A 52-year-old woman with rheumatoid arthritis experienced painless spontaneous hemorrhage, tearing, and necrosis of the skin on the lower laternal aspect of both legs. Thereapy with systemic prednisone had been used to suppress active synovitis for almost four years and was associated with moon facies, hirsutism, truncal obesity, thinning of the skin, extensive purpura on the extremities, and other manifestations of hypercortisolism. There was no evidence of rheumatoid vasculitis, trauma, or blood dyscrasia. The leg lesions may represent an exaggerated type of steroid-induced purpura resulting

The sites of 92 rheumatoid nailfold lesions were compared with sites of pressure as indicated by tissue blanching during gripping. A close correspondence between sites of blanching and sites of vasculitis was found, and this is discussed in relation to the possible role of vascular compression and blood stasis in the development of rheumatoid vasculitis.

Laser nephelometric estimation of IgM in the eluate fractions following Sepharose 6B chromatography has enabled the calculation of the proportion of low molecular weight IgM (7S IgM) in normal and pathological sera. This figure has then been used to determine the absolute amount of 7S IgM. Twenty-seven of 36 (75%) patients with rheumatoid arthritis had 7S IgM with a mean value of 17 mg/100 ml (170 mg/l) (range 2.5-59 mg/100 ml). No sera from 10 healthy controls were found to contain 7S IgM. Patients with active rheumatoid arthritis had significantly more 7S IgM than those with inactive disease

Nine biopsy specimens taken from the scalps of five children and four adults with subcutaneous necrobiotic granulomas were reviewed. This histopathologic features were similar in both groups. Four of the five children presented clinically with multiple, firm, bound-down subcutaneous nodules limited to the scalp but no associated systemic disease. In addition to the scalp nodules, one child had typical cutaneous lesions of granuloma annulare over the acral areas. Two of the adult patients, however, had severe rheumatoid arthritis with multiple rheumatoid nodules on the arms in addition to the s

Fundus lesions are said not to be found in direct relationship to rheumatoid arthritis. A vasculitis or retinopathy if present is said to be secondary to concomitant hypertension or due to some connective tissue disease other than rheumatoid arthritis. The case is presented of a 64-year-old female with sero-positive rheumatoid arthritis. During a period of exacerbation of her arthritis she developed anterior ischaemic optic neuropathy (A.I.O.N.) resulting in blindness of one eye, followed two weeks later by a temporary occlusive episode in a cilioretinal arteriole in the other eye. The patient

A study of five patients with severe rheumatoid vasculitis treated with cyclophosphamide was undertaken to determine whether immune complexes were present in serum and if their levels correlated with disease activity and response to treatment. Circulating immune complexes were measured by various techniques including the Clq binding and Raji cell radioimmunoassays and determination of the presence of cryoglobulins. Elevated levels of circulating immune complexes, hypocomplementemia, and high titer rheumatoid factor were present during active vasculitis. Clinical and serologic remissions were i

The C1q binding assay and the nephelometric monoclonal rheumatoid factor assay were able to discriminate 79% and 57% respectively of rheumatoid arthritis (RA) patients from healthy blood donors. In addition these assays could distinguish those patients with active arthritis from those with inactive disease, and the C1q binding assay correlated significantly with other laboratory indices of the rheumatoid process, including the erythrocyte sedimentation rate, low molecular weight or 7S IgM, and the rheumatoid factor titre. High levels of C1q binding were also seen in rheumatoid vasculitis. Both

Mild acidic treatment increases the rheumatoid factor titre of some sera and synovial fluids (SF) in rheumatoid arthritis (RA), juvenile RA (JRA) and most frequently in rheumatoid vasculitis. This unmasking of 'hidden' RF in serum and SF samples correlated with the RF-immune complexes (RF-IC) and complexed C4 present in the 3% polyethylene glycol (PEG) precipitates, indicating that by means of 'hidden' RF measurements RF-ICs are possibly detected. This method seems to provide a diagnostic tool for detecting RF-ICs in RA and other related diseases.

Rheumatoid arthritis is a multisystem disease, with many clinical forms bearing close resemblance to systemic lupus erythematosus, dermatomyositis or polyarteritis nodosa. Although the involvement of the integument is not as disabiling as the joint disease, the extent of disability may be of sufficient magnitude to necessitate therapeutic intervention. Two patients are presented whose skin involvement was suggestive of cutaneous angiitis and who responded dramatically to treatment using sulfasalazine.

Rheumatoid arthritis (RA) is a systemic illness defined and classified according to strict criteria of the American Rheumatism Association. Cutaneous disease may complicate RA or the drugs used in its treatment. Cutaneous vasculitis is potentially a prognostic factor in patients with RA. Controversy exists regarding the role of corticosteroids in the development of rheumatoid vasculitis. Two illustrative cases of RA with cutaneous complications are presented and the joint and cutaneous manifestations are reviewed herein.

In three cases of cerebral amyloid angiopathy there was also a chronic cerebral vasculitis characterized by segmental fibrinoid necrosis, chronic adventitial inflammatory infiltrates, obliterative "endarteritis" and hyaline arteriolar change, resembling rheumatoid vasculitis. Two of these cases had rheumatoid arthritis, and one had unspecified "arthritis" at the onset of dementia. Both vasculitis and amyloidosis involved the leptomeningeal and cerebral cortical vessels. In the two autopsy-verified cases, the vascular disease was limited to the brain. In the third case, only a brain biopsy was

Circulating immune complexes have been shown to be common in rheumatoid disease and to relate to severe systemic manifestations of disease. Hypocomplementaemia and complexes detected by anticomplementary activity are found in systemic rheumatoid vasculitis and in other complications such as Felty's syndrome. By contrast elevated complement levels and complexes detected by platelet aggregating activity are common in early and in continuing active rheumatoid disease. The common presence of circulating immune complexes suggests some therapeutic manoeuvres which might be helpful in systemic rheuma

Twelve years after the onset of ankylosing spondylitis, a patient developed severe seropositive rheumatoid arthritis with subcutaneous nodules. This was subsequently complicated by Sjögren's syndrome and rheumatoid vasculitis. The literature is reviewed, and the nature of the association between ankylosing spondylitis and rheumatoid arthritis is discussed.

Four patients with severe anterior scleritis that had not responded to conventional therapy were treated with cytotoxic immunosuppressive alkylating agents alone (one case) or in combination with prednisone (three cases). Two of the patients had Wegener's granulomatosis, one pyoderma gangrenosum, and one rheumatoid vasculitis. All patients responded well to this therapy with a decrease in inflammation and a healing of the sclera. Other systemic manifestations of the underlying diseases often responded well also.

With the great interest in recent years in the use of immunosuppressive/antiinflammatory drugs for the treatment of severe systemic inflammatory disorders, it is surprising that, except in Wegener's granulomatosis, the use of methotrexate (MTX) in disseminated vasculitides has been neglected. We have treated three patients with MTX, each with a different form of severe, corticosteroid-resistant, life-threatening, disseminated vasculitis. One had a nonspecific vasculitis characterized by excruciating muscle pain, weakness, and atrophy; the second had rheumatoid vasculitis with extensive skin ul
