A narrative review of our developing knowledge about paediatric spondylodiscitis based on existing literature.

Paediatric spondylodiscitis (SD) is an infection involving the vertebral bodies and adjacent intervertebral discs in populations below 16 years old. It is rarely confined to a single compartment; more often, it spreads to other areas in the spinal column and may cause other infections, such as epidural abscesses, subdural abscesses, septic arthritis of the facet joints, paravertebral abscesses and even meningitis. SD predominantly affects children from 6 to 48 months old, and the lumbar spine seems to be most at risk; no specific markers are currently available for a biological diagnosis of SD

Clinical characteristics, pathogenesis, treatment, and prognosis of bone and joint infections in neonates: a single-center report of 20 cases.

BACKGROUND: Bone and joint infections (BJIs) in neonates are rare but serious conditions associated with significant orthopedic complications. Diagnosis is challenging due to nonspecific clinical manifestations, often leading to delayed treatment and poor outcomes. This study aimed to analyze the clinical characteristics, management, and outcomes of neonates with BJIs to improve early diagnosis and guide clinical decision-making. METHODS: We performed a single-center, retrospective review of 20 neonates (aged ≤ 35 days) hospitalized with BJIs between 2017 and 2025. Data on demographics, clinic

Never Too Old for Arthroscopic Surgery: Woman with Low Virulence Pathogenic Shoulder Osteomyelitis - A Case Report.

The incidence of advanced osteomyelitis following arthroscopic shoulder surgery is rare, with few documented cases. As the need for shoulder interventions in elderly populations increases, managing these complex infections poses a significant surgical challenge. This report details the case of an 85-year-old woman who presented with fever and a fistulating portal site infection of the right shoulder. Diagnostic workup confirmed methicillin-resistant Staphylococcus aureus (MRSA). She was successfully treated with arthroscopic irrigation, debridement, and synovectomy, followed by a three-month c

[Infectious sacroiliitis during pregnancy and the postpartum period: A case report and literature review].

INTRODUCTION: Infectious sacroiliitis is a rare condition, accounting for 1-2% of septic arthritis cases. Its risk factors remain poorly defined, although several studies suggest an association with pregnancy and the postpartum period. METHODS: Based on a case of postpartum infectious sacroiliitis, we conducted a literature review to describe the clinical, microbiological, and therapeutic features of this entity. RESULTS: A systematic search, without date or language restrictions, identified 36 articles published between 1933 and 2024. The median age of patients was 26 years [24-32], and the

Antimicrobial and immune-modulating effects of recombinant pancreatitis-associated protein in the treatment of MRSA-induced severe septic arthritis.

Septic arthritis is a severe infection caused by bacteria directly invading the synovial joints. It leads to inflammatory damage to articular cartilage and may result in significant functional impairment, with a high recurrence rate. The rapid spread of antibiotic-resistant bacterial strains has further exacerbated the threat to human health, particularly in septic arthritis cases. Based on the antibacterial activity and regenerative properties of pancreatitis-associated protein (PAP), the present study aims to investigate the therapeutic potential of exogenous recombinant PAP for treating met

Primary Septic Arthritis of the Hip Caused by Neisseria meningitidis Serogroup W135 in an Infant.

Septic arthritis in children is a medical and surgical emergency, most commonly caused by Staphylococcus aureus. Primary septic arthritis due to Neisseria meningitidis is extremely rare; the absence of meningeal signs often leads to significant diagnostic and therapeutic delay. We report a rare case of primary hip arthritis caused by Neisseria meningitidis serogroup W135 in an infant. A six-month-old infant with no significant medical history presented with fever and right hip pain following a flu-like syndrome. Clinical examination revealed a pseudoparalytic right limb and a fever of 39°C. La

Persistent CD4+ T cell hyporesponsiveness during recovery from prolonged symptomatic SARS-CoV-2 infection.

Symptoms of acute SARS-CoV-2 infection often resolve quickly but are sometimes associated with persistent immune dysfunction. The factors that predispose individuals to compromised immune function have not been well defined. We investigated CD4+ T cell phenotype and function in a small cohort of individuals who recovered from mild to moderate SARS-CoV-2 infection without hospitalization and were divided into short or prolonged symptom duration groups. Five individuals with prolonged symptom duration showed marked downregulation of CD4 on CD3+CD8- T cells (CD4low group) and a poor response to T

Septic Arthritis of the Hip in a Young Athlete: A Diagnostic Delay Caused by Cognitive Bias.

Septic arthritis requires urgent diagnosis and treatment; however, acute hip pain in young athletes may be misinterpreted as a sports-related musculoskeletal condition, resulting in delayed recognition. We report a 21-year-old male rugby player who presented with rapidly worsening left hip pain after a match and difficulty bearing weight. Laboratory tests showed elevated inflammatory markers. Plain radiographs were normal, while magnetic resonance imaging demonstrated a hip joint effusion. Fluoroscopy-guided aspiration yielded approximately 11 mL of cloudy yellow synovial fluid, but antibiotic

Shotgun Metagenomics Reveals Skin Microbiome Composition and Function in Infant Atopic Disease.

BACKGROUND: Atopic dermatitis (AD), food sensitization (FS), and food allergy (FA) frequently co-occur in infancy, but the factors underlying distinct atopic phenotypes remain unclear. Although FLG null mutations are major genetic risk factors for AD, they explain only part of disease heritability, suggesting a potential role for the skin microbiome. This study examined how early-life skin microbiome composition and its interaction with host genetics contribute to distinct atopic phenotypes in infancy. METHODS: We analyzed > 1000 skin swabs from 429 infants in the VITALITY cohort using deep s
