A correlation has previously been observed between the presence of enhancing complexes and cutaneous vasculitis in rheumatoid arthritis. Two parients with seropositive juvenile rheumatoid arthritis are described in whom enhancing complexes were detected before the appearance of cutaneous vasculitis. Their contrasting response to penicillamine is discussed in relation to the role of of rheumatoid factor and antinuclear antibodies.

The synovial fluid beta1a (C3) and beta1e (C4) expressed by their ratio to corresponding serum concentrations) were studied in 31 cases of seropositive rheumatoid arthritis (RA+), 5 cases of seronegative rheumatoid arthritis (RA-1), and 15 cases of osteoarthrosis (OA) before osmic acid synoviorthesis on knees. This was repeated after synoviorthesis in the synovial fluid (SF) of 24 RA+, 4 RA-, and 10 OA patients. The following studies were undertaken: (a) the relationship between these components; (b) their correlation with the SF protein concentration and rheumatoid factor titre, when present.

In an earlier study, the mean plasma carcinoembryonic antigen (CEA) level of patients with seropositive rheumatoid arthritis (RA) was found to be significantly higher than that of normal, control subjects. Five cases of aortic incompetence and nodular seropositive rheumatoid arthritis are presented. Four cases underwent aortic valve replacement. Two of these had granulomatous involvement of the aortic cusps similar to subcutaneous rheumatoid nodules, and another showed a nonspecific fibrosis. IgG antiglobulins in human sera were measured by single radial diffusion in agar after elution from ethyl chloroformate insolubilized immunosorbents of rabbit or human IgG. With both immunosorbents seronegative and seropositive rheumatoid sera contained significantly higher levels of IgG antiglobulins than did sera from healthy adults. Attempts to measure antiglobulins interacting specifically with the pFc' half of the Fc region were not successful due to an unacceptably high degree of non-antibody binding.

Persons having positive tests for rheumatoid serum factors in population samples in Leigh (Lancashire) and Wensleydale (Yorkshire) were investigated for evidence of infection. Using a new in vitro method of measuring the chemotaxis of polymorphonuclear leukocytes from peripheral blood, a chemotactic index has been calculated. Complexes of IgG—rheumatoid factor (RF) and IgG—β1C were demonstrated in the synovial membrane obtained from patients with rheumatoid arthritis (RA) by a mixed immunofluorescence method that shows simultaneously the two components of the complex. IgG—RF was found only in the synovia of cases with circulating RF, while IgG—β1C deposits could be detected in both seropositive and seronegative cases. These findings are discussed in connection with the pathogenesis of rheumatoid synovitis in the light of the immune complex pathogenetic mechanism.

Complement-mediated lysis of sheep erythrocytes coated with optimal concentrations of rabbit IgG hemolysin was inhibited by euglobulin fractions from the sera of patients with seropositive rheumatoid arthritis.
