Proteomic dysregulation associated with cerebral amyloid angiopathy.

INTRODUCTION: Cerebral amyloid angiopathy (CAA) is characterized by impaired vascular amyloid-β clearance, distinct from Alzheimer's disease (AD), underscoring the need to clarify its molecular basis for predicting therapy-related amyloid-related imaging abnormalities. METHODS: We analyzed post-mortem brain proteomics from Religious Orders Study and Rush Memory and Aging Project (ROS/MAP) and cerebrospinal fluid (CSF) proteomics from Alzheimer's Disease Neuroimaging Initiative (ADNI) to identify CAA-associated proteins and network modules, delineate their biological processes and cellular ori

Isolated jejunal amyloidosis presenting as acute mechanical small bowel obstruction: a rare diagnostic challenge - a case report.

INTRODUCTION: Gastrointestinal (GI) involvement in amyloidosis is uncommon and usually occurs as part of systemic disease. Localized jejunal amyloidosis is exceptionally rare and may mimic neoplastic or inflammatory disorders because of its nonspecific presentation. CASE PRESENTATION: A 77-year-old man with no significant medical history presented with acute abdominal pain, distension, and bilious vomiting. Contrast-enhanced CT demonstrated jejunal obstruction associated with circumferential wall thickening and multiple intraluminal polypoid lesions. Enteric magnetic resonance imaging confirm

A budget impact analysis for the addition of rare disease genes to secondary findings screening during genome-wide sequencing.

PURPOSE: To estimate the potential budget impact to payers of returning secondary findings (SFs) for rare diseases identified through genome-wide sequencing (GWS) in the United States (US) and Canada. METHODS: A population-based model was used to estimate the number of patients that would be identified in a given jurisdiction in Canada or the US for the 11 diseases on the ACMG-SFv3.2 list classified as "inborn errors of metabolism" or "miscellaneous phenotypes", as well as the budget impact of treating those patients over 5 years. Uncertainty was assessed using one-way sensitivity and scenari

Concurrent fibrinogen Aα-chain amyloidosis and AA amyloidosis in a Humboldt penguin (Spheniscus humboldti).

Amyloidosis is a disease characterized by the deposition of abnormally folded proteins as amyloid fibrils in tissues and occurs in various mammals and birds. In some cases, amyloids derived from two different precursor proteins may coexist in the same individual, a condition known as double amyloidosis. Amyloid A (AA) amyloidosis is known to occur frequently in birds; however, little is known about other types of amyloidosis. In the present study, we report a case of amyloidosis in a Humboldt penguin involving two distinct types of amyloids. Histopathological examination, transmission electron

Interventricular septal thickness for cardiomyopathic phenotype enrichment in patients with established left ventricular hypertrophy.

We evaluated whether interventricular septal thickness in diastole (IVSd) ≥ 15 mm enriches cardiomyopathic phenotypes among adults with established left ventricular hypertrophy (LVH) and whether broader clinical-echocardiographic features improve triage. We studied 1,127 adults undergoing echocardiography from 2017 to 2019 who met established LVH criteria (sex-specific left ventricular mass index elevation and both IVSd and posterior wall thickness ≥ 13 mm). The primary phenotype was hypertrophic cardiomyopathy, Fabry disease, or amyloidosis. The composite phenotype occurred in 108 patients (9

Childhood onset, indolent progression, and risk of hepatic rupture: the natural history of hereditary lysozyme amyloidosis.

BACKGROUND: Lysozyme amyloidosis (ALys) is a rare systemic amyloidosis caused by pathogenic variants in LYZ. We describe the natural history and outcomes for this disease in the largest cohort yet published. METHODS: All patients diagnosed with ALys at the National Amyloidosis Centre between 1988 and 2025 were included. Diagnosis was established by immunohistochemistry or proteomics with the identification of a variant in LYZ. Trends in data were analysed longitudinally. RESULTS: Thirty-five patients were included (median age: 45 years). Twenty-five (71%) carried the Asp67His (p.Asp85His) va

Quantitative Proteomics of Plasma-Derived Extracellular Vesicles Reveals a Plasminogen-Centered Protein Network in Transthyretin Cardiac Amyloidosis.

BACKGROUND: The prognosis of patients with transthyretin cardiac amyloidosis (ATTR) remains poor; this is primarily due to late diagnosis, when irreversible damage has already occurred. Circulating levels of transthyretin or other plasma markers have little value for diagnosis. Although extracellular vesicles (EVs) (key mediators of intercellular communication) may reflect disease-specific molecular changes, their protein cargo has not yet been explored in the context of ATTR cardiomyopathy (ATTR-CM). OBJECTIVES: We evaluated whether cardiac amyloidosis alters the proteome of circulating EVs

Unilateral vitreous amyloidosis as the presenting feature of hereditary transthyretin amyloidosis (TTR p.Val50Met): a case report and focused literature review.

BACKGROUND: Hereditary transthyretin amyloidosis (ATTRv) is an autosomal dominant systemic disorder characterized by extracellular amyloid deposition. Ocular manifestations typically present bilaterally, and unilateral vitreous amyloidosis without systemic symptoms or family history is rare. Few reports have documented the laterality pattern of vitreous amyloidosis or its role as an early indicator of systemic disease. CASE PRESENTATION: We present a 67-year-old Chinese man who experienced progressive left eye vision loss over 1 year. Dense vitreous opacities in the left eye initially mimicke

Demographic Representation in Clinical Trials for Transthyretin-Associated Cardiac Amyloidosis: A Systematic Review.

BACKGROUND: Underrepresentation of minority groups in clinical trials exacerbates health disparities and limits the generalizability of findings. Transthyretin amyloid cardiomyopathy (ATTR-CM) is a condition that disproportionately affects certain racial and ethnic groups, yet the extent to which trial enrollment reflects disease burden remains unclear. Misalignment between disease prevalence and trial representation may delay treatment development and increase the economic burden associated with late diagnosis and suboptimal management. To evaluate racial and ethnic representation in complete

Targeting protein misfolding and aggregation in neurodegenerative diseases: challenges for the identification and validation of targets for small molecules.

INTRODUCTION: Neurodegenerative diseases such as Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis (ALS), and related proteinopathies are characterized by the misfolding, aggregation, and accumulation of specific proteins that disrupt neuronal homeostasis, leading to neuronal death. Despite major advances in structural biology and molecular neuroscience, identifying and validating druggable targets in neurodegenerative diseases remains challenging. Many involved proteins are intrinsically disordered, exhibit conformational heterogeneity, and form highly dynamic

Rapid therapeutic response in refractory lichen amyloidosis treated with upadacitinib: a case report.

Lichen amyloidosis (LA) is an intractable, chronic pruritic skin disorder characterized by amyloid deposition within the dermal papillae. It manifests as severely itchy, hyperpigmented papules and cobblestone plaques mainly distributed on the extremities. Conventional treatment modalities often yield partial or temporary therapeutic effects with unstable efficacy. This paper describes the case of a 60-year-old Chinese male patient presenting with a 3-year history of progressive, refractory lichen amyloidosis involving bilateral limbs. Over a 2-year period, the patient had failed multiple succe

Clinical manifestations and cardiac abnormalities in systemic Amyloid A Amyloidosis.

BACKGROUND: Cardiac involvement in systemic amyloid A (AA) amyloidosis is traditionally considered rare. Respectively, contemporary data describing cardiac characteristics of patients with systemic AA amyloidosis are scarce. METHODS: All patients with a diagnosis of systemic amyloidosis were identified from electronic medical records of a tertiary medical center. Patients with non-AA amyloidosis, concomitant multiple myeloma, lack of histological confirmation, or absence of cardiac evaluation were excluded. Probable cardiac involvement was diagnosed based on predefined criteria. RESULTS: The

Lower mitochondrial DNA copy number in ATTRV50M Swedish carriers.

BACKGROUND: Hereditary transthyretin (TTR) amyloidosis caused by TTR p.Val50Met (ATTRV50M) is a progressive systemic disease caused by misfolding of the protein transthyretin (TTR). The conversion of TTR into an amyloidogenic state has recently been found to be strongly influenced by a non-native disulfide bond, thus also a redox environment. Variation in mitochondrial DNA (mtDNA) copy number serves as an indirect marker of redox imbalance and mitochondrial dysfunction. MtDNA is moreover strictly maternally inherited, supporting previous findings where maternal inheritance of the pathogenic va
