CONTEXT: Male carriers of balanced reciprocal translocations can experience impaired spermatogenesis and adverse pregnancy outcomes. The impact of translocation fragment size and breakpoint location on these outcomes remains unclear.
AIMS: To investigate whether the location and size of translocated fragments relate to spermatogenic function, embryo aneuploidy ratio, and pregnancy outcomes in couples with male chromosomal translocation.
METHODS: In total, 103 couples with male reciprocal translocation were included and these couples underwent 116 pre-implantation genetic testing cycles. Pati

Lyme disease, caused by Borrelia burgdorferi, is the most common tick-borne infection in the United States. Arthritis is a major clinical manifestation of infection, and synovial tissue damage has been attributed to the excessive pro-inflammatory responses. The secretory leukocyte protease inhibitor (SLPI) promotes tissue repair and exerts anti-inflammatory effects. The role of SLPI in the development of Lyme arthritis in C57BL/6 mice, which can be infected with B. burgdorferi, but only develop mild joint inflammation, was therefore examined. SLPI-deficient C57BL/6 mice challenged with B. burg

Lyme disease is caused by Borrelia species that are transmitted by Ixodes ticks prevalent in parts of the United States and Europe. A Lyme vaccine containing the outer surface protein A (OspA) antigens from the single Borrelia species most prevalent in the United States was marketed in the 1990s, but was withdrawn because of unproven concerns about safety, which led to insufficient sales. Since then, the incidence of Lyme disease has increased in the United States owing to the geographical spread of infected ticks. Lyme disease due to multiple different species of Borrelia is also widely preva

The spirochete Borrelia burgdorferi causes Lyme disease. In some patients, an excessive, dysregulated proinflammatory immune response can develop in joints leading to persistent arthritis even after antibiotic therapy. In such patients, persistence of antigenic B. burgdorferi peptidoglycan (PGBb) fragments within joint tissues may contribute to immunopathogenesis pre- and post-antibiotic treatment. In live B. burgdorferi cells, the outer membrane shields the polymeric PGBb sacculus from exposure to the immune system. However, unlike most diderm bacteria, B. burgdorferi releases PGBb turnover p

Background/Objectives: Lower-extremity weakness in older adults is often overlooked, yet it can have reversible or medical causes that contribute to increased falls. Common factors include vision disturbances, impaired balance due to otolith dysfunction, arthritis-related immobility, and lower-extremity neuropathy. This case presents a unique diagnostic challenge in evaluating bilateral lower-extremity weakness and recurrent falls in an older adult, highlighting the complexity of diagnosing conditions with overlapping symptoms. Case Presentation: The patient, a woman with a history of a neuroe

BACKGROUND: Lyme arthritis is a common manifestation of late-stage Lyme disease in pediatric and adolescent patients. Patients with Lyme arthritis typically present with atraumatic knee effusion and may undergo magnetic resonance imaging (MRI) to aid in diagnosis. The incidence of meniscal pathology on MRI in association with Lyme arthritis is unknown. This study aims to evaluate the incidence of meniscal pathology on MRI in young patients with Lyme arthritis.
METHODS: Patients (<18 years old) presenting with a unilateral knee effusion from 2009 to 2019 with a positive Lyme antibody serologic

Persistent symptoms after an acute infection is an emerging public health concern, but the pathobiology of such conditions is not well understood. One possible scenario involves the persistence of lingering antigen. We have previously reported that patients with postinfectious Lyme arthritis often harbor the peptidoglycan (PG) cell wall of Borrelia burgdorferi, the Lyme disease agent, in the synovial fluid of their inflamed joints after treatment. However, it is not yet known how B. burgdorferi PG persists, in what form, or if it may play a role in other postinfectious complications after Lyme

Lyme disease, caused by Borrelia burgdorferi, is the most common vector-borne disease in the United States, and the range of its tick vector continues to expand. Most Lyme disease cases are diagnosed with the onset of the erythema migrans rashes, which can be single or multiple and vary from a homogeneous erythema to bull's-eye patterns. Serologic antibody testing is of low sensitivity at onset but becomes highly sensitive after a few weeks. Early dissemination may lead to neurologic and cardiac complications. Mono- or oligoarticular arthritis may develop in untreated patients. Antibiotic trea

Post-treatment Lyme disease (PTLD) occurs in a portion of patients after initial antibiotic treatment of Lyme disease (LD) and is often characterized by arthralgia without synovitis. Rheumatoid factor (RF) and anti-citrullinated protein antibodies (ACPA) are often used to assess joint pain in this setting; however, their clinical utility remains unknown. Our objective was to define the frequency of these autoantibodies in a large cohort of carefully characterized patients with PTLD meeting a research case definition and to determine the clinical implications of these tests. RF and ACPA were te

Apicomplexan protozoan parasite Babesia microti (Bm) and spirochete Borrelia burgdorferi are tick-transmitted pathogens that cause babesiosis and Lyme disease and increasingly cause coinfections. More pronounced Lyme arthritis occurs during the acute phase of coinfection compared to B. burgdorferi alone. In susceptible C3H mice, Bm parasitemia was quantified by microscopic examination of blood smears while live imaging of bioluminescent B. burgdorferi N40 strain-infected mice allowed monitoring of disseminated infection. Furthermore, proteomic analyses of blood samples unveiled nuanced tempora

Lyme borreliosis (LB) is the most common tick-borne disease in Europe and can manifest as localized erythema migrans (EM) or further disseminate into Lyme neuroborreliosis (LNB), Lyme arthritis (LA) or other manifestations. However, public health surveillance in Denmark and Ireland only captures LNB, while published LB data for Sweden only includes LNB. To enhance the understanding of LB disease burden in these countries, this study aimed to estimate LB incidence by clinical manifestation using the ratio of each manifestation to LNB. These ratios were derived using random effects meta-analysis

We present a 55-year-old patient with rheumatoid arthritis, concomitant small vessel vasculitis, and autoimmune inflammatory myositis who developed a multilocular annular erythema under immunosuppressive therapy. Clinically highly suspicious for Lyme borreliosis, histologically only a minimal perivascular and interstitial dermatitis with discrete mucin deposits was found. Infectious pathogenesis was proven by the detection of borrelia DNA using PCR. This case extends the spectrum of histologically invisible dermatoses and illustrates that in immunocompromised patients, clinically prototypical

Borrelia burgdorferi (or Borreliella burgdorferi), the extracellular spirochete responsible for Lyme disease, elicits a type I interferon (IFN-I) response critical for the development of Lyme arthritis. However, the specific pathogen-associated molecular pattern (PAMP) driving this response remains unidentified. Previous studies have reported that B. burgdorferi culture supernatants significantly stimulate macrophage IFN-I responses, but the responsible component was unknown. In this study, we identified cyclic-di-adenosine monophosphate (c-di-AMP) as the critical PAMP for the induction of IFN

UNLABELLED: Establishment of immunoglobulin diversity is contingent on recombination that occurs both at the Fab and at the Fc regions of the immunoglobulin, and this process is time dependent. Based on this principle, we questioned whether Lyme disease stage can be distinguished by quantification of immunoglobulin class and IgG isotype specific to VlsE in serum from clinically characterized patients. We used an enzyme immunoassay to categorize serologic antibodies to VlsE antigen as well as machine-learning techniques to train and integrate multiple predictors to identify likely disease stage

OBJECTIVE: A previous case series described the development of new autoimmune, inflammatory arthritis (IA) developing within 2 years of Lyme disease (LD). This study aimed to estimate the incidence of IA following LD using administrative claims data. Influenza, an infection not typically associated with post-infectious IA, was used for comparison.
METHODS: We conducted a retrospective cohort study using the Johns Hopkins Health System administrative claims data from 01/2013-05/2024. Patients with LD and influenza were identified using International Classification of Diseases (ICD) codes, with

Lyme disease has many different manifestations, including arthritis. There is no evidence that the pathogen Borrelia burgdorferi expresses toxins, so the presence of the organism is insufficient to cause inflammation and disease. Thus, the interaction between the causative pathogen and the many human immune system mechanisms responding to the organism is the apparent cause of inflammation and tissue damage/dysfunction. This review discusses many of the aspects of the relationship between organisms and host responses to summarize the many means by which the immune response can cause the synovit

Lyme disease (LD), a zoonotic infectious disease caused by Borrelia burgdorferi (B. burgdorferi), can affect various organs, including the skin, heart, nervous system, and joints. Lyme arthritis (LA) is the most common and severe late-stage presentation of LD, often presenting with intermittent joint swelling and pain. Although antibiotics are effective in most patients with LA, some patients may continue to experience arthritis symptoms for months or years after standard treatment, which poses a serious threat to their quality of life. Therefore, more effective treatments are urgently needed.
