BACKGROUND: Rheumatoid arthritis (RA) is an autoimmune disease for which better biomarkers are needed, especially in seronegative cases. This study evaluates complete blood count (CBC)-derived inflammatory indices - neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), monocyte-to-lymphocyte ratio (MLR), systemic immune-inflammation index (SII), and systemic inflammation response index (SIRI) - for RA diagnosis and disease activity assessment, with comparisons between seropositive and seronegative RA.
METHODS: We conducted a retrospective case-control study of 230 RA patie

Rheumatoid arthritis (RA) is a chronic, immune-mediated disease with symmetric polyarticular pain and swelling. It includes seropositive RA, marked by specific antibodies, and seronegative RA, without these antibodies. The relationship between immune cells and both types of RA has not been fully elucidated. Therefore, we conducted a two-sample Mendelian randomization analysis with bidirection to evaluate the causal relationship between immune cells and both seropositive and seronegative RA. We utilized data from various genome-wide association studies and employed multiple analytical methods,

Still’s disease (SD) and rheumatoid arthritis (RA), particularly seronegative RA, may overlap clinically and biologically, complicating diagnosis. We explored data-driven overlaps between SD, seronegative RA, and seropositive RA using unsupervised clustering. We retrospectively included 312 adults: 98 SD from the multicentric AURAL Still cohort and 214 RA (93 seropositive, 121 seronegative). Baseline clinical and laboratory data were analysed using factor analysis of mixed data followed by k-means clustering. SD patients were younger than RA patients and more frequently presented with fever an

INTRODUCTION/OBJECTIVES: To investigate longitudinal trends in the proportion of seronegative rheumatoid arthritis (RA) and to compare clinical outcomes between seronegative and seropositive patients in a Japanese single-centre cohort.
METHOD: We included 4,354 RA patients with a disease duration < 4 years from the Institute of Rheumatology, Rheumatoid Arthritis cohort. Seronegativity was defined as the absence of both rheumatoid factor (RF) and anti-citrullinated peptide (CCP) antibody. We assessed the association between enrolment period and seronegativity using multivariable logistic regre

Previous observational studies have indicated a link between rheumatoid arthritis (RA) and airway hyperresponsiveness diseases (AHDs). However, the causality of this relationship remains uncertain. Our research aims to investigate the causal relationship between RA and AHDs. Using publicly available genome-wide association study summary statistics, we selected single nucleotide polymorphisms associated with RA (including overall RA, seropositive RA and seronegative RA), and AHDs (including asthma, bronchitis, allergic rhinitis and chronic obstructive pulmonary disease [COPD]) as instrumental v

BACKGROUND: This study seeks to investigate associations of osteoporosis (OP) with arthritis and to assess causality using Mendelian randomization (MR).
METHODS: Firstly, an observational study was conducted using the National Health and Nutrition Examination Survey (NHANES). The relationship between OP and arthritis was analyzed by weighted multivariate logistic regression. Furthermore, a Mendelian randomization analysis was performed to examine the causal effect of arthritis on OP. Genetic data on OP and arthritis were derived from independent genome-wide association studies (GWAS). Arthrit

OBJECTIVE: Early diagnosis of seronegative rheumatoid arthritis (SNRA) is often challenging due to lack of reliable serological markers. The aim of this study was to explore protein biomarkers of SNRA and identify their potential as therapeutic targets.
METHODS: Third-eight seropositive (SPRA) patients and 20 SNRA patients were enrolled in our omics study. Differentially expressed proteins (DEPs) between the two groups were identified via the Astral mass Analyzer for quantitative proteomics using data-independent acquisition (DIA) mass spectrometry, and the dataset of GSE93272 was used for th

Rheumatoid vasculitis (RV) is an uncommon but potentially fatal extra-articular manifestation of rheumatoid arthritis (RA), characterized by severe systemic inflammation and high mortality despite advances in disease-modifying therapies. We describe a 52-year-old woman with long-standing seropositive RA and irregular use of methotrexate who presented with progressive digital ischemia and extensive necrotic cutaneous ulcers, in the absence of active synovitis. Histopathology revealed small-vessel vasculitis, accompanied by markedly elevated rheumatoid factor and anti-cyclic citrullinated peptid

We report the case of a 55-year-old woman with seropositive rheumatoid arthritis (RA) treated with methotrexate and rituximab (RTX) who developed severe inflammatory low back pain one month after her fourth RTX cycle. MRI of the lumbosacral spine revealed L4-L5 infectious spondylodiscitis with vertebral endplate destruction, anterior epidural extension, and a right psoas abscess. CT-guided aspiration confirmed methicillin-sensitive Staphylococcus aureus infection. Empirical IV therapy with ciprofloxacin and gentamicin was initiated per local protocol. Following microbiological confirmation, ci

Large granular lymphocytic leukemia (LGLL) is often associated with autoimmune diseases, with rheumatoid arthritis diagnosed in approximately one-third of the patients. Nevertheless, other rheumatologic conditions should also be considered. We describe a male patient in his 50 s with a long-standing clinical history suggestive of palindromic rheumatism, who was diagnosed with LGLL leukemia during an evaluation for fever and neutropenia.

INTRODUCTION: Chronic hepatitis B (CHB) frequently coexists with metabolic dysfunction-associated steatotic liver disease (MASLD). This study aimed to determine whether MASLD is a risk factor for incident rheumatoid arthritis among middle-aged patients with CHB in a real-world setting.
METHOD: A population-based cohort study was conducted using the Korean National Health Insurance Service, including 114,425 middle-aged CHB individuals (aged 40-65 years) classified as having either MASLD or non-SLD. Cox proportional hazards regression and Fine-Gray subdistribution hazard models were applied to

OBJECTIVES: To report a case of AA developing during golimumab and leflunomide treatment for seropositive rheumatoid arthritis, with subsequent improvement following initiation of selective Janus kinase 1 (JAK1) inhibition.
METHODS: A 55-year-old woman developed progressive, non-scarring alopecia while rheumatoid arthritis disease activity remained well controlled. Clinical and trichoscopic findings were consistent with AA, and laboratory investigations were unremarkable. The patient received serial intralesional triamcinolone at approximately 4-8 week intervals over a 14-month period, with p

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis is a multisystem autoimmune disease that may present diagnostic challenges, particularly in patients with coexisting autoimmune conditions. Although the coexistence of rheumatoid arthritis and ANCA-associated vasculitis is recognised, vasculitis more commonly develops several years after rheumatoid arthritis onset. We describe a 68-year-old man who developed rapidly progressive glomerulonephritis due to myeloperoxidase (MPO)-ANCA-associated microscopic polyangiitis within 6 months of a diagnosis of seropositive rheumatoid arthri

OBJECTIVE: To assess whether a panel of peripheral blood biomarkers associated with idiopathic pulmonary fibrosis (IPF) is also associated with interstitial lung disease (ILD) in patients with rheumatoid arthritis (RA) using three independent cohorts.
METHODS: We first assessed the association of a panel of IPF-associated biomarkers with prevalent ILD among two separate RA cohorts (n = 93 and n = 71). Concentrations of eight IPF-related biomarkers (eotaxin, Flt-3L, interleukin-8, macrophage-derived chemokine, monocyte chemoattractant protein 1, and matrix metalloproteinase 2/7/9 [MMP-2/7/9])
