An Association Between Haemochromatosis Genotypes and Venous Leg Ulcers in Australian Individuals.

Chronic venous disease underlies the pathogenicity of venous leg ulcers. However, it is unknown why some people with chronic venous disease are more susceptible to the development of venous leg ulcers (VLUs) than others. While there is evidence of a genetic component, our understanding of how specific genetic variants influence the development of chronic wounds and more specifically (VLUs) is limited. The aim of this study was to determine if specific genetic variants associated with the iron overload disorder haemochromatosis, HFE p.C282Y and HFE p.H63D, predispose to the development of VLUs.

Potential application of mono-, dual-, and triple-target GLP-1 receptor agonists in improving the prognosis of patients with diabetic foot ulcers.

Diabetic foot ulcer (DFU), a severe chronic complication of diabetes mellitus, poses a major public health threat due to its high rates of disability, recurrence, and all-cause mortality. The mortality rate of DFU patients is closely related to cardiovascular events, indicating that their treatment should go beyond local wound management and focus on cardiovascular risk intervention. Glucagon-like peptide-1 receptor agonists (GLP-1 RA), known for their cardiovascular protective effects demonstrated in cardiovascular outcome trials, offer new treatment opportunities for DFU patients. However, t

Clinical efficacy of combined treatment for lower extremity venous reflux and iliac vein obstruction.

BackgroundChronic venous disease (CVD) arises from venous obstruction, reflux, or both. Iliac vein stenting is standard for obstructive lesions, but the incremental benefit of concurrently treating deep venous reflux remains uncertain. We evaluated the efficacy of combining iliac vein stent placement with deep venous valve reconstruction in CVD patients presenting with both iliac obstruction and reflux.MethodsFrom October 2015 to May 2025, we retrospectively analyzed a prospectively maintained cohort of 74 patients with lower-limb CVD and iliac vein stenosis and stratified them by reflux patte

Thermography-Assisted Grading in Wagner Classification: A New Approach for Diabetic Foot Assessment. A Cross-Sectional Study.

BACKGROUND: Diabetic foot is a serious complication of diabetes mellitus that generates lifelong consequences on the health and quality of life of affected patients. One of the first grading systems developed for diabetic foot was the Wagner classification system. Despite its prolonged use in the medical field, accurate clinical assessment requires an experienced evaluator to minimize errors and bias. Using infrared thermography, a technology that quantitatively measures temperature changes in areas of interest related to altered vascular flow derived from inflammatory processes, could diminis

Effect of cold saline application during sharp debridement on pain in patients with diabetic foot: a randomised, controlled, single-blind trial.

OBJECTIVE: Debridement is the removal of nonviable, contaminated, or foreign material from within or near a wound until healthy adjacent tissue is exposed. In sharp debridement, patients are expected to experience greater levels of pain because of cell damage. This study evaluated the effect of applying cold 0.9% sodium chloride (NaCl) during sharp debridement on pain in patients with diabetic foot. METHOD: A randomised, controlled, single-blind trial was performed with inpatients randomised to two equally sized groups: an experimental group and a control group. During sharp debridement, cold

Management of essential thrombocythaemia-associated toe ulcer with a novel topical macrophage-modulating cream containing extract of Plectranthus amboinicus.

A novel macrophage-modulating cream containing proprietary extracts from Plectranthus amboinicus and Centella asiatica has been approved for the treatment of diabetic ulcers in some countries. It has been reported to suppress M1 macrophages and facilitate the transition from M1 to M2 macrophages, reducing inflammation and leading to faster wound healing. Based on its mode of action, it has also been used in non-diabetic wounds. This report describes the treatment of a patient with essential thrombocythaemia (ET) presenting initially as a toe ulcer, and which was successfully managed with the c

Distinct microRNA signatures in Platelet-Rich Plasma-Derived extracellular vesicles predict healing outcomes in chronic diabetic foot ulcers.

BACKGROUND: Diabetic foot ulcers (DFUs) present a major challenge due to impaired angiogenesis and chronic inflammation. Autologous platelet-rich plasma (PRP) is a widely used therapy, but clinical outcomes remain inconsistent. We hypothesized that the microRNA (miRNA) cargo of platelet-derived extracellular vesicles (PRP-EVs) drives this therapeutic variability. METHODS: Ten patients with refractory DFUs were enrolled. Autologous PRP-EVs were isolated, and wound healing-associated miRNAs were quantified via qRT-PCR. Clinical wound closure was monitored weekly, with the primary efficacy endpo

A narrative review of clinical decision support systems in offloading footwear for diabetic foot ulcers.

BACKGROUND: Offloading footwear is a critical intervention for preventing and treating diabetic foot ulcers (DFUs), as it reduces plantar pressure (PP) and promotes healing. However, decision-making around its prescription remains fragmented, with variability in feature selection, limited personalization, and diverse evaluation approaches. While guidelines, knowledge-based systems, and machine learning (ML) applications have explored elements of decision-making, no clinical decision support system (CDSS) currently exists to provide feature-level prescriptions. METHODS: We conducted a narrative

A single-dose of PDGFB circular RNA enables sustained growth factor expression to accelerate diabetic wound healing.

Diabetic foot ulcer (DFU) is one of the most serious complications of diabetes and lack effective treatment options. Although platelet-derived growth factor-B (PDGFB) has been approved for the treatment of diabetic wounds, it is difficult to sustainably deliver PDGFB to the wound site of DFU owing to its poor stability and easy degradation. To address these limitations, we developed a lipid nanoparticle (LNP)-encapsulated PDGFB circular RNA (LNP-circPDGFB) formulation designed to achieve sustained local expression and release of PDGFB for enhanced diabetic wound healing. The therapeutic circRN

Future Directions for Diabetic Foot Ulcer Interventions: A Scoping Review of Continuous Glucose Monitoring-based Telemedicine.

OBJECTIVES: Nonadherence to diabetes treatment leads to persistent hyperglycemia, a key driver of nonhealing diabetic foot ulcers (DFUs). Continuous glucose monitoring (CGM) and telemedicine have independently improved glycemic management, glycated hemoglobin levels, and time in range. In this study we review the various components of telemedicine and CGM to gain insight that could contribute to future directions for enhancing DFU interventions. METHODS: A scoping review was conducted through searches involving PubMed, Embase, CINAHL, and Scopus. RESULTS: Sixteen studies were included in thi

Role of Neutrophil-to-Lymphocyte Ratio and Platelet-to-Lymphocyte Ratio in Predicting In-Hospital Mortality in Patients With Diabetic Foot Ulcers Undergoing Amputation: A Retrospective Study.

BACKGROUND: Diabetic foot ulcers (DFUs) are a severe complication of diabetes and a major cause of non-traumatic lower limb amputations worldwide. Patients undergoing major lower limb amputations (MLLAs) for DFUs often have poor prognoses, with high in-hospital mortality rates. Inflammatory biomarkers, such as the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR), have emerged as potential prognostic indicators in various clinical settings; however, their predictive value in this high-risk population remains underexplored. METHODOLOGY: This retrospective observationa

An off-the-shelf bioactive EV delivering cryogel orchestrating immunomodulatory reprogramming and angiogenesis for diabetic ulcer healing.

Chronic diabetic foot ulcers (DFUs) are marked by persistent inflammation, oxidative stress and hypoxia, leading to impaired healing and limited treatment success. While extracellular vesicles (EVs) are promising in modulating the immune microenvironment and promoting regeneration, their clinical use is constrained by instability and loss of function over storage. Alternatively, scaffolds that deliver oxygen have shown regenerative potential and can act as a delivery system for EVs. Here, we report a multifunctional Bioactive Oxygenated Off-the-Shelf EV Therapy (BOOST) that integrates lyophili

Glucolipid metabolic disorders and ferroptosis in diabetic ulcers: pathogenic crossroads and therapeutic opportunities.

Ferroptosis, an iron-dependent form of regulated cell death driven by lipid peroxidation, has emerged as a key player in the pathogenesis of Diabetic Ulcers (DUs). Concurrently, Glucolipid Metabolic Disorders (GLMDs), driven by persistent hyperglycemia and lipotoxicity, constitute a core pathological basis for tissue damage and dysfunction of reparative cells. The interplay between ferroptosis and GLMDs represents a promising yet underexplored research area. This review synthesizes recent advances in understanding the molecular mechanisms underlying their interaction, focusing on how GLMDs dri
