Abatacept versus hydroxychloroquine for prevention of rheumatoid arthritis in individuals with palindromic rheumatism: a randomized open-label trial.

A substantial proportion of individuals with palindromic rheumatism develop rheumatoid arthritis (RA). This randomized, open-label, multicenter trial aimed to assess whether 2 years of treatment with abatacept (n = 34; 125 mg subcutaneous injections weekly during the first year and every 2 weeks during the second year) compared with oral hydroxychloroquine (n = 36; 5 mg kg-1 per day) could reduce the frequency of RA development in individuals with palindromic rheumatism positive for rheumatoid factor and/or anticitrullinated protein antibody. The primary outcome was the development of persiste

Rheumatoid arthritis and airway hyperresponsiveness: A GWAS-based mendelian randomization study.

Previous observational studies have indicated a link between rheumatoid arthritis (RA) and airway hyperresponsiveness diseases (AHDs). However, the causality of this relationship remains uncertain. Our research aims to investigate the causal relationship between RA and AHDs. Using publicly available genome-wide association study summary statistics, we selected single nucleotide polymorphisms associated with RA (including overall RA, seropositive RA and seronegative RA), and AHDs (including asthma, bronchitis, allergic rhinitis and chronic obstructive pulmonary disease [COPD]) as instrumental v

Fulminant Rheumatoid Vasculitis With Digital Necrosis and Cutaneous Ulcerations: A Case Report.

Rheumatoid vasculitis (RV) is an uncommon but potentially fatal extra-articular manifestation of rheumatoid arthritis (RA), characterized by severe systemic inflammation and high mortality despite advances in disease-modifying therapies. We describe a 52-year-old woman with long-standing seropositive RA and irregular use of methotrexate who presented with progressive digital ischemia and extensive necrotic cutaneous ulcers, in the absence of active synovitis. Histopathology revealed small-vessel vasculitis, accompanied by markedly elevated rheumatoid factor and anti-cyclic citrullinated peptid

Staphylococcus aureus Pyogenic Spondylodiscitis in a Patient With Rheumatoid Arthritis Receiving Rituximab.

We report the case of a 55-year-old woman with seropositive rheumatoid arthritis (RA) treated with methotrexate and rituximab (RTX) who developed severe inflammatory low back pain one month after her fourth RTX cycle. MRI of the lumbosacral spine revealed L4-L5 infectious spondylodiscitis with vertebral endplate destruction, anterior epidural extension, and a right psoas abscess. CT-guided aspiration confirmed methicillin-sensitive Staphylococcus aureus infection. Empirical IV therapy with ciprofloxacin and gentamicin was initiated per local protocol. Following microbiological confirmation, ci

Improving Lipid Screening Adherence in Inflammatory Arthritis Using a Two-Cycle Plan-Do-Study-Act (PDSA) Quality Improvement Intervention.

INTRODUCTION: Autoimmune diseases, such as rheumatoid arthritis (RA) and spondylarthritis (SpA), including ankylosing spondylitis (AS) and psoriatic arthritis (PsA), are characterized by chronic systemic inflammation and are associated with significant cardiovascular morbidity and mortality. Dyslipidemia is a well-established contributor to this increased cardiovascular risk and represents an important comorbidity in patients with rheumatologic disease. STUDY AIM: This pre-post interventional quality improvement (QI) study used two iterative Plan-Do-Study-Act (PDSA) cycles at a high-volume, a

From Dyspnea to Diagnosis: Neutrophilic Pleural Effusion With Elevated Adenosine Deaminase As the First Clue to New-Onset Rheumatoid Arthritis.

Rheumatoid arthritis (RA) rarely announces itself with a pleural effusion, yet when it does, the biochemical and pleural fluid profile often leads to a suspicion of infectious etiology at first. We describe a 44‑year‑old woman whose initial manifestation of seropositive RA was a large, unilateral, neutrophil‑rich, adenosine‑deaminase (ADA)-positive pleural effusion. The case draws attention to a diagnostic pitfall: elevated pleural fluid ADA-although classically associated with tuberculous pleuritis-can also occur in rheumatoid pleuritis, highlighting that ADA levels must be interpreted in the

Mounting Challenges: A Tragic Case of Rheumatoid Arthritis-Associated Interstitial Lung Disease in an Incarcerated Patient, Highlighting Healthcare Access Barriers in Correctional Settings.

Rheumatoid Arthritis-associated Interstitial Lung Disease (RA-ILD) is a serious extra-articular manifestation associated with significant morbidity and mortality. We present the case of a 48-year-old incarcerated male with seropositive rheumatoid arthritis who was clinically stable on tofacitinib and methotrexate for over one year. Following the discontinuation of tofacitinib, which the patient attributed to correctional facility formulary changes and restrictions, he developed progressive respiratory symptoms. He presented to the hospital two months later with acute hypoxic respiratory failur

Paraneoplastic Arthritis Mimicking Late-Onset Rheumatoid Arthritis in an Older Smoker: A Diagnostic Challenge.

Paraneoplastic rheumatic syndromes may present with inflammatory arthritis that closely mimics rheumatoid arthritis (RA), leading to diagnostic delay. We describe an older male with progressive inflammatory polyarthritis and a low-titer positive rheumatoid factor (RF), initially diagnosed with late-onset seropositive RA. Although his arthritis improved with glucocorticoids and hydroxychloroquine, persistent respiratory symptoms prompted further evaluation. High-resolution computed tomography (HRCT) of the chest revealed progressive fibrotic interstitial lung disease (ILD) with an indeterminate

Cavitary Pulmonary Nodules Associated With Leflunomide Therapy in Rheumatoid Arthritis: A Case Report and Literature Review.

Rheumatoid arthritis (RA) may present with extra-articular pulmonary manifestations, including rheumatoid lung nodules. Leflunomide is an effective disease-modifying antirheumatic drug, but increasing case reports suggest an association with accelerated pulmonary nodulosis. We describe a 61-year-old woman with long-standing seropositive RA who developed left-sided lower chest pain, generalized weakness, fatigue, and loss of appetite after switching from methotrexate to leflunomide 20 mg/day. CT imaging demonstrated multiple bilateral cavitary nodules predominantly in the lower lobes. Extensive

Inhalant Exposures and the Risk of Rheumatoid Arthritis and Rheumatoid Arthritis-Associated Interstitial Lung Disease.

Rheumatoid arthritis (RA) is a systemic autoimmune disease with substantial extraarticular morbidity and mortality, particularly from rheumatoid arthritis–associated interstitial lung disease (RA-ILD). Although advances in disease-modifying therapies have improved joint outcomes and overall survival, RA-ILD remains a major contributor to premature death. Emerging evidence supports the mucosal origins hypothesis, which proposes that RA-related autoimmunity may be initiated at extraarticular sites, particularly the lungs following inhalational exposures. This narrative review synthesizes the cur

Non-ST-Segment Elevation Myocardial Infarction (NSTEMI) in a High-Risk Rheumatoid Arthritis Patient on Long-Term Baricitinib: Drug Signal or Risk Factor Convergence?

Rheumatoid arthritis (RA) is associated with increased cardiovascular (CV) morbidity and mortality, partly related to chronic systemic inflammation and accelerated atherosclerosis. Janus kinase (JAK) inhibitors, including baricitinib, are effective targeted therapies for RA, but their use in older patients and those with baseline CV risk factors requires careful risk assessment. We report a clinically detailed case of non-ST-elevation myocardial infarction (NSTEMI) during long-term baricitinib therapy and use it to discuss real-world CV risk stratification, monitoring, and pharmacovigilance in

Utility of the Early Sjögren Antibody Panel as a Diagnostic Marker for Sensory Neuropathy.

INTRODUCTION/AIMS: .: Primary Sjögren syndrome (SS) is a chronic autoimmune disease characterized frequently by sensory neuropathy (SN). Carbonic Anhydrase-6 (CA-6), Parotid Secretory Protein (PSP), and Salivary Protein-1 (SP-1) antibodies (Early Sjögren antibodies [ESA]) are found in 45% of SS patients who lack traditional antibodies (Ro/La). But there is a lack of information regarding whether ESA are associated with SN, and if there are any identifying characteristics associated with ESA in SN. Our goal was to close this knowledge gap. METHODS: All SN patients tested for ESA from May 2023-

Coexistence of Sjögren's disease and Takayasu arteritis in a woman: a case-based review.

Sjögren's disease (SjD) is a chronic systemic autoimmune disorder primarily affecting the exocrine glands, while Takayasu arteritis (TAK) is a large-vessel vasculitis involving the aorta and its major branches. Although SjD frequently overlaps with other autoimmune diseases, its coexistence with TAK has been reported only rarely. A 47-year-old Turkish woman with seropositive SjD presented with constitutional symptoms, upper extremity claudication, and asymmetric peripheral pulses. Laboratory evaluation revealed elevated inflammatory markers while antiphospholipid antibodies were negative. Comp
