ACG Clinical Guideline: Preventive Care for Patients With Cirrhosis.

Cirrhosis affects more than 2 million Americans and drives substantial healthcare utilization, impaired quality of life, and mortality. Because early-stage disease carries a favorable prognosis and decompensated patients can recompensate, systematic delivery of preventive care is essential. Under the auspices of the American College of Gastroenterology Practice Parameters Committee, a writing group of hepatology experts and 2 Grading of Recommendations, Assessment, Development, and Evaluation methodologists developed PICO questions, conducted librarian-assisted searches of PubMed, Embase, and

Association between Plasma Uric Acid Level and Mortality Rate in Children with Sepsis and Acute Kidney Injury.

PURPOSE: Acute kidney injury (AKI) is a common condition in hospitalized patients that can have a significant impact on outcomes, including an increase in overall complications and mortality rates. Criteria such as serum creatinine level, urinary output, and scoring systems such as KDIGO in acute conditions do not have acceptable specificity and sensitivity to evaluate kidney function. Therefore, this study was conducted to determine the relationship between plasma uric acid level and mortality rate in patients with sepsis and acute kidney failure. MATERIALS AND METHODS: In this descriptive-a

Low-Volume Plasma Exchange for Fulminant Hepatic Failure Secondary to Hepatitis A in a Young Adult: A Case Report.

Acute liver failure accounts for a very fatal and severe complication. It is defined as acute liver injury with hepatic encephalopathy (HE) and an elevated international normalized ratio (INR above 1.5). There are multiple factors that can lead to acute liver failure. These different etiologies can vary depending on certain geographical areas where they are more prevalent than other factors in the area. Hepatitis caused by the Hepatitis A virus (HAV) is one of the leading causes of acute liver failure worldwide, especially in developing countries. Artificial Liver Support System (ALSS), on the

Spontaneous Recovery from Hepatitis A-Induced Acute Liver Failure in a Child: A Rare Case Report from Hargeisa Group Hospital, Somaliland.

BACKGROUND: Hepatitis A-induced acute liver failure (ALF) is a rare but leading cause of pediatric ALF in developing countries. ALF is defined as severe acute liver injury with coagulopathy and hepatic encephalopathy occurring within 26 weeks of symptom onset in patients without pre-existing chronic liver disease. It carries high mortality, accounting for approximately 8% of liver transplants worldwide, with reported survival rates ranging from 30-60% in the absence of transplantation. Fewer than 1% of patients with hepatitis A progress to ALF. CASE PRESENTATION: A 5-year-old girl from Wajaal

From hepatitis A to acute liver failure in a resource-limited setting: a case report.

Hepatitis A virus (HAV) infection is usually self-limited, but a minority of cases progress to acute liver failure (ALF), particularly, in resource-limited settings. We report a previously healthy 19-year-old woman who presented with 20 days of jaundice, dark urine, and hepatic encephalopathy. Baseline tests showed total bilirubin 18.3 mg/dL, aspartate aminotransferase 281 U/l, alanine aminotransferase 341 U/l, international normalized ratio (INR) 1.23, and positive anti-HAV immunoglobulin M/immunoglobulin G. Although she did not meet formal ALF criteria at presentation, within 72 hours, she p

Recompensation and Further Decompensation After Index Decompensation in Patients with Autoimmune Hepatitis: A Real-World Study.

BACKGROUND AND AIMS: There are limited data on hepatic recompensation in autoimmune hepatitis (AIH)-related decompensated cirrhosis. We evaluated incidence and predictors of recompensation and further decompensation, and assessed the impact of recompensation on survival. METHODS: In this retrospective analysis of a prospectively maintained database, we included patients with AIH-related decompensated cirrhosis, confirmed by liver biopsy, who were treated with immunosuppression. Recompensation, defined by Baveno VII criteria (including etiological suppression), was analyzed using Fine and Gray

Trends in the Clinical Severity of Pediatric Acute Hepatitis Due to Hepatitis A Virus and Nonhepatitis A Virus Etiologies: A Call for Action.

OBJECTIVE: Hepatitis A virus (HAV) remains a major cause of acute hepatic illness in children in low-and middle-income countries. With changing epidemiology and increasing hospitalization among older children, comparative data distinguishing HAV from other causes of pediatric acute hepatitis remain limited. This study evaluated clinical characteristics, laboratory derangements, disease severity, and outcomes of pediatric acute hepatitis, comparing HAV with non-HAV etiologies and identifying predictors of adverse outcomes. MATERIALS AND METHODS: This retrospective hospital-based study included

Losartan-Associated Sub-fulminant Hepatitis: A Case Report.

Losartan, a commonly prescribed angiotensin II receptor blocker, is generally well-tolerated but can rarely cause severe drug-induced liver injury (DILI), leading to acute liver failure. We report a 43-year-old man with hypertension and type 2 diabetes who developed jaundice, fatigue, and right upper quadrant pain six weeks after starting losartan (50 mg daily). Despite prompt discontinuation of the drug, he progressed to acute liver failure with hepatic encephalopathy, coagulopathy (international normalized ratio (INR): 2.8), and a Model for End-Stage Liver Disease (MELD) score of 28, meeting

Clinical and in-hospital outcomes of alcohol-related pancreatitis in the setting of concomitant alcohol-related hepatitis: A retrospective cohort study.

BACKGROUND: Alcohol misuse is a major cause of acute pancreatitis. When alcohol related hepatitis coexists, the combination of hepatic and pancreatic injury may exacerbate systemic inflammation and organ dysfunction. We evaluated whether concurrent AH influences in hospital outcomes among adults admitted with alcohol induced AP. METHODS: We performed a retrospective cohort study using the NIS database from 2019-2021, identifying adults with alcohol induced AP by ICD 10 codes. Patients were stratified by the presence of concurrent AH. The primary outcome was in hospital mortality. Secondary ou

Active learning for the accelerated discovery of complex concentrated NiCoCr alloys in additive manufacturing.

BACKGROUND: According to the World Health Organization, air pollution is closely associated with climate change and, in particular, with global warming. In addition to melting of ice and snow, rising sea level, and flooding of coastal areas, global warming is leading to a tropicalization of temperate marine ecosystems. Moreover, the effects of air pollution on airway and lung diseases are well documented as reported by the World Allergy Organization. METHODS: Scientific literature was searched for studies investigating the effect of the interaction between air pollution and climate change on

INTRODUCTION: Essential precondition for the development of cervical cancer is a persistent human papillomavirus (HPV) infection. The majority - approximately 70% - of cervical carcinomas is caused by two high-risk HPV types (16 and 18). Recently, two vaccines have been approved to the German market with the potential to induce protection against HPV 16 and HPV 18 among additional low-risk virus types. OBJECTIVES: To analyse whether HPV vaccination is effective with regard to the reduction of cervical cancer and precursors of cervical carcinoma (CIN), respectively? Does HPV vaccination repres

Acute liver failure and liver transplantation.

Sudden and rapid deterioration of hepatic function leading to jaundice, coagulopathy and encephalopathy defines acute liver failure (ALF). Timing of jaundice to encephalopathy classifies ALF into hyperacute, acute and sub-acute liver failure and prognosticates outcome. There exists a significant geographical heterogeneity in the etiology of ALF; paracetamol toxicity is the most common cause in the west, whereas viral hepatitis predominates Asian countries. Hepatitis A is the leading cause for ALF in India. ALF due to yellow phosphorus poisoning is observed commonly in southern India. Clinical

[Acute Liver Failure].

Acute liver failure (ALF) is a severe, potentially reversible form of liver insufficiency, which is defined by the occurrence of hepatic coagulopathy and hepatic encephalopathy in patients with no previous hepatic disease. Acute liver failure is preceded by severe acute liver injury (ALI) with an increase in transaminases, jaundice, and deterioration in general condition over a period of hours to weeks. Every year 200-500 people develop ALF in Germany, most frequently on the background of toxic liver injury (e.g. drug induced liver injury). Other potential causes include viral infections (e.g.

Influence of Obesity Class on Clinical Outcomes in Alcoholic Hepatitis: A National Cohort Study of Mortality, Complications, and Resource Use.

BACKGROUND & AIMS: Alcoholic hepatitis (AH) is a severe manifestation of alcoholic liver disease with high morbidity and mortality. This study used the 2016-2020 National Readmission Database to investigate how obesity influences AH outcomes. METHODS: Adult hospitalizations were categorized as those without obesity, Class 1 obesity (BMI 30-34.9), Class 2 obesity (BMI 35-39.9), or Class 3 obesity (BMI ≥ 40). We compared mortality, complications, and resource utilization across these groups using regression models. RESULTS: Among 82 367 AH admissions, 4.09% had Class 1 obesity, 2.73% had Class

Fatal Liver Failure After Herpes-Simplex-Virus-2-Induced Acute Necrotizing Hepatitis: A Case Report.

Herpes simplex virus (HSV) infections are common in the European population, typically presenting with mucocutaneous and anogenital manifestations. However, disseminated infections and organ involvement are rare, usually occurring in immunocompromised individuals, particularly after hematopoietic stem cell or solid organ transplantation. HSV1/2-induced hepatitis is infrequent but can result in acute liver failure (ALF) and increased mortality. We present a case of fulminant ALF caused by disseminated primary HSV2 infection in a fifty-year-old male with rheumatoid arthritis treated with the JAK

Vibrio cholerae Bacteremia causing severe lactic acidosis and multiorgan failure in alcoholic cirrhosis: A report of a fatal case.

Non-O1, non-O139 Vibrio cholerae (NOVC) infections can cause sepsis in patients with liver cirrhosis. NOVC bacteraemia is a rare condition, with the highest number of cases reported in Asia. Despite increasing recognition of non-O1, Non-O139 Vibrio cholerae as emerging pathogens associated with gastroenteritis, wound infections, and septicaemia, there is limited knowledge on disease patterns, complications, and survival dynamics of the disease. Here, NOVC strains were isolated from an elderly female with a known history of alcoholic cirrhosis, who presented with severe abdominal pain and alter

Thyroid storm with concurrent autoimmune hepatitis: a case report on diagnostic challenges and the role of therapeutic plasma exchange.

SUMMARY: Thyroid storm represents a severe expression of thyrotoxicosis and is commonly associated by multiple organ dysfunction and liver abnormality. Thyrotoxicosis with concurrent autoimmune hepatitis is rare but could confound liver dysfunction, hence complicating diagnosis and subsequent management. We present the case of a 37-year-old woman with thyroid storm complicated by multiple organ failure and resistant hyperthyroidism. Despite initial medical therapy, her condition deteriorated with rising bilirubin and worsening right heart failure. Therapeutic plasma exchange (TPE) was initiate

What's new in the critical care management of acute liver failure: a focus on global accessibility.

INTRODUCTION: Acute liver failure (ALF) is a rare syndrome characterized by acute severe liver dysfunction and hepatic encephalopathy. The etiology of ALF varies according to age and geographical distribution and hence management depends on the etiology and clinical condition. AREAS COVERED: While high-income-countries (HIC) most often see acetaminophen (APAP) and drug-induced liver injury (DILI) as causes of ALF, low-middle-income countries (LMIC) face a higher burden of viral hepatitis (particularly hepatitis A and E virus) and herbal/traditional/toxin medicine-related ALF transplant-free s

Hepatitis A-Induced Fulminant Hepatic Failure Linked to Autoimmune Hepatitis in a Child Amidst Diagnostic Challenges of Dengue and Malaria.

Hepatitis A virus (HAV) typically causes a self-limiting illness in children. Rarely, it can progress to fulminant hepatic failure (FHF), and even less commonly, may be followed by features suggestive of autoimmune hepatitis (AIH). The diagnostic overlap can be particularly challenging in tropical regions, where endemic infections such as dengue and malaria may present with similar clinical features. We report a previously healthy five-year-old girl who presented with a four-day history of high-grade fever, abdominal pain, vomiting and lethargy. Examination revealed pallor, icterus, hepatosple
