Lifestyle strategies and nutraceuticals in chronic venous disease: an evidence-weighted synthesis and clinical positioning.

BACKGROUND AND AIMS: Chronic venous disease (CVD), including chronic venous insufficiency and venous leg ulcers (VLU), is common and substantially impairs quality of life. Compression is the cornerstone of conservative care, but lifestyle measures and selected nutraceuticals may provide additional benefit. This review aims to synthesize and clinically position evidence on adiposity, weight management, diet-related factors, and venoactive nutraceuticals across CVD phenotypes. METHODS: Evidence from epidemiological studies, clinical cohorts, randomized trials, and meta-analyses was summarized u

Smart Dressings for Pre-Ulcer Risk Surveillance and Closed-Loop Chronic Wound Management: Advances, Discrepancies, and Translational Horizons.

Chronic ulcers, including diabetic foot ulcers, pressure ulcers, and venous leg ulcers, remain a major clinical challenge due to their progressive tissue deterioration and high recurrence rates. Current preventive strategies largely rely on intermittent assessment and often fail to detect early pathological changes before visible tissue damage occurs. Recent advances in flexible electronics and wearable biointerfaces have enabled smart dressings capable of continuous physiological surveillance. However, existing studies and reviews mainly focus on individual sensing technologies, while lacking

In vivo biomechanical and morphological alterations in the Achilles tendon, ankle joint, and ankle plantar flexor muscles in individuals with diabetes mellitus without neuropathy.

BACKGROUND: Chronic hyperglycemia accelerates the accumulation of advanced glycation end-products and collagen cross-linking, which may impair collagen remodeling, alter tissue morphology, and increase muscle-tendon stiffness. These changes may restrict ankle mobility and elevate forefoot plantar loading, increasing susceptibility to skin breakdown and ulcers in individuals with diabetes. Because many muscle-tendon impairments in those with diabetes are attributed to neuropathy, whether these changes are present before clinically detectable neuropathy remains unclear. This study investigated b

Abnormally low expression of tumor necrosis factor stimulated gene 6 (TSG-6) in obese type 2 diabetic mice contributes to altered inflammation and delayed cutaneous wound healing.

Type 2 diabetes mellitus (T2DM) is a chronic disorder associated with obesity and hyperglycemia. T2DM is linked to significant clinical complications such as delayed wound healing and diabetic foot ulcers. Tumor necrosis factor-stimulated gene 6 (TSG-6) is a protein with multiple functions including regulation of cytokine binding to glycosaminoglycans in extracellular matrices and on vascular endothelial walls. Loss of TSG-6 in knockout mice increases skin inflammation and delays wound healing. Here, in both a genetic model (db/db mice) and a dietary model that simulates human T2DM (mice treat

Multifunctional pH-responsive HA-c-FZ1 hydrogel presents a promising therapeutic strategy for diabetic skin wounds.

BACKGROUND: Diabetic foot ulcers remain a major clinical challenge, driven by the convergent effects of microbial infection, chronic inflammation, oxidative stress, and inadequate neovascularization that collectively disrupt tissue repair. Although the integrin αvβ3 agonist peptide FZ1 exhibits promising pro-healing activity, limited delivery control and insufficient functional versatility have restricted its clinical translation. To overcome these constraints, a pH-responsive hyaluronic acid (HA)-based hydrogel, hyaluronic acid conjugated with FZ1 peptide (HA-c-FZ1), was developed to enable i

Refractory Adult-Onset Localized Cutaneous Langerhans Cell Histiocytosis Successfully Treated with Surgical Intervention: Case Report.

Langerhans cell histiocytosis (LCH) is a rare clonal proliferative disorder that frequently mimics common inflammatory dermatoses, often resulting in delayed diagnosis. We report a highly deceptive clinical presentation. A 36-year-old man presented with a five-year history of painful, refractory ulcerated plaques on his bilateral groins. Previous empirical therapies failed completely. A targeted skin biopsy eventually revealed classic atypical Langerhans cells with reniform nuclei, demonstrating striking immunohistochemical positivity for CD1a and CD207. Comprehensive systemic investigations c

Marjolin's ulcer developing in a post-traumatic scar: management with negative pressure wound therapy and split-thickness skin grafting.

Marjolin's ulcer is a rare but aggressive malignant transformation arising in chronic wounds or scars. We report the case of a man in his 60s who developed a large, non-healing ulcer at the site of a previous traumatic injury and reconstructive surgery performed three decades earlier. Clinical suspicion of malignancy led to wide local excision with sentinel lymph node biopsy, followed by staged reconstruction. Negative pressure wound therapy (NPWT) was used to optimise the wound bed, which was complicated by a local infection requiring targeted antibiotic therapy. Subsequent split-thickness sk

Clinical improvement of pemphigus-like mucocutaneous disease in a patient with CLL following initiation of zanubrutinib: a case report.

Pemphigus is an autoimmune bullous disease. PNP/PAMS represents a rare subtype of autoimmune bullous disease, and typically associated with an underlying neoplasm, particularly lymphoproliferative disorders. Current management of PNP primarily involves glucocorticoids, immunosuppressants, and rituximab. This article reports a 67-year-old male who presented with generalized erythema, blisters, and oral erosions with pain for one month. Initially diagnosed with pemphigus, he received methylprednisolone treatment, which led to slight improvement in skin lesions but minimal resolution of oral ulce

Extracellular vesicles in wound healing and scar formation: molecular regulation of macrophages, fibroblasts, and their crosstalk.

Extracellular vesicles (EVs) mediate communication between immune and stromal cells during cutaneous repair by transferring proteins, lipids, nucleic acids, and other bioactive components. This review examines their effects on macrophage inflammatory and stress-adaptation programs, fibroblast activation and fate, and macrophage-fibroblast crosstalk in wound healing and pathological scarring. In chronic and metabolically impaired wounds, stem- and stromal-cell EVs commonly reduce persistent inflammation, improve macrophage autophagy and redox balance, and support fibroblast migration, prolifera

Diagnostic value of neutrophil-to-lymphocyte ratio for identifying severe great saphenous vein reflux in patients with chronic venous insufficiency: A retrospective cohort study.

ObjectivesThe role of inflammatory markers in chronic venous insufficiency (CVI) has been examined primarily within the CEAP clinical classification framework or at a disease-present versus disease-absent level. CEAP, however, is weighted toward symptoms and skin findings and does not directly reflect the underlying hemodynamic severity of reflux. This study evaluated the diagnostic accuracy and optimal cutoff values of five preoperative inflammatory markers in differentiating severe (Grade IV; reflux time ≥4 s) from moderate (Grade III; reflux time >1 to <4 s) great saphenous vein reflux thro

Beyond the parasite: skin-intrinsic cytolytic circuits drive disease in cutaneous leishmaniasis and reveal targets for host-directed therapy.

SUMMARYCutaneous leishmaniasis (CL) is a globally prevalent parasitic disease in which tissue pathology often arises primarily from immune-mediated damage rather than uncontrolled parasite replication. Despite effective Th1 responses and the production of IFN-γ, lesions often fail to resolve, highlighting an uncoupling of protective immunity from disease pathogenesis. This review synthesizes evidence from experimental models and human studies demonstrating that skin-intrinsic cytolytic inflammatory circuits-rather than parasite burden-are dominant drivers of lesion persistence and treatment fa

Persistent Hyperprocalcitoninemia as the Initial Diagnostic Clue to Sporadic Medullary Thyroid Carcinoma.

UNLABELLED: Medullary thyroid carcinoma (MTC) is a rare neuroendocrine tumour arising from parafollicular C cells, and serum calcitonin remains its most reliable biochemical marker. Procalcitonin (PCT), widely used in the evaluation of bacterial infections, is usually undetectable under non-infectious conditions. Persistent elevation of PCT in the absence of clinical or microbiological evidence of infection is uncommon and may represent an early clue to underlying endocrine malignancy. We report the case of a 48-year-old woman who presented with markedly elevated PCT levels (>20 ng/ml) detecte

Advances in Wound Healing Strategies for Diabetic Foot Ulcers: From Conventional to Regenerative Therapies.

Diabetic Foot Ulcer (DFU) is a serious and chronic complication of uncontrolled diabetes, significantly increasing the risk of amputation and mortality in diabetic patients. Conventional wound healing techniques often fail to manage chronic wounds in DFU due to several limitations, which eventually raises the need for advanced, innovative, and efficient treatment strategies for the better management of DFU. Many novel interventions, such as growth factor therapy, stem cell therapy, gene therapy, nanotechnology, bioengineered skin substitutes, 3D-bioprinting, and regenerative medicine, have sho

Optimizing deferoxamine delivery through the skin for pressure ulcers.

INTRODUCTION: Deferoxamine (DFO) is an iron-chelator, approved for systemic treatment of iron overload. New research finds local applications to mechanistically correct ischemia-driven hypoxia that underlies chronic wound pathology resulting in therapeutic angiogenesis. Development of composite hydrogels and hybrid biomaterials that combine natural and synthetic polymers for enhanced mechanical integrity, antimicrobial function, and controlled drug release confirms that the field is moving toward multifunctional, bio-responsive wound therapies. Incorporating deferoxamine via reverse-micelle te
