The longitudinal orthopaedic history of 107 unselected patients with rheumatoid arthritis undergoing reconstructive surgery was studied to define the relationship between serology and surgery. A 43-year-old woman with seronegative rheumatoid arthritis developed a selective IgA deficiency during aurothioglucose treatment. No antibodies against IgA were detectable in the serum; normal numbers of IgA, IgG and IgM positive cells were demonstrated by immunofluorescence studies on a biopsy specimen from the small intestine. These data would suggest a defect in IgA secretion rather than in production by plasma cells.

Seronegative rheumatoid arthritis (RA) patients have increased proportions of OKT-4+ (helper) T cells and diminished proportions of OKT-8+ (suppressor/cytotoxic) T cells in the peripheral blood. An increased level of glycoproteins (seromucoid, protein-bound hexoses, protein-bound hexosamines, protein-bound sialic acids) was found in the blood serum from patients with rheumatoid arthritis (RA) as compared with healthy individuals. Higher glycoprotein content was observed in the seropositive RA patients than in the seronegative ones. The numbers of pokeweed mitogen (PWM)-induced IgM-, IgG- and IgA-secreting cells (SC) in mononuclear cell cultures from patients with seropositive and seronegative rheumatoid arthritis (RA) were reduced compared to those of control persons. Lactate concentration was studied in 383 synovial fluid specimens from patients with various arthritides. The highest concentrations of lactate occurred in non-gonococcal septic synovial fluids.
