic minority groups, most literature has characterized several cutaneous manifestations (CM) of IBD in patients of white skin, with a lack of studies describing these complications in patients with other skin tones. METHODS: Our study aimed to determine the rates of various CM of IBD in skin of color using a health care database to identify white and non-white p

Rocky Mountain Spotted Fever Presenting as an Ischemic Stroke: A Rare Manifestation.

Rocky Mountain spotted fever (RMSF) is among the most severe tick-borne rickettsial infections encountered in the United States. Its presentation ranges from the classic triad of fever, headache, and rash to central nervous system (CNS) conditions, including meningitis, encephalitis, and seizures. Notably, RMSF presenting as an ischemic stroke due to CNS vasculitis is rare, which can prove fatal if recognition is delayed. We present the case of a woman in her 50s with a history of well-controlled diabetes mellitus type 2 and hypertension, who presented with fever, chills, body aches, and rash.

Cutaneous manifestations of scrub typhus: a comprehensive review of diagnostic clues in a neglected tropical disease.

Scrub typhus, caused by Orientia tsutsugamushi, is a re-emerging zoonotic infection with significant diagnostic reliance on its cutaneous manifestations. The disease, transmitted by larval trombiculid mites, is endemic to the 'Tsutsugamushi Triangle' and extends to other regions globally. Dermatological features are pivotal in early recognition, with eschar formation being pathognomonic. Eschars, resembling 'cigarette burns', develop as necrotic lesions surrounded by erythema and often resolve with minimal scarring. Maculopapular rashes, although variably reported, predominantly affect the tru

Coronary Ectasia and Ventricular Arrhythmia in a Patient With Prior Tick Exposure and Borrelia burgdorferi IgG Seroreactivity: A Case Report.

Lyme borreliosis is a multisystem infectious disease caused by Borrelia burgdorferi and transmitted to humans through the bite of infected Ixodes ticks that most commonly affects the skin, joints, nervous system, and, less frequently, the heart. Lyme carditis typically presents with atrioventricular conduction abnormalities, whereas coronary vascular involvement is rarely described. We report a case of a 74-year-old man with exertional chest discomfort, dyspnea, and palpitations several months after an untreated tick bite. Electrocardiography demonstrated ventricular bigeminy with repolarizati

Probable Coxsackievirus Infection in an Immunocompetent Adult With Genital and Oral Ulcerations Mimicking Herpes Simplex Virus Infection and Behçet's Disease.

Disseminated viral infections in immunocompetent adults are uncommon and may mimic autoimmune or bacterial conditions, including Behçet's disease and disseminated herpes simplex virus (HSV) infection. We report a case of a 33-year-old immunocompetent woman who developed severe, painful vulvar ulcerations, oral ulcers, and a painful maculopapular rash involving both lower extremities that evolved into pustules. She was afebrile and hemodynamically stable but demonstrated a marked systemic inflammatory response, including leukocytosis (white blood cell count: 21.3×10⁹/L; 85% neutrophils) and ele

Recurrent Lower Extremity Pseudocellulitis in a Liver Transplant Recipient: Suspected Tacrolimus-Associated Drug Eruption.

Cutaneous drug eruptions can closely mimic cellulitis, creating a diagnostic challenge in transplant recipients receiving multiple potentially contributory medications. We report recurrent lower-extremity pseudocellulitis in a 55-year-old liver transplant recipient whose clinical course raised suspicion for a tacrolimus-associated drug eruption. Over 11 months, she was hospitalized 10 times and received multiple therapeutic and prophylactic antimicrobial regimens without sustained improvement. Repeated microbiologic studies, including bacterial, fungal, and acid-fast bacillus tissue cultures,

Superficial Skin Ulcers as an Initial Manifestation of Systemic Lupus Erythematosus.

Cutaneous vasculitis is frequently present in patients with systemic lupus erythematosus (SLE), rheumatoid arthritis, and Sjögren's syndrome and is less common in dermatomyositis, systemic sclerosis, and relapsing polychondritis. Given the low prevalence of vasculitic involvement in this disease, the skin lesions described so far have had minimal semiological characterization. This has limited the provision of clear guidelines regarding their treatment, with most therapeutic interventions being extrapolated from idiopathic chronic leukocytoclastic vasculitis. We present the case of a female pa

Case Report: A case of cutaneous polyarteritis nodosa in a child following a streptococcal infection.

BACKGROUND: Polyarteritis nodosa is a rare systemic necrotizing vasculitis that is often overlooked and misdiagnosed in clinical practice. Patients may exhibit fever, rash, myalgia, and further symptoms; in severe instances, this may result in damage to the kidney, heart, and other important organs, and may even be life-threatening. Consequently, prompt diagnosis and intervention might mitigate the occurrence of complications and improve patient prognosis. PATIENT PRESENTATION: An 11-year-old girl was admitted to our hospital with multi-joint pain for 7 days, accompanied by worsening fever fo

Skin Manifestations Among Individuals With Hepatitis C Infection.

Hepatitis C, an infection of the liver caused by the hepatitis C virus (HCV), is a growing global health concern, with an increasing annual incidence of primary disease and a notable likelihood of developing extrahepatic manifestations, including dermatological conditions. Studies have explored the relationship between hepatitis C infection and dermatologic manifestations, but there has been a limited number of comprehensive review manuscripts. This review aims to provide a comprehensive summary of studies conducted over a 10-year period, highlighting the relationship between hepatitis C infec

Eosinophilic Granulomatosis with Polyangiitis (EGPA) is a rare systemic vasculitis with eosinophilic inflammation and variable clinical presentations. Although skin manifestations are frequent, current classification criteria do not include them, which may underestimate their diagnostic value. This prospective observational study aimed to assess systemic and skin involvement as well as eosinophilia, anti-neutrophil cytoplasmic antibody (ANCA), and Anti-nuclear antibodies (ANA) serum levels in 20 EGPA patients followed for one year at the University Hospital of Messina, Italy, before starting M

Dilemmas in the Management of Digital Ulcers in Systemic Sclerosis-Mixed Connective Tissue Disease: Lessons From a Case Report.

Systemic sclerosis (SSc) is a rare autoimmune disorder marked by fibrosis and microvascular injury, commonly manifesting as digital ulcers and skin thickening. Lower limb ulcers are uncommon but can contribute to significant morbidity, need prolonged healing, and carry the risk of progression to necrosis or amputation, particularly in systemic sclerosis-mixed connective tissue disease (SSc-MCTD). We report a case of a 57-year-old woman with Raynaud's phenomenon, inflammatory small-joint arthritis, alopecia, photosensitive rash, and exertional dyspnea. Localized Immune-Complex Small-Vessel Vasculitis Presenting as Unilateral Myositis After Hepatitis A.

Immune-complex small-vessel vasculitis (ICSVV) is a subtype of vasculitis characterized by inflammation and necrosis of small blood vessels caused by deposition of circulating immune complexes in vessel walls, followed by complement activation and recruitment of inflammatory cells. It typically targets postcapillary venules in the skin, but other small vessels in muscles, kidneys, gastrointestinal tract, and other organs can also be affected. Vasculitic myositis is usually bilateral and symmetric, with only a few reports of muscle-limited disease. Hepatitis A virus (HAV) is an uncommon trigger

Hypocomplementemic Urticarial Vasculitis: A Case Report.

Urticarial vasculitis (UV) is an inflammatory condition that affects small vessels, generating urticarial lesions with wheals lasting >24 hours. It can be divided into two main groups: normocomplementemic (NUV) and hypocomplementemic urticarial vasculitis (HUV). The latter is a rare condition, whose association with autoimmune diseases, primarily systemic lupus erythematosus (SLE), makes its diagnosis difficult. We present a 67-year-old female patient with a family history of SLE. She presented with disseminated dermatosis of eight months' duration with wheals lasting >24 hours. Due to a suspe

Long-term risk of malignancies in ANCA-associated vasculitis.

OBJECTIVES: Patients with anti-neutrophil cytoplasmic antibodies-associated vasculitis (AAV) have an increased malignancy risk, largely attributed to immunosuppressive therapy. Given the latency period for malignancy development, data on long-term incidence in large AAV cohorts remain limited. This study aimed to assess the cumulative malignancy incidence in patients with AAV from European Vasculitis Society (EUVAS) clinical trials (1995-2012). METHODS: We analysed 848 patients with AAV from 17 European countries. Data were collected via questionnaires sent to the principal investigators of 7

Longitudinal changes on cranial magnetic resonance imaging in relapsing giant cell arteritis.

OBJECTIVE: There is a need for better tools to monitor disease activity in giant cell arteritis (GCA). Prior studies demonstrated that vascular enhancement on cranial vessel wall magnetic resonance imaging (vw-MRI) decreases with treatment of GCA, but whether enhancement increases during relapse is not well known. This study examined changes on vw-MRI during relapse of cranial GCA. METHODS: Patients with active GCA acquired cranial vw-MRIs at enrollment and months 1, 6, and 12 and if suspected relapse occurred. Neuroradiologists graded vw-MRI enhancement for several structures. Changes in MRI

Haptoglobin as a novel predictor of visceral involvement and relapse in adult IgAV patients.

INTRODUCTION: IgA vasculitis (IgAV) can present as skin-limited or systemic disease, which can be severe in adults. Predictive markers for visceral involvement are suboptimal. Considering haptoglobin's role as an acute phase reactant, we evaluated whether its differential expression in IgAV patients' skin and leukocytes is also reflected systemically in a larger cohort of adult IgAV patients. Additionally, soluble form of haptoglobin scavenger receptor CD163 was measured in IgAV patient serum. METHODS: We re-analyzed RNA sequencing data from leukocytes and skin biopsies of treatment-naïve adu

Systemic Involvement of Leukocytoclastic Vasculitis in Skin of Color.

Leukocytoclastic vasculitis (LCV) is an immune-mediated small-vessel vasculitis with various underlying causes. We present a unique case of LCV in an elderly man with Fitzpatrick phototype V skin to highlight multi-organ signs associated with this rare pathology. Our patient presented with an evolving, diffusely spread rash following respiratory symptoms accompanied by muscle weakness, scant hemoptysis, and decreased visual acuity. A comprehensive diagnostic approach, including skin biopsy, revealed LCV with associated eosinophilia. Treatment through a multidisciplinary approach included syste

A case and literature review of axial spondyloarthritis and immunoglobulin A vasculitis: Rare association or under-recognized?

Axial spondyloarthritis (axSpA) is a chronic inflammatory arthritis affecting the spine and sacroiliac joints, often accompanied by extra-musculoskeletal manifestations involving the eyes, gut, and skin. Other organ systems, including the heart (aortic insufficiency), lungs (upper-lobe predominant interstitial fibrosis), and kidneys (nephritic syndrome), may also be affected. Immunoglobulin A vasculitis (IgAV), formerly known as Henoch-Schönlein purpura, is the most common systemic vasculitis in children and is typically self-limited. It is characterized by palpable purpura, arthritis or arthr
