Lyme borreliosis is the most common tick-borne disease in the northern hemisphere. It is a zoonosis caused by several species of Borrelia burgdorferi sensu lato and transmitted by the bite of infected ticks of the Ixodes ricinus complex. Lyme borreliosis in North America and Europe differs in certain respects, likely reflecting the different Borrelia species that cause human disease in these locations. The earliest manifestation of Lyme borreliosis is the skin lesion erythema migrans, which develops at the tick bite site, typically 7-14 days after the bite. Some untreated patients will then (w

INTRODUCTION: Ischemic stroke in young adults is uncommon and is frequently associated with rare etiologies, including autoimmune diseases and vasculitis. Takayasu arteritis (TA) is a chronic inflammatory large-vessel arteriopathy involving the aorta and its major branches and may result in cerebral ischemia due to arterial stenosis or thrombosis.
CASE PRESENTATION: We report the case of a 26-year-old woman with a history of suspected rheumatoid arthritis and Lyme disease who presented with acute left-sided hemiparesis and dysarthria. At admission, large-vessel vasculitis had not yet been sus

Lyme borreliosis (LB), the most common European tick-borne disease, can manifest as an erythema migrans (EM) rash or as disseminated LB. The prospective Burden of Lyme Disease (BOLD) study evaluated the frequency of LB clinical manifestations, including signs, symptoms, and treatment patterns in 14 healthcare practices in endemic regions of six European countries: the Czech Republic, Germany, Poland, Slovakia, Slovenia, and Sweden. Between April 2021 and December 2022, patients with suspected LB were evaluated using predefined case definitions that were applied by investigators to identify med

BACKGROUND: Lyme arthritis (LA) is a manifestation of Lyme disease characterized by joint swelling and pain. LA's clinical presentation mimics pathologies such as septic arthritis (SA) and toxic synovitis (TS). Despite similar presentations, the treatments vary; LA is treated medically, septic arthritis requires surgical debridement, and TS resolves with conservative therapy. Similar presentations can lead to misdiagnosis, subjecting patients to unnecessary interventions. The purpose of this study was to identify physical and laboratory findings that may help differentiate between LA, SA, and

Lyme disease (LD) is the most prevalent vector-borne disease in the Northern Hemisphere. The bacterial pathogen responsible for the disease is transmitted to humans and other mammals via the arthropod vector Ixodes spp. whose hematophagy lends itself to the acquisition and transmission of this pathogen. The spirochete pathogen responsible for LD belongs to the genus Borrelia burgdorferi sensu lato complex, who can be found in North America, South America, Eurasia, and Africa. The initial clinical presentation of LD typically manifests with an erythema migrans skin lesion (bull's-eye rash), and

BACKGROUND: In France, surveillance of Lyme borreliosis (LB) is based on general practitioners of a sentinel network and the national hospital discharge database (PMSI). Given the known limitations of the PMSI for epidemiological surveillance, we assessed the performance of its algorithm in identifying hospitalised LB cases in three university hospitals, in terms of sensitivity and positive predictive value (PPV).
METHODS: We identified patients hospitalised during 2017-2018 with positive laboratory results for LB from hospital laboratory databases. Simultaneously, we screened in the PMSI LB

Lyme disease is a growing and prominent human health problem caused by a group of spirochaetal bacteria that belong to the Borrelia genus. Persistent Lyme disease infection produces a multi-system disorder that may result in severe arthritis, carditis, neurological problems, and even death. Preventing severe disease requires immediate treatment, but current approaches to diagnose Lyme disease are indirect, serology-based assays that may fail early in infection. All Lyme disease-causing Borrelia species shed distinct and unique fragments of their peptidoglycan cell wall during growth. We exploi

In this large multi-center cohort of children evaluated for Lyme disease in a Lyme-endemic emergency department, we assessed the diagnostic accuracy of clinician suspicion and subsequent clinical decision-making for children presenting with monoarthritis. Among 1,582 children with monoarthritis evaluated for Lyme disease, 623 (39%) had Lyme arthritis and 32 (2%) had septic arthritis. Overall, 313 (20%) had an invasive joint procedure (arthrocentesis or arthroscopy), 194 (12%) received parenteral antibiotics, and 376 (24%) were hospitalized. Clinician suspicion had moderate discriminative abili

The prevalence of tick-borne disease continues to rise in the United States. Lyme disease is the most prevalent, classically presenting as monoarthralgia or polyarthralgia and effusions. Although most cases of Lyme arthritis are well managed with antibiotics, in some cases, refractory symptoms and arthritis persist. In addition, the tick-borne disease, alpha-gal syndrome (AGS), can have long-term sequelae, including immunoglobulin E-mediated hypersensitivity to mammalian products. Classically, AGS presents as a new onset of food allergy or intolerance to bovine products. As interest in biologi

Lyme disease is a spirochete infection acquired after inoculation of Borrelia burgdorferi with the bite of an Ixodes tick, commonly encountered in the Northeastern United States, usually characterized by erythema migrans, arthritis, and meningitis. Bannwarth syndrome is an uncommon manifestation of Lyme neuroborreliosis (LNB) in the United States. It can present with various clinical features, which often make the diagnostic process challenging. In our case, the subacute onset of flaccid leg weakness with diminished deep tendon reflexes and facial weakness mimicked Guillain-Barré syndrome (GBS

BACKGROUND: Lyme arthritis is the most common late manifestation of disseminated Lyme disease in the United States. While most patients respond to antibiotic therapy, a subset develops postantibiotic Lyme arthritis (PALA), characterized by persistent noninfectious inflammatory arthritis. Data describing predictors of PALA and treatment patterns remain limited.
METHODS: We conducted a retrospective observational cohort study of patients with polymerase chain reaction (PCR)-confirmed native joint Lyme arthritis treated at our institution from 2004 to 2025. Clinical, laboratory, imaging, treatme

Lyme arthritis, a manifestation of Lyme disease, is triggered by the spirochetal bacterium Borrelia burgdorferi (Bb), which is transmitted through the bite of the Ixodes tick. Although multiple studies have been conducted on the complex host immune response in Lyme arthritis, the spatial gene expression environment in the joint tissue remains unexplored. Here, we applied spatial transcriptomics to ankle joints of C3H mice infected with Bb, profiling tissues at peak inflammation (two weeks post infection) and after antibiotics (four weeks post-infection) during inflammation resolution. Analysis

BACKGROUND: Electronic health records provide opportunities for improving Lyme disease surveillance by collecting detailed clinical information and timely, scalable disease incidence estimation. Medical record review can improve understanding of clinical manifestations, how likely and possible cases compare, and potential misclassification of possible cases.
METHODS: The SubLyme Network is a collaboration among 5 healthcare systems, a coordinating center, and the Centers for Disease Control and Prevention. The sites each conducted stratified random sampling of 500 potential acute Lyme disease

BACKGROUND: Lyme arthritis (LA) in children typically manifests as monoarthritis of a large joint. Although the prognosis is generally good, persistent arthritis occurs in 10%-23% of patients despite antibiotic treatment. Most described pediatric LA cohorts are from the United States, with limited European data available. This study aimed to describe clinical features and outcomes of pediatric LA in Europe using a large Swiss cohort.
METHODS: Children diagnosed with LA at the University Children's Hospital Zurich, Switzerland between 1 January 2006 and 31 December 2020 were included. Data on

Lyme borreliosis is the most common vector-borne disease in the temperate Northern Hemisphere, with approximately 476,000 diagnoses annually in the United States and over 130,000 reported cases across Europe each year, figures that substantially underestimate true incidence. In 1998, LYMErix became the first licensed vaccine against Lyme borreliosis. Its withdrawal four years later was not driven by evidence of vaccine failure but by the collapse of public confidence, and regulatory endorsement that any successful vaccine programme requires. The field was without a licensed product for twenty-

BACKGROUND: Powassan virus (POWV) is a tick-borne, neurotropic arbovirus endemic to Canada and the United States and is transmitted by Ixodes ticks, which also carry Lyme disease. It is an increasingly recognized cause of neuro-invasive infection in humans, for which treatment is supportive. Prognosis in POWV encephalitis is variable, ranging from complete recovery to death. As with other arboviral encephalitides, the mainstay of diagnosis is serological testing. However, molecular diagnostics play an increasingly important role, particularly in the case of immunocompromised hosts.

PURPOSE OF REVIEW: Lyme disease is the most common vector-borne infection in North America, and the incidence continues to rise as the geographic range of Ixodes ticks expands. While most patients can be cured with early antibiotic therapy, Lyme arthritis persists as a challenging late manifestation, with a subset developing postantibiotic inflammatory disease that mimics autoimmune arthritis.
RECENT FINDINGS: Recent studies have advanced our understanding of Lyme arthritis. Even after bacterial eradication, in some patients, residual antigens in the synovial fluid can sustain immune activati

INTRODUCTION: Infection with Treponema pallidum, or syphilis, is often called the "Great Imitator" because of its propensity for atypical presentations and non-specific symptoms. In addition, syphilis has been shown to create false-positive results for common tests, including Lyme disease, HIV, and multiple autoimmune conditions. One that has not been documented is anti-cyclic citrullinated peptide (anti-CCP). Anti-CCP is a highly specific test for Rheumatoid Arthritis. However, in this case, syphilis led to false-elevations in anti-CCP.

CASE: A 43-year-old man with a history of previously treated Lyme arthritis presented with persistent right knee pain for 2 years. Imaging demonstrated synovitis and large subchondral cystic lesions in the proximal tibia and distal femur, suggestive of postinfectious Lyme arthritis (PILA). He underwent right knee arthroscopic synovectomy and transosseous endoscopic bone grafting. Notable improvements in knee pain and range of motion were observed at 1 year postoperatively.
CONCLUSION: Transosseous endoscopic bone grafting of subchondral bone cysts can improve symptoms and restore periarticula

BACKGROUND: Post-treatment Lyme disease syndrome (PTLDS) and rheumatoid arthritis (RA) are both characterized by chronic inflammation and immune dysregulation; however, whether they share underlying molecular immune mechanisms remains unclear.
METHODS: Peripheral blood mononuclear cell (PBMC) transcriptomic datasets from the GEO database were analyzed to identify common differentially expressed genes (DEGs) between PTLDS and RA. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses were subsequently performed. Feature genes were screened using least absolut
