Clozapine-associated systemic inflammatory reaction with adult-onset Still's disease-like features and marked interleukin-18 elevation in a patient with schizophrenia: a case report.

BACKGROUND: Clozapine can trigger early inflammatory adverse reactions ranging from transient fever to organ-dominant syndromes. It remains unclear whether this spectrum can also present as a systemic adult-onset Still's disease (AOSD)-like phenotype. CASE PRESENTATION: A 54-year-old Japanese woman with long-standing treatment-resistant schizophrenia, psychogenic polydipsia with water intoxication, and overweight/obesity underwent olanzapine-to-clozapine cross-titration. Under the standard Japanese Clozaril Patient Monitoring Service procedure, clozapine was increased to 75 mg/day by day 23.

Reported Clinical Practice in Still Disease in France in 2024: A Descriptive Analysis and Comparison With Current Different National Guidelines.

OBJECTIVES: Still disease, which includes systemic juvenile idiopathic arthritis (sJIA) and adult-onset Still disease (AOSD), has historically had age-related treatment differences. With the advent of bDMARDs and evolving disease knowledge, management approaches have shifted. Our study aims to describe 2024 reported clinical practices for Still disease in France, and compare them with 2017 and 2024 national guidelines to identify practice gaps. METHODS: This study is a French subanalysis of the European JIR-CliPS project, which evaluates reported prescribing practices for inflammatory disease

Adult-onset Still's disease presenting with periorbital erythematous rash and progressive interstitial lung disease.

A man in his 80s with chronic obstructive pulmonary disease presented with fever, arthralgia and periorbital erythema resembling a heliotrope rash, accompanied by progressive interstitial lung disease (ILD). Laboratory investigations revealed neutrophilic leucocytosis and markedly elevated C-reactive protein, ferritin and Krebs von den Lungen-6. Dermatomyositis, particularly clinically amyopathic dermatomyositis, was considered in the differential diagnosis; however, myositis-specific autoantibodies were negative and histopathological findings supported a diagnosis of adult-onset Still's disea

Factors associated with elevated serum IgG4 in rheumatic diseases: A retrospective single-center observational study.

Elevated serum immunoglobulin G4 (IgG4) levels have been reported in various rheumatic diseases; however, the factors associated with IgG4 elevation remain incompletely understood. This study aimed to evaluate the frequency of elevated serum IgG4 in different rheumatic diseases and to explore its association with disease classification based on typical inflammatory characteristics and autoantibody status. This retrospective single-center study included 255 patients with rheumatic diseases who underwent serum IgG4 testing between January 2017 and December 2022. Patients were categorized for exp

Examination of HLA-DRB1*15-linked Candidate Antigens in Still Disease With and Without Lung Disease and Drug-Associated Immune Reactions.

OBJECTIVE: Lung disease (LD) in systemic juvenile idiopathic arthritis and adult-onset Still disease (Still-LD) is a severe manifestation strongly associated with HLA-DRB1*15 alleles and drug-associated immune reactions (DAIR), including eosinophilia, non-Still disease rashes, and elevated liver function tests. Despite the high morbidity and mortality of these phenomena, pathogenesis remains poorly understood. This study investigates whether Still-LD and DAIR are associated with pathogenic antigens through hypersensitivity reactions to Aspergillus fumigatus, for which HLA-DRB1*15 is a known ri

Refractory macrophage activation syndrome in adult-onset Still's disease: response to interferon-γ blockade and JAK inhibition.

A female patient in her mid-20s presented with fevers, rash and polyarthralgia and was ultimately diagnosed with macrophage activation syndrome (MAS), a widely recognised form of secondary haemophagocytic lymphohistiocytosis in the setting of autoinflammatory disease such as adult-onset Still's disease. Extensive infectious and malignancy workup was negative. Her clinical course was complicated by persistently active disease despite initial immunomodulator therapies, including corticosteroids and anti-interleukin-1 therapy (anakinra). Further escalation of therapy with tocilizumab and etoposid

Secondary Haemophagocytic Lymphohistiocytosis Triggered by Adult-Onset Still's Disease in Mid-trimester Pregnancy Initially Presenting as an Upper Respiratory Tract Infection: A Case Report.

Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder, characterised by fever, arthritis, and an evanescent rash. A severe complication is macrophage activation syndrome (MAS), also known as secondary haemophagocytic lymphohistiocytosis (HLH), a life-threatening state of uncontrolled immune activation that may lead to multiorgan failure. Diagnosis during pregnancy is particularly challenging because of overlapping clinical and biochemical features. We report a 32-year-old African female primigravida at 16 weeks' gestation, presenting with sore throat, dysphagia, chest pai

Delayed Diagnosis: Adult-Onset Still's Disease Initially Mistaken for Tuberculosis in a Nigerian Man.

Adult-onset Still's disease (AOSD) is a rare autoimmune disorder characterised by fever, rash, joint pain, and elevated ferritin levels. It is extremely difficult to diagnose due to its overlap with other autoimmune and infective causes. If left untreated, systemic inflammation can rarely trigger an exaggerated immune response, leading to secondary haemophagocytic lymphohistiocytosis (HLH) or macrophage activation syndrome (a secondary subtype of HLH). This case demonstrates AOSD complicated by HLH in a Nigerian man in his 30s with fever, shortness of breath, night sweats, and joint pain, wher

Successful salvage treatment with baricitinib for macrophage activation syndrome complicating adult-onset Still's disease during interleukin-6 inhibition: A case report and literature review.

Adult-onset Still's disease (AOSD) is a systemic autoinflammatory disorder, and macrophage activation syndrome is a life-threatening hyperinflammatory complication. Interleukin-1 and interleukin-6 inhibitors have improved outcomes in refractory AOSD, but optimal management of macrophage activation syndrome, particularly when it develops during biologic therapy, remains uncertain. We report a case of a 49-year-old woman with articular-predominant AOSD who developed fulminant macrophage activation syndrome while receiving high-dose glucocorticoids, tacrolimus, and the interleukin-6 receptor inhi

Comorbidity of autoimmune diseases in patients with autoimmune pulmonary alveolar proteinosis.

BACKGROUND: Autoimmune diseases coexisting with autoimmune pulmonary alveolar proteinosis (APAP) are rare. Therefore, the clinical characteristics of complications of these diseases are not well known. METHODS: We examined clinical data of APAP patients with autoimmune diseases as a retrospective study in a single center. RESULTS: Among 100 APAP patients, ten (10.0%) had autoimmune diseases including rheumatoid arthritis (n = 3), polymyalgia rheumatica (n = 1), adult-onset Still's disease (n = 1), polymyositis (n = 1), immune thrombocytopenia (n = 1), autoimmune hepatitis (n = 1), dermatomyo

Causal Associations Between Nine Autoimmune Diseases and Idiopathic Thrombocytopenic Purpura: A Mendelian Randomization Study.

INTRODUCTION: Clinical evidence suggests the presence of comorbidity between Autoimmune diseases (ADs) and secondary Idiopathic thrombocytopenic purpura (ITP), whereas the underlying causal relationship remains to be established. This study employed Mendelian Randomization (MR) analysis to investigate potential causal relationships between multiple common ADs and the risk of ITP. METHODS: Using criteria of P<5×10-8 , P<5×10-6 , and F-statistic>10, and excluding single nucleotide polymorphisms (SNPs) with confounding, palindromic sequences, and linkage disequilibrium as instrumental variables

Development of a predictive tool for hemophagocytic lymphohistiocytosis/macrophage activation syndrome risk in adult-onset Still's disease.

BACKGROUND: Hemophagocytic lymphohistiocytosis/macrophage activation syndrome (HLH/MAS) is one of the most common complications of adult-onset Still's disease (AOSD), and a leading cause of organ failure and mortality in these patients. Early diagnosis and management are crucial but can be challenging. Our study aims to develop a tool for predicting the risk of HLH/MAS in AOSD patients. METHODS: The study recruited AOSD patients admitted to Ruijin Hospital from May 2018 to May 2024. Clinical features and laboratory test results were collected within the first 3 days of hospitalization. Predic

Progressive macrocytic anaemia over 14 years leading to the diagnosis of VEXAS syndrome in a patient initially diagnosed with adult-onset Still's disease.

VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a recently recognised autoinflammatory disease caused by somatic mutations in UBA1. Its clinical features include recurrent fever, systemic inflammation affecting multiple organs, and progressive haematologic abnormalities such as macrocytic anaemia. Because these manifestations overlap with other autoinflammatory diseases, including adult-onset Still's disease, the diagnosis can be challenging. In this case report, we describe a patient initially diagnosed with adult-onset Still's disease who was ultimately found to

Adult-Onset Still's Disease Presenting With Persistent Hyperpigmented Scaling Rash in a Young Ethiopian Woman: A Case Report From a Resource-Limited Setting.

BACKGROUND: Adult-onset Still's disease (AOSD) is a rare systemic autoinflammatory disorder characterized by high-spiking fever, arthritis, rash, and systemic inflammation. Because there is no single diagnostic test, diagnosis relies on compatible clinical features, fulfillment of classification criteria, and exclusion of infectious, malignant, and autoimmune conditions. Atypical cutaneous manifestations may further delay recognition, particularly in resource-limited settings. CASE PRESENTATION: We report the case of a 22-year-old Ethiopian woman who presented with a five-month history of inf

A Case of Adult-Onset Still's Disease Diagnosed Following Treatment for Scrub Typhus.

A 28-year-old woman, previously diagnosed with seronegative rheumatoid arthritis, was admitted with fever, vomiting, and diarrhea. She developed septic shock with disseminated intravascular coagulation and was treated with antibiotics, noradrenaline, prednisolone, and thrombomodulin. On day 3, a rash appeared, and polymerase chain reaction detected Orientia tsutsugamushi deoxyribonucleic acid, thus confirming the diagnosis of scrub typhus. Her symptoms resolved after minocycline treatment. After tapering the prednisolone, fever, rash, and arthralgia recurred. Laboratory tests revealed elevated

Acute Adult-Onset Still Disease Presenting as Fever of Unknown Origin and Severe Myalgia in a Resource-Limited Tertiary Hospital in Brazil.

This case highlights the diagnostic challenges of managing a patient who initially presented with a fever of unknown source and subsequently met the criteria for fever of unknown origin (FUO) after more than 3 weeks of persistent fever despite an extensive diagnostic workup in a resource-limited tertiary hospital within Brazil's public health system. A man in his sixth decade presented with prolonged high-grade fever and severe lower-limb myalgia, initially raising suspicion for leptospirosis due to rural exposure and occupational history. Extensive evaluation-including blood cultures, serolog

Diagnostic performance of a dual western blot for the simultaneous detection of anti-HTLV-1 and anti-HIV-1 antibodies.

INTRODUCTION: There is no consistent definition of burnout. It is neither a defined diagnosis in ICD-10 nor in DSM-IV. Yet it is diagnosed by office-based doctors and clinicians. Mainly due to reimbursement reasons, diagnoses like depression are used instead of burnout diagnoses. Therefore burnout has a very high individual, social and economic impact. OBJECTIVES: How is burnout diagnosed? Which criteria are relevant? How valid and reliable are the used tools? What kind of disorders in case of burnout are relevant for a differential diagnosis? What is the economic effect of a differential diagn

Challenges for the effectiveness of infectious disease interventions during global change.

BACKGROUND: Sequential treatment of ankylosing spondylitis (AS) that includes tumour necrosis factor-α antagonists (anti-TNF agents) has been applied in most of the Western countries. Existing cost-effectiveness (CE) models almost exclusively presented the incremental CE of anti-TNF agents using a closed cohort while budget impact studies are mainly lacking. Notwithstanding, information on impact on total population health and societal budget as well as on actual incremental CE for a given decision time span are important for decision makers. This study aimed at quantifying, for different deci
