Clinicopathologic Features of Penile Ulceration in Malignant Atrophic Papulosis: A Case Report and Review of the Literature.

Degos disease, or malignant atrophic papulosis (MAP), is a rare vasculopathic disorder that commonly involves the skin, gastrointestinal tract, and central nervous system. Diagnosis is made through recognition of characteristic histopathologic features of cutaneous lesions. Here, we report a 58-year-old male who initially presented with a penile eschar exhibiting vascular dilation, hyalinization of superficial dermal vessels, and hemosiderin deposition. Later, he developed scattered erythematous papules with central porcelain-white scarring; biopsies of these lesions exhibited features histopa

15 Years of Intermittent Therapy With Hydroxychloroquine Without Any Loss of Efficacy in Reticular Erythematous Mucinosis.

Reticular erythematous mucinosis (REM) is a rare dermatological condition characterized by erythematous, reticulated patches and plaques with a slightly infiltrated appearance. REM is classified among cutaneous mucinoses, which are characterized by the accumulation of mucin in the subcutaneous tissues, leading to the formation of characteristic reticulated patches on the skin. The pathogenesis is still debated, but the association with sun exposure seems to play an important role. We present the case of a 54-year-old individual with a history of recurrent erythema on the chest and back. The pa

Folliculosebaceous Cystic Hamartoma With Unusual Mucinosis: A Report of a Rare Case.

We report a case of a clinically indistinct pink papule on the midline upper back of a 77-year-old woman. Histopathological examination of the lesion revealed a unique variant of folliculosebaceous cystic hamartoma (FSCH) characterized by mucin deposits in sebaceous lobules. FSCH is a rare lesion involving a cystically dilated follicle surrounded by sebaceous glands embedded within a stromal matrix composed of mesenchymal features. Follicular mucinosis is an uncommon inflammatory disorder typically presenting as benign, idiopathic lesions in children or mycosis fungoides-associated lesions in

[Myxedema psychosis, a rare complication of untreated hypothyroidism].

Myxedema psychosis, also known as myxedema madness, is a rare neuropsychiatric manifestation of untreated or inadequately treated hypothyroidism. We report the case of a 46-year-old woman presenting to the emergency department with acute psychiatric symptoms, including confusion, hallucinations, and delusions. Physical examination revealed classic signs of hypothyroidism, such as periorbital edema, dry skin, and diffuse hair loss. Laboratory results showed a markedly elevated thyroid-stimulating hormone (TSH) level of 45.76 mU/L and a severely reduced free thyroxine (T4) level (< 1.0 pmol/L),

Acral Persistent Papular Mucinosis in the United States: Case Series and Literature Review.

BACKGROUND: Acral persistent papular mucinosis (APPM) is a localized variant of lichen myxedematosus (LM) characterized by asymptomatic, flesh-colored papules primarily distributed on the hands and forearms. This chronic dermatosis, distinct from generalized mucinosis due to its lack of systemic involvement, remains underreported in medical literature. OBJECTIVE: In this study, we present two cases of APPM to the limited pool of documented cases in the United States, highlighting its emerging recognition. METHODS: This is a case series of two patients presenting with asymptomatic papular eru

Primary cutaneous mucinoses in connective tissue diseases as a diagnostic and prognostic challenge.

Mucin deposition in the skin, a hallmark feature in dermatopathology, is frequently associated with connective tissue diseases such as lupus erythematosus, dermatomyositis, and, to a lesser extent, scleroderma. Although mucin deposition commonly accompanies specific skin lesions in lupus erythematosus, dermatomyositis, and sometimes scleroderma, its occurrence as a distinct clinical manifestation with defining histopathologic features is rare. Such presentations that are classified as primary cutaneous mucinoses not only aid in diagnosis but may also have prognostic implications. This study sy

Hyalase in Dermatology: Applications Beyond Filler Management.

BACKGROUND: Hyaluronic acid (HA) is a key extracellular matrix component in the skin. Hyaluronidase is an enzyme that breaks down HA into monosaccharides. The FDA has approved this enzyme for hypodermoclysis, drug absorption enhancement, and subcutaneous urography. The most common application of this enzyme is the dissolution of HA fillers. AIM: This study aimed to explore the various applications of hyaluronidase beyond its common medical uses. Moreover, we illustrate the mechanism by which this enzyme is helpful in conditions such as drug extravasation, scleroderma, scleromyxedema, and kelo

Livedo Reticularis With Perineural and Intraneural Mucin Deposition in a Child With AT-III Deficiency: A Case Suggestive of Neuropathia Mucinosa Cutanea?

INTRODUCTION: Cutaneous mucinoses comprise a heterogeneous group of disorders characterized by abnormal mucin deposition. Neuropathia mucinosa cutanea (NMC) is an exceedingly rare entity, defined by hypertrophic dermal nerves containing mucin within and around the perineurium. Its pathogenesis and nosologic status remain uncertain. CASE REPORT: We report the case of a 15-year-old Romanian girl with severe antithrombin III (AT-III) deficiency and systemic thromboses, who presented with acute ischemic stroke and later developed livedo reticularis and acral discoloration. Skin biopsy of a toe re

Sclerosing diseases of the skin.

Sclerosing skin diseases comprise a group of distinct dermatological conditions characterized by fibrotic changes that may severely impair patients' quality of life. These conditions often present with cutaneous manifestations and, in some cases, may extend to extracutaneous tissues, potentially resulting in significant morbidity and mortality. This review explores the current understanding of the four most prevalent sclerosing skin diseases - localized scleroderma (morphea), systemic sclerosis (SSc), scleromyxedema, and scleredema adultorum of Buschke - as well as sclerotic conditions induced

Hypersensitivity Reaction and a Single-Bag Rapid Desensitization to Idursulfase.

Idursulfase is the first-line and only available enzyme replacement therapy (ERT) for Mucopolysaccharidosis type II (MPS II), or Hunter Syndrome. Deficiency in the lysosomal enzyme iduronate-2-sulfatase leads to progressive skeletal deformities, neurologic deterioration, airway obstruction, and cardiomyopathy. In severe cases, these deformities can lead to death during teenage years (Stapleton et al. 2017). Continuous treatment with ERT is essential to prevent irreversible changes. However, 16 out of 108 (15%) patients had hypersensitivity reactions to idursulfase during clinical trials. Hyper

Scleromyxoedema and Thyroid Disease: A Case Report.

Scleromyxoedema (SM) is a rare cutaneous mucinosis characterised by waxy papules, diffuse skin induration, and systemic involvement such as gastrointestinal, cardiopulmonary, or neurological complications. The disease is strongly linked to the presence of monoclonal gammopathy. Traditionally, thyroid dysfunction is considered an exclusion criterion for the diagnosis of SM; however, clinical observations suggest that this association may occur in certain cases. We present the case of a 70-year-old woman with a history of hypothyroidism on levothyroxine for three months, who presented with progr

Atypical papular mucinosis with thyroid cancer: comparison of isotretinoin and erbium:YAG laser treatment.

Papular mucinosis (PM), or lichen myxoedematosus, is a rare skin disorder characterized by mucin deposition in the dermis, resulting in papules and nodules. It can present as scleromyxoedema, often with systemic complications, or localized PM, which typically follows a benign course. Atypical cases may present with unique features. We present an atypical case of PM in a 23-year-old woman with concurrent Hashimoto thyroiditis and papillary thyroid carcinoma. The patient presented with numerous progressive papules on her chest and abdomen, alongside systemic symptoms, including fatigue and weigh

Unusual Cutaneous Manifestations in a Patient with a History of Hepatitis B: A Case of Scleromyxedema and Literature Review.

Scleromyxedema (SM) is a rare primary cutaneous mucinosis characterized by systemic papules and scleroderma-like manifestations, often associated with monoclonal gammopathy. We present the case of a 37-year-old male with SM who developed yellowish plaques on the neck and back over three years. Histopathological examination revealed mucin deposition, fibroblast proliferation, and fibrosis, supporting the diagnosis. Serum protein electrophoresis identified IgG-λ type paraproteinemia. Notably, the patient had a history of hepatitis B virus (HBV) infection, suggesting a potential association. Foll

Nodular-Type Lichen Myxedematosus: A Rare Variant With Exclusive Involvement of the Hands-Report of Two Cases With a Comprehensive Review of the Literature.

Lichen myxedematosus (LM) is an uncommon cutaneous mucinosis characterized by the deposition of mucin and fibroblast proliferation in the dermis. This condition can be classified into 2 forms: a diffuse/generalized LM, also known as scleromyxedema, associated with monoclonal gammopathy and systemic implications, and a localized form, primarily affecting the skin. Within the localized form, nodular-type LM is a rare variant presenting as firm, skin-colored to pinkish mucinous nodules. In this article, we report 2 new cases of nodular-type LM with exclusive involvement of the hands and provide a
