Effect of trace elements and nutrients on 21 autoimmune diseases: a Mendelian randomization study.

BACKGROUND: Numerous clinical studies have observed a close relationship between serum trace elements, nutrients, and autoimmune diseases. However, whether there is a genetic causal effect between serum trace elements, nutrients, and autoimmune diseases remains unclear. OBJECTIVE: This study aims to investigate the causal effects of serum trace elements and nutrients on 21 autoimmune diseases using Mendelian randomization (MR). METHODS: Single nucleotide polymorphisms for the exposure factors (serum trace elements and vitamins) were obtained from the published UK Biobank database and genome-

A Diagnosis of Exclusion - Adult-Onset Still's Disease: A Case Report.

Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder characterized by high spiking fevers, arthralgia or arthritis, and a distinctive salmon-colored evanescent rash. Diagnosis is clinical but can pose a challenge for providers given its rarity and variable clinical presentation. Here, the authors present the case of a 58-year-old Caucasian male exhibiting fever, myalgias, hyperferritinemia, polyarthralgia, and diffuse pruritic rash. While a rash is part of the typical presentation for AOSD, the diffuse pruritus seen in this patient is atypical. Several differentials were

Evaluation of Myocarditis in Patients With Still Disease: Clinical Findings From the Multicenter International AIDA Network Still Disease Registry.

OBJECTIVE: We aimed to (1) evaluate the cardiac involvement, with a focus on myocarditis, in patients with Still disease included in the multicenter Autoinflammatory Disease Alliance (AIDA) Network Still disease registry; and (2) assess the predictive factors for myocarditis by deriving a clinical risk patient profile for this severe manifestation. METHODS: A multicenter observational study was established, in which consecutive patients with Still disease in the AIDA Network Still disease registry were characterized by cardiac involvement. Cardiac involvement was defined according to the pres

Non-Classical Complications of Adult-Onset Still's Disease: A Multicenter Spanish Study.

Objective: To investigate the prevalence and clinical spectrum of atypical or non-classical complications in adult-onset Still's disease (AOSD) beyond macrophage activation syndrome (MAS) and to identify factors linked to their occurrence. Methods: Multicenter cross-sectional study of AODS cases included in the Spanish registry on Still's disease. Results: This study included 107 patients (67% women), of whom 64 (59.8%) developed non-classical complications. These include macrophage activation syndrome in 9.5%, atypical skin manifestations in 38.8%, cardiac involvement in 22.7% (comprising per

Evaluation of performance of the Still Activity Score for assessment of Adult-onset Still's Disease: Comparative study with Systemic Feature Score and Modified Pouchot-Activity Score.

BACKGROUNDS: Adult-onset Still's Disease (AOSD) is a systemic inflammatory disorder. There is no definitive AOSD activity indicator. Two of the currently used disease activity scores are the Modified Pouchot Activity Score (mPoS), and Systemic Feature Score (SFS). Another scoring system has been recently introduced, named the Still Activity Score (SAS). AIMS: In this single-center cross-sectional study, we aimed to compare the performance of the SAS with the mPoS and SFS, both of which have been used for a long time for measuring disease activity in patients with AOSD. METHOD: 69 patients ag

Achievement and Usefulness of Intermediate Treatment Targets for Still Disease Proposed by the European Alliance of Associations for Rheumatology and the Paediatric Rheumatology European Society.

OBJECTIVE: Intermediate treatment targets in Still disease have been proposed by the European Alliance of Associations for Rheumatology (EULAR) and the Paediatric Rheumatology European Society (PReS) in 2024. This study aimed to evaluate the utility of the targets in clinical practice. METHODS: Consecutive patients with adult-onset Still disease (AOSD) based on the Yamaguchi criteria who visited Keio University Hospital from April 2012 until May 2024 were retrospectively reviewed. We assessed the achievement rates of the following treatment targets: day 7 (resolution of fever and reduction of

Long-Term Health Consequences of SARS-CoV-2- Trigger for Still Disease in Adults.

Still disease is a rare inflammatory disorder often triggered by infections or malignancies, with SARS-CoV-2 as a possible factor. This paper explores a 56-year-old patient who developed adult-onset Still disease after COVID-19. Symptoms included fever, arthralgia, rash, hepatomegaly, and joint swelling. Diagnosis followed Yamaguchi criteria and treatment with glucocorticoids and immunosuppressants led to recovery. The paper discusses similarities between Still disease and severe COVID-19 that suggest shared inflammatory mechanisms, particularly IL-1 activation. Early diagnosis and treatment a

Endogenous Lipoid Pneumonia in Adult Autoinflammatory Disease.

We describe one of the first cases of endogenous lipoid pneumonia (ELP) in an adult patient with the clinical picture of adult-onset Still disease (AOSD) and a low penetrance genetic background of tumor necrosis factor receptor-associated periodic syndrome (TRAPS). This case highlights the complex pathophysiology of lung involvement in autoinflammatory diseases, operating at the interface of the innate and adaptive immune system. This case presents a 53-year-old immunocompromised woman with treatment refractory autoinflammatory disease and history of macrophage activation syndrome (MAS), prese

Ruxolitinib targets JAK-STAT signaling to modulate neutrophil activation in refractory macrophage activation syndrome.

Macrophage activation syndrome (MAS) is believed to be caused by inappropriate proliferation and activation of the mononuclear phagocytic system. Adult-onset Still disease (AOSD) is characterized by neutrophil activation and a cytokine storm, which can lead to the severe and potentially life-threatening complication of MAS. RNA sequencing revealed that neutrophils may play a distinct role and enhance the innate immunity of patients with AOSD with MAS (AOSD-MAS). In the CpG-induced secondary hemophagocytic lymphohistiocytosis (HLH) model, the depletion of neutrophils significantly reduced cytok

TREATMENT OF HEMOPHAGOCYTIC LYMPHOHISTIOCYTOSIS IN THE INTENSIVE CARE UNIT: TOO LATE AFTER RESORTING TO INVASIVE ORGAN SUPPORTS?

Hemophagocytic lymphohistiocytosis (HLH) is a severe hyperinflammatory syndrome that can lead to early death from multiple organ failure. In secondary HLH, etiological treatment is an emergency, but sometimes impossible until complementary results (microbiology, histology) are available. A "bridge to etiological treatment" with anti-inflammatory drugs (anti-JAK, anti-cytokines or etoposide) is then essential. This study was conducted to determine the right time to start such treatment. Methods: We conducted a retrospective study at the University Hospital of Marseille on ultraselected (HS scor

Macrophage activation syndrome-associated adult onset Still disease treatment: a scoping review of case reports and case series.

OBJECTIVE: One of the most lethal complications of adult-onset Still disease (AOSD) is macrophage activation syndrome, which can be present later in the case of uncontrolled AOSD or concurrently with the first diagnosis of AOSD. Up to the present, there has not been a standard guideline of treatment for AOSD associated with macrophage activation syndrome (MAS). Therefore, this systematic scoping review aimed to examine the available literature and provide an overview of treatment based on the available literature, mostly case reports and case series. METHODS: A search strategy combined terms

Diagnosis and outcomes of fever of unknown origin cases with an erythrocyte sedimentation rate of 100 mm/h or more: An International ID-IRI (Infectious Diseases - International Research Initiative) Observational Retrospective Cohort Study.

Herein, we aimed to analyze the final diagnosis in a well-defined cohort of fever of unknown origin (FUO) cases whose erythrocyte sedimentation rate (ESR) was 100 mm/h or more during the admission. The subgroup of the FUO patients with an ESR of 100 mm/h or more during the FUO evaluation, was extracted from the study database of a previously published multicenter study (European Journal of Clinical Microbiology & Infectious Diseases. April 15, 2023;42 (4):387-98). Data for 139 patients (17.6%, 139/788 of the original cohort) who fulfilled the study inclusion criteria, were obtained from 29 cen

Challenges on diagnosis and treatment of refractory adult-onset still disease in Sub-Saharan Africa: a case report.

BACKGROUND: Adult-onset-Still's disease is a rare, multi-systemic condition with difficult and delayed diagnosis, characterized by a diversity of signs and symptoms related to hyperactivation of macrophages and neutrophils. Although diagnosis in the classical form is easily realized, there are atypical presentations that can be challenging, particularly in the absence of diagnostic means. CASE PRESENTATION: We report the case of a 28-year-old single African woman who had been experiencing prolonged fever and diffuse inflammatory joint pain for months, and who had manifested odynophagia and a

Lymphoma risk in autoimmune diseases with multiple medication use: analysis from the LIFE Study.

Autoimmune diseases (AIDs) are associated with an increased risk of lymphoma. Although treatment strategies have changed substantially in AIDs, recent evidence regarding this association is lacking. Particularly, the impact of novel immunosuppressive/immunomodulatory agents and the cumulative effect of multiple medication use on lymphoma risk have not been well investigated. To address these, we used the LIFE Study database, which contains health care claims data from 2014 to 2023 in 15 municipalities in Japan. Of 2,229,423 individuals, 211,592 had AIDs and 530 subsequently developed lymphoma.
