Claw lesions in dairy cattle pose a significant risk to dairy farmers worldwide in terms of animal welfare concerns and economic profitability. However, the use of different data sources, classification systems, and definitions of reference groups limits the comparison across herds and decreases the usability of the recordings for phenotypic and genetic analyses. In South Africa, information on claw lesions is not routinely collected by dairy farmers and data are limited to hoof trimmers recording lesions during preventative trimming or as needed by producers. Records of the most common claw l

Staphylococcal infection is as common as streptococcal infection as a cause of infection-related glomerulonephritis (IRGN). It is seen more frequently in the pediatric population and is relatively rare in adults. Glomerular disease manifests as either nephritic or nephrotic syndrome. A high index of suspicion is necessary for diagnosing this condition as it can be mistaken for other common conditions in the elderly. IRGN may present as volume overload and may masquerade as cardiorenal syndrome type 1 in patients with preexisting heart failure, emphasizing the importance of distinguishing betwe

CASE: Septic arthritis is an infection of the joint space that can lead to joint destruction, disability, and death. Mortality risk is increased by old age, comorbidities such as decreased kidney function, and involvement of multiple joints. We report a rare case of bilateral septic arthritis of the wrists in a 101-year-old woman who recovered without complications.
CONCLUSION: This case illustrates the necessity of monitoring pain in other joints in cases of monoarticular septic arthritis to rule out polyarticular involvement to prevent long-term complications, morbidity, and mortality.

OBJECTIVE: The human microbiota is implicated in the development and progression of rheumatoid arthritis (RA). Given the increased RA burden in women, and well-known correlations between the vaginal microbiota and local inflammation, we seek to understand the vaginal microenvironment in the context of RA pathology.
METHODS: Self-collected vaginal swabs and questionnaires on dietary and health practices were obtained from 36 RA and 50 demographically-matched control women, 18-63 years of age. Additionally, medication regimen and disease activity and severity were captured for the RA cohort. Va

Acute rheumatic fever (ARF) is an abnormal immunologic response to group A streptococcus (GAS) infections, most commonly tonsillopharyngitis. While ARF most often occurs in children, it is rare in adults. Carditis is a recognized manifestation of ARF, though it is uncommon in adult presentations. We present a case of a 35-year-old male with fever, migratory polyarthritis, and subclinical carditis in the form of a prolonged PR interval, mild aortic regurgitation (AR), and trace mitral regurgitation (MR) detected on echocardiography. Further investigations revealed an erythrocyte sedimentation r

Group A Streptococcus (Streptococcus pyogenes) infections are increasing globally and can be invasive despite immunocompetence. Septic arthritis requires an early diagnosis and prompt treatment. Because of its high specificity, Gram staining of the synovial fluid is crucial to early diagnoses; however, its sensitivity is low, and around-the-clock laboratory testing is often impractical. We report a case of oligoarticular septic arthritis in an immunocompetent man in his 60s. Point-of-care Gram staining of the synovial fluid revealed chains of Gram-positive cocci. An invasive streptococcal infe

UNLABELLED: Migratory arthritis is classically associated with acute rheumatic fever (ARF), but may signify other medical conditions. We aimed to track the final diagnoses of this obscure presentation, and to distinguish clinical variables that differentiate ARF from other conditions in order to facilitate the clinical diagnosis of ARF. Medical records were reviewed of all the patients aged 0-18 years who presented with migratory arthritis during 2015-2023 at the emergency department, pediatric wards, and the rheumatology unit of a large tertiary medical center. Primary diagnoses, demographic,

INTRODUCTION: Septic arthritis of large joints is associated with high morbidity and mortality, necessitating prompt empiric antibiotic therapy tailored to local and current pathogen patterns. The need for antimicrobial stewardship further highlights the importance of updated data on pathogens and resistance trends to guide effective empiric treatment.
METHODS: This study retrospectively investigated the distribution of causative pathogens and their antimicrobial resistance profiles in septic arthritis of native knee and shoulder joints from January 2007 to December 2024 at a tertiary hospita

Group B Streptococcus (GBS) is a well-recognized pathogen in neonates and older adults but is rare in adolescents, particularly as a cause of musculoskeletal infection. We describe the case of a previously healthy 16-year-old female patient with GBS sacroiliac septic arthritis and osteomyelitis presenting with progressive hip and back pain complicated by acute urinary incontinence. Initial evaluation was confounded by nonspecific symptoms, under-recognized inflammatory markers, and incidental spinal imaging findings, resulting in diagnostic delay. Blood cultures ultimately identified GBS, and

Background:Streptococcus pneumoniae is a well-known pathogen responsible for respiratory and invasive diseases; however, central nervous system (CNS) involvement in the form of bacterial myelitis is exceedingly rare, particularly in immunocompetent adults. Moreover, the association between pneumococcal infections and reactive arthritis is scarcely documented. We report an unusual case of pneumococcal myelitis complicated by reactive arthritis in an elderly patient with no evident immunosuppression. Case Presentation: A 68-year-old man with a medical history of hypertension, benign prostatic hy

Background Invasive pneumococcal infections, such as bacteremia, remain associated with substantial morbidity and mortality, particularly among individuals with underlying health conditions. The primary objective of this study was to evaluate the association between patient comorbidities, particularly chronic cardiac disease, and the development of complicated pneumococcal bacteremia. Secondary objectives included assessing vaccine coverage and clinical outcomes among high-risk patients. Methods We retrospectively reviewed medical records of patients admitted to SSM Health and St. Louis Univer

Streptococcus pneumoniae (SP) is a common pathogen responsible for a broad spectrum of infections and the leading cause of community-acquired pneumonia; however, invasive and life-threatening disease is rare in immunocompetent individuals. We describe the case of a 60-year-old male who presented with a constellation of neurological and systemic symptoms involving the respiratory, nervous, and musculoskeletal systems, and was subsequently diagnosed with invasive pneumococcal disease (IPD). Despite the severity of his illness, the patient made a remarkable recovery following prompt initiation of

Streptococcus suis is a major bacterial pathogen in the swine industry, causing meningitis, arthritis, and other diseases in infected pigs. It also poses significant public health risks due to its zoonotic potential, particularly in individuals with skin lesions. Current detection methods, including traditional culture-based techniques and PCR assays, are time-consuming, labor-intensive, and lack sufficient accuracy. To address these limitations, this study aimed to develop a rapid and precise detection method for S. suis. By leveraging whole-genome sequencing (WGS) and multiple sequence align

BACKGROUND: Streptococcus suis is a zoonotic pathogen that causes meningitis, septicaemia, endocarditis, arthritis, and pneumonia in human beings. With the increasing prevalence of S. suis infections and a general decline in the effectiveness of antibiotics, the development of novel drugs that have effect on S. suis is extremely urgent. Emodin, a natural anthraquinone derivative of Rheum palmatum L., Reynoutria japonica Houtt., Polygonum multiflorum Thunb. and Cassia obtusifolia L., has been reported to exert anti-S. suis effect; however, the specific mechanism of the anti-S. suis action by ta

BACKGROUND: Streptococcus suis is an emerging pathogen causing invasive zoonotic infections in humans. Antimicrobial resistance (AMR) to penicillin, macrolides, and tetracyclines has emerged in S. suis-infected swine. AMR among zoonotic S. suis strains causes a critical concern for human infection and antimicrobial treatment options. Thus, the study aims to delineate the clinical characteristics and to explore the changing pattern of the antimicrobial susceptibility (AST) of S. suis infection in humans.
METHODS: We conducted a chart review of adult patients with culture-confirmed S. suis infe

INTRODUCTION: Osteoarthritis (OA) is the most prevalent form of arthritis and affects over 528 million people worldwide. Degenerative joint disease involves cartilage degradation, subchondral bone remodeling, and synovial inflammation, leading to chronic pain, stiffness, and impaired joint function. Initially regarded as a "wear and tear" condition associated with aging and mechanical stress, OA is now recognized as a multifaceted disease influenced by systemic factors such as metabolic syndrome, obesity, and chronic low-grade inflammation. Recent studies have focused on the gut-joint axis to

The ocular surface microbiome plays a crucial role in maintaining immune homeostasis, and its disruption may contribute to mucosal inflammation and autoimmunity. This pilot exploratory study investigated and compared the ocular surface bacterial microbiome in patients with Stevens-Johnson syndrome (SJS), Sjögren's disease (SjD), and healthy controls using next-generation sequencing (NGS) and correlated these findings with dry eye parameters. Conjunctival swabs were collected from sixteen individuals: ten with SJS, three with SjD, and three healthy controls. Dry eye parameters were employed to

Goose gout, a metabolic disorder characterized by systemic urate deposition in visceral organs (e.g., liver, kidneys) and joints of goslings, is strongly associated with goose astrovirus (GAstV) infection and environmental stressors such as high humidity. This disease severely compromises growth performance and survival rates, yet no licensed prophylactic or therapeuti interventions are currently available. Here, we investigated the efficacy of SIA, a Chinese herbal formula comprising Saposhnikovia divaricata, Isatis indigotica and Astragalus membranaceus against GAstV-induced gout in goslings
