Septic arthritis following joint injection/aspiration is an uncommon and challenging orthopaedic disease. We report a unique case of a 58-year-old non-diabetic, non-obese female with no prior shoulder surgery who developed septic arthritis of the right shoulder due to the anaerobic bacterium Cutibacterium acnes (C. acnes), identified from deep-tissue samples obtained during arthroscopic debridement. The case is notable for a history of pseudogout and multiple recent intra-articular injections in the affected shoulder. The patient received four shoulder joint (glenohumeral) ozone/prolozone inje

Secondary capillary leak syndrome (SCLS) is a rare and frequently underrecognized cause of progressive edema, hypoalbuminemia, and noncardiogenic pulmonary edema. We present a case of a 55-year-old man with seronegative rheumatoid arthritis and antisynthetase syndrome on chronic immunosuppression who developed progressive bilateral lower-extremity edema and pulmonary edema in the absence of cardiac, renal, or hepatic disease. Extensive evaluation excluded alternative etiologies. The patient met diagnostic criteria for SCLS and demonstrated clinical improvement with albumin-assisted diuresis. T

Methotrexate (MTX) is a folate antagonist widely used as a disease-modifying antirheumatic drug (DMARD). Despite its efficacy and broad therapeutic range, its narrow safety margin and reliance on renal clearance confer risk for severe multisystem toxicity, particularly in patients with renal impairment, comorbidities, or dosing errors. We describe a 74-year-old male with coronary artery disease, type 2 diabetes, stage 3a chronic kidney disease, obesity, and seronegative rheumatoid arthritis who presented with progressive weakness, painful oral and genital ulcers, and three weeks of watery diar

Benralizumab, an anti-interleukin-5 receptor α monoclonal antibody, induces near-complete eosinophil depletion and provides substantial benefit in severe eosinophilic asthma. We report an 80-year-old woman with severe eosinophilic asthma who was treated with benralizumab from August 2020. Peripheral blood eosinophils rapidly decreased to 0/μL, asthma exacerbations ceased, and lung function improved. Over the subsequent four years, while asthma remained well controlled, she developed a temporally ordered sequence of clinical events, including elderly-onset seronegative rheumatoid arthritis, bio

BACKGROUND: Magnetic resonance cholangiopancreatography (MRCP) is useful to assess exocrine pancreatic function by combining rapid imaging acquisition with the administration of secretin, a gastrointestinal hormone that stimulates the secretion of bile and pancreatic juice. However, extensive data on this method are lacking. This study aimed to determine whether MRCP with secretin administration is able to simultaneously detect alterations of both the pancreatic ducts and exocrine pancreatic function.
METHODS: All subjects older than 18 years who underwent magnetic resonance imaging (MRI) and

A 28-year-old woman, previously diagnosed with seronegative rheumatoid arthritis, was admitted with fever, vomiting, and diarrhea. She developed septic shock with disseminated intravascular coagulation and was treated with antibiotics, noradrenaline, prednisolone, and thrombomodulin. On day 3, a rash appeared, and polymerase chain reaction detected Orientia tsutsugamushi deoxyribonucleic acid, thus confirming the diagnosis of scrub typhus. Her symptoms resolved after minocycline treatment. After tapering the prednisolone, fever, rash, and arthralgia recurred. Laboratory tests revealed elevated

Psoriatic arthritis is a chronic inflammatory disease associated with psoriasis, and its diagnosis can be challenging owing to nonspecific symptoms, absence of reliable biomarkers, and occasional delay in skin manifestations. Herein, we report a case of psoriatic arthritis that initially presented as an acute finger inflammation mimicking infection. A 46-year-old woman developed sudden swelling and pain in the left ring finger during chemotherapy for cervical cancer. Based on the results of the physical examination, laboratory tests, and magnetic resonance imaging, pyogenic flexor tenosynoviti

Hemophagocytic lymphohistiocytosis (HLH) is a disorder characterized by dysregulated immune activation triggered by various etiologies, leading to excessive inflammatory responses, cytokine storm, and subsequent organ dysfunction. While the presence of hemophagocytic cells in septic shock is generally considered a physiological response that seldom requires specific intervention, the finding of abundant hemophagocytes along with Escherichia coli(E. coli) on a bone marrow smear warrants careful assessment for possible HLH. We herein present a fatal case of HLH secondary to an E. coli urinary tr

BACKGROUND: Whipple's disease (WD) is a rare systemic infection caused by Tropheryma whipplei, often misdiagnosed as seronegative rheumatoid arthritis (RA) due to overlapping clinical features. Delayed diagnosis may lead to multisystem complications, particularly in patients treated with immunosuppressive or biologic therapies.
CASE PRESENTATION: We describe two cases of WD initially misdiagnosed as refractory seronegative rheumatoid arthritis. Both patients were middle-aged men with persistently elevated inflammatory markers and poor responses to multiple biologic treatments. One developed c

BACKGROUND: Microplastics (MPs) are among the many ubiquitous environmental contaminants of emerging concern for both aquatic and terrestrial species. Bats have integral roles in aquatic-terrestrial food webs on almost every continent, are exposed to a wide variety of environmental contaminants, and yet have received limited investigation about the threat of MPs. While MPs have been detected in numerous bird species and in bats of the Amazon, there are no published studies documenting the dietary MP exposure of North American bats that consume many terrestrial and aquatic arthropods or the pos

Cases involving both the induced membrane technique and intramedullary beaming are generally rare. Here, we report such a case in an 83-year-old man who suffered from left midfoot pain. Pyogenic arthritis was suspected based on clinical findings, and curettage was performed, revealing an extensive bone defect. The patient was clinically diagnosed with seronegative rheumatoid arthritis. Therefore, the patient underwent both induced membrane technique and intramedullary beaming for the extensive bone defect in the talus and navicular regions caused by seronegative rheumatoid arthritis. The patie

This report describes the case of an 85-year-old woman who was found to have seronegative rheumatoid arthritis during preoperative investigations for knee replacement surgery for osteoarthritis. Follow-up for knee osteoarthritis was continued for many years without any symptoms involving other joints. Before total knee arthroplasty for osteoarthritis, preoperative investigations revealed a highly inflammatory state. After differential diagnosis, seronegative rheumatoid arthritis was diagnosed. Even in preoperative investigations before total knee arthroplasty for very old patients with osteoar

Calcium pyrophosphate deposition (CPPD) disease is a rheumatic disorder frequently observed in elderly individuals, often presenting with symptoms resembling polymyalgia rheumatica (PMR). Despite differing treatment strategies for these conditions, a definitive method for distinguishing between them remains unestablished. We report the cases of four elderly patients initially presenting with polymyalgia (PM), ultimately diagnosed with CPPD disease. Early-stage CT scans in all cases demonstrated chondrocalcinosis (CC) in the shoulder and hip joints. These findings suggest that PM in CPPD diseas

OBJECTIVES: Although with great advances, the biomarkers used in the diagnosis of rheumatoid arthritis (RA) are still challenging, especially in seronegative RA. Recently, procollagen C-endopeptidase enhancer (PCOLCE) was revealed as a potential autoantigen of seronegative RA by our group. This study aimed to further analyse the B-cell autoantigenic epitopses of PCOLCE and the diagnostic value of its antibody in seronegative RA.
METHODS: The B-cell epitopes of PCOLCE were elucidated by the Immune Epitope Database and molecular mimicry analyses. The anti-citrullinated PCOLCE271-284 antibody (a

Organizing pneumonia (OP) is a pulmonary inflammatory disorder involving the alveolar airspaces and ducts and resulting in interstitial lung disease. It is usually corticosteroid responsive. There have been reports of corticosteroid-resistant and refractory cases of organizing pneumonia (OP) requiring alternative treatment options, including immunosuppressants and cytotoxic agents. There are only a handful of cases of organizing pneumonia (OP) treated successfully with intravenous immunoglobulins (IVIg). We describe the novel case of a patient with a history of seronegative rheumatoid arthriti

OBJECTIVES: To evaluate clinical prognosis following the introduction of a first targeted therapy (TT) according to the serological profile of rheumatoid arthritis (RA) and to analyze differences in efficacy of TT.
METHOD: This single-center retrospective study included patients with RA who received a first TT between 2000 and 2020. Patients were seropositive (IgM and/or IgA rheumatoid factors plus anti-CCP) or seronegative (without autoantibodies). Various data were collected at baseline and during follow-up. The primary endpoint was remission (assessed by DAS28) at one and two years.

A 36-year-old man with a history of arthritis, initially diagnosed as seronegative rheumatoid arthritis, developed new-onset SLE complicated by HLH and TEN. The patient presented with fevers, abdominal pain, vomiting, fatigue, rash, and significant weight loss. Despite multiple hospital visits and antibiotic treatments, his symptoms persisted. On admission, he exhibited extensive erythema, targetoid macules, full-thickness desquamation, and hemorrhagic crusting, covering about 30% of his body surface area. Laboratory findings revealed pancytopenia, positive ANA, anti-chromatin, dsDNA, hypocomp
