Similar Long-Term Incidence of Hepatocellular Carcinoma in Chronic Hepatitis B Initiating Entecavir Versus Tenofovir Alafenamide: A REAL-B Study.

Data on potential differences in hepatocellular carcinoma (HCC) incidence between entecavir (ETV) and tenofovir alafenamide (TAF) in chronic hepatitis B (CHB) patients are limited. This retrospective multinational study included treatment-naïve adults with CHB without a history of HCC who initiated ETV or TAF therapy since 2016 at 23 centres in 4 countries. Baseline characteristics were balanced using inverse probability of treatment weighting (IPTW) and propensity score matching (PSM). HCC incidence and the association between antiviral regimen and HCC were analysed using competing-risk analy

Risk of Hepatocellular Carcinoma or Hepatic Decompensation Associated With Steatotic Liver Disease in People With and Without HIV.

BACKGROUND: Steatotic liver disease (SLD) is highly prevalent in people with HIV (PWH), but factors influencing progression to hepatocellular carcinoma (HCC) and hepatic decompensation (HD) remain unclear. It is unknown if the risk of these outcomes associated with SLD differs by HIV status. METHODS: Patients from the Veterans Aging Cohort Study-HIV (includes matched people without HIV [PWoH]) who underwent an ultrasound, computed tomography, or magnetic resonance imaging between 2001-2017 were included. Patients with diagnosed cirrhosis or chronic viral hepatitis were excluded. SLD was defin

Beyond fibrosis: the molecular roadmap from MASLD to hepatocellular carcinoma.

Metabolic dysfunction-associated steatotic liver disease (MASLD) has overtaken viral hepatitis as the fastest-growing cause of hepatocellular carcinoma (HCC) in most high-income countries. What makes this epidemic clinically treacherous is a feature that distinguishes MASLD-related HCC from other aetiologies: roughly half of all cases arise in livers that have never progressed to cirrhosis, slipping through the net of conventional surveillance strategies. The molecular journey from fatty liver to malignancy unfolds through at least six interlocking programmes: lipotoxic rewiring of hepatocyte

Hepatitis-Associated Aplastic Anemia in Children and Adolescents: A Comprehensive Review.

Hepatitis-associated aplastic anemia (HAAA) is a rare but life-threatening subtype of acquired aplastic anemia (AA) that predominantly affects children and adolescents, characterized by bone marrow failure developing weeks to months following acute hepatitis. Although HAAA accounts for a small proportion of AA cases, it presents unique diagnostic and therapeutic challenges and remains poorly understood. Most cases are seronegative, with no identifiable viral pathogen, supporting an immune-mediated pathophysiology. Emerging evidence implicates dysregulated cytotoxic T-cell activation, reduced r

Distinct immune signatures define hepatitis D- versus hepatitis B-associated hepatocellular carcinoma.

BACKGROUND: Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality worldwide. Hepatitis D is the most severe form of viral hepatitis. However, the clinical, virological, and systemic inflammatory characteristics of HDV-associated HCC remain insufficiently characterized. METHODS: In this single-center retrospective study, we identified 162 patients with HCC due to chronic hepatitis D (HDV-HCC; n=54) or chronic hepatitis B (HBV-HCC; n=108). Plasma levels of 92 soluble inflammatory mediators (SIMs) were quantified using proximity extension assay in HCC patients [HBV-HCC (n

Host restriction factors in HBV-related liver disease and hepatocellular carcinoma: mechanisms of viral-host interplay and implications for therapy.

Chronic hepatitis B virus (HBV) infection is a leading cause of liver cirrhosis and hepatocellular carcinoma (HCC), imposing a substantial global health burden. The natural history of persistent HBV infection progresses from chronic inflammation to fibrosis, cirrhosis, and malignant transformation, driven in part by the stability of covalently closed circular DNA (cccDNA) within hepatocyte nuclei. Nucleos(t)ide analogues (NUCs) effectively suppress viral replication but rarely lead to a functional cure, necessitating long-term therapy and carrying a residual risk of resistance. PEG-interferon-

Complete remission of minimal change disease after antiviral monotherapy in a patient with immune-active chronic hepatitis B: A case report.

RATIONALE: Minimal change disease (MCD) is characterized by diffuse podocyte foot process effacement on electron microscopy, minimal structural changes on light microscopy, and the absence of immune complex deposition on immunofluorescence. In adults, MCD accounts for approximately 10% to 25% of nephrotic syndrome cases and is typically idiopathic, although secondary causes such as infections, medications, malignancies, and immune disorders have been reported. The association between hepatitis B virus (HBV) infection and glomerular disease has been well established in membranous nephropathy an

Hepatic Progenitor Cells and the Ductular Reaction: Pathophysiological Role in the Progression of Liver Disease.

Ductular reaction (DR), a defining histological hallmark of diverse acute and chronic liver injuries, is fundamentally driven by the activation of hepatic progenitor cells (HPCs) and their complex, spatiotemporal bidirectional crosstalk within the hepatic niche. As bipotent precursor cells, HPCs initiate compensatory regeneration when mature hepatocyte proliferation is compromised. However, DR is not a uniform process but a multiphase dynamic reaction: transient early activation facilitates parenchymal restoration, whereas persistent, dysregulated HPC expansion fuels progressive fibrogenesis.

Hepatitis A-Associated Very Severe Aplastic Anemia in a Young Adult Female: A Case Report.

BACKGROUND: Hepatitis‑associated aplastic anemia (HAAA) is a rare but life‑threatening immune‑mediated complication of acute viral hepatitis, and hepatitis A virus is an uncommon cause of this syndrome. CASE PRESENTATION: We report the case of a 24‑year‑old woman who developed acute hepatitis A followed by rapidly progressive pancytopenia fulfilling the criteria for very severe aplastic anemia. Her clinical course was complicated by febrile neutropenia, transfusion dependence, gastrointestinal bleeding, and Stenotrophomonas maltophilia bacteremia. Bone marrow biopsy demonstrated marked hypoce

Histopathologic spectrum of focal liver lesions diagnosed by ultrasound-guided percutaneous liver biopsy and predictors of hepatocellular carcinoma: A single-center experience from Sub-Saharan Africa.

BACKGROUND: Focal liver lesions (FLLs) encompass a wide spectrum of benign and malignant pathologies, and accurate diagnosis is essential for appropriate management. Although advances in imaging have improved lesion characterization, histopathologic assessment remains the diagnostic gold standard for indeterminate lesions. Data on the histopathologic spectrum and diagnostic utility of ultrasound-guided percutaneous liver biopsy (US-PLB) in sub-Saharan Africa (SSA) are limited. This study aimed to characterize the histopathologic findings of US-PLB performed for FLLs at a tertiary referral cent

Hepatitis A Virus (HAV) Recurrence and De Novo Hepatitis A Acquired at Time of Liver Transplantation: Comparison of Graft HAV Infection and Viral Kinetics.

Hepatitis A virus (HAV) infection is an uncommon indication for liver transplantation, and donor-derived or transfusion-associated HAV infection is rare. Data describing HAV recurrence or early acquisition following liver transplantation, particularly with longitudinal quantitative viral kinetics, are limited. We describe two liver transplant recipients with graft HAV infection occurring in different immunological contexts. One patient underwent emergency liver transplantation for HAV-related acute liver failure and subsequently developed recurrent HAV infection of the graft. The second patien

Numerous linked cases of acute genotype F hepatitis B virus infections in people living with HIV or on pre-exposure prophylaxis.

OBJECTIVES: At least 2.7 million people living with HIV are infected with HBV worldwide. Some antiretroviral treatments and tenofovir disoproxil fumarate-based Pre-Exposure Prophylaxis (PrEP) protect against HBV infection. However, emerging antiretroviral (ARV) strategies without any activity against HBV may promote spread of this virus in the future. In this context, we studied recent cases of acute HBV infection in people at risk of sexually transmitted infections. Our objective was to better understand HBV circulation and the dynamics of its transmission. METHODS: This retrospective study

Immune Profiles Elicited by Different HBV Vaccine Modalities and Their Associations with Serological and Intrahepatic Viral Outcomes.

Therapeutic vaccination for chronic hepatitis B aims to restore HBV-specific T-cell immunity and contribute to a functional cure. We systematically compared the immune profiles and antiviral efficacy of four vaccine modalities: recombinant HBsAg (rHBsAg), antigen-antibody immune complex (IC), a DNA vaccine encoding HBsAg (DNA-HBsAg), and an HBsAg-Fc fusion DNA vaccine (DNA-HBsAg-Fc). In naïve mice, the rHBsAg vaccine predominantly induced anti-HBs IgG responses, whereas the IC vaccine elicited strong anti-HBs IgG and detectable CD4+ T-cell responses. In contrast, DNA vaccines preferentially in

Profile of migrant people with non-medically supervised use of pregabalin: a scoping review.

INTRODUCTION: Pregabalin is a gabapentinoid drug used to treat neuropathic pain, epilepsy and anxiety. Over recent years, prescriptions and non-medically supervised use of pregabalin have increased in several European countries, especially within the French-speaking regions. These increases have been reported in young migrants, particularly those from Maghreb. However, the available peer-reviewed literature is poor with respect to the main characteristics of migrant people with non-medically supervised use of pregabalin and their trajectory of use. This scoping review explored both indexed sci

[Extreme values of alpha-fetoprotein in a patient with autoimmune hepatitis].

We present the case of a 29-year-old woman with a 5-year history of liver function test abnormalities who consulted for polyarthritis, fever, and jaundice lasting one month. Initial studies showed predominantly direct hyperbilirubinemia and elevated transaminases, alkaline phosphatase and gamma-glutamyl transferase. Viral hepatitis was ruled out by negative serology and toxic hepatitis by absence of exposure to hepatotoxic drugs. The antibody panel showed elevated antinuclear antibodies and positive anti-Ro titers. She also presented elevated polyclonal immunoglobulin G levels. Given the chron

Global Underutilization of Antiviral Therapy and Survival Outcomes in Hepatitis B Virus-Related Hepatocellular Carcinoma: A Systematic Review and Meta-Analysis of 78 Studies and 68,243 Patients.

Antiviral therapy with nucleos(t)ide analogues is recommended for patients with viremic hepatitis B virus (HBV)-related cirrhosis regardless of levels of viremia or liver enzymes. However, hepatocellular carcinoma (HCC) is not an antiviral treatment criterion in the U.S. HBV treatment guidelines, though prior studies have reported improved survival with antiviral treatment in HBV-related HCC. To inform guidelines, we estimated antiviral treatment rates and their association with survival and HCC recurrence rates in HBV-related HCC. We conducted a systematic review and meta-analysis of studies

Multidimensional regulation of hepatic macrophages in chronic liver disease: from metabolic reprogramming to intercellular and interorgan crosstalk.

Chronic liver diseases (CLDs), including metabolic dysfunction-associated steatotic liver disease (MASLD), alcohol-associated liver disease (ALD, viral hepatitis, liver fibrosis, cirrhosis and hepatocellular carcinoma (HCC)), impose enormous global public health burdens. Hepatic macrophages act as core orchestrators governing the initiation and progression of all CLD subtypes. This review systematically integrates recent advances regarding hepatic macrophage ontogeny, spatial heterogeneity, metabolic rewiring, multi-cellular communication, and systemic neuro-immune crosstalk along the gut-live

Spleen stiffness to rule out high-risk varices in advanced chronic liver disease: an individual patient data meta-analysis.

BACKGROUND & AIMS: Management of esophageal varices (EV) is of critical importance in compensated advanced chronic liver disease. We conducted an individual patient data (IPD) meta-analysis to (i) investigate the performance of spleen stiffness measurement (SSM)-based algorithms using vibration-controlled transient elastography (VCTE) to rule out high-risk varices (HRV); (ii) develop a new model for diagnosing any grade EV. METHODS: We systematically searched databases for studies reporting endoscopy and SSM-VCTE data in adult patients, collecting their IPD. The main outcome was ruling out HR
