Unraveling Bone-Skin Crosstalk Enables miRNA Nanoformulation for Cutaneous Neurovascular Reconstruction in Diabetic Mice.

The clinical intractability of diabetic foot ulcers stems from a profound uncoupling of cutaneous neurovascular networks, rendering standard metabolic and topical interventions largely palliative. Paradoxically, remote orthopedic trauma robustly accelerates distal skin repair, yet the systemic molecular mediators driving this "bone-skin crosstalk" remain undefined, precluding its translation into non-invasive therapies. Here, we establish that macroscopic bone fracture expedites diabetic wound healing through the systemic release of exosomal miR-130b-3p, a potent orchestrator of coupled angiog

Serine synergizes with lipopolysaccharide to induce macrophage pyroptosis through extracellular Hsp90α and early skin immune microenvironment disruption in diabetic foot.

BACKGROUND: The pathogenesis of early-stage skin lesions in diabetic foot (DF) remains poorly understood, and cannot be fully explained by conventional theories. Skin microbiota dysbiosis has recently emerged as a critical factor, but the underlying mechanisms remain unclear. METHODS: In this study, we integrated metabolomics and metagenomics analyses of skin samples to investigate metabolic dysregulation driven by microbial dysbiosis. RESULTS: We identified elevated serine as a key metabolic alteration strongly correlated with a dysbiotic microbiota structure. Functionally, we demonstrate t

Training a phage to expand its host range: directed evolution against Staphylococcus aureus from diabetic foot infections.

Diabetic foot infections (DFIs) are a major complication of diabetes frequently involving multidrug-resistant Staphylococcus aureus. Their persistence and therapeutic complexity underscore the urgent need for alternatives to conventional antibiotics. Phage therapy offers a promising solution, though its clinical application is often limited by the narrow host range of individual phages. Here, SAVM02, a recently characterized staphylococcal Kayvirus, was trained using directed evolution against DFI isolates. After 20 iterative passages on a mixed panel of susceptible and non-susceptible S. aure

Effectiveness and safety of alprostadil injection in the treatment of patients with type 2 diabetes complications.

BACKGROUND: Type 2 diabetes mellitus (T2DM) has become a global public health crisis with a steady upward prevalence, and China alone has over 100 million adult T2DM patients. Long-term persistent hyperglycemia damages microvasculature and peripheral nerves, leading to disabling complications such as diabetic foot (DF) and diabetic peripheral neuropathy (DPN). These complications severely reduce patients' quality of life and increase medical burdens, thus creating an urgent need for effective adjunctive therapeutic interventions. OBJECTIVES: To evaluate the efficacy and safety of alprostadil

Proposed dense variational autoencoder model integrated with contrastive learning for foot ulcer classification.

The most serious complication of diabetes, Diabetic Foot Ulcer (DFU), can result in chronic infection, damage to tissues, and even amputation if not identified in a timely way. It is even more dangerous for disabled people. Accurate and timely diagnosis is therefore essential for improved patient outcomes. However, it is difficult to perform a manual assessment of DFU images due to variations in the textures of ulcers, light effects, and severities. Herein is presented an enhanced deep learning framework DenseNet121 with Variational Autoencoder and Contrastive Learning (DenseVAE-CL) which inco

Diabetic foot ulcer classification using an enhanced coordinate attention integrated ConvNext model.

Diabetic foot ulcers (DFUs) pose a significant complication of diabetes with the potential to lead to amputation if not effectively managed. Current DFU treatments require rigorous monitoring by both healthcare professionals and patients, which is challenging due to the high costs associated with diagnosis, treatment and long-term care. A major limitation of these approaches is their limited capacity to identify highly relevant pattern connections and broad contextual correlations resulting inaccuracies in classifying regions of interest. This research introduces an attention enhanced deep lea

Photo-controlled spatiotemporal sequential release of MXenes/NO gas from bilayer (clean-cure) hydrogel promotes healing of MRSA biofilm-infected diabetic ulcer wounds.

Despite the success in designing intelligent materials for diabetic ulcer wound (DUW) healing, it is often challenging to consider the entire healing process, from early antibacterial and anti-inflammatory effects to late matrix remodeling and angiogenesis characteristics, by on-demand releasing of multiple therapeutics. Herein, we designed a versatile bilayer (GCM/Clean-GDA/Cure) hydrogel dressing with light-controlled, spatiotemporal sequential release of MXene nanosheets (MX NSs)/nitric oxide (NO) gas for the healing of methicillin-resistant Staphylococcus aureus (MRSA) biofilm-infected DUW

Investigating plantar soft tissue elasticity modulation using air insoles during walking.

BACKGROUND: In the forefoot region, diabetic foot ulcer (DFU) is more likely to develop due to elevated plantar pressure and repetitive loading. This study investigated the effects of different forefoot insole pressures and walking durations on the elasticity of the plantar tissue to identify conditions that may help prevent DFU. METHODS: With thirteen healthy participants, we conducted a controlled walking exercise at a speed of 3.6 mph on a treadmill, comparing three different inner pressures of 80, 160, and 240 mmHg with two different walking durations of 10 and 20 min. We measured the elas

Circular RNAs in Diabetic Foot Ulcers: A Scoping Review of Clinical, Preclinical, and In Silico Evidence on Diagnostic and Therapeutic Potentials.

BACKGROUND AND OBJECTIVE: Diabetic foot ulcers (DFUs) involve chronic inflammation, impaired angiogenesis, oxidative stress, and disrupted fibroblast-keratinocyte interactions. Circular RNAs (circRNAs), a category of stable non-coding RNAs, have become essential regulators of these processes; nevertheless, their comprehensive functions in DFUs are still inadequately characterised. This scoping review integrated clinical, preclinical, and in silico evidence on circRNAs in DFUs to assess their diagnostic, mechanistic, and therapeutic potential. METHODS: Systematic searches of MEDLINE, EMBASE, W

Diabetic Ray Amputation Wounds Managed With Local Flap Surgery and Negative Pressure Wound Therapy: A Case Series.

A new surgical technique for ray amputation of the diabetic foot is described. The procedure provides soft tissue reconstruction using a local flap in combination with negative pressure wound therapy to close these wounds safely and effectively. This article reports on 12 patients treated with this technique in a Tertiary Hospital/General Hospital in Singapore and compares their outcomes to those of 12 comparative cases from neighbouring institutions under the same health group who received conventional dressings. The new technique led to a higher number of healed wounds (12 vs. 4) and a short

Nurses' Perceptions of Fundamental Care in Preventing Venous Ulcer Recurrence: A Qualitative Focus Group Study.

Venous ulcers are the most prevalent among those affecting the lower limbs, with high economic and quality of life impact. Due to the chronicity of venous disease, healing and recurrence cycles are common. Patient adherence to self-care activities to prevent recurrence is very low, so it is crucial to reflect on nurses' experiences with this phenomenon. A qualitative and descriptive study was developed to analyse nurses' perceptions about preventing venous ulcer recurrence. According to the inclusion criteria, focus groups with nurses were implemented. Three focus groups were conducted through
