Accuracy and operational performance of computer-aided detection software use cases for TB screening: a retrospective comparative evaluation in Ho Chi Minh City, Viet Nam.

BACKGROUND: Human reader variability and workforce constraints limit the quality and scale of chest X-ray (CXR) screening for tuberculosis (TB) in many high-burden settings. Although the WHO endorses computer-aided detection (CAD) software, most evaluations have focused on the use case where CAD software entirely replaces human readers. Evidence on alternate CAD software deployment models for TB screening and head-to-head comparisons remains limited. METHODS: A test library of 1466 CXR images was curated after an active case finding initiative in Ho Chi Minh City, Viet Nam. Images were proces

The Proteome of Plasma Extracellular Vesicles in Latent and Active Tuberculosis Reveals Phase-Specific Composition and Immunometabolic Insights.

Tuberculosis (TB) is an infectious disease that affects millions of people worldwide. Extracellular vesicles (EVs) are mediators of intercellular communication that participate in the immunopathogenesis of TB. Here, we characterized plasma EVs from healthy individuals and from individuals with latent (LTB) or active TB (ATB). We found increased total protein concentrations in EVs from LTB and ATB individuals compared to healthy controls, without significant differences in EV concentrations, but reduced exosome abundance in LTB. Proteomic and bioinformatic analysis showed that LTB EVs were enri

Targeting Latent Tuberculosis Using Hypoxia-Activated Prodrugs of Bedaquiline Analogues.

The increasing emergence of antimicrobial resistance (AMR) is reducing the effectiveness of existing antibiotics against bacterial infections, including tuberculosis (TB), a deadly disease caused by Mycobacterium tuberculosis (M.tb). Approximately 90% of infected individuals develop latent tuberculosis infection, for which current treatment options are limited. Latent tuberculosis infection is characterised by the formation of hypoxic granulomas, which restrict oxygen and drug penetration, highlighting the need for therapies effective under hypoxic conditions. In this study, a hypoxia‑activate

Trials, tribulations, and formulations in access to new TB drugs for children: learning from rifapentine.

Historically, there has been considerable delay between stringent regulatory authority approval for new TB drugs in adults, and subsequent trials, registration, and access for children. This has been due to the exclusion of older children from phase 3 trials, the failure to study novel drugs in younger children earlier on, and delays in developing and testing paediatric formulations. Specific barriers include the failure to develop paediatric investigational plans early on, regulatory barriers, inefficient trial design and implementation, limited site capacity, and lack of advocacy and funding

Small-molecule activators of the Staphylococcus aureus ClpC/ClpP AAA+ protease.

The central AAA+ ClpC/ClpP protease in Gram-positive bacteria is crucial for virulence and stress resistance and has been recognized as drug target. Natural cyclic peptides deregulate the essential Mycobacterium tuberculosis ClpC1 and cause cell death. Similarly, overactivated mutants of the non-essential Staphylococcus aureus ClpC homolog cause uncontrolled proteolysis and severe toxicity in vivo. However, no chemical modulators of S. aureus ClpC have been described. Here, using a biochemical high-throughput screen, we identify eight distinct bona fide small molecules that stimulate ClpC ATPa

Anti-mycobacterial activity of phage-encoded lysterases.

Mycobacterium abscessus is a non-tuberculous Mycobacterium (NTM) that is often associated with respiratory infections in persons with cystic fibrosis. High levels of antibiotic resistance make M. abscessus infections difficult to treat, highlighting the urgent need for alternative therapeutic strategies. Mycobacteriophage therapy-treatment of infections with viruses that infect Mycobacterium hosts-shows considerable promise, but the narrow host specificity of the current mycobacteriophage repertoire limits this approach to personalized therapies. Here, we characterize the anti-M. abscessus act

The dual burden of disease: Depression among patients with multidrug resistance tuberculosis in low-income and lower- middle-income countries: A systematic review and meta-analysis.

INTRODUCTION: Multidrug-Resistant Tuberculosis (MDR-TB) continues to pose a serious global health challenge, particularly in low-income and lower-middle-income countries where the disease burden, fragile health systems, and socioeconomic hardships are most pronounced. These conditions not only complicate treatment but also worsen patients' vulnerability to depression. Although numerous primary studies have investigated the prevalence of depression among individuals with MDR-TB, their findings have been inconsistent. To address this gap, the present study seeks to estimate the pooled prevalence

MicroRNA-1246 regulates macrophage lipid metabolism and is involved in resistance to human Mycobacterium tuberculosis infection.

We determined whether microRNAs (miRNAs) regulate resistance to Mycobacterium tuberculosis (Mtb) infection in healthy household contacts (HHCs) of patients with tuberculosis (TB). As shown in our previously published study, we recruited 460 HHCs from 443 index cases of active pulmonary TB, with 293 (64.8%) remaining IGRA-negative throughout a two-year follow-up. Whole-transcriptome sequencing at baseline (0 months) and follow-up (24 months) and confirmation studies identified miRNA-1246 (downregulated) and miRNA-125b (upregulated) as potential candidates for the development of resistance in no

Exploring Biomarkers and Regulatory Mechanisms Associated with the Innate Immune Cell Barrier in Pulmonary Tuberculosis Using Bioinformatics and Machine Learning.

BACKGROUND: Pulmonary tuberculosis (PTB) development is linked to dysregulated immune responses, with the innate immune cell barrier playing a crucial role in both disease initiation and progression. METHODS: The acquisition of transcriptomic datasets pertaining to PTB was conducted using the GEO database. Modules associated with the innate immune cell barrier were identified using weighted gene co-expression network analysis. The intersection of the differentially expressed genes and the module genes yielded a set of overlapping genes. Biological pathways were characterized and candidate gen

Convergent gold-catalyzed synthesis of a partially protected MTX-capped octasaccharide motif of Mycobacterium tuberculosis lipoarabinomannan.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains one of the leading causes of death from a single infectious agent worldwide. The emergence of multidrug-resistant (MDR) and extensively drug resistant (XDR) strains has further compounded the need for effective intervention strategies and rapid diagnostic approaches. One of the striking features of Mtb is its remarkably thick and waxy cell wall characterized by the presence of arabinogalactan (AG), lipoarabinomannan (LAM), peptidoglycan and mycolic acids. Further, arabinose and galactose residues in AG and LAM occur in the

Computed Tomographic Features Associated with Pulmonary Melioidosis versus Pulmonary Tuberculosis in Diabetic Patients: A Retrospective Comparative Cohort Study.

Pulmonary melioidosis is frequently mistaken for pulmonary tuberculosis because of overlapping clinical and radiological features, often delaying appropriate treatment. We conducted a retrospective comparative cohort study at a tertiary care center in southern India between January 2018 and August 2025 to compare chest computed tomography (CT) features of microbiologically confirmed melioidosis and tuberculosis in diabetic adults who underwent CT within 30 days of symptom onset. A total of 151 patients were included: 46 with melioidosis and 105 with tuberculosis. Cavitation (n = 81/105 [77.1%]

Inhibition of the MEK-ERK signaling pathway reverses acquired osimertinib resistance in NSCLC attributed to AXL overexpression.

Osimertinib, a third-generation epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI), is the standard first-line treatment for patients with EGFR-activating mutation of late-stage non-small cell lung cancer (NSCLC), but the emergence of acquired resistance limits its efficacy in real use. The overexpression of anexelekto (AXL) is known to be associated with acquired resistance to osimertinib. However, the specific mechanisms of AXL-mediated resistance are not well explained, nor of the specific therapeutic approaches in patients for this. In this study, we identified that acti

Epigenetic insights into anti-tuberculosis drug-induced liver injury: A systematic review of DNA methylation as a potential biomarker.

Drug-induced liver injury (DILI) remains a major clinical challenge that may cause acute liver failure, chronic liver disease, and treatment discontinuation. DNA methylation, a key epigenetic mechanism regulating gene expression, has emerged as a potential biomarker of DILI susceptibility; however, clinical evidence is largely confined to anti-tuberculosis drug-induced liver injury (ATDILI). This systematic review evaluated associations between DNA methylation patterns and DILI risk, with emphasis on ATDILI. Searches of PubMed, Scopus, and Embase identified eight eligible case-control studies

Pancreatic Tuberculosis Mimicking Pancreatic Cancer in a Young Immunocompetent Man: A Case Report and Literature Review.

BACKGROUND: Pancreatic TB is extremely rare, even in endemic areas. Clinically and radiographically, it may resemble a malignant process. Herein, we present a case of pancreatic TB who underwent surgery with a preoperative diagnosis of pancreatic malignancy. CASE PRESENTATION: A 34-year-old Afghan man presented with a one-year history of intermittent epigastric pain, accompanied by nausea and vomiting. Physical examination showed stable vital signs with mild epigastric tenderness. Laboratory examinations revealed mild anemia with mildly increased liver enzymes and lipase. The CA19-9 level in

Outcomes in Patients with Tuberculous Meningitis: A Prospective Cohort Study in North India.

Tuberculous meningitis (TBM) is the most severe form of extrapulmonary tuberculosis, carrying the highest mortality among all forms. This multicentric prospective cohort study conducted in North India aimed to determine the incidence of complications, functional outcome (modified Rankin scale [mRS]), and mortality in TBM. Patients 12 years old and older with TBM were followed up until completion of 9 months of antitubercular treatment. Patients with concomitant HIV infection, other infections causing meningitis, or multidrug-resistant tuberculosis were excluded. The primary outcome was mRS at

A global community of practice to improve public-private engagement in TB: the TBPPM Learning Network.

BACKGROUND: In many high-TB-burden settings, most people seek initial care from private providers who are often not integrated into national TB programmes. Public-private mix (PPM) strategies are therefore critical, yet difficult to operationalise. Communities of Practice (CoPs) can act as health system enablers and facilitate knowledge exchange, but empirical evidence on their long-term function in disease specific contexts is limited. METHODS: We conducted a mixed-methods descriptive analysis of the TBPPM Learning Network (LN) from 2019 to 2025. Quantitative data from routine platform analy

The burden of multimorbidity in patients with TB-HIV: a retrospective cohort analysis.

BACKGROUND: Multimorbidity (i.e., the co-existence of two or more long-term health conditions [LTCs]) is increasing in high-TB-burden countries. Few studies have estimated the prevalence of multimorbidity in patients with TB, and none in TB-HIV. This retrospective cohort study used HIV clinic data from the Infectious Diseases Institute in Kampala, Uganda, to determine the prevalence and patterns of multimorbidity in patients with TB-HIV at the time of TB diagnosis, and its effect on TB treatment outcomes. METHODS: Adults living with HIV and diagnosed with TB between 1 January 2014 and 14 Nove
