Systemic Spiroplasma turonicum Infection Manifested as Knee Arthritis, France, 2025.

We report on an immunocompromised patient in southeast France who sought care for fever, knee arthritis, and skin erythema. Blood and joint samples were positive for Spiroplasma turonicum, an intracellular gram-negative bacterium generally found in plants, insects, and ticks. Clinicians should be aware of possible S. turonicum infection in immunocompromised patients.

Development of Novel Antimicrobial Peptides and Their Engineering into Nanoparticles to Improve Performance.

Antimicrobial peptides (AMPs) are promising alternatives to antibiotics, but discovering potent, low-toxicity candidates and improving their delivery remain challenging. In this study, novel AMPs were identified by constructing and screening a synthetic random peptide library using a bacterial surface display system. Rational design generated derivative peptides, among which WP-4 and WP-6 showed high antimicrobial activity, good biocompatibility, rapid bactericidal effects, and low propensity for resistance development. WP-6 also showed good in vivo therapeutic potency in a murine Escherichia

Serine protease inhibitor A3 is a soluble SIRPα ligand that suppresses inflammation and antitumor immunity.

SIRPα is an inhibitory receptor with the canonical ligand CD47. Here, we show that human serine protease inhibitor A3 (SERPINA3) and its mouse ortholog SERPINA3k function as soluble binders and activators of SIRPα, suppressing SIRPα+ macrophages, neutrophils, dendritic cells and natural killer cells. SERPINA3(k) treatment alleviates lipopolysaccharide-induced pneumonia and ameliorates experimental autoimmune encephalomyelitis and rheumatoid arthritis. Although wild-type SERPINA3 provokes inflammation in cardiomyocytes and brain vascular endothelial cells, the SERPINA3D2 mutant, identified thro

Developing a cps-genotyping scheme for the emerging zoonotic pathogen Streptococcus parasuis through genetic analysis of capsular polysaccharide synthesis gene loci.

BACKGROUND: Streptococcus parasuis is an emerging zoonotic pathogen responsible for pneumonia, meningitis, peritonitis, pleuritis, nephritis, cellulitis, cholecystitis, sepsis, and arthritis in humans. The continuous increase in reported human cases of S. parasuis infection indicates its growing threat to public health and highlights the urgent need to monitor its epidemiology. The lack of high-throughput typing tools has seriously impeded the epidemiological surveillance of S. parasuis. Serotyping based on antigenic differences in capsular polysaccharides (CPSs) synthesized by cps loci is wid

Description of Corynebacterium nasorum sp. nov. and Corynebacterium hallucis sp. nov. isolated from human nasal passages and skin.

Strains of two novel Corynebacterium species were cultured from samples of human nostrils and skin collected in the USA and Botswana. These strains demonstrated growth on Columbia Colistin-Nalidixic Acid agar with 5% sheep blood and in liquid media (brain-heart infusion and tryptic soy broth) supplemented with Tween 80, a source of the fatty acid oleic acid. Cells were Gram-positive, non-spore-forming, non-motile bacilli that showed catalase but not oxidase activity. Major fatty acids in both of these species were C18 : 1 ω9c (oleic acid), C16 : 0 (palmitic acid) and C18 : 0 (stearic acid). An

Cell DiffErential Expression by Pooling (CellDEEP) highlights issues in differential gene expression in scRNA-seq.

Accurate identification of differentially expressed genes (DEGs) in single-cell RNA sequencing data remains challenging. Single-cell-specific statistical models often report large numbers of candidate genes but can exhibit inflated false positive rates, whereas pseudobulk approaches improve false discovery control at the cost of reduced sensitivity. To overcome the noise and bias that other tools have, and allow the user to have more control of the DEG process, we present CellDEEP, which uses a cell aggregation (metacell) approach. This tool provides a framework for flexible selection of pooli

Global, regional and national burden of pertussis in 1990-2023: a Global Burden of Disease study.

Despite long-standing immunization programs, pertussis remains a major cause of vaccine-preventable mortality, particularly among infants. Here we present a primary assessment of global pertussis incidence, mortality and disability-adjusted life years across 204 countries and territories from 1990 to 2023. We show that despite short-term pandemic-period fluctuations, the dominant global pattern from 1990 to 2023 was long-term decline: age-standardized mortality decreased by 61.2% and age-standardized incidence by 41.6%. Relative to the anomalously low pandemic-period estimates, incidence and m

Disseminated Ureaplasma urealyticum infection presenting as systemic inflammatory syndrome in ocrelizumab-treated patients with multiple sclerosis: a case series.

BACKGROUND: Ocrelizumab, an anti-CD20 monoclonal antibody, is widely used to treat multiple sclerosis (MS), inducing profound B-cell depletion and disease remission. However, it may rarely lead to opportunistic infections. We report two patients with MS on ocrelizumab who developed unique complications of anti-CD20 antibody therapy. One patient developed fulminant endophthalmitis, a novel complication that has not been previously reported. CASE PRESENTATION: A 38-year-old female of South Asian descent and a 46-year-old White female, both with MS on ocrelizumab, developed disseminated Ureaplas

Neurological manifestations and vasculopathy in PLCG2-associated antibody deficiency and immune dysregulation (APLAID): a case series.

BACKGROUND: Autosomal dominant pathogenic variants in PLCG2 are associated with autoinflammation and PLCG2-associated antibody deficiency and immune dysregulation (APLAID), characterized by immunological abnormalities and multi-system inflammation. Descriptions of associated neurological manifestations and vascular anomalies are limited. We describe three patients with APLAID, vasculopathies, neuroinflammation, and cerebral vascular tortuosity. These cases highlight underrecognized and clinically important manifestations of APLAID. CASE PRESENTATIONS: All three patients had de novo variants i

Septic Arthritis of the Knee Caused by Neisseria meningitidis Serogroup Y.

Neisseria meningitidis is a notifiable pathogen in Japan when isolated from any sterile site. We describe a rare case of primary meningococcal arthritis without meningitis or bacteremia. A 48-year-old man presented with acute septic arthritis of his knee. A synovial fluid analysis using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry revealed gram-negative diplococci, identified as N. meningitidis. Further antimicrobial susceptibility testing showed the isolate was pan-susceptible, allowing de-escalation from ceftriaxone to ampicillin therapy. The patient recovered

Evaluation of the BioFire joint infection panel in Taiwan: Impact of regional non-target pathogens and joint type on its performance.

OBJECTIVES: The BIOFIRE® Joint Infection (BJI) panel, a rapid multiplex polymerase chain reaction assay, is used to diagnose joint infections. Here we used it to evaluate the clinical performance and impact of antimicrobial resistance (AMR) detection in Taiwan's unique microbial landscape. METHODS: Adult patients with suspected septic arthritis or prosthetic joint infection were prospectively enrolled from four hospitals in southern Taiwan. Synovial fluid specimens were tested using a BJI panel and routine bacterial culturing. Performance was assessed by calculating the positive percent agree

Burden of Five Musculoskeletal Disorders in ASEAN Countries, 1990-2023: A Global Burden of Disease Study 2023 Analysis.

Background/Objectives: Five musculoskeletal disorders (MSDs), including gout, low back pain, neck pain, osteoarthritis, and rheumatoid arthritis represent a growing public health burden; however, evidence from the Association of Southeast Asian Nations (ASEAN) remains limited. This study aimed to assess the burden of these five MSDs across ten ASEAN countries. Methods: Using data from the Global Burden of Disease Study 2023, we estimated the burden using prevalence and disability-adjusted life years (DALYs). National trends from 1990 to 2023 were assessed using average annual percent change (A

Septic arthritis attributed to a spider bite - a case report and review of the literature.

Individuals who travel to tropical or subtropical areas are exposed to various infectious, environ-mental, and climatic risk factors, as well as traffic accidents, security problems, natural disasters, and direct contact with potentially dangerous plants or animals. Accidental bites by venomous spiders during long journeys are always dangerous to health and life, but it is not usually recognized that nonvenomous spiders also pose such a substantial threat to the lives of local inhabitants and travelers. Here, we present an unusual case of a 41-year-old male patient who was admitted to the Univ

Disseminated Tuberculosis With Ocular Involvement After 12 Years of Anti-TNF-α Therapy.

BACKGROUND: Ocular tuberculosis (TB) is an uncommon yet potentially vision-threatening manifestation of Mycobacterium tuberculosis, particularly difficult to diagnose in immunocompromised individuals. The increasing use of anti-tumor necrosis factor-α (anti-TNF-α) agents has markedly heightened the risk of TB reactivation, often with disseminated or extrapulmonary involvement despite negative baseline screening. CASE PRESENTATION: We describe a 52-year-old woman with rheumatoid arthritis receiving long-term adalimumab and methotrexate who developed disseminated TB with pulmonary, splenic, and

Etanercept-Induced Cutaneous Necrotizing Vasculitis in a Patient With Rheumatoid Arthritis: A Case Report and Literature Review.

Tumor necrosis factor-alpha (TNF-α) inhibitors are effective therapies for rheumatoid arthritis (RA) but may rarely induce paradoxical autoimmune complications, including vasculitis. We report the case of a 53-year-old woman with seropositive RA and secondary Sjögren's syndrome who developed bilateral dependent petechial and purpuric lesions of the lower extremities after approximately 33 months of etanercept therapy at a dose of 25 mg twice weekly. At symptom onset, her RA was well controlled, with no clinical or ultrasonographic synovitis and a Disease Activity Score in 28 joints of 2.1. Ski

Adult-Onset Still Disease Triggered by Recent Cytomegalovirus Infection: A Diagnosis of Exclusion and a Review of the Literature.

Adult-onset Still disease (AOSD) is a rare systemic autoinflammatory disorder characterized by quotidian fever, inflammatory arthritis, evanescent rash, neutrophilic leukocytosis, and hyperferritinemia. Because no pathognomonic diagnostic test exists, diagnosis relies on characteristic clinical features and exclusion of infectious, malignant, and autoimmune disorders. Viral infections, including cytomegalovirus (CMV), have been proposed as potential immunologic triggers in genetically susceptible individuals, although causality remains unproven. We report a 47-year-old Hispanic woman with hype

Glucocorticoid regulation of the human noncoding genome.

Glucocorticoids (GCs) are widely used anti-inflammatory and immunosuppressive agents known to induce substantial changes in gene expression, yet their effects on noncoding RNAs such as long noncoding RNAs (lncRNAs) or microRNAs (miRNAs) remain poorly characterized. Here, we present the first comprehensive and systematic analysis of GC regulation of the noncoding genome across nine human primary cell types from both hematopoietic and nonhematopoietic lineages. In vitro GC treatment was applied to each cell type, with transcriptomic profiling (total RNA-seq and small RNA-seq) at 2 and 6 h posttr
