Rheumatological diseases (RDs) are characterized by autoimmune-mediated organ damage, with the lungs being an important target organ. Almost all compartments of the pulmonary system may be involved, with considerable heterogeneity in the frequency, severity, and spectrum of pulmonary manifestations. This heterogeneity may result in delayed diagnosis and treatment, leading to adverse outcomes. The objective of this study was to characterize the clinical, demographic, serological, and radiological features of pulmonary manifestations across a spectrum of rheumatic diseases. This cross-sectional

Rheumatoid vasculitis (RV) is an uncommon but potentially fatal extra-articular manifestation of rheumatoid arthritis (RA), characterized by severe systemic inflammation and high mortality despite advances in disease-modifying therapies. We describe a 52-year-old woman with long-standing seropositive RA and irregular use of methotrexate who presented with progressive digital ischemia and extensive necrotic cutaneous ulcers, in the absence of active synovitis. Histopathology revealed small-vessel vasculitis, accompanied by markedly elevated rheumatoid factor and anti-cyclic citrullinated peptid

OBJECTIVES: Rheumatoid arthritis (RA) is a chronic autoimmune disease with complex etiopathogenesis, and variable clinical course. The human gastrointestinal (GI) tract is a dynamic ecosystem in which micro-organisms influence metabolism, immune signalling, and pathogen resistance. Understanding these interactions is key to exploring the microbiota's role in RA. This review examines how intestinal and oral microbiota differ in RA patients and influence disease activity and treatment response.
METHOD: A literature search was conducted using PubMed, from inception until January 3, 2025. Studies

BACKGROUND: Sjögren's disease (SjD) is a chronic autoimmune disease featured by lymphocytic infiltration of exocrine glands and numerous organ involvements, but central nervous system involvement is relatively rare. Due to the variety of symptoms, diagnosis is particularly difficult under certain conditions, which may delay the optimal timing of treatment. Increased clinical awareness and effective pharmacotherapies are urgently needed.
CASE PRESENTATION: A case of a 39-year-old female was initially considered to have an intracranial occupancy based upon rapidly progressing nervous symptoms i

Low-dose methotrexate (MTX) (5-25 mg/week) is widely used in the treatment of autoimmune diseases and is considered relatively safe. There have been reports of toxicities caused by MTX, but the characteristics remain unclear. Through a retrospective case series at our hospital, we describe the clinical features, management, and outcomes of adverse reactions caused by MTX. This is a retrospective, single-center case series of patients with rheumatoid arthritis (RA) who developed adverse reactions while receiving low-dose MTX (5-25 mg/week) between March 2022 and August 2025. Exclusion criteria

Rheumatic diseases are complex systemic disorders characterized by multi-organ involvement and diverse laboratory abnormalities. The diagnostic approach involves a comprehensive evaluation of clinical and serological markers and the exclusion of conditions with overlapping presentations, such as other rheumatic diseases and systemic infections. Visceral leishmaniasis (VL) is a protozoan infection that primarily affects the reticuloendothelial system. Direct effects of parasite infiltration of the reticuloendothelial system, combined with the subsequent host immune response, produce clinical an

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Epstein-Barr virus (EBV) is a ubiquitous herpesvirus, increasingly implicated in the pathogenesis of several autoimmune diseases, such as multiple sclerosis (MS), systemic lupus erythematosus (SLE), and rheumatoid arthritis (RA). These diseases have both, shared and disease-specific immunopathogenic pathways involving EBV. Shared mechanisms include the role of EBV latent proteins in triggering immune dysfunction, molecular mimicry between viral and self-antigens, infection of autoreactive B-cells, and dysfunction of type 1 interferon (IFN-1) responses. In MS, EBV is associated with a CNS-compa

Rheumatoid Arthritis (RA) is among the most common systemic diseases of connective tissue, affecting 1,5% of the population. In recent years about 30 000 new cases of RA have been reported annually. The rheumatoid process has a progressive course, leading to system-wide destruction of bones and joints, along with damage to many other organs and systems. The treatment team involved in managing cases of RA must be composed of specialists from many different fields: orthopedic surgeons, rheumatologists, physiotherapists, social workers, occupational therapists, and visiting nurses.<br /> The trea

Tripterygium glycosides (TG) is a widely used preparation in the treatment of rheumatoid arthritis (RA), nephrotic syndrome and diabetic nephropathy. Although the clinical efficacy is definite, the side-effects on reproductive system limit its wide application. It is of great significance to take measures to alleviate its reproductive toxicity and expand its clinical use. The mechanism of TG-induced reproductive toxicity involves oxidative stress, inflammation, apoptosis, and metabolism imbalance, which lead to adverse effects on male and female reproductive organs. To mitigate these effects,

Rheumatoid arthritis (RA) is a systemic autoimmune disease that affects millions of people worldwide and is characterized by persistent inflammation, pain, and joint destruction. In RA, the dysregulation of the immune system is well documented. However, the genetic basis of the disease is not fully understood, especially when extra-articular organs are involved. Interstitial lung disease (ILD) is a major cause of morbidity and mortality in patients with RA. Notably, RA-ILD shares several risk factors with idiopathic pulmonary fibrosis (IPF), namely male gender, smoking history, usual interstit

UNLABELLED: Musculoskeletal disorders are age-related illnesses that substantially impact the locomotor system. These problems can lead to serious complications, such as fractures, which can severely compromise the quality of life of patients. With the aging of the global population, the incidence and prevalence of musculoskeletal disorders are gradually increasing. Proliferation, differentiation, and cellular fate are extensively regulated by the BMP signaling pathway, which is expressed in approximately all organ systems. Musculoskeletal diseases are generally influenced by BMP signaling, wh

Hemophagocytic lymphohistiocytosis (HLH) is an uncommon but potentially life-threatening condition where the body's immune system becomes overactive, causing widespread inflammation and organ damage. In this report, we present the case of a 21-year-old woman with a history of rheumatoid arthritis who developed secondary HLH, triggered by acute interstitial pneumonitis (AIP). This case sheds light on the importance of early recognition and intervention in managing this rare condition. It emphasizes the need to consider HLH in patients presenting with unexplained systemic inflammation similar to

The new coronavirus pandemic has been ongoing for nearly five years. In addition to the severe symptoms in the acute phase, it is accompanied by long-term complications and sequelae involving the respiratory, neurological, immune, circulatory, and gastrointestinal systems for several months or even years, which is called the Long COVID. Many studies have suggested that systemic chronic inflammation caused by residual viral components may be one of the pathophysiologic mechanisms of Long COVID. In this paper, we will review the autoimmune diseases caused by chronic inflammation. In particular,

INTRODUCTION: Autoimmune diseases often present in a systemic manner, affecting various organs and tissues. Involvement of the central and peripheral nervous system is not uncommon in these conditions and is associated with high morbidity and mortality. Therefore, early recognition of the neuropsychiatric manifestations associated with rheumatologic diseases is essential for the introduction of appropriate therapies with the objective of providing a better quality of life for individuals.
OBJECTIVE: To provide a literature review of the neuropsychiatric manifestations related to Systemic Lupu

BACKGROUND: Sjögren's disease is characterised by mucosal dryness, fatigue, chronic pain, systemic organ involvement, and elevated autoreactive IgG antibodies. There are no approved disease-modifying treatments. Therefore, we aimed to evaluate nipocalimab, a neonatal Fc receptor blocker that reduces circulating IgG, including autoantibodies, in patients with Sjögren's disease.
METHODS: This phase 2, double-blind, multicentre trial enrolled individuals with moderate-to-severe, active Sjögren's disease (ie, Clinical European League Against Rheumatism Sjögren's Syndrome Disease Activity Index [C

BACKGROUND: Periostin is an extracellular matrix protein that participates in tissue remodeling and inflammation and may be related to cardiometabolic abnormalities. However, its association with metabolic and lipid disorders in newly diagnosed type 2 diabetes mellitus (T2DM) patients with dyslipidemia is poorly defined. This study evaluated serum periostin and its associations with glycemic, lipid, and atherogenic measures. We also performed exploratory molecular docking to investigate its predicted interactions with metformin and atorvastatin.
METHODS: This was a case-control study with 45

PURPOSE OF REVIEW: Primary cutaneous lymphomas (PCL) are an uncommon malignancy of the lymphocytes, primarily presenting with dermatologic lesions. Central nervous system(CNS) metastatic manifestations, are even rarer. This review focus mainly on three aspects namely early suspicion of CNS involvement, selection of cases for CNS chemo-prophylaxis and lastly, the rare occurrence from skin straight to brain without other organ involvement.
RECENT FINDINGS: Primary extranodal large B-cell lymphomas are very heterogeneous. Recent molecular data have thrown some light on such divergent clinical be
