Multifunctional nanocomposite with stage-responsive behavior for wound healing.

Chronic wounds, particularly refractory diabetic ulcers, typically fail to heal due to a self-perpetuating inflammatory state driven by persistent bacterial infection, severe oxidative stress, and impaired microvascular regeneration. Conventional static dressings cannot accommodate the spatiotemporal, multiphase complexity of tissue repair. Here, a stage-responsive hierarchical nanocomposite (SNF@GQDs@Arg) was engineered by covalently anchoring graphene quantum dots (GQDs) onto radially porous silica nanoflowers (SNF), followed by surface adsorption of L-arginine (L-Arg). The resulting platfor

Wuguchong Bioactive Peptides Promote Wound Healing via Hedgehog Pathway‑Mediated Fibroblast Modulation.

The skin undertakes multiple critical physiological functions, including barrier protection and thermoregulation. However, chronic nonhealing ulcers have emerged as a major clinical challenge. In wound repair, skin healing is a highly coordinated, time-sequential process wherein fibroblasts-key dermal cells-play irreplaceable roles in proliferation, migration, differentiation, and extracellular matrix (ECM) synthesis. When external environmental factors induce fibroblast dysfunction, this can lead to impaired epithelialization, thereby resulting in persistent wound nonclosure, which may repres

Update of the S1-guideline atypical fibroxanthoma (AFX) and pleomorphic dermal sarcoma (PDS).

Atypical fibroxanthoma (AFX) and pleomorphic dermal sarcoma (PDS) are rare cutaneous neoplasms representing histomorphological, genetic as well as epigenetic variants of one disease spectrum. Both tumors manifest as nonspecific, often ulcerated nodules in chronically sun-damaged skin. AFX is a well-demarcated, often rapidly growing tumor. PDS tumors are poorly circumscribed and characterized by a more aggressive infiltrative growth pattern. Histologically, both are composed of spindle-shaped and epithelioid tumor cells with pleomorphic nuclei as well as atypical multinucleated tumor giant cell

Persistent CCL3 expression sustains neutrophil recruitment and contributes to delayed wound healing in advanced age.

BACKGROUND: Delayed wound healing in advanced age is associated with prolonged inflammation, but the mechanisms that impair inflammatory resolution in this context remain poorly defined. Neutrophils are essential for early repair, but their persis tence is predicted to disrupt resolution and delay healing. RESULTS: Here we show that aged mice exhibit impaired wound closure accompanied by a sustained neutrophil infiltrate from days 7-14 post-injury, long after neutrophils have resolved in young wounds. In aged skin, macrophage abundance and neutrophil-macrophage interactions were reduced, cons

Upadacitinib in refractory acquired reactive perforating collagenosis with atopic dermatitis and chronic kidney disease: a case report.

BACKGROUND: Acquired reactive perforating collagenosis (ARPC) is a rare perforating dermatosis characterized by transepidermal elimination of degenerated collagen fibers. It frequently develops secondary to systemic comorbidities, including diabetes mellitus, chronic kidney disease (CKD), and atopic dermatitis (AD), which limit therapeutic options and contribute to unpredictable outcomes. Here, we report a case of ARPC with concurrent AD in a patient with type 2 diabetes mellitus and stage 4 CKD, who experienced acute paradoxical exacerbation within 24 h of dupilumab initiation and achieved su

Microbial diversity in leg ulcer and skin in Sickle Cell disease.

INTRODUCTION: Leg ulcers are a common, painful, and difficult-to-treat complication of sickle cell disease due to their prolonged healing process. The microbial composition of these wounds remains poorly understood. This study investigated the microbiota diversity in sickle cell disease leg ulcers and associations with laboratory parameters. METHODS: Samples were collected from leg ulcers, intact skin adjacent to the ulcers, and intact skin from healthy controls. Microbiome profiling was performed by sequencing the V4 region of the 16S ribosomal RNA (rRNA) gene. The laboratory results of pati

Microneedles for Chronic Refractory Wounds: A Review of Dynamic Regulation Strategies and Therapeutic Applications.

Chronic refractory wounds (CRWs), characterized by delayed healing due to disrupted phases of hemostasis, inflammation, proliferation, and remodeling, pose significant clinical challenges with diverse etiologies such as diabetic ulcers and radiation-induced ulcers. Traditional therapies like debridement and skin flap transplantation face limitations. Microneedle technology has emerged as a promising approach, enabling painless transdermal drug delivery to the dermis while enhancing bioavailability and enabling microenvironment-responsive interventions. In this context, this review highlights a

Spontaneous hypopigmentation mutant in threespine stickleback fish exhibits changes in the gut microbiome consistent with inflammatory bowel diseases.

Albinism is a large group of disorders characterized by a loss of pigment in ectoderm-derived tissues - most noticeably impacting the skin, hair, and eyes. Albinism is categorized as either syndromic or non-syndromic. Syndromic albinisms include a wide range of disorders with mutations impacting not just pigmentation but other cellular functions. These include, non-exclusively, melanogenesis disorders, endosome trafficking disorders like lysosomal storage disorders, and cell migration disorders. Mutations leading to syndromic albinisms can cause multi-organ system symptoms due to broad disrupt

Targeting calciphylaxis: the emergence of vitamin K as a potential treatment.

Calciphylaxis, or calcific uraemic arteriolopathy, is a rare, life-threatening vascular disorder predominantly affecting patients with advanced chronic kidney disease (CKD), especially those on dialysis. It is characterized by arteriolar calcification, thrombosis, and painful ischemic skin lesions that frequently ulcerate, contributing to high morbidity and mortality. Calciphylaxis arises from medial arterial calcification, endothelial injury, and a pro-inflammatory, pro-thrombotic milieu. Disruption of vitamin K-dependent pathways, particularly impaired γ-carboxylation of matrix Gla protein (

Recommendations for microbiological sampling in infected hard-to-heal wounds: an Italian association for cutaneous ulcers (AIUC) perspective.

Chronic and hard-to-heal wounds represent a significant and growing healthcare burden, frequently complicated by infection and impaired healing. Despite the availability of international guidelines, microbiological assessment in clinical practice remains highly heterogeneous, with substantial variability in sampling techniques, timing, and interpretation of results. This variability occurs within a broader One Health context, where antimicrobial resistance and microbial ecology are shaped by interconnected human, environmental, and healthcare factors. The present recommendations, developed by

Mind the Gap: A survey of chronic wound datasets, methods, and clinical translation.

Chronic wounds, including diabetic foot ulcers, venous ulcers, arterial ulcers and pressure ulcers, represent a growing global health challenge with significant morbidity, mortality, and economic burden. Advancements in medical image analysis have shown promise in automating wound detection, segmentation, and measurement. However, translation of these research advances into clinical practice through mobile applications remains limited. This survey presents systematic analysis of the relationship between chronic wound datasets, deep learning methods, and mobile wound assessment applications. We

Stereotactic Body Radiation Therapy for Management of Locally Advanced Periorbital Tumors: Efficacy and Toxicity Profile.

BACKGROUND: The management of locally advanced periorbital skin tumors with orbital invasion is challenging. Orbital exenteration historically provides reliable local control but causes major functional and cosmetic morbidity. We evaluated stereotactic body radiation therapy (SBRT) as an eye-sparing alternative, focusing on local control, vision preservation, and toxicity. METHODS: This retrospective series includes 26 consecutive patients with locally advanced periorbital tumors treated prospectively with SBRT. Tumors involved the canthi (zones III-IV) in 20 patients, with multi-zone involve

The overlooked surface: cutaneous manifestations of inflammatory bowel disease.

Inflammatory bowel disease (IBD) is characterized by a chronic, idiopathic inflammation of the intestinal tract. The primary types include ulcerative colitis (UC), which affects the colon and rectum with continuous mucosal and submucosal lesions solely and Crohn's disease (CD), which may involve any segment of the gastrointestinal tract with discontinuous lesions. Among extraintestinal manifestations (EIMs), the skin is most frequently involved. Erythema nodosum (EN) and pyoderma gangrenosum (PG) are identified as the most prevalent cutaneous presentations. This review explores the underlying

Systemic Sclerosis Presenting With Long-Standing Peripheral Neuropathy and Delayed Multisystem Manifestations: A Case Report.

Systemic sclerosis (SSc) is a rare autoimmune connective tissue disorder characterized by immune dysregulation, microvascular injury, and progressive fibrosis involving the skin and internal organs. Although Raynaud phenomenon and cutaneous sclerosis are the hallmark manifestations, atypical presentations may significantly delay diagnosis. Peripheral neuropathy is an uncommon neurological manifestation and rarely precedes the diagnosis of SSc by several years. We report the case of a 62-year-old female who presented with a 20-year history of progressive distal paresthesia involving both upper

Regression of severe sclerotic chronic graft-versus-host disease after a second haploidentical hematopoietic stem cell transplantation: a case report.

Sclerotic chronic graft-versus-host disease (Scl-cGVHD) is a disabling fibrotic manifestation of cGVHD and frequently exhibits poor responsiveness to conventional immunosuppressive therapy. Therapeutic options remained limited, particularly in pediatric patients. An 8-year-old boy with relapsed acute myeloid leukemia underwent matched sibling donor allogeneic hematopoietic stem cell transplantation (allo-HSCT) in January 2021. Four months later, he developed progressive cutaneous cGVHD that evolved into severe Scl-cGVHD, with diffuse skin thickening, painful fissures, ulceration, and multiple

A Rare Case of Pyogenic Granuloma on the Lip in a 10-Year-Old Girl: Case Report and Literature Review.

Oral pyogenic granulomas (PGs) are well documented in the literature and are a commonly encountered lesion in routine clinical practice. However, their extragingival occurrence is rare and often has an unusual presentation. The clinical diagnosis of uncommon extragingival PG can be quite challenging. In the present case report, a 10-year-old female presented with a soft, tender swelling of 2 × 1.5 centimetres, which was pink, smooth-surfaced, sessile, and prone to bleeding and was provisionally diagnosed as PG. The excision was performed using a minimally invasive treatment modality of 980-nm
