Detection of circulating and intra-articular soluble immune complexes, has been done in 28 rheumatoid arthritis, 8 mixed connective tissue diseases and 8 other connective tissue diseases. The method used, is based on a competition reaction in the antibody dependent lymphocytotoxicity reaction (ADC). In rheumatoid arthritis serum immune complexes were detected in 75% of the seropositive cases and 66% of the seronegative ones. The mean level of complexes is higher in the former form of the disease. In a given patient, the concentration of immune complexes in joint fluid, is higher than in serum.

Particulate extracellular aggregates of immunoglobulin can be demonstrated in many whole synovial fluids from patients with rheumatoid arthritis but not in other diseases. These occur only in seropositive patients and correlate with more active inflammation and lowered synovial fluid C3. Aggregates were associated with intracellular inclusions but were more abundant and always contained both IgG and IgM. No aggregates were seen in 50 control fluids. These aggregates are compatible with immune complexes and should be included in studies of immunologic events in rheumatoid fluids.

In a consecutive series of patients with advanced, seropositive rheumatoid arthritis, 112 severely damaged knees were reconstructed with a stabilizing, total resurfacing prosthesis in 80%, a hemiprosthesis in 13% (3% biocompartmental) and a hinge prosthesis in 7%. Agranulocytosis, which must be differentiated from leukopenia, was seen as the most severe side effect of levamisole in the treatment of rheumatoid arthritis (RA). Eight female patients with drug-induced agranulocytosis (five patients with definite seropositive rheumatoid arthritis (RA), three patients with upper respiratory infections) were studied for the presence of HLA-B27 and antinuclear antibodies (ANA). A new procedure, the radioimmunoprecipitation--P.E.G. assay (RIPEGA) is proposed for the detection of anti-IgG antibodies (anti-IgG Ab). This sensitive and reproducible test was performed by incubation of radio-labelled human IgG (125I-IgG) with patient's sera. Separation of free from complexed 125I-IgG was achieved by a 7% P.E.G. precipitation. Preliminary results demonstrated anti-IgG Ab in 84.4% of the cases of sero-positive rheumatoid arthritis (R.A.), 85% of seronegative R.A., 43% of other forms of inflammatory arthritis, 50% of unclassified arthritis and in 10.5% of the control subjects.

D-penicillamine treatment of 50 patients with rheumatoid arthritis with negative ANF tests, was followed by means of protein disk electrophoresis of the urine. Twenty-six ankles of 23 patients with seropositive or negative arthritis and 24 ankles of 12 cadavers as a control group were studied by contrast synoviography. The main features in the patients were capsule enlargement, hypertrophic synovitis, and connections to tendon sheaths and to the posterior subtalar joint. None of the controls showed capsule distension or irregular outline of the joint cavity, but connections to tendon sheaths and/or to the posterior subtalar joint were present. The immuno-modulatory effect of Levamisole in the treatment of rheumatic diseases was studied in an open trial.
