A Case of Pericarditis With Pericardial Effusion Due to Lyme Disease.

Acute pericarditis with pericardial effusion caused by Lyme disease is very rare and requires excluding other more common possible causes of acute pericarditis. Common causes of acute pericarditis include viral and bacterial infections, connective tissue disorders such as rheumatoid arthritis, systemic lupus erythematosus, or scleroderma, or trauma to the heart, such as from a heart attack or heart surgery. While Lyme disease is mainly known for cardiac effects such as heart blocks and arrhythmias, it can also rarely cause pericarditis with pericardial effusion. In this case report, we present

Surveillance of Lyme neuroborreliosis and Lyme borreliosis: estimates of disease burden in Southern Sweden 2009-2022.

BACKGROUND: Despite recommendations by EU, surveillance of Lyme neuroborreliosis (LNB) is still not fully implemented in all member states, Sweden being one of them. Laboratory-based notifications on positive Borrelia antibody index (AI) in paired cerebrospinal fluid-serum samples alone have been suggested for surveillance of LNB. OBJECTIVES: We aim to describe the epidemiology of Lyme borreliosis (LB) and LNB in Southern Sweden and assess if laboratory-based surveillance of LNB alone risks to underestimate the incidence in different populations. METHODS: Using register data, we categorized

Long-term survival of Babesia microti and Borrelia burgdorferi in C3H/HeJ mice and their effect on Lyme arthritis and babesiosis manifestations.

Babesia microti is increasingly co-transmitted with Lyme disease-causing Borrelia burgdorferi by ticks in the endemic regions of the United States. Infection in mice by this parasite mirrors human babesiosis, such as anemia, splenomegaly, alterations in splenic leukocyte balance, and diminished humoral immunity associated with enhanced Lyme disease manifestations at the acute phase. To evaluate the long-term survival of B. burgdorferi and B. microti in mice and their effects on pathogenesis, we conducted a 16-week infection experiment in C3H/HeJ mice. All mice infected with B. microti, irrespe

Changes in the Regional Incidence of Lyme Disease Across the United States From 2004 to 2019: A PHIS Database Study.

BACKGROUND: Lyme disease is the most common vector-borne disease in United States children. It was traditionally thought to be limited to the Northeast and upper Midwest due to the prevalence of tick and deer vectors in these regions. We hypothesize that as Lyme disease cases in children continue to increase throughout the country, nontraditional regions such as the Midwest and South may have an increased rate of Lyme disease compared with the Northeast region. METHODS: The Pediatric Health Information System (PHIS) database was queried for patients 19.0 years or younger who were treated for

The potential to improve Lyme disease diagnostics through quantification of immunoglobulin class switching patterns.

N. Nair, A. Marques , E. J. Horn, G. Brown et al., J Clin Microbiol 63:e0034725, 2025, https://doi.org/10.1128/jcm.00347-25 present data to demonstrate that infection by Borrelia burgdorferi, the primary causative agent of Lyme disease in the USA, leads to immunoglobulin class switching in antibodies specifically against the immunodominant antigen, VlsE (variable major protein-like sequence, expressed), in a predictable pattern between the early acute (<1 month illness duration) to late-stage Lyme arthritis stages. Detection of anti-VlsE isotypes in sera was highly specific to Lyme disease pat

Identification of hub gene and immune infiltration in Lyme disease revealed by weighted gene co-expression network analysis and machine learning.

INTRODUCTION: Lyme disease (LD), caused by the spirochete Borrelia burgdorferi (Bb), is a multisystem disorder with early symptoms such as erythema migrans and late manifestations including arthritis and neuroborreliosis. The molecular mechanisms driving tissue damage and inflammatory dysregulation in LD remain incompletely characterized. Given the central role of peripheral blood mononuclear cells (PBMCs) in orchestrating immune responses, we aimed to identify optimal feature genes (OFGs) within PBMCs associated with LD pathogenesis and delineate their immune infiltration patterns using integ

Non-radiographic axial spondyloarthritis developing after Lyme arthritis: a case report and review.

BACKGROUND: Lyme disease is a tick-borne illness that can result in a spectrum of musculoskeletal complications, ranging from infection-related arthritis to post-infectious, immune-mediated conditions. Although prior reports have linked Lyme disease to systemic autoimmune diseases, such as rheumatoid arthritis and psoriatic arthritis, non-radiographic axial spondyloarthritis (nr-axSpA) has not previously been described following Lyme disease. CASE PRESENTATION: A previously healthy patient developed monoarthritis of the ankle, with inflammatory synovial fluid and a positive Lyme Western blot,

Antibiotics for Rheumatologic Diseases: A Critical Review.

Antibiotics have been traditionally used to treat patients with infectious diseases. However, recent investigations have highlighted their immunomodulating features. Additionally, they have been used to treat patients with rheumatologic diseases of proven infectious etiology. Thus, an emerging body of literature is developing on the potential role of antibiotics in managing patients with rheumatologic diseases, which are primarily characterized by autoimmune-driven inflammation. We critically review the potential use of antibiotics in rheumatology, focusing on both their direct antimicrobial a

Rabies virus-vectored Lyme disease vaccine provides long-term protection against tick-transmitted Borrelia burgdorferi.

Lyme disease (LD) cases have doubled globally. LD is a tick-borne illness caused by the Borrelia burgdorferi sensu lato (Bb). If untreated, Bb can disseminate to distal organs, causing carditis, arthritis, and meningitis. Currently, no FDA-approved human LD vaccine exists on the market. This study used two approaches to incorporate OspA into the rabies virus (RABV) vaccine vector. We used the RABV-glycoprotein tail (RVG tail) and the Hendra virus (HeV) glycoprotein tail (HVG tail) to incorporate OspA, creating BNSP333-OspA-RVG and BNSP333-OspA-HVG, respectively. Both vaccines produced type-1 b

Synovial chemokine and cytokine profiles in horses with and without systemic Borrelia burgdorferi infection.

BACKGROUND: Lyme arthritis, a well-documented subtype of arthritis in humans and dogs, is caused by Borrelia burgdorferi and spread primarily through ticks (Ixodes spp.). While Lyme disease in horses is known to cause a variety of clinical signs, its involvement in the development of equine arthritis is controversial. As climate change enables tick populations to expand, more horses could be exposed to B. burgdorferi; therefore, a greater understanding of how Lyme disease affects joints is critical to preserving soundness. OBJECTIVES: To determine if systemic, naturally occurring Lyme disease

Guidelines for Lyme borreliosis: treatment.

The aim of the clinical practice guideline is to support healthcare professionals in the diagnosis and management of patients with proven or suspected Lyme borreliosis (LB). The guideline provides an evidence-based algorithm for antibiotic therapy in confirmed LB cases and outlines recommendations for complex presentations where the three diagnostic criteria are not fully met, suggesting a possible challenge antibiotic therapy in specialized settings. Updated in accordance with international literature and previous French recommendations (HAS, 2022), the guidelines detail antibiotic selection

Guidelines for Lyme borreliosis: clinical manifestations.

Lyme borreliosis (LB) is a tick-borne zoonosis caused by spirochetes belonging to the Borrelia burgdorferi sensu lato (Bb sl) complex. In Europe, multiple pathogenic species-including B. afzelii, B. garinii, and B. burgdorferi sensu stricto-are responsible for a wide diversity of clinical manifestations. The disease may present in various stages-localized, early disseminated, or late disseminated-depending on the time elapsed since the tick bite and the organs involved, such as the skin, joints, or nervous system. Erythema migrans (EM) is the most frequent clinical presentation, accounting for

Toll-like receptor 1 polymorphism is associated with impaired immune tolerance, dysregulated inflammatory responses to Borrelia burgdorferi, and heightened risk of post-infectious Lyme arthritis.

INTRODUCTION: Clinical presentation of Lyme disease is largely due to host immune response to infection. Previously, we identified a variant (1805GG) in the TLR1 gene, a key immune sensor for Borrelia burgdorferi, which was associated with excessive inflammation and severe disease. Herein we examined the mechanism by which this variant leads to dysregulated immunity. METHODS: We found that patients with post-infectious Lyme arthritis, a condition characterized by marked persistent synovitis in joints, have a higher frequency of TLR1-1805GG compared to those whose arthritis resolves with antib

Lyme arthritis: Demographic characteristics and Borrelia ospA genospecies in synovial fluid. A 17-year cohort study in Sweden.

Lyme arthritis (LA) is a recognized manifestation of Lyme borreliosis in Sweden, but data on the causative Borrelia species are limited. This study aimed to describe the demographics of LA and identify Borrelia burgdorferi sensu lato (B. burgdorferi s.l.) genospecies present in synovial fluid (SF). Patient data were collected from the test requisition form accompanying consecutive SF samples submitted to the Clinical Microbiology Laboratory in Lund, Sweden, for Borrelia DNA detection between 2007 and 2023. Detection was performed using 16S rDNA PCR, and genospecies were determined through ospA

Evaluating the neuroprotective potential of Mucuna pruriens and Boswellia serrata in Huntington's disease: an integrative bioinformatics approach.

Hepatitis E causes large-scale epidemics in endemic areas. The disease, during epidemics, has increased incidence and severity in pregnant women. Sporadic acute viral hepatitis (AVH) is common in endemic areas. The relationship of sporadic AVH and pregnancy has not been well studied. Over a 3-year period we prospectively studied 76 pregnant women and 337 non-pregnant women of childbearing age with sporadic acute viral hepatitis for aetiology, clinical course and outcome of disease. The aetiology in sporadic AVH was hepatitis A virus (HAV) in six (1.5%), hepatitis B virus (HBV) in 62 (15%), hep

Co-development and testing of an extended community pharmacy model of service delivery for managing osteoarthritis: protocol for a sequential, multi-methods study (PharmOA).

BACKGROUND: Osteoarthritis is a common, painful and disabling long-term condition. Delivery of high-quality guideline-informed osteoarthritis care that successfully promotes and maintains supported self-management is imperative. However, osteoarthritis care remains inconsistent, including under use of core non-pharmacological approaches of education, exercise and weight loss. Community pharmacies are an accessible healthcare provider. United Kingdom government initiatives are promoting their involvement in a range of long-term conditions, including musculoskeletal conditions. It is not known w

Exploring the potential extended role of community pharmacy in the management of osteoarthritis: A multi-methods study with pharmacy staff and other healthcare professionals.

INTRODUCTION: Osteoarthritis is the commonest form of chronic joint pain, which patients often self-manage before seeking healthcare advice. Patients frequently seek advice from community pharmacies, and a recent policy has recommended integrating community pharmacies into long-term condition pathways. This study explored community pharmacy teams' (CPs) and other healthcare professionals' (HCPs) views on community pharmacies providing an extended role for osteoarthritis management, identifying potential barriers and facilitators to this. METHODS: A multi-methods study comprising surveys of CP
