Fulminant Rheumatoid Vasculitis With Digital Necrosis and Cutaneous Ulcerations: A Case Report.

Rheumatoid vasculitis (RV) is an uncommon but potentially fatal extra-articular manifestation of rheumatoid arthritis (RA), characterized by severe systemic inflammation and high mortality despite advances in disease-modifying therapies. We describe a 52-year-old woman with long-standing seropositive RA and irregular use of methotrexate who presented with progressive digital ischemia and extensive necrotic cutaneous ulcers, in the absence of active synovitis. Histopathology revealed small-vessel vasculitis, accompanied by markedly elevated rheumatoid factor and anti-cyclic citrullinated peptid

What to do when the first TNF inhibitor fails in rheumatoid arthritis: stratified expert recommendations from a scoping review and Delphi consensus.

BACKGROUND: Despite widespread use of tumor necrosis factor inhibitors (TNFi) as first-line therapy in rheumatoid arthritis (RA), up to 40% of patients fail initial treatment. Subsequent therapeutic choices remain poorly structured, with limited evidence-based guidance to inform individualized post-TNFi decision-making. OBJECTIVE: To develop evidence-informed, profile-based recommendations to guide treatment selection after inadequate response to a first TNFi in RA, combining evidence and expert consensus. DESIGN: Delphi-based consensus study informed by a PRISMA-guided scoping review (ScR)

The Hidden Enemy: A Case Report of Early-Onset Rheumatoid Vasculitis Masquerading as Peripheral Artery Disease.

Rheumatoid vasculitis (RV) is a rare, severe extra-articular manifestation of rheumatoid arthritis (RA), typically occurring after long-standing erosive disease. Advances in RA therapy have reduced its incidence; however, RV can occasionally present early in high-risk patients. We report a man in his thirties from Central India with a 3-year history of inadequately treated seropositive RA, who presented with progressive polyarthritis, neuropathy, and distal ischemic changes. Over five months, he developed worsening joint pain and swelling, followed by tingling, numbness, and subsequently dry g

Livedoid Vasculopathy Mimicking Rheumatoid Vasculitis in a Patient With Active Rheumatoid Arthritis.

Livedoid vasculopathy (LV) is a rare thrombo-occlusive disorder of the dermal microcirculation that may clinically resemble inflammatory vasculitis. In patients with rheumatoid arthritis (RA), the development of painful lower extremity ulcers often raises concern for rheumatoid vasculitis, making histopathological evaluation essential for accurate diagnosis and treatment selection. We present a case of a 45-year-old man with an eight-year history of seropositive RA receiving methotrexate, sulfasalazine, and baricitinib who developed painful lower extremity ulcers, edema, and episcleritis in th

Fatal intrahepatic haemorrhage in a patient with rheumatoid vasculitis: an autopsy case report.

Rheumatoid vasculitis (RV) is an extra-articular complication characterised by small-to-medium vessel vasculitis associated with rheumatoid arthritis, leading to various organ involvements. However, there are few reports of RV associated with aneurysms causing intra-abdominal haemorrhage. Although the incidence of RV has recently decreased, its prognosis remains poor. We herein report a case of RV in a patient with a 1.5-year history of treatment for late-onset rheumatoid arthritis. The patient died of intrahepatic haemorrhage caused by the rupture of a hepatic artery aneurysm. RV can be chall

Rheumatoid variable vessel vasculitis concurrently affecting small, medium and large vessels.

Rheumatoid vasculitis (RV) is a rare and severe extra-articular complication of rheumatoid arthritis (RA), typically occurring in patients with longstanding and erosive RA. It most commonly involves small and medium sized vessels, but it can also affect large vessels. In this case report, we present a patient with de novo RV who developed multisystem vasculitis that affected small, medium and large vessels prior to the diagnosis of RA. The patient had markedly elevated rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) antibodies, confirming the diagnosis of RV. This case

Right colic artery pseudoaneurysm in a patient with rheumatoid arthritis: A case report.

To describe the clinical presentation, diagnosis, and endovascular treatment of a right colic artery (RCA) pseudoaneurysm in a patient with rheumatoid arthritis, highlighting its rarity as a complication of rheumatoid vasculitis. A woman in her early 50s with rheumatoid arthritis presented with a three-day history of epigastric pain, vomiting, and melena. Examination revealed anemia, purpuric rashes, and mild abdominal tenderness. Computed tomography (CT) angiography identified an RCA pseudoaneurysm. Following resuscitation and blood transfusion, selective superior mesenteric artery angiograph

Clinical heterogeneity and prognostic determinants in rheumatoid vasculitis: a systematic analysis of organ-specific manifestations, therapeutic outcomes, and biomarker correlations.

BACKGROUND: Rheumatoid vasculitis (RV), a severe extra-articular complication of rheumatoid arthritis (RA), with current evidence limited to fragmented case reports and a lack of consensus on diagnostic/therapeutic protocols. This study systematically evaluated the clinical heterogeneity, prognostic determinants, and management challenges of RV through the comprehensive analysis of case reports. METHODS: We conducted a systematic review of 112 RV cases from PubMed, Scopus, and Embase in the past 10 years. After dual-reviewer screening and exclusion of confounded cases (concurrent autoimmune d

Vasculitic fasciitis characterizes a distinct subset of vasculitic myopathy with interferon-gamma signature.

Vasculitic myopathy (VM) represents a nonspecific manifestation of various vasculitic syndromes that presents with myalgia, leg tenderness, and muscle weakness. Most cases of VM develop in the context of polyarteritis nodosa (PAN), antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis, or rheumatoid vasculitis, or manifest as single organ vasculitis. Muscle inflammation in VM is poorly understood, and most studies essentially report the presence of vasculitic changes demonstrated by microscopic analysis of skeletal muscle tissue. However, no detailed characterization or in-depth ana

Mononeuritis Multiplex as an Initial Presentation of Rheumatoid Arthritis: A Rare Case Report from Bangladesh.

Rheumatoid vasculitis (RV) is an uncommon extra-articular manifestation of Rheumatoid Arthritis (RA) that typically affects patients with a more aggressive form of the disease. The diagnosis of rheumatoid vasculitis is suggested based on clinical presentation, laboratory findings, and confirmed through biopsy. We describe the case of a 28-year-old man who initially presented with myalgia and weakness in all four limbs. Subsequently, he developed hypertension and testicular tenderness. Neurological examination revealed painful sensory-motor neuropathy involving the bilateral median and ulnar ne

Rheumatoid vasculitis with multianeurysmal bleeding and thrombosis in a patient with long-standing rheumatoid arthritis.

Rheumatoid vasculitis (RV) is a rare but severe complication of long-standing, seropositive rheumatoid arthritis (RA). We present a case of a woman in her 70s with a 32-year history of RA who developed life-threatening vascular complications, including digital ulcers, mononeuritis multiplex, deep vein thrombosis, pulmonary embolism and haemorrhagic shock due to multiple visceral artery aneurysms. CT angiography revealed active mesenteric bleeding, and angiography confirmed aneurysms in the celiac territory. She underwent successful coil embolisation and stabilisation through multidisciplinary

Rheumatoid vasculitis presenting in the absence of rheumatoid arthritis: a case-based review.

Rheumatoid vasculitis (RV) is an extra-articular manifestation (EAM) of rheumatoid arthritis (RA), which is typically observed in long-standing, severe disease. The skin and peripheral nerves are most commonly affected, although the clinical presentation can be highly variable. We report an unusual case of a 58-year-old male who presented with peripheral neuropathy, constitutional symptoms, and Raynaud's phenomenon and was subsequently diagnosed with RV despite having no history of RA. Rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) antibodies were strongly positive, an
