Mechanisms of immune escape and extramedullary tropism in leukemia cutis.

The mechanisms that lead to extramedullary tropism of acute myeloid leukemia (eAML) remain obscure and no specific therapeutic approaches for this entity exist. Because the long-term survival of eAML is poor, a deeper understanding of the immune microenvironment and leukemia phenotypes underlying this entity is warranted. Here, we performed bulk and single-cell transcriptome profiling of 23 eAML biopsies from 10 patients with isolated extramedullary disease in skin and subcutaneous tissue. Unlike normal healthy skin, we found leukemia cutis to be heavily immune infiltrated; in extramedullary r

Iatrogenic Dextrose Hyperosmolar Extravasation Injury in Suspected Endogenous Hyperinsulinism.

Hyperosmolar dextrose infused through peripheral venous access can extravasate into surrounding soft tissue and produce local injury. We present the case of a 44-year-old woman with a history of seizure disorder and hyperlipidemia who underwent anterior lumbar interbody fusion (ALIF). On the day of admission, she experienced a witnessed generalized seizure on the floor that terminated following administration of intravenous lorazepam. Point-of-care glucose obtained during the event was 39 mg/dL and corrected with intravenous dextrose. She was transferred to the intensive care unit (ICU), where

Epithelioid Angiosarcoma Presenting With Bilateral Hemothorax and Hemoptysis: A Case Report.

We report a case of a 77-year-old man presenting with bilateral hemothorax, hemoptysis, and multiple bilateral pulmonary nodules and lung infiltrates. Histopathological and immunohistochemical analysis of a surgically resected nodule revealed epithelioid angiosarcoma, and the overall radiological and operative findings favored pulmonary origin. No other primary sites outside the thorax could be identified. The patient was planned for palliative chemotherapy, but his condition deteriorated, and he died before treatment could be initiated. Angiosarcoma is a rare and highly aggressive endothelial

Pleomorphic Dermal Sarcoma With Rare Metastasis to the Colon.

A 76-year-old male presented with a firm ulcerated plaque on his right forehead. Initial superficial biopsy at an outside practice found an atypical dermal spindle proliferation suggestive of atypical fibroxanthoma (AFX). The superficial nature of the specimen limited subcutaneous evaluation. The patient was referred for Mohs micrographic surgery, and the specimen underwent pathologic evaluation. This specimen again revealed an atypical dermal spindle proliferation diffusely positive for CD10 and negative for CD34, Desmin, p63, pancytokeratin, and SOX10, now with infiltration into the subcutan

Cervical Spine Involvement in Morphea Patients With Subclinical Neurologic Signs.

Morphea is a rare skin disease that involves the fibrosis of subcutaneous tissues. Several subtypes of morphea, such as linear and deep variants, may affect musculoskeletal structures and produce neurologic manifestations. However, morphea is differentiated from systemic scleroderma by the absence of internal organ damage. Although the prognosis of a morphea diagnosis is generally favorable, systemic involvement can be severe. However, early detection may improve disease management. The relationship between morphea and its involvement in the cervical spine remains incompletely characterized. A

Late breast cancer recurrence concurrent with silicone breast implant rupture 17 years after mastectomy and immediate implant-based reconstruction in a 53-year-old woman: a case report.

BACKGROUND: Late locoregional breast cancer recurrence and silicone implant rupture can produce similar cutaneous and periprosthetic abnormalities. However, positron emission tomography-computed tomography (PET/CT) may mask the true malignant tumor due to implant-related inflammation, resulting in false-negative results. Cases where breast cancer recurrence and silicone implant rupture coexist have rarely been reported. We report histologically confirmed recurrence with silicone implant rupture 17 years after mastectomy and immediate implant-based reconstruction for ductal carcinoma in situ (D

Eosinophilic Fasciitis in Children: A Case Report.

Eosinophilic fasciitis (EF), also known as Shulman's disease, is a rare inflammatory and fibrosing disorder characterized by inflammation and eosinophilic infiltration of the fascia, leading to skin thickening and induration. It typically presents with limb edema progressing to induration and sclerosis and is frequently, but not invariably, associated with peripheral blood eosinophilia. Diagnosis relies on a combination of clinical, laboratory, imaging, and histopathological findings. We report the case of a 16-year-old female patient with a documented history of hypereosinophilic syndrome, mo

Uncovering the Cause of Pneumonia: Reactivation of HHV-6B in an Immunocompromised Patient.

UNLABELLED: Human herpesvirus 6 (HHV-6) is a ubiquitous virus that commonly infects the paediatric population and establishes latency in the host following primary infection. As in other herpesviruses, HHV-6 may reactivate when the immune system becomes severely compromised, such as after chemotherapy or haematopoietic cell transplantation. We report a case of an 83-year-old male, previously diagnosed with multiple myeloma and treated with lenalidomide plus bortezomib, who was admitted to the internal medicine department with bilateral pulmonary thromboembolism. During his stay, he developed s

Orbital atypical lymphocytic panniculitis preceding conjunctival marginal zone lymphoma in the same patient with undifferentiated connective tissue disease: a case-based review.

The association between autoimmune diseases and lymphoproliferative disorders is well established. It is also recognized that the skin and deep subcutaneous tissue can be affected by lymphocytic infiltrates in autoimmune diseases, a hallmark of lupus erythematosus profundus or lupus panniculitis (LP). To present an unusual lymphoproliferative disorder in a patient with undifferentiated connective tissue disease (UCTD). A 60-year-old female was diagnosed in 2019 with UCTD. A Case of Cutaneous T-cell Lymphoma: The Importance of a Dermatology Consultation.

Mycosis fungoides (MF), the predominant variant of cutaneous T-cell lymphoma (CTCL), typically manifests as pruritic patches, plaques, and tumors, often mimicking cellulitis. Misdiagnosis of cellulitis has led to substantial avoidable healthcare costs, unnecessary hospitalizations, increased risk of nosocomial infections, and antibiotic resistance. We report a case of cellulitis misdiagnosis in a patient with a history of MF. A 68-year-old male referred from plastic surgery due to suspected worsening of left lower leg cellulitis despite recent antibiotic treatment presented to the hospital. Wh

Screening and transcriptomic analysis of anti-Sporothrix globosa targeting AbaA.

INTRODUCTION: Sporotrichosis is a fungal disease caused by a complex of Sporothrix schenckii, leading to chronic infections of the epidermis and subcutaneous tissue in both humans and animals. METHODS: Through virtual screening targeting the key gene abaA to screen out the small-molecule drugs to treat Sporotrichosis. To further validate the antifungal activity of small-molecule drugs, growth curves, minimum bactericidal concentration (MBC), and minimum inhibitory concentration (MIC) for Sporothrix globosa (S. globosa) and Sporothrix schenckii (S. schenckii) were measured. In addition, we hav

Hidradenitis Suppurativa Mimicking Tuberculosis Scrofuloderma: The Role of Dermatologic Ultrasound as a Diagnostic Tool.

Hidradenitis suppurativa (HS) is a chronic inflammatory disease of the apocrine glands, typically presenting with cycles of flare-ups and periods of improvement. It predominantly affects women and is commonly found in areas such as the axillae, inguinal, and inframammary regions. We present a case of a 25-year-old male from Brazil who presented with recurrent cervical lesions over two years, initially suspected to be skin tuberculosis (scrofuloderma). Diagnostic workup included polymerase chain reaction (PCR), tuberculin skin test (TST), chest X-ray, and histological analysis, all of which rul

Wnt/β-Catenin-Activated Nonpilomatrical Carcinoma of the Skin: A Case Series.

Among skin epithelial tumors, recurrent mutations in the APC/CTNNB1 genes resulting in activation of the Wnt/β-catenin pathway have been reported predominantly in neoplasms with matrical differentiation. In the present study, we describe the morphologic, immunohistochemical, and genetic features of 16 primary cutaneous carcinomas harboring mutations activating the Wnt/β-catenin pathway without evidence of matrical differentiation, as well as 4 combined tumors in which a similar Wnt/β-catenin-activated carcinoma component was associated with Merkel cell carcinoma (MCC) or pilomatrical carcinoma

Clinical analysis of 10 cases with subcutaneous panniculitis-like T-cell lymphoma and tissue AURKA expression.

Источник пометил публикацию как отозванную

BACKGROUND: Due to its rarity, subcutaneous panniculitis-like T-cell lymphoma (SPTCL) is often misdiagnosed as benign panniculitis, and there are no standardized treatment guidelines for SPTCL. Aurora kinase A (AURKA) plays a regulatory role in both mitosis and meiosis. Cells treated with an AURKA inhibitor showed severe mitotic delay, which triggered apoptosis. MATERIALS AND METHODS: Ten cases of SPTCL were collected in this study, and immunohistochemistry was performed to detect AURKA expression in the skin tissues of these cases. Control groups were set as follows: 1) 10 cases of inflammat

Indurated Plaques on the Legs: Think Lymphoma.

Dear Editor, Primary cutaneous diffuse large B-cell lymphoma, leg-type (PCDLBCL-LT) is a rare and aggressive neoplasm. A timely diagnosis may prevent fatal outcomes; physicians should take this entity into consideration when assessing non-specific lesions on the lower limbs. We present a 69-year-old woman with a 1-month history of a firm plaque on her left leg. Physical examination revealed an asymptomatic, indurated, smooth, and erythematous plaque on the pretibial region of her left extremity (Figure 1, a). The rest of the physical examination was normal.
