Global, regional, and national burden of tetanus in 204 countries and territories, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Tetanus is a life-threatening, vaccine-preventable disease. Its incidence decreased substantially following global initiatives such as the Expanded Programme on Immunization (EPI) and Maternal and Neonatal Tetanus Elimination (MNTE). Despite these efforts, tetanus elimination has not been achieved, and significant disparities in morbidity and mortality persist across various regions. Therefore, we sought to investigate the global, regional, and national burden of tetanus from 1990 to 2023, using data from the Global Burden of Disease Study (GBD) 2023. METHODS: We estimated the glo

ATR inhibition induces NF-κB-dependent immunogenicity in Merkel cell carcinoma independent of cGAS-STING.

There is a major unmet need to develop therapies for patients whose tumors do not respond to PD-1 blockade. Inhibition of the ATR-mediated DNA replication checkpoint has been reported to augment antitumor immunity, although the underlying mechanisms remain poorly understood, particularly in cGAS-STING-deficient tumors. Here, we investigated the effects of ATR inhibition (ATRi) on tumor immunogenicity in Merkel cell carcinoma (MCC), an aggressive skin cancer commonly driven by Merkel cell polyomavirus (MCPyV). Among 92 primary MCC tumors, Ki-67 positivity was observed in 50% of MCPyV-positive a

Whole-Body MRI of Inflammatory and Infectious Musculoskeletal Diseases: Emphasis on Chronic Nonbacterial Osteomyelitis and Systemic Inflammatory Disorders.

Whole-body magnetic resonance imaging (WBMRI) has emerged as the primary imaging tool for multifocal inflammatory and infectious musculoskeletal diseases. By extending coverage from the vertex to the toes in a single, radiation-free examination, WBMRI enables the detection of clinically occult lesions, disease-specific pattern recognition across bone marrow, joints, entheses, and muscles, and objective monitoring of disease activity and treatment response. This review describes the WBMRI findings-with detailed imaging features and distribution patterns-in chronic nonbacterial osteitis, SAPHO s

Human T-Cell Leukaemia Virus Type 1-Associated Myelopathy in Patients with Rheumatoid Arthritis: A Case Series Highlighting Functional Disability and Diagnostic Challenges.

OBJECTIVES: Although rheumatoid arthritis (RA) and human T-cell leukaemia virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) occasionally coexist, their clinical characteristics and impact on patient outcomes remain poorly elucidated. METHODS: We conducted a retrospective case series of five patients with RA complicated by HAM/TSP (HAM-RA) enrolled in the HTLV-1 Positive Rheumatoid Arthritis Miyazaki Registry. Data on clinical characteristics, disease course, functional status, quality of life and HTLV-1-associated parameters were reviewed. Age- and sex-matched

Successful use of a Janus-kinase (JAK) inhibitor in chronic chikungunya arthritis refractory to conventional synthetic DMARDs: a case report.

Chronic chikungunya arthritis is a debilitating post-viral inflammatory condition that clinically mimics rheumatoid arthritis and often requires long-term treatment with disease-modifying antirheumatic drugs. Treatment options are limited when conventional synthetic disease-modifying antirheumatic drugs are not tolerated or lose efficacy. We report the case of a 50-year-old Brazilian woman with serologically confirmed chikungunya virus infection who developed chronic chikungunya arthritis with a rheumatoid arthritis-like phenotype. Following an acute febrile illness accompanied by rash and neg

FBXL21 regulates diurnal proteostasis in skeletal muscle by targeting DNAJB6 and client proteins.

Circadian regulation of proteostasis, a key determinant of muscle health, remains poorly understood. Here, we identify DNAJB6, an Hsp40 (DnaJ) co-chaperone, as a substrate of the circadian E3 ligase FBXL21. FBXL21 mediates the ubiquitination-dependent proteasomal degradation of both DNAJB6 and its client proteins, including Desmin. In contrast, myopathy-causing mutations of DNAJB6 confer resistance to FBXL21-directed degradation. Fbxl21 KO C2C12 cells display aberrant Desmin accumulation, and show aggravated cytoplasmic accumulation of TDP-43, another DNAJB6 client protein, in response to heat

Combined pneumonia and glandular tularemia following a tick bite in an immunocompromised patient.

Francisella tularensis can cause a broad spectrum of clinical manifestations depending on the route of inoculation. In northeastern France, tick bites are a frequent route of transmission. Typically, vector-borne inoculation results in glandular or ulceroglandular forms, whereas aerosol exposure causes pulmonary forms. We report the case of a 72-year-old immunocompromised woman (rheumatoid arthritis treated with methotrexate) who developed pneumonia six days after a tick bite, diagnosed four weeks later with a glandular tularemia. Although Francisella tularensis was not isolated from the lungs

LongTerm Outcomes of Suspected Transient Synovitis of the Hip in NonAspirated Patients.

BACKGROUND: Transient synovitis is a common cause of pediatric hip pain that must be distinguished from septic arthritis. Although the literature includes outcome data for aspirated hips, long-term outcomes in patients managed presumptively without aspiration remain limited. This study aimed to characterize the long-term clinical outcomes of children with presumed transient synovitis managed without hip aspiration. METHODS: A retrospective study of pediatric patients diagnosed with transient synovitis and managed presumptively without aspiration. Collected variables included demographics, cli

Bilateral Keratouveitis and Secondary Glaucoma in a Young Dog With Immune-Mediated Polyarthritis and Systemic Inflammatory Response Syndrome.

A 7-month-old intact female mixed-breed dog presented emergently for inability to walk, lethargy, and vomiting. The patient was diagnosed with immune-mediated polyarthritis (IMPA) and secondary systemic inflammatory response syndrome (SIRS). Bilateral keratouveitis and secondary glaucoma developed within 24 h of presentation. Thorough diagnostic testing failed to identify an underlying etiology. Despite aggressive medical management, the patient developed rapid vision loss. Due to poor visual prognosis and anticipated need for long-term medical management, both eyes were enucleated. Histopatho

Early features of cellular ageing impair differentiation capacity in human lymph node fibroblasts prior to rheumatoid arthritis onset.

OBJECTIVE: Cellular ageing of the immune system has been suggested to play a role in the pathogenesis of rheumatoid arthritis (RA). RA onset is preceded by a preclinical phase of systemic autoimmunity without overt tissue inflammation, allowing the investigation of predisease cellular dysfunction in at-risk individuals. Lymph nodes (LNs) are crucial for (auto)antibody development, with LN fibroblasts playing a key role in regulating immune homeostasis. Here, we investigated age-associated alterations in LN fibroblast differentiation capacity during the at-risk and established stages of RA. ME

Acute respiratory deterioration in rheumatoid arthritis-associated interstitial lung disease associated with pulmonary Mycobacterium abscessus infection: a case report.

Nontuberculous mycobacteria (NTM) are important pathogens in patients with chronic lung diseases, including rheumatoid arthritis-associated interstitial lung disease (RA-ILD). Although acute exacerbations of ILD associated with NTM infection have been reported sporadically, cases involving pulmonary Mycobacterium abscessus infection remain extremely limited. We describe a fatal case of acute respiratory deterioration in a patient with RA-ILD following pulmonary infection with M. abscessus. A 76-year-old man with longstanding RA-ILD developed pulmonary NTM disease initially caused by Mycobacter

Extraintestinal Reprogramming of Colon-Derived CD4+ Intraepithelial Lymphocytes Reveals a Mechanism Underlying the Gut-Joint Axis.

OBJECTIVE: The gut-joint axis is a central paradigm in spondyloarthritis, yet the cellular mechanisms linking intestinal and joint immunity remain poorly understood. One potential link is TCRαβ+ CD4+ intraepithelial lymphocytes in the colon (cIELs) that can traffic to extraintestinal tissues, including the joint. Within an inflamed joint, trafficked cIELs display both regulatory and inflammatory capacity. We sought to determine whether this functional identity is determined by an impaired epithelium or shaped by the joint environment. METHODS: We combined colonoscopy-guided photoconversion in

Septic arthritis of ankle with granulomatous change in the abscess wall and bone changes in the lower end of tibia by Mycobacterium abscessus- a case report.

Chronic granulomatous infections are frequently diagnosed as tuberculosis, often supported by histopathological findings. Rapidly growing mycobacteria (RGM), a type of non-tuberculous mycobacteria, can also produce granulomatous reactions that resemble tuberculosis. RGMs are resistant to standard antituberculous treatment, and microbiological evidence is crucial for determining optimal therapy. This report presents a case of septic arthritis of the right ankle featuring granulomatous reactions in the abscess wall and bone changes in the lower end of the tibia, initially diagnosed clinically as

Delayed and multisystem immune-related adverse events in MSI-high colon cancer treated with nivolumab plus ipilimumab.

Immune checkpoint inhibitors (ICIs) can cause immune-related adverse events (irAEs) involving multiple organs. However, delayed-onset, multisystem irAEs after treatment discontinuation are uncommon. Here, we present a rare case of a man in his 50s with microsatellite instability-high colon cancer who developed multisystem irAEs following treatment with nivolumab plus ipilimumab. The patient developed adrenocorticotropic hormone deficiency after four cycles of combination therapy, which improved with hydrocortisone supplementation. Following eight cycles of nivolumab monotherapy, treatment was

Arthritis models for studying chikungunya virus joint inflammation.

Chikungunya virus (CHIKV) joint inflammation remains poorly understood, despite its clinical burden. This review surveys in vitro joint models which were originally developed for arthritis research and examine how they can be repurposed to study CHIKV joint pathology. We outline key considerations, limitations, and recommendations for adapting these models for infection studies, including facilitating infection susceptibility across joint models, tuning matrix composition, addressing cellular incompleteness through modeling relevant immune cells, improving culture longevity and reproducibility

Agreement of Multimodal Large Language Models and Novice Readers With Expert-Based Radiographic Scoring of Presumptive Septic Carpal Arthritis in Calves.

Septic arthritis is an important cause of morbidity in calves, and radiographic interpretation may vary between observers, especially when structured scoring systems are used by less experienced readers. This single-center retrospective reader study evaluated whether general-purpose multimodal large language models (LLMs) could support rubric-based radiographic scoring of presumptive septic carpal arthritis in calves. Fifty calves aged 0-3 months with clinical findings consistent with presumptive septic carpal arthritis were included, and one carpal radiograph per case was assessed. Two novice
