BACKGROUND: Although pleural tissue debris is occasionally encountered in the drainage systems of patients undergoing catheter or chest tube drainage, its clinical utility has not been reported. Here, we report two cases in which pleural tissue debris retrieved during catheter drainage enabled a definitive diagnosis of malignant pleural effusion (MPE) and molecularly guided treatment decisions.
CASE PRESENTATION: The first case involved a 67-year-old woman with rheumatoid arthritis-associated interstitial lung disease who developed a right pleural effusion suggestive of lung cancer with pleur

The coexistence of active rheumatoid arthritis and active primary biliary cholangitis represents a distinct therapeutic challenge. Evidence-based guidance for treatment selection in this clinical setting remains limited, and published experience with tocilizumab is restricted to isolated case reports. We report a 59-year-old woman with highly active, seropositive rheumatoid arthritis and marked cholestatic biochemical activity, in whom diagnostic evaluation revealed active primary biliary cholangitis in the absence of advanced liver fibrosis. Following multidisciplinary assessment, methotrexat

Rheumatoid arthritis (RA), systemic lupus erythematosus/lupus nephritis (SLE/LN), and systemic sclerosis (SSc) remain major causes of chronic inflammation, disability, organ damage, and treatment burden despite therapeutic advances. RNA therapeutics offer programmable transcript suppression, network-level microRNA modulation, transient restoration of protective proteins, and targeting-ligand functions. Existing reviews largely catalogue modalities or individual delivery systems; this review instead asks whether a candidate forms an interpretable translational program across target biology, dis

Tumor necrosis factor-alpha (TNF-α) inhibitors are effective therapies for rheumatoid arthritis (RA) but may rarely induce paradoxical autoimmune complications, including vasculitis. We report the case of a 53-year-old woman with seropositive RA and secondary Sjögren's syndrome who developed bilateral dependent petechial and purpuric lesions of the lower extremities after approximately 33 months of etanercept therapy at a dose of 25 mg twice weekly. At symptom onset, her RA was well controlled, with no clinical or ultrasonographic synovitis and a Disease Activity Score in 28 joints of 2.1. Ski

Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) may precede overt articular manifestations, posing significant diagnostic and therapeutic challenges. The usual interstitial pneumonia (UIP) pattern is the most common radiologic phenotype and is associated with a poorer prognosis and a progressive fibrosing course.

Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is associated with substantial morbidity and mortality. We compared international and regional clinical practice guidelines to identify agreement, differences, and gaps in RA-ILD care. We conducted a narrative comparative review of five guideline groups: ERS/EULAR, ACR/CHEST, EULAR, PANLAR, and ALAT. Recommendations were assessed across screening, diagnosis, monitoring, and treatment, together with their methodological approaches. Diagnostic recommendations showed the greatest agreement: ILD-focused guidelines emphasized high-r

Patients with immune-mediated inflammatory rheumatic diseases (IMIRDs) may be more vulnerable to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection because of underlying immune dysregulation. However, it remains unclear which factors determine coronavirus disease 2019 (COVID-19) severity in this population, especially in Eastern European settings. We conducted a retrospective, multicenter observational study including 211 patients with IMIRDs diagnosed with COVID-19 between 01.03.2020 and 31.12.2024. We collected clinical, demographic, and laboratory data within the first 7

We report a case of elbow pigmented villonodular synovitis (PVNS) combined with seronegative rheumatoid arthritis (SNRA) and review relevant literature to summarize the characteristics, treatment, and outcomes of this comorbidity. The patient was a 72-year-old female. She developed swelling and pain of the right elbow in 2002, and underwent surgical treatment. Postoperative histopathology confirmed PVNS. In 2007, she developed swelling and pain in the left elbow and underwent synovectomy, and synovectomy again yielded pathology consistent with PVNS. From 2021 onwards, she experienced recurrent

Background/Objectives: Neurological manifestations in rheumatoid arthritis (RA) are common, yet the clinical relevance of central nervous system (CNS) abnormalities detected on neuroimaging remains uncertain. We aimed to determine the prevalence and patterns of cranial magnetic resonance imaging (MRI) findings in RA patients treated with rituximab (RTX) and to assess whether these abnormalities are associated with disease-specific mechanisms or with age- and comorbidity-related factors. Methods: In this retrospective observational study, 84 RA patients who received RTX and had available pre-tr

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is an immune-mediated neuropathy that rarely co-occurs with rheumatoid arthritis (RA). When CIDP proves refractory to first-line therapies, management becomes particularly challenging, especially in patients with multiple autoimmune conditions. A 43-year-old Chinese male with a 3-year history of seropositive RA in sustained remission presented with acute progressive quadriparesis, bilateral ptosis, and facial diplegia. Cerebrospinal fluid analysis revealed albuminocytologic dissociation, and nerve conduction studies demonstrated

The protozoan parasite Toxoplasma gondii is the cause of the widespread but sometimes disregarded parasitic illness toxoplasmosis. Cytokines play an indispensable role in fostering an environment that is more conducive to pregnancy. Their intricate network of interactions allows for the observation of numerous modulatory consequences, such as cell migration, invasion, and gene expression. Among these, CCL2 (MCP-1) and CXCL16 have emerged as promising immunological biomarkers reflecting disease activity, inflammation, and tissue involvement. Monocyte chemotactic protein 1 (MCP-1), another name

PURPOSE: To describe a case of rheumatoid arthritis (RA)-associated orbital and intracranial inflammatory disorder (OIID).
BACKGROUND: RA is a systemic autoimmune disease associated with a multitude of extra-articular manifestations. Cigarette smoking increases the risk of RA vasculitis and adversely influences the expression of the proto-oncogene p53 in RA. Although ocular manifestations of RA are well known, orbital and intracranial inflammatory presentations are much less common.
CASE PRESENTATION: A 63-year-old woman with seropositive RA and a significant tobacco smoking history presente

OBJECTIVES: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) affects 5-10% of patients with RA. While lung ultrasound (LUS) shows promise for RA-ILD detection, its role remains inconclusive. We aimed to assess its performance as a screening tool in at-risk RA patients.
METHODS: We made a cross-sectional study of RA patients, including i) individuals >50 years old with ≥1 risk factor for ILD, and ii) patients with new-onset respiratory symptoms. All patients underwent clinical examination, pulmonary function testing (PFT), LUS, and chest high-resolution computed tomography (H

BACKGROUND: Interstitial lung diseases (ILDs) are a heterogeneous group of disorders in which epithelial injury, inflammation and fibrosis lead to progressive loss of lung function. Several ILDs other than idiopathic pulmonary fibrosis (IPF), including connective tissue disease-associated ILD (CTD-ILD), fibrotic hypersensitivity pneumonitis and fibrotic nonspecific interstitial pneumonia, may develop a progressive fibrosing phenotype, and distinguishing fibrotic from non-fibrotic disease is clinically important. BACKGROUND: Disseminated herpes simplex virus (HSV) infection is an uncommon but serious condition, typically seen in immunocompromised hosts. Cytopenias, including neutropenia and thrombocytopenia, are rare manifestations, particularly in patients not actively receiving immunosuppressive therapy.
CASE PRESENTATION: We report a 75-year-old female with a history of rheumatoid arthritis (RA), previously on methotrexate and low-dose prednisone, who presented with painful oral and genital ulcers, mucositis, and rapidly progressive bicytopenia (neutropenia and thrombocytopenia). Her recent brief r

BACKGROUND & AIMS: Dietary interventions may improve rheumatoid arthritis (RA) outcomes through anti-inflammatory and metabolic pathways. This study aimed to evaluate the effects of the Dietary Approaches to Stop Hypertension (DASH) diet on disease activity and treatment outcomes in patients with seropositive RA.
METHODS: In this 12-week parallel-group randomized controlled trial, 82 patients with seropositive RA were randomly assigned to a DASH diet intervention (n = 40) or a control group maintaining their usual diet (n = 42). The primary outcome was change in disease activity assessed usin
