Seroprevalence and risk factors of hepatitis E virus infection in a rural Argentine community: a cross-sectional study.

BACKGROUND: Hepatitis E virus (HEV) is a leading cause of acute viral hepatitis worldwide. Rural populations and pig production systems in Argentina may be at increased risk of HEV exposure; however, epidemiological data from these settings remain scarce. METHODS: A cross-sectional study was conducted among residents of a rural community. Serum samples from 90 residents were tested for anti-HEV immunoglobulin G. Participants completed structured questionnaires and residential addresses were georeferenced to evaluate exposures and spatial distribution. RESULTS: Rural residents reported greate

Disparate tetraparvovirus species linked to hepatitis.

The role of human tetraparvoviruses (PARV4) in cases of acute illness is poorly characterized, and the contribution of animal-associated tetraparvoviruses to human disease is unknown. In this study, we applied metagenomic and viral-enrichment next-generation sequencing to plasma from patients with diagnostically unresolved acute illness in Georgia (non-A-C hepatitis) and Bangladesh (acute febrile illness), and two tetraparvovirus genomes were recovered. Specimen HEP004 from a Georgian patient with acute hepatitis contained a virus clustering within Tetraparvovirus ungulate4, a species associat

The Association between Vitiligo and Infections: A Systematic Review.

Vitiligo is an acquired depigmentation disease with an unknown etiology. Several theories on its etiology have been proposed, including autoimmune mechanisms, oxidative stress, genetic factors, environmental factors, and lack of melanocyte growth factor. Recent studies have demonstrated a possible correlation between microbial infections and the development or consequences of vitiligo. This study aims to investigate the potential association between microbial infections and vitiligo, focusing on identifying any explainable correlations between these two. A systematic search was performed on al

Stigma related to hepatitis B virus: A barrier to global viral hepatitis elimination.

Chronic hepatitis B virus (HBV) infection remains a major global health challenge, affecting over 300 million people worldwide and contributing substantially to liver-related morbidity and mortality. Despite the availability of effective vaccination, diagnostic tools, antiviral therapy, and surveillance strategies, progress toward the World Health Organization's hepatitis elimination targets has been slower than anticipated. HBV-related stigma has emerged as a critical and under-recognized barrier undermining efforts across the entire HBV care cascade. Stigma associated with HBV is multi-facet

Hepatitis B reactivation in HBsAg-/anti-HB-negative patients diagnosed with hematological malignancy: A case series.

BACKGROUND: Hepatitis B virus (HBV) reactivation is a well-recognized complication of immunosuppressive therapy in patients with hematologic malignancies. It usually occurs in those with chronic or resolved HBV infection identified by positive hepatitis B surface antigen (HBsAg) or hepatitis B core antibody (anti-HBc). Reactivation in patients who test negative for both markers is exceedingly rare and poses a diagnostic challenge. METHODS: We report on three patients with hematologic malignancies who developed HBV reactivation despite negative baseline serology. Cases were identified and mana

Serum IgM crosslinks hepatitis B subviral particles into supramolecular immune complexes.

Hepatitis B virus (HBV)-infected cells release hepatitis B surface antigen (HBsAg)-rich subviral particles (SVPs) in large excess over infectious virions. SVPs bind antibodies against HBsAg (anti-HBs), including neutralizing antibodies, but the physical products of antibody capture in human blood are uncharacterized. Here we used cryo-electron tomography (cryo-ET) to image HBsAg-enriched serum from a patient with severe acute Hepatitis B and observed three-dimensional assemblies of intact IgM-SVP immune complexes. These associations persisted at a lower particle density and were validated by a

Development of dihydroquinolizinone (DHQ)-based inhibitors for suppression of hepatitis B viral infections.

Chronic hepatitis B virus (HBV) infection is a serious public health burden, and current treatments fail to achieve satisfactory cure rates. RG7834 is a novel therapeutic agent with a differentiated mechanism of action (MOA) compared to current standard treatments. However, its Phase I clinical trial was terminated due to neurotoxicity issues. Furthermore, the pharmacokinetic performances of RG7834 are unsatisfactory, leaving significant room for improvement. This article reports some attempts and efforts based on the RG7834 molecule, including enhancing antiviral activity, improving pharmacok

Empowerment in advanced liver disease: UK experiences of care from the perspectives of people with lived experience, carers and healthcare professionals.

ObjectivesLiver disease is a leading cause of morbidity and mortality, with alcohol-related liver disease, metabolic-associated steatotic liver disease, and viral hepatitis comprising most cases in the UK. Advanced liver disease presents challenges due to unpredictable disease trajectories, high symptom burden, psychosocial impacts, and stigma. This study aimed to develop an explanatory theory of care experiences in decompensated advanced liver disease from the perspectives of individuals living with the condition, carers, and healthcare professionals, with public involvement embedded througho

[Effects of gambogic acid on liver function in rats with chronic hepatitis B by adjusting Sirt1/FoxO1 pathway].

This study investigated the effects of gambogic acid(GA) on liver function in rats with chronic hepatitis B(CHB) via the silent information regulator 2 homolog 1(Sirt1)/forkhead box transcription factor O1(FoxO1) signaling pathway. A CHB rat model was established using hepatitis B virus(HBV) antigen. Another 10 healthy rats injected with an equal volume of physiological saline served as the control group. After successful modeling, rats were divided into a CHB group, low-dose GA(L-GA, 50 mg·kg~(-1) GA by gavage) group, high-dose GA(H-GA, 100 mg·kg~(-1) GA by gavage) group, and GA+EX527(100 mg·

Chronic Hepatitis B: Current Understanding and Clinical Implications of Immune Exhaustion.

Chronic hepatitis B (CHB) infection leads to progressive dysfunction and exhaustion of antiviral immune cells, which is a primary obstacle to clinical cure. Although the phenotypic hallmarks of exhausted T cells are well described, the hierarchical trajectory, subset-specific heterogeneity, and underlying mechanisms of exhaustion across the full spectrum of immune cells in the unique hepatic microenvironment remain poorly defined. This review provides a comprehensive and critical synthesis of recent advances in immune exhaustion during CHB. We delineate the common and distinct exhaustion progr

Viral persistence and immune dysfunction in chronic hepatitis B: Mechanisms of post-treatment antigen asynchrony.

Chronic hepatitis B (CHB) remains a major global health burden. Following antiviral therapy, the dynamics of hepatitis B virus (HBV) DNA and hepatitis B surface antigen (HBsAg) may become discordant, a phenomenon referred to as post-treatment asynchrony. This review summarizes the three principal forms of post-treatment asynchrony and examines their underlying mechanisms. We integrate the limitations of current antiviral targets, host immune dysfunction, virological characteristics, and host genetic factors into a unified framework to explain the discordant dynamics between HBsAg and HBV DNA.

Evaluating the potential of treatment options for chronic hepatitis B.

INTRODUCTION: Chronic hepatitis B (CHB) remains a leading cause of cirrhosis and hepatocellular carcinoma worldwide, but long-term or even lifelong viral suppression with nucleos(t)ide analogues (NAs) seldom clears hepatitis B surface antigen (HBsAg), so-called 'functional cure.' In recent years, a shift in treatment strategy from viral suppression toward cure has emerged, catalyzed by three revised international hepatitis B guidelines (2025-2026) and the first-in-class phase 3 evidence that a finite antiviral therapy, the antisense oligonucleotide bepirovirsen, can achieve a functional cure i

Safety, Tolerability and Antiviral Activity of GST-HG131 in Chronic Hepatitis B: A Randomized, Double-Blind Phase IIa Study.

BACKGROUND AND AIMS: Chronic hepatitis B (CHB) remains a major global health burden. Current therapies rarely achieve a functional cure due to low rates of HBsAg clearance, the persistence of the viral replication template-covalently closed circular DNA (cccDNA)-and the need for lifelong treatment. GST-HG131 is a first-in-class, orally bioavailable small-molecule inhibitor of PAPD5/7, key host factors involved in HBV mRNA stabilization and HBsAg production. We evaluated the safety, tolerability and antiviral activity of GST-HG131 in patients with CHB receiving nucleos(t)ide analogue (NUC) ther

Toll-like Receptor 7 and Interferon-α Synergy Establishes a Positive Feedback Loop to Potentiate Antiviral Immunity in Chronic Hepatitis B.

BACKGROUND AND OBJECTIVES: Current therapies for chronic hepatitis B (CHB) rarely achieve functional cure, underscoring the need for novel immunotherapies. Toll-like receptor 7 (TLR7), a key innate immune sensor, has emerged as a major drug development focus. This study aims to characterize stage-specific TLR7 expression and functions in CHB, investigate its association with antiviral therapy response, and evaluate synergistic effects of TLR7 activation combined with interferon-α (IFN-α). METHODS: We conducted cross-sectional and longitudinal clinical analyses of CHB patients, complemented by

A divergent hepatitis A virus lineage in wild western lowland gorillas and central chimpanzees supports localized circulation and expands HAV diversity.

Hepatitis A virus (HAV) is a ubiquitous human pathogen that may cause acute liver disease. Despite the availability of effective vaccines, high prevalence rates are still reported in Southeast Asia and Africa, where sanitation conditions are poor. Although seven genotypes are known, including three in humans, three in non-human primates (NHP), and one recently identified in alpacas, the diversity, ecology, and evolutionary history of HAV remain poorly understood. In a pilot study of virome molecular characterization of wild African apes by using oligonucleotide probes enrichment, HAV sequences
