Homoeopathic Management of a Lower Leg Ulcer with Cellulitis: An Evidence-Based Case Report.

BACKGROUND: Cellulitis is an acute bacterial infection involving the dermis and subcutaneous tissue, characterized by erythema, warmth, swelling, and pain. It commonly follows disruption of skin integrity and may progress to ulceration. Leg ulcers are chronic or acute breaches in skin continuity of the lower limb, frequently associated with infection, inflammation, delayed healing, and significant morbidity. When cellulitis complicates a leg ulcer, the risk of tissue damage and prolonged recovery increases. Although antibiotics constitute standard management, integrative approaches are increas

The clinical and cost analysis of laser therapy for healing hard-to-heal leg ulcers: a self-control service evaluation study.

OBJECTIVE: Hard-to-heal (chronic) leg ulcers (LUs) can impair quality of life and, in severe cases, may require amputation. Although there are a few studies on the use of class III laser therapy with LUs, studies using powerful class IV lasers are scarce. The present study uses a class IV therapeutic laser (K-Cube 4; K-Laser; K-Laser USA, LLC, US) which uses four wavelengths (660nm, 800nm, 905nm and 970nm) of diodes in one device to target haemoglobin, water and cytochrome c oxidase simultaneously, with different frequency modulation and high power levels to enhance clinical efficacy and produ

Stromal-Derived Factor-1 (SDF-1/CXCL12) and Skin Wound Healing Research at the Intersection Between Regenerative Biology and Medicine.

The history of stromal-derived factor-1 (SDF-1), alias CXCL12, started serendipitously and relatively late in the cytokine cDNA cloning era (1975-2000) and evolved at the biological level from progenitor cell-specific chemokine in the bone marrow to multifunctional cytokine with growth factor-like and tissue-regenerative activities. This evolution was parallelled by the integration of SDF-1/CXCL12 within the protein families of chemokines, cytokines and cell growth-promoting recombinant products having the potential for clinical applications. Here, we use this central position of CXCL12 as sma

Microneedles for the treatment of migraine and orofacial pain: A narrative review.

BACKGROUND: Migraine is a common neurological disorder and a primary headache condition. Despite its central origin, migraine pain may be referred to the orofacial region via trigeminal pathways, resulting in phenotypic overlap with other orofacial pain (OFP) conditions. OFP represents a highly prevalent and heterogeneous group of pain disorders that substantially impair quality of life. Microneedle (MN) technology enables minimally invasive, localized, or transdermal drug delivery and has emerged as a promising mechanism-guided strategy for the management of migraine and other OFP conditions.

A 43-Year-Old Man With Isolated Cutaneous Lymphomatoid Granulomatosis Presenting as a Chronic Necrotizing Ulcer of the Upper Arm.

BACKGROUND We report a 43-year-old man with ulcerative lesions of the right arm without pulmonary symptoms that was diagnosed as cutaneous lymphomatoid granulomatosis, a rare Epstein Barr virus-associated angiocentric and angiodestructive lymphoproliferative disorder that most commonly involves the lungs. Cutaneous involvement usually occurs in association with pulmonary disease, while isolated cutaneous disease is rare and most often presents with papules, nodules, or plaques. Necrotizing ulcers are uncommon and can closely mimic other dermatologic conditions. CASE REPORT A 43-year-old man pr

Primary subcutaneous inguinal malignant melanoma mimicking a soft tissue sarcoma.

INTRODUCTION AND IMPORTANCE: Malignant melanoma typically arises from cutaneous or mucosal melanocytes and is often recognized through visible pigmentation changes. Rarely, melanoma may present as a deep or subcutaneous soft tissue mass without epidermal involvement, thereby mimicking sarcomas or other mesenchymal tumors. Such atypical presentations are exceedingly rare and may delay diagnosis and treatment. CASE PRESENTATION: A 66-year-old woman with well-controlled hypertension presented with a painless, mobile, left inguinal mass that had been present for approximately 10 years but recentl

Diabetic Foot Infections: A Toolkit for Wound Care Clinicians.

Diabetic foot infections (DFIs) are a serious complication of diabetes, leading to high rates of hospital admission, amputation, and early death-particularly in persons living in resource-limited communities. Although the updated International Working Group on the Diabetic Foot guidelines in 2024 provide well-laid-out strategies for managing DFIs, these tend to be better suited for use in well-resourced centers. This case-based review discusses and applies these guidelines for an approach to DFI in real-world scenarios. It focuses on supporting health care providers working in community and re

The function and clinical application potential of stem cell-derived exosomes in skin wound healing: A review of recent research advances.

The healing of skin wounds, particularly chronic wounds such as diabetic ulcers, presents significant clinical challenges, including inadequate infection control, imbalanced inflammatory regulation, and insufficient angiogenesis with traditional therapies. When these complications arise, effective treatment strategies are urgently needed. Stem cell-derived exosomes (SC-Exos; referred to herein as exogenously applied SC-Exos unless stated otherwise) are nanoscale vesicles containing bioactive components such as proteins and nucleic acids, with low immunogenicity and high biocompatibility. A Triple-Layer Amniotic Membrane Dressing Drives Robust Wound Healing: In-Depth Protein Profiling and In Vivo Validation in Rat and Human Subjects.

In chronic ulcers, early intervention to obtain wound closure is a critical requirement for optimal reparative success. Human dried amniotic membrane (HDAM) has been widely used as a wound covering due to three characteristics: its content of highly enriched extracellular matrix (ECM) and bioactive cytokines, its long shelf life, and its suitable biomechanical properties adequate for handling in the dried state. The ECM composition can be enhanced by the creation of a sandwich composed of three distinct layers of the amniotic membrane without creating any detrimental manufacturing shortcomings

Renal-Limited Cryofibrinogen-Associated Glomerulonephritis Diagnosed Using Electron Microscopy.

Cryofibrinogen-associated glomerulonephritis (CryoFiGN) is a rare, diagnostically challenging cause of nephrotic syndrome, and its clinical presentation can vary considerably. Herein, we report the case of a 76-year-old man who presented with nephrotic syndrome; biopsy showed a C3-dominant membranoproliferative pattern of injury. Electron microscopy revealed organised subendothelial microtubular deposits (40-80 nm), confirming CryoFiGN, despite inconclusive results on mass spectrometry. The patient progressed to end-stage kidney disease and later developed systemic vasculitis with skin ulcers

Beyond rejection: engineering immune responses to redefine skin graft survival.

Skin grafting remains a fundamental reconstructive technique in reconstructive and dermatologic surgery for restoring skin integrity following burns, trauma, chronic ulcers, and excision of malignancies. However, a graft survival rate is determined by multiple factors and by a delicate balance between inflammatory and reparative immune system processes. Controlled inflammation promotes wound healing while excessive or prolonged immune activation has the potential to result in cytotoxicity and complement activation which will eventually lead to graft rejection. Early rejection is driven by anti

Synergistic mechanochemical regulation in PNIPAM/tannic acid hydrogels for enhanced diabetic wound healing.

Skin, the body's primary protective barrier, faces formidable repair hurdles in chronic wounds like diabetic ulcers. Hyperglycemia-induced oxidative stress accumulates reactive oxygen species (ROS), inhibiting keratinocyte and fibroblast migration to delay re-epithelialization and granulation tissue formation; impaired myofibroblast function further weakens skin contraction, collectively hindering wound closure. To address these limitations, a multifunctional hydrogel (PGA-TA) with integrated antioxidant and thermoresponsive contraction properties was developed. In this study, N-isopropylacryl
