The coexistence of active rheumatoid arthritis and active primary biliary cholangitis represents a distinct therapeutic challenge. Evidence-based guidance for treatment selection in this clinical setting remains limited, and published experience with tocilizumab is restricted to isolated case reports. We report a 59-year-old woman with highly active, seropositive rheumatoid arthritis and marked cholestatic biochemical activity, in whom diagnostic evaluation revealed active primary biliary cholangitis in the absence of advanced liver fibrosis. Following multidisciplinary assessment, methotrexat

Tumor necrosis factor-alpha (TNF-α) inhibitors are effective therapies for rheumatoid arthritis (RA) but may rarely induce paradoxical autoimmune complications, including vasculitis. We report the case of a 53-year-old woman with seropositive RA and secondary Sjögren's syndrome who developed bilateral dependent petechial and purpuric lesions of the lower extremities after approximately 33 months of etanercept therapy at a dose of 25 mg twice weekly. At symptom onset, her RA was well controlled, with no clinical or ultrasonographic synovitis and a Disease Activity Score in 28 joints of 2.1. Ski

We report a case of elbow pigmented villonodular synovitis (PVNS) combined with seronegative rheumatoid arthritis (SNRA) and review relevant literature to summarize the characteristics, treatment, and outcomes of this comorbidity. The patient was a 72-year-old female. She developed swelling and pain of the right elbow in 2002, and underwent surgical treatment. Postoperative histopathology confirmed PVNS. In 2007, she developed swelling and pain in the left elbow and underwent synovectomy, and synovectomy again yielded pathology consistent with PVNS. From 2021 onwards, she experienced recurrent

Background/Objectives: Neurological manifestations in rheumatoid arthritis (RA) are common, yet the clinical relevance of central nervous system (CNS) abnormalities detected on neuroimaging remains uncertain. We aimed to determine the prevalence and patterns of cranial magnetic resonance imaging (MRI) findings in RA patients treated with rituximab (RTX) and to assess whether these abnormalities are associated with disease-specific mechanisms or with age- and comorbidity-related factors. Methods: In this retrospective observational study, 84 RA patients who received RTX and had available pre-tr

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is an immune-mediated neuropathy that rarely co-occurs with rheumatoid arthritis (RA). When CIDP proves refractory to first-line therapies, management becomes particularly challenging, especially in patients with multiple autoimmune conditions. A 43-year-old Chinese male with a 3-year history of seropositive RA in sustained remission presented with acute progressive quadriparesis, bilateral ptosis, and facial diplegia. Cerebrospinal fluid analysis revealed albuminocytologic dissociation, and nerve conduction studies demonstrated

The protozoan parasite Toxoplasma gondii is the cause of the widespread but sometimes disregarded parasitic illness toxoplasmosis. Cytokines play an indispensable role in fostering an environment that is more conducive to pregnancy. Their intricate network of interactions allows for the observation of numerous modulatory consequences, such as cell migration, invasion, and gene expression. Among these, CCL2 (MCP-1) and CXCL16 have emerged as promising immunological biomarkers reflecting disease activity, inflammation, and tissue involvement. Monocyte chemotactic protein 1 (MCP-1), another name

PURPOSE: To describe a case of rheumatoid arthritis (RA)-associated orbital and intracranial inflammatory disorder (OIID).
BACKGROUND: RA is a systemic autoimmune disease associated with a multitude of extra-articular manifestations. Cigarette smoking increases the risk of RA vasculitis and adversely influences the expression of the proto-oncogene p53 in RA. Although ocular manifestations of RA are well known, orbital and intracranial inflammatory presentations are much less common.
CASE PRESENTATION: A 63-year-old woman with seropositive RA and a significant tobacco smoking history presente

OBJECTIVES: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) affects 5-10% of patients with RA. While lung ultrasound (LUS) shows promise for RA-ILD detection, its role remains inconclusive. We aimed to assess its performance as a screening tool in at-risk RA patients.
METHODS: We made a cross-sectional study of RA patients, including i) individuals >50 years old with ≥1 risk factor for ILD, and ii) patients with new-onset respiratory symptoms. All patients underwent clinical examination, pulmonary function testing (PFT), LUS, and chest high-resolution computed tomography (H

BACKGROUND & AIMS: Dietary interventions may improve rheumatoid arthritis (RA) outcomes through anti-inflammatory and metabolic pathways. This study aimed to evaluate the effects of the Dietary Approaches to Stop Hypertension (DASH) diet on disease activity and treatment outcomes in patients with seropositive RA.
METHODS: In this 12-week parallel-group randomized controlled trial, 82 patients with seropositive RA were randomly assigned to a DASH diet intervention (n = 40) or a control group maintaining their usual diet (n = 42). The primary outcome was change in disease activity assessed usin

Elderly-onset rheumatoid arthritis (EORA) and polymyalgia rheumatica (PMR) are among the most frequent inflammatory rheumatic diseases in patients over 50, yet their clinical differentiation, particularly when EORA presents with polymyalgic onset, poses a common diagnostic challenge. This systematic review aimed to identify the pathophysiological, clinical, imaging and treatment-response features that distinguish EORA from PMR. MEDLINE, CENTRAL and Scopus (inception to January 2026), with supplementary searches of LILACS, Google Scholar and trial registries (June 2026) were searched, including

Haemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome caused by uncontrolled immune activation. In adults, it is usually secondary to an underlying condition, such as infection, malignancy, or autoimmune disease. Identifying this trigger is essential, as treatment depends on the cause. Visceral leishmaniasis (VL) is a systemic parasitic infection that may mimic or trigger HLH, particularly in immunosuppressed patients. We report a 59-year-old man with seropositive rheumatoid arthritis, for which he received immunosuppressive treatment with methotrexate and

Reverse total shoulder arthroplasty (RTSA) in very young adults with rheumatoid arthritis (RA) is uncommon because of concerns regarding implant longevity and lifetime revision burden, and outcomes data in patients younger than 30 years remain limited. We report the case of a 25-year-old woman with seropositive RA who developed rapid, destructive monoarticular involvement of the right shoulder despite treatment with disease-modifying antirheumatic drugs (DMARDs) and biologic therapy. Imaging demonstrated complete glenohumeral joint space obliteration, humeral head collapse, erosive changes, an

Tenosynovitis is a recognized feature of rheumatoid arthritis (RA), yet it typically presents alongside symmetric polyarticular synovitis. Isolated focal tenosynovitis as the primary and initial manifestation of RA is uncommon and frequently leads to diagnostic delays. We present the case of a 62-year-old female who presented with localized right ankle pain in the absence of systemic joint involvement or elevated inflammatory markers. While the rheumatoid factor was negative and both the erythrocyte sedimentation rate and C-reactive protein levels were within normal limits, musculoskeletal ult

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized primarily by inflammatory polyarthritis, although extra-articular manifestations, including pericardial disease, are well recognized. While pericardial effusions are relatively common, large symptomatic effusions causing cardiac tamponade as the initial manifestation of RA are exceedingly rare. We report a 75-year-old woman with hypertension and dyslipidemia who presented with progressive dyspnea and fatigue one month after an episode of right-hand monoarthritis with an initially unrevealing rheumatologic evaluation. OBJECTIVE: We examined associations of post-traumatic stress disorder (PTSD) and other mental health disorders (OMH) with rheumatoid arthritis (RA), accounting for effects of smoking.
METHODS: We conducted a matched case-control study, identifying incident RA cases and controls using national Veteran Health Administration data (2006-2019). PTSD/OMH exposures prior to RA diagnosis were identified using administrative/diagnostic codes. Conditional logistic regression examined associations of RA (vs. no-RA) with PTSD (±OMH) and OMH. Relative Excess Risk of Interaction was calculated between OMH/

OBJECTIVE: Autoimmune processes in rheumatoid arthritis (RA) begin years before clinical symptom onset. This study aimed to analyse inflammation-related proteins in individuals during the pre-symptomatic phase, in relation to healthy controls and patients with early RA, and their relationship to seroconversion.
METHODS: Plasma samples collected prior to symptom onset from individuals who later developed RA (n = 419), healthy population controls (n = 49), and patients with early RA at diagnosis (n = 68) were obtained from a biobank. All samples were analysed for 92 inflammation-related protein

BACKGROUND: Plasma calprotectin is a marker of neutrophil activity. Its relation to the clinical phenotype and autoantibodies in rheumatoid arthritis (RA) is only partly understood. We therefore aimed to investigate the association between calprotectin and clinical parameters, CRP, ESR, anti-CCP2 and other anti-modified protein autoantibodies (AMPAs) in patients with RA.
METHODS: Rheumatoid factor (RF), anti-cyclic citrullinated peptide 2 (CCP2) isotypes and plasma calprotectin were centrally measured in a cross-sectional cohort of 4521 Scandinavian RA patients using fluoroenzyme immunoassays
