[Borrelia as a differential diagnosis to monoarthritis].

In this case report, a young woman presented with recurring knee monoarthritis. Arthroscopy revealed synovitis, and histology showed chronic plasma cell-dominated inflammation. A positive serum-borrelia IgG titre supported the suspicion of Lyme arthritis despite a negative specific Borrelia PCR result on a paraffin-embedded synovial biopsy. Treatment with phenoxymethylpenicillin, initiated more than two years after symptom onset, resulted in rapid and near-complete remission. The case underscores the diagnostic challenges of Lyme arthritis and the importance of considering the diagnosis in mon

Lyme disease in a warming world: Rising cases, shifting patterns, and clinical implications.

Lyme disease, the most common vector-borne illness in the United States, is increasingly affecting regions beyond historically endemic areas. Warming temperatures and evolving ecosystems have contributed to the expansion of tick populations and altered seasonal activity, raising the likelihood of human exposure in areas previously considered low-risk. Clinicians must maintain a high index of suspicion for Lyme disease, even in patients without traditional geographic exposure. This article reviews the impact of environmental changes on the distribution and epidemiology of Lyme disease. It also

B. burgdorferi infection alters synovial macrophage responses in a human ex vivo model of Lyme Arthritis.

INTRODUCTION: Borrelia burgdorferi (Bb) infection can persist in protected sites like articular joints, presenting as migratory oligoarthritis characteristic of Lyme Arthritis (LA). LA is characterized by intermittent synovitis and joint swelling, but why acute infection is relapsing-remitting and not persistent remains unknown. We hypothesize that the immune response is unique to the joint space, particularly the response of Resident Synovial Macrophages (RSMs), and may be important in understanding the clinical course of acute LA and beyond. This study aimed to investigate the polarization a

Artificial intelligence-assisted spatial correction of augmented reality projections; implementing radiation mapping during radioguided robotic surgery.

The aim of this study was to investigate the effect of micro ribonucleic acid (miR)-195 on myocardial infarction (MI) in rats via regulating the transforming growth factor-β1 (TGF-β1)/Smad signaling pathway. A total of 36 Sprague-Dawley rats were randomly divided into a normal group (n=12), a model group (n=12) and an miR-195 antagomir group (n=12). In the normal group, the heart was exposed only, and normal saline was intraperitoneally injected after operation. In the model group, the acute MI model was established. In the miR-195 antagomir group, the acute MI model was also established, and

Analysis of vaccine adverse events associated with rheumatoid arthritis: A pharmacovigilance study based on the VAERS database.

Vaccination is a principal method of infectious disease prevention, but its association with rheumatoid arthritis (RA) remains controversial. This study assessed vaccine‑associated RA using the Vaccine Adverse Event Reporting System (VAERS). Data from 1990 to 2026 were extracted. Descriptive statistics, Weibull fitting, and disproportionality analysis using four methods, including the reporting odds ratio (ROR), along with subgroup analyses by age, sex, and pre‑/post‑COVID, were performed. Among 11,532,185 reports, 34,498 RA‑related adverse events (AEs) (0.03%) involved 5006 subjects. Females

Frequency and prognostic outcomes of emergency diagnosis in 13 non-neoplastic conditions in England: A population-based cohort study using linked electronic health records of 1.7 million patients.

BACKGROUND: In patients with cancer, diagnosis through or soon after the use of emergency hospital care ('emergency diagnosis') is associated with advanced disease and poor prognosis. The varied and complex reasons for emergency diagnoses-including rapid disease progression, patient factors, and system delays-are unlikely to be unique to cancer. However, little is known about the frequency and prognostic outcomes of emergency diagnosis in other conditions. We aimed to document the frequency of emergency diagnoses across various non-neoplastic conditions and examine the association of emergency

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A person-centred consultation intervention to improve shared decision-making about, and uptake of, osteoporosis medicines (iFraP): a pragmatic, parallel-group, individual randomised controlled trial protocol.

BACKGROUND: Good quality shared decision-making (SDM) conversations involve people with, or at risk of osteoporosis and clinicians collaborating to decide, where appropriate, which evidence-based medicines best fit the person's life, beliefs, and values. We developed the improving uptake of Fracture Prevention drug treatments (iFraP) intervention comprising a computerised Decision Support Tool (DST), clinician training package and information resources, for use in UK Fracture Liaison Service consultations. Two primary objectives to determine (1) the effect of the iFraP intervention on patient-

Utility of Cytokine Biomarkers for the Diagnosis of Pediatric Pyogenic Musculoskeletal Infections.

BACKGROUND: Pyogenic musculoskeletal infections, such as septic arthritis and osteomyelitis, require prompt recognition and treatment but can be challenging to distinguish from Lyme or inflammatory arthritis. Our goal was to identify cytokine biomarkers of musculoskeletal infections. METHODS: Using 2 multicenter prospective cohorts of children undergoing emergency department evaluation for musculoskeletal infection, we selected children ≤21 years of age with a musculoskeletal infection (cases) matched by age and sex to children with Lyme and inflammatory arthritis (controls). We performed a 4

Self-management interventions for common long-term conditions in low- and middle-income countries: a synthesis of current evidence.

BACKGROUND: In addition to endemic communicable diseases, resource-poor health systems in low- and middle-income countries (LMICs) are buckling under an increasing prevalence, morbidity, and mortality attributable to non-communicable diseases (NCD) - 'NCDs crisis.' Supported self-management (SM) is a recommended approach for improving patient outcomes. There is yet no robust evidence on the effectiveness of SM interventions for people with long-term conditions (LTCs) in LMICs. METHODS: Underpinned by a comprehensive search of seven databases, we conducted a systematic review and evidence synt

Acute Monoarthritis: Diagnosis in Adults.

Acute monoarthritis, characterized by pain or swelling in a single joint, is a diagnostic challenge in the primary care setting. Intra-articular conditions typically manifest with reduced active and passive range of motion, whereas patients with periarticular conditions such as tendinitis or bursitis often maintain passive range of motion. When evaluating a patient with acute monoarthritis, it is essential to remember that many polyarthritic processes can initially present in a single joint. A broad differential diagnosis for monoarthritis should include septic arthritis, osteoarthritis, gout,

Lyme neuroborreliosis in Japan: Borrelia burgdorferi sensu lato as a cause of meningitis of previously undetermined etiology in hospitalized patients outside of the island of Hokkaido, 2010-2021.

BACKGROUND AND PURPOSE: Clinical manifestations of Lyme borreliosis (LB), caused by Borrelia burgdorferi sensu lato (Bbsl), include erythema migrans, Lyme neuroborreliosis (LNB), carditis, and arthritis. LB is a notifiable disease in Japan with <30 surveillance-reported LB cases annually, predominately from Hokkaido Prefecture. However, LB, including LNB, may be under-diagnosed in Japan since diagnostic tests are not readily available. We sought to determine if LNB could be a cause of previously undiagnosed encephalitis or meningitis in Japan. METHODS: Investigators at 15 hospitals in 10 pref
