Nitroblue tetrazolium (NBT) test, widely performed to investigate the physiology and function of blood polymorphonuclear leukocytes, was evaluated using blood and synovial fluid cells of 20 patients with rheumatoid arthritis and 19 patients with osteoarthritis. There were no significant differences among two joint diseases and healthy controls in NBT test values on blood polymorphonuclear leukocytes. Evidence developed over the years has suggested that lymphocyte depletion and removal of plasma factors can ameliorate rheumatoid arthritis. In our studies of 40 patients, a subset of patients that respond best to 20 therapeutic lymphoplasmapheresis over 11 weeks has emerged. These are functional Class III patients with seropositive, erosive progressive disease who have little deformity. OM 8930, a bacterial product with immunostimulatory capacities, was studied for its efficacy in the long-term treatment of rheumatoid arthritis (RA). Ten (10) patients with active seropositive rheumatoid arthritis were treated intermittently for six months with the agent and were evaluated according to clinical, laboratory and immunological parameters. DNA levels were measured in synovial fluids and sera of 106 patients with rheumatoid arthritis (RA), osteoarthritis (OA), gout, pseudogout, and posttraumatic arthritis (TRA). Synovial tissue cells from patients with seropositive rheumatoid arthritis bind dexamethasone in a similar fashion to the classical steroid receptors on other cells. In contrast, synovial fluid cells and blood PMNLs from these patients bind dexamethasone with much reduced affinity. Some component of plasma may interfere with the binding of dexamethasone to its receptor.

Synovectomy of the knee was performed in a 27-year-old female with seropositive rheumatoid arthritis of 6 years duration. The photometric latex test (PLT) for the detection of rheumatoid factors (RF), developed and clinically evaluated in adult rheumatoid arthritis, was applied in a study of juvenile rheumatoid arthritis (JRA). A total of 40 individuals with JRA, 27 with other collagen diseases and 24 in a control group were examined. A solid-phase radioimmunoassay was developed to detect immunoglobulin (Ig)E antibodies that bound to human IgG. IgE-rheumatoid factor activity was found in the serum of 18 of 20 patients with seropositive rheumatoid arthritis, 1 of 4 patients with seronegative rheumatoid arthritis, 3 of 32 patients with seronegative rheumatoid arthritis, 3 of 32 patients with asthma, and in 1 patient with hypocomplementemic vasculitis and iodide sensitivity. Data of 46 patients with juvenile chronic arthritis were evaluated. All of them were under the age of 16 at the time of study. Patients with juvenile ankylosing spondylitis, psoriatic arthritis, and arthritis associated with inflammatory bowel disease were excluded from this series. A double-blind study was made on the effectivenss of erbium169 synoviorthesis in the digital joints of 7 patients with erosive, seropositive rheumatoid arthritis. The study involved 70 joints observed over a 12-month period: 20 pairs of metacarpophalangeal joints (MCP) and 15 pairs of proximal interphalangeal joints (PIP) were injected with erbium169 in one joint and saline in the other one.
